1.0 Policy

ItisthepolicyofCSUChannelIslandsthatentryintoaconfinedspacewillbedoneinaccordancewithOSHAstandardsandotherregulatoryguidelines. Noindividualshallenteraconfinedspacewithout followingtheConfinedSpaceEntryProgram.

2.0 Purpose/Scope

2.1 Purpose ToestablishaclearandconsistentprocedureforconfinedspaceentryontheCSUChannelIslandscampus.

2.2 Scope TherequirementsofthisdocumentapplytoallCSUChannelIslandsemployeesandemployeesofauxiliaryorganizations.

3.0 Definitions

• AlternateEntryProcedure: Aprocedurethatmaybeusedtoenteraconfinedspaceiftheonlyhazardpresentinthespaceisatmosphericinnatureand:

§ Theatmospherichazardcanbecontrolledbymechanicalventilationalone;and

§ Thepermitspacewillnotbecomeimmediatelydangeroustolifeandhealth(IDLH)ifthemechanicalventilationshouldfail.

• Attendant: AnindividualstationedoutsideoneormorepermitrequiredconfinedspaceswhomonitorstheauthorizedentrantsandwhoperformsallAttendant'sdutiesasassignedinthisprogram.

• AuthorizedEntrant: ACSUChannelIslandsemployeewhoisauthorizedbythecampustoenterapermit requiredconfinedspace.

• ConfinedSpace: Aconfinedspacemeansaspacethat:

1. Islargeenoughandsoconfiguredthatanemployeecanbodilyenterandperformwork;and

2. Haslimitedorrestrictedmeansforentryorexit(forexample,manholes,vaults,andpitsarespacesthatmayhavelimitedmeansofentryorexit);and

3. Isnotdesignedforcontinuousemployeeoccupancy.• EntryPermit: Thewrittendocumentthatallowsandcontrolsentryintoapermitrequired

confinedspace.

• EntrySupervisor: Thepersonresponsiblefordeterminingifacceptableentryconditionsarepresentatapermitspacewhereentryisplanned,forauthorizingentryandoverseeingentryoperations,andforterminatingentryasrequiredbythisprogram.

• HazardousAtmosphere: Anatmospherethatmayexposeemployeestotheriskofdeath,incapacitation, impairmentofabilitytoself-rescue,oracuteillnessresultingfromthefollowing:

1. Flammablegas,vapor,ormistinexcessof10percentofitslowerflammablelimit(LFL);

2. AirbornecombustibledustataconcentrationthatmeetsorexceedsitsLFL;3. (note:thisconditionmaybeapproximatedasaconditioninwhichthedustobscures

visionatadistanceof5feetorless)4. Atmosphericoxygenconcentrationbelow19.5percentorabove23.5percent;5. Atmosphericconcentrationofanysubstancethatcouldresultinanexposure

greaterthanthePermissibleExposureLevel(PEL).6. Anyotheratmosphericconcentrationthatisimmediatelydangeroustolifeand

health(IDLH).• ImmediatelyDangeroustoLifeorHealth(IDLH): Anyconditionthatposesanimmediate

ordelayedthreattolifeorthatwouldcauseirreversibleadversehealtheffectsorthatwouldinterferewithanindividual'sabilitytoescapeunaidedfromaconfinedspace.

• LowerFlammableLimit(LFL): Theminimumvaporconcentrationofaflammableliquidinair,belowwhichflamepropagationdoesnotoccuroncontactwithanignitionsource.

• Non-PermitConfinedSpace: Aconfinedspacethatdoesnotcontainor,withrespecttoatmospherichazards,havethepotentialtocontainanyhazardcapableofcausingdeathorseriousphysicalharm.

• OxygenDeficientAtmosphere: Anatmospherecontaininglessthan19.5percentoxygenbyvolume.

• OxygenEnrichedAtmosphere: Anatmospherecontainingmorethan23.5percentoxygenbyvolume.

• PermitRequiredConfinedSpace: Aconfinedspacethathasoneormoreofthefollowingcharacteristics:

1. Containsorhasapotentialtocontainahazardousatmosphere;2. Containsamaterialthathasthepotentialforengulfinganentrant;3. Hasaninternalconfigurationsuchthatanentrantcouldbetrappedorasphyxiated

byinwardlyconvergingwallsorbyafloorwhichslopesdownwardandtaperstoasmallercross-section;or

4. Containsanyotherrecognizedserioussafetyorhealthhazard.

• Testing: Theprocessbywhichhazardsthatmayconfrontentrantsofapermitspaceareidentifiedandevaluated.

• Ventilation: Aprocessusedtocontroltheatmospherichazardsofconfinedspacesbyreplacingunsafeairwithclean,breathableair.

5.0 Responsibilities

5.1 Environmental Health and Safety 1. EstablishandupdatethewrittenConfinedSpaceEntryProgram.2. Providetrainingasnecessary.3. Reviewrecordsofconfinedspaceentryoperations.4. Provideconsultationtodepartmentsthatenterconfinedspaces.

5.2 Facilities Services 1. Ensurethatallemployeeswhoareinvolvedwithaconfinedspaceentry(including

AuthorizedEntrant,EntryAttendant,andEntrySupervisor),havereceivedtrainingasspecifiedinthisplan.

2. Provideandmaintainequipmentthatisneededforconfinedspaceentryincluding(butnotlimitedto):atmospherictestingequipment,protectivebarriers,ventilationdevices,andcommunications/rescueequipment.

3. EnsurethatallconfinedspaceentriesinvolvingFacilitiesServicesemployeesareconductedaccordingtothisprocedure.

4. Maintainrecordsofallconfinedspaceentries.5. Maintaininventoryofconfinedspaces.6. Postconfinedspacesasappropriateandpreventunauthorizedentry.

5.3 Police Department 1. Dispatchtrainedofficerstoprovideimmediatemedicalassistanceintheeventof

aninjury.2. Summonexternalresourcesforconfinedspacerescueormedicalassistanceas

needed.

5.4 Communication Services (Information Technology) 1. Ensurethatallemployeeswhoareinvolvedwithaconfinedspaceentry(including

AuthorizedEntrant,EntryAttendant,andEntrySupervisor)havereceivedtraining.2. EnsurethatallconfinedspaceentriesinvolvingCommunicationsServices

employeesareconductedaccordingtothisprocedure.

5.5 Confined Space Entry Supervisor 1. Ensurethatallpersonsinvolvedinaconfinedspaceentryareproperlytrainedand

followtheproceduresoutlinedinthisplan.2. Evaluateandclassifytheconfinedspacepriorto.Thesupervisorisresponsiblefor

verifyingthesafetyofthespace.3. Confirmthatrescueresourcesareavailable.4. CompletetheConfinedSpaceEntryPermit.5. Ensurethatcanceledpermitsareproperlyfiled.

Note: TheEntrySupervisordoesnotneedtobepresentduringtheentireconfinedspaceentryoperation.

5.6 Confined Space Entrant(s) 1. Knowthehazardsinthespace2. Knowthesignsandsymptomsofexposuretohazardspresent.3. Musthavecompletedtraining4. MuststayinconstantcontactwiththeAttendant(i.e.sight,voice,orradio)duringa

PermitRequiredConfinedSpaceentry.5. Mustevacuateimmediatelywhenbecomeawareofaproblemororderedtodoso

byattendant.6. UsetheappropriatesafetyequipmentasspecifiedontheEntryPermit.

5.7 Entry Attendant 1. Knowthehazardsinthespace2. Musthavecompletedtraining3. ObtainandinstalltherequiredsafetyequipmentforPermitRequiredConfined

Spaceentries.4. Monitorpedestrianbarrierstoprotectentrant(s)fromexternalhazards.5. Monitortheatmospherewithintheconfinedspaceduringtheentireentry

operation.6. Mustnotenterspace;mustremaininpositionuntilrelievedbyanothertrained

Attendant.7. Ensurethatalllockout/tagoutmeasures(ifimplemented)remaininplace.8. Responsiblefororderinganevacuationifasituationdevelopsthatcouldendanger

theEntrant9. Inanemergency,mustcallforhelpandperformnon-entryrescue.

6.0 ConfinedSpacesonCampus1. EH&SandFacilitiesServiceswilljointlydeterminewhichtypesofspacesorspecific

spacesareclassifiedas"ConfinedSpaces". Further,spaceswhicharenormallyexpectedtobe"PermitRequiredConfinedSpaces"willbeidentified. Alist andmapsofthesespacescanbefoundinAppendixC.

2. Theonlyindividualsauthorizedtodowngradetheentryrequirementsforaparticularspacearedesignated"EntrySupervisors".

7.0 ConfinedSpaceEntryProcedures

7.1 Space Evaluation

Note: RefertoAppendixAforConfinedSpaceEntryDecisionFlowChart

1. AllPermitRequiredConfinedSpacesaretobeevaluatedbyanEntrySupervisorpriortoauthorizinganentry(refertoAppendixAforalistofthesespacesoncampus).

2. Theprimarypurposeoftheevaluationwillbetodetermineif:

a) thespacecanbereclassifiedtoa"Non-PermitConfinedSpace"b) theentrywillbevia"AlternateEntryProcedures"orc) theentrywillbevia"PermitRequiredEntryProcedures"

7.2 Reclassification to Non-Permit Confined Space

Tobereclassifiedasa"Non-PermitConfinedSpace,"thefollowingconditionsmustexist:

a) Thespaceposesnoactualorpotentialatmospherichazardb) Allhazardswithinthespaceareeliminatedwithoutentry

7.3 Reclassification to Alternate Entry

Tobereclassifiedasan"AlternateEntry"space,thefollowingconditionsmustexist:

a) Theonlyhazardinthespaceisthepotentialforahazardousatmosphere;andb) Thepotentialhazardcanbeeliminatedthroughtheuseofcontinuousforced

ventilation;andc) Previousandcurrentatmosphericmonitoringdata(gastests)must

demonstratethat(a)and(b)aretrue(i.e.therehasneverbeenarecordofanatmospherichazardduringanentry).

• Iftheaboveconditionsaretrue(a,b,andc),andthegastest(s)indicatethattheatmosphereisnon-hazardous,isthenthespacemaybeenteredusingthe"AlternateentryProcedures"below.

• Ifthepotentialforatmospherichazardcannotbeeliminatedwithventilation,thespacemustbeenteredusingthe"PermitRequiredEntryProcedures"

7.4 Alternate Entry Procedures

1. EntrySupervisorshallensurethatspacehasbeenclassified.

2. Ifanentrancecovermustberemoved,theopeningshallbepromptlyguardedbyrailing, temporarycover,orothertemporarybarrierthatwillpreventanaccidentalfallthroughtheopening,andwillprotecteachemployeeworkinginthespacefromforeignobjectsenteringthespace.

3. Allhazardoussourcesofenergyshallbelockedand/ortaggedout.

4. Ensurethattheairhasbeentestedforhazardousaircontaminantsandthatthetestswerenegative.

a. Note:thisshouldhavebeencompletedpriortoreclassifyingspaceas"AlternateEntry"). Logthetestresultsonthe"ConfinedSpaceEntryPermitandRecordSheet"(onlypre-entryreadingsneedtobeentered). Thegasdetectorshouldbeusedduringtheentireentryprocedure. Testforthefollowingcontaminants,intheorderlisted:

b. Oxygencontent;

c. Flammablegasesandvapors;and

d. Potentialtoxicaircontaminants.

5. EntrySupervisorshallfilloutthe"ConfinedSpaceEntryPermitandRecordSheet"(AppendixC).

6. Setupadequateventilationforthespacethatwillbeentered. Ensurethatthesupplyairistakenfromacleansourceandthattheventilationisallowedtooperateforatleast5minutespriortoenteringthespace. Iftheventilationceasestooperate,allEntrantsmustleavethespace immediately.

7. TheEntrantshallimmediatelyevacuatetheconfinedspaceifhe/shebecomesawareofapreviouslyunrecognizedhazard. Ifthisoccurs,theEntrancePermitshallbecanceled,andthespacemustbereevaluatedbyanEntrySupervisorpriortore-entry.

Note: 1. TheEntrySupervisormayactasanentrantduring"AlternateEntryProcedure"entries.

8. AnattendantorStandbypersonisstillnecessaryfor"AlternateEntryProcedure"entries.

9. Whentheconfinedspaceentryhasconcluded,thecompletedRecordSheetshallbefiledandkeptonfileforaminimumofone(1)year.

7.5 Permit Required Entry procedures 1. Asaminimum,anEntrySupervisor,EntryAttendantandEntrantshallbe

designatedtoparticipateineachPermitRequiredConfinedSpaceEntry. Note:TheEntrySupervisormayactastheEntryAttendantaswell.

2. Acquire,inspectandsetupandsafetyequipmentrequiredbythepermitincludingblowers,fullbodyharness,rescuetripod,andtrafficcontrolsystems.

3. TheEntrysupervisorshallestablishappropriaterescueproceduresspecifictothespaceentryandshalllisttheseproceduresonthepermit.

4. Allhazardoussourcesofenergyshallbelockedand/ortaggedout.

5. Ifanentrancecovermustberemoved,theopeningshallbepromptlyguardedbyrailing,temporarycover,orothertemporarybarrierthatwillpreventanaccidentalfallthroughtheopening,andwillprotecteachemployeeworkinginthespacefromforeignobjectsenteringthespace.

6. TesttheairforhazardouscontaminantsandentertheresultsontheConfinedSpaceEntryPermitandRecordSheet.Continuouslysampleandrecordresultsthroughouttheentire entryprocedure.

7. TheEntrySupervisorshallcompleteandsigntheConfinedSpaceEntryPermit(allsections)andRecordpriortospaceentry. Thepermitconditions,entryprecautions,andrescueproceduresshallbereviewedwiththeAttendantandEntrantpriortoentry.

8. Priortoentry,theAttendantshallverifythattheentryconditionsareacceptable.

9. TheEntrantmustwearretrievalequipmentduringtheentireentryoperation.

10. TheAttendantwillcontinuouslymonitortheportablegasdetectorandrecordthereadingseveryhour(minimum).

11. Animmediateevacuationofthespaceshallbeorderedifthesafetyequipmentfailsorifthespacebecomes,orhasthepotentialtobecome,immediatelyhazardous.

12. Uponcompletionofthejob,theEntrySupervisorshallcancelthepermit(line12)andensurethatthecompletedpermitisplacedintheproperdepartmentfile.

8.0 HazardControl

8.1 Monitoring Confined Space Air Quality 1. PriortoenteringaPermitRequiredorAlternateEntryconfinedspace,prepare

thegasdetectorbyensuringthedevicehasbeenrecentlycalibrated(checkmanualforfrequency)andisingoodworkingorder.

2. Conducta"bumptest". Breatheintothegasdetectorprobeandlookforadecreaseintheoxygenconcentration-thedetectorshouldalarmduetothelackofoxygen.

3. Sampletheairqualityofthespacebyslightlymovingthelid,orbytestingthespacethroughtheholeinthelid(ifavailable)beforecompletelyopeningthespace.

4. Lowertheprobeslowly,allowingtimefortheinstrumenttodetectatmosphericchangesatdifferentverticalheightswithinthespace.

5. Measureinthefollowingorder:

1. Oxygen2. LEL3. H2S4. CO

RecordtheresultsontheConfinedSpaceEntryPermitRecordSheet.

6. ThegasdetectorshallremainonduringtheentireentryoperationshallberegularlyinspectedbytheAttendant. TheAttendantshallalsorecordthegasreadingsontheRecordSheetataminimumofone(1)hourintervals.

7. Ceaseentryoperationsandremoveentrypersonnelifthefollowingconcentrationsareexceededatanytime:

• Oxygenreadinglessthan19.5%orgreaterthan22.5%

• Combustiblegasreadinggreaterthan9%LEL

• H2Sreadinggreaterthan9ppm

• COreadinggreaterthan34ppm

8.2 Ventilating a Confined Space 1. Setuponeormoreblowerstoprovideadequateventilationforthespace.

Ventilationmustbeforceddraftdischargeofcleanairintospace(notexhaustofspace).

2. Ensurethattheventilationairsupplyisfromacleansource.3. Allowenoughtimeforblowerstoclearthespacebeforeentering.4. Ensurethattheblowersremainonduringtheentireentryoperation.Iftheblower

fails,theEntrantmustleavethespaceimmediately(Note:thisappliestobothPermitRequiredandAlternateEntryprocedures.)

9.0 EmergencyRescuefromPermitRequiredConfinedSpaces

Historically,manyrescuersarekilledwhileattemptingaconfinedspacerescue. Inlightofthisfact,Emergencyrescueswithinconfinedspacesshouldonlybeattemptedbypersonswhoareproperlytrainedandhavetheproperrescueequipment.

1. AllrescuesattemptedbyCSUChannelIslandspersonnelwillbeNon-EntryRescues(i.e.rescuerwillnotenterthespaceatanytimeduringrescue).

2. RetrievalequipmentisrequiredforallPermitRequiredverticalentriesgreaterthan5ft.Retrievalequipmentshallinclude(butnotbelimitedto)thefollowing:

• Rescueharness(chestorfullbody)• Rescuetripod• Ropeorcable(inspectedfordamage)

3. EntrySupervisorwillnotifythePoliceDepartmentbydialing911inanemergency.

4. Allpersonsinvolvedintherescuemusthavereceivedtrainingandpracticedarescuewithinthelast12months.

5. AtleastonememberoftherescueteammusthavetraininginCPRandFirstAid.

10.0 Training1. AllCSUChannelIslandsemployeeswhoparticipateinPermitRequiredor

AlternateEntryProceduresincludingEntranceSupervisors,Attendants,andEntrants,mustreceivetrainingintherequirementsofthisprogram.

2. AllCSUChannelIslandsemployeeswhoaredesignatedasrescuepersonnelmustbetrainedinproperrescuetechniquesaswellasCPRandFirstAid.

11.0 Contractors3. ContractorsthatenterconfinedspacesontheCSUChannelIslandscampusmust

complywiththeConfinedSpaceStandardasdefinedinSection5156-5158ofTitle8oftheCaliforniaCodeofRegulations.

4. CSUChannelIslandswillnotifycontractorsofconfinedspacesandhazardsthereinwhencontractedworkwillrequireentry. AcontractornotificationformcanbefoundinAppendixB.

5. CSUChannelIslandsisnotresponsibleforemployeesofcontractorswhomayenterUniversityconfinedspaces.

This is a “NON-PERMIT REQIURED CONFINED SPACE”. All hazards have been eliminated

CHECKLIST INSTRUCTIONS: The Entry Supervisor shall answer and initial each checklist item as it is completed. The Entry Supervisor shall authorize work to begin by signing below, only after all checklist items have been appropriately addressed. The Entry Supervisor shall cancel the permit by signing below after work is completed, or if conditions arise that are out of compliance with the checklist YES NO N/A Initial A. Have all personnel been appropriately trained and instructed in CSE procedures? B. Have emergency communications and action procedures been identified and explained? C. Are the air blowers in use sufficient to maintain an atmosphere free from contaminants? H. Is the gas detector within calibration limits and operating properly? I. Is the space and surrounding area free of harmful vapors and gases? J. Has the appropriate fall protection / retrieval equipment been installed and inspected? K. Has the body harness been properly donned and inspected? L. Is a fire extinguisher available? M. Is explosion proof equipment required (i.e. lighting, radios, blowers, etc)? N. Is an appropriately maintained First Aid Kit available? O. Is the entry area secured and marked with barriers? P. Is traffic control equipment in place?
