In the practice of cos-metic dentistry, if the pa-tient’s face is to retain a « well-proportioned » ap-pearance, the following 2 rules must be respected:

1. The incisal plane must be parralel to the inter-pupil lary l ine,

2. The interincisal mid-l ine must be paral lel to the facial midl ine.

In order to manufacture prostheses that are na-tural-looking and estheti-

cally pleasing, the dental technician must apply these two key rules.

However, given the ab-sence of a way of ac-curately communicating the patient’s facial data to the dental laboratory (interpupillary line and facial midline), various esthetic adjustments are often unavoidable, which has a negative impact on the efficiency of the den-tal practitioner / dental technician partnership.

Ditramax® is the first de-vice enabling a patient’s facial data to be transfer-red to the laboratory. It is especially useful in ante-rior maxillary restoration work and major cosme-tic treatment. Ditramax®

THE DITRAMAX ® DENTAL DEVICE THE MAIN STEPS IN USING THE DITRAMAX ® SYSTEM FREQUENTLY ASKED QUESTIONS

The 2 lines that have been marked out bring the 3 fa-cial characteristics of the patient that need to be ta-ken into account to ensure dental prostheses are fit-ted in the most esthetically pleasing way possible:

1. THE HORIZONTAL LINEas seen from the front

Indicates the orientation of the interpupillary line,

2. THE HORIZONTAL LINEas seen from the side

Shows the orientation of the Camper’s plane,

3. The vertical line

Indicates the position of the facial midline.

6. Remove the device from the patient’s mouth, place it on its stand and remove the Camper’s rods. Screw the tracing mount and place the working cast into the indendations of the silicone.

2. Adjust the align-ment of the interpu-pillary line by mo-ving the frame to the right or left until the rule is horizonta-ly aligned with the pupils. Lock into po-sition using knob 2.

7. Using the stylus, trace a horizontal line (thus transferring the interpupillary line and Camper’s plane) and a verti-cal line (represen-ting the facial mi-dline) upon the cast.

3. Center the frame on the facial mi-dline (as seen from the front) by turning knob 3 in the appro-priate direction util the central point of the rule is aligned with the facial mi-dline.

8. The cast is ready to be sent to the la-boratory. It contains all the information necessary to en-sure the resultant prosthesis will be as natural-looking and esthetically pleasing as possible.

4. Ensure the facial midline is centered laterally by adjusting the frame until the ends of the 2 Cam-per’s rods are the same distance from their respective tra-gus. Lock into posi-tion using knob 4.

1. Get the patient to bite upon the bite fork coated with silicone. Slide the rule up or down until perfect verti-cal alignment with the pupils. Lock the rule into position using knob 1.

5. Adjust the device until it is aligned with Camper’s plane by tilting the device un-til the 2 rods are pa-rallel to Camper’s plane (that inter-sects the tragus and the subnasal point). Lock into position using knob 5.

The device for transferring patients’ facial data to the laboratory

Is Ditramax® a facebow?

No. The facebow, which enables the maxillary cast to be mounted onto the articulator, is a very useful tool with regard to occlu-sal rehabilitation, but from an esthetic point of view it often causes problems. In many patients, the Frank-fort plane (the reference plane for the articulator) is not parallel to the inter-pupillary line. Therefore, crowns that are in the right position on the arti-culator end up being in a slanted position in the pa-tient’s mouth. The key aim of Ditramax® is to ensure that prostheses are fitted in the most esthetically pleasing way possible.

Is Ditramax® compatible with all articulators ?

Yes. Ditramax® works by tracing a line along the base of the working cast. Therefore it does not have any impact on the arti-culator, regardless of the type of articulator used.

Is the taking of measure-ments using Ditramax® a difficult or lengthy process ?

Ditramax® is designed to be extremely easy to use. There are two main steps to carry out:

1. Taking the esthetic fa-

cial data of the patient,

2. Transferring this data onto the working cast by means of tracing lines with the stylus.

These 2 steps take under 10 minutes to complete.

Do any specific details or instructions have to be gi-ven to the laboratory ?

Yes. As the Ditramax® sys-tem is quite new, we re-commend that the dental technician is given speci-fic instructions on how this facial data is to be used. You will receive a guide for this purpose with your Ditramax® pack. We re-commend you send this guide to the laboratory along with the first casts that have been marked using the Ditramax® sys-tem. Are the measurements ta-ken reliable ? Can they be reproduced ?

A major advantage of the Ditramax® system is that it is based around a bite fork rather than being tis-sue-borne, as is the case for the facebow. It is the-refore « as one » with the patient’s head when the measurements are being

taken, thus delivering re-liable measurements that can be reproduced.
