Example Post Discharge

Workflow:

Scheduled Visit

Patient at home clicks on appointment reminder email or

logs into hospital patient portal to install virtual visit app.

SBR System matches patient with appropriate virtual clinic.

Receptionist takes call, makes sure patient’s audio/video is

set up properly and takes any last minute details.

Receptionist transfers patient to appropriate physician

queue and places patient into Virtual Waiting Room.

Patient is shown educational videos in waiting room.

Surveys or other reading materials can also be presented.

Doctor is alerted about patient readiness, can view waiting

queue on mobile device/tablet and can place calls.

When ready, the doctor takes patient out of waiting room.

SBR system manages call setup on both ends.

If needed, doctor can bring in other clinicians by name, role

or skill. Calls can also be handed off.

The patient, doctor and resource are able to connect, and

have sharing capabilities of the EMR or PACs system.

Patient can be transferred back to receptionist for routing to

additional providers or for scheduling follow-up.

Clinicians are always in control of their availability.

Carts, rooms, clinics, ED’s, etc can also be mapped to

available resources.

Example Clinical Consults

Workflow:Ad Hoc Requests

Visiting nurse, assisted living attendant, ER clinician or

remote clinic is in need of real time consult for assistance.

skill we do not

have currently

on site.

Interpreter

For instance, patient does not speak English so interpreter

is needed.

Or a consult with a specialist or tertiary facility is needed.

Triage form can be used then parsed by SBR system to

automatically determine where to route consult request.

Request is routed to the appropriate facility, department,

physician, and placed in a service request queue.

Appropriate covering Intensivist is notified of waiting

consult request.

Call can be taken on mobile device if Intensivist is not near

video terminal.

Call can also be transferred to desktop or cart, and imagery

and other information shared as needed.
