This man was empowered to advocate for himself with the City. He was able to convince the City to make a local fishing hole accessible for his wheelchair.

Nursing Home Transition Program

People with Disabilities are often a large percentage of

those who are chronically homeless through no fault of

their own

Some time ago Chris was homeless in

Cedar City

• After a divorce Chris, who’d lost his leg in a vehicle accident, was forced to move from home to home depending on the generosity of friends.

• At times he even lived on the street.

Chris Continued: Insult to Injury

• These stays at friends were punctuated by long stays in Nursing Homes.

• Where he recovered from injuries sustained from living in places that did not accommodate for his disability.

Red Rock Center’s Nursing Home

Transition Coordinator• Jordi met Chris while he

was in a Nursing Home.

• She provided advocacy, resources and encouragement.

• Chris provided the enthusiasm and willingness to work to leave the Nursing Home.

• And….

Now Chris has an accessible apartment to call home.
