Case-control studies
• Compare groups retrospectively
• Identify cause of the disease, and a suitable control group of those without it– Match for 3-4 characteristics– Compare the groups’ exposure to the
variable(s) of interest– Calculate the risk for each group
Steps in a case-control study
• Identify cases • Select controls
– Preferably matched to cases by 3 or 4 characteristics
• Obtain information about– Relevant exposures– Confounders

Sources of bias in a Case Control Study
• Selection bias• Eg. Controls selected from hospital population
instead of general population
• Information bias– Misclassification of participants : disease or
exposure status• Eg. Parents of kids with osteosarcoma may be more
likely to remember earlier exposures because they will have thought about them more

Cohort studies• Take a group (“cohort”) and follow it
forward (prospectively) through time• Powerful design
– Natural history of disease
– \Determine whether exposure precedes outcome
– Exposure measurement more valid– Dynamic disease measures

Cohort studies are good when:
• We know which exposures are worth investigating
• We can minimise loss to follow-up
• The conditions under study are relatively common
They comprise the strongest observational design for establishing whether a cause-and-effect relationship exists

- Randomization: assures equal distribution of confounders between study and control groups
- Restriction: subjects are restricted by the levels of a known confounder
- Matching: potential confounding factors are kept equal between the study groups
- Stratification for various levels of potential confounders
- Multivariable analysis (does not control for effect modification)
Effect modification can be assessed by:
- Stratification for various levels of potential confounders
- Multivariable analysis (by assessing interaction)
More importantly, NOT by adjustment in multivariable analysis
Reverse causation can be assessed by:
- Mendelian randomization
What if you are interested in the rare outcome?
• Heart failure in adults before age 50

• Cases: Adults with premature heart failure (18-50 years)– General medicine vs. cardiology
• Controls: Adults 18-50 without heart failure– Who are the appropriate controls?
• Potential predictors: based on questionnaire demographic, behavioral, co-morbid risk factors
• Hypothesis: Patients with hypertension early in adulthood are more likely to have premature heart failure.
