• Light housekeeping/laundry

• Bed tuck service

• A phone call to family to notify them that their loved

one is home


Extended Care Plans Available

Care can be extended beyond the one-time Hospital to

Home package to help clients maintain their independence.

Continued care plans include a nurse assessment of the

client’s needs, a personal care plan based on this

assessment and a fall prevention safety check of the

client’s home.
