RISK MANAGEMENT TOOLS : TAKE 5 ASSESSMENT



Take 5, essentially a last minute personal risk assessment has been designed to encourage all of us to identify hazards associated with all tasks before work commences.

The proactive process aims to enhance our safety culture through continual self-evaluation and is based on the principle of ‘ENGAGING THE MIND BEFORE THE HANDS’.

All of us must take time before the job to conduct a simple, mental review of each step of our task to identify all hazards and what is needed to control them.



Before starting any task, ask yourself the following questions:

1.Do I understand the task that I am going to do?2.Do I know the hazards associated with this task?3.Do I understand the risks involved when proceeding with this task?4.Are there adequate controls in place to prevent me from being hurt?5.Am I sure that I can complete this task safely?

If the answer to any of these questions is no, the task MUST NOT proceed and you must seek immediate instruction from your supervisor.

The Take 5 Risk Assessment card has been developed and is available to be used for guidance while implementing this tool into your daily work.
