Useful References1 This chapter is included for purpose of general information pertinent to certain codes
& other requirements that can be referred to2 British & US codes references have been in general, included as used to be profes-
sional practice in past decades3 Currently in addition to those codes, EN codes have also been in practice4 Tried to enlist as many codes references, as could be practically, possible based on

Software List in General1 OLGA 2000-dynamic/slugging study,
a (Olga is a Norwegian petroleum software originally, developed by IFE for Statoil in 1983.

2 INFOWORKS-drainage network modeling, (Potential use of software for urban drainage forecasting)
3 FRED- Shell FRED (fire, risk, explosion & dispersion) software, a Fault tree+ Fault tree, event tree & markov analysis software. b (Fault Tree Analysis uses tree structures to decompose system level failures into
combinations of lower level events and Boolean gates to model respective interactions.
c To address safety and ways failures or undesirable events that could occur, while trying to avoid them can be very challenging.
d To improve system safety and reliability, designers and analysts have to make better informed risk assessment decisions based on quantified evaluations of risks and effects, associated with alternative designs and other risk mitigating actions.
e These challenges are more compounded, where manual methods of fault isolation and risk analysis in large and complex systems are not viable.
f Using failure probability and system reliability data can assist in addressing undesirable events or challenges.
g Often times, Fault Trees are used in reliability and safety risk assessments to represent graphically, logical interactions and probabilities of occurrence of component failures and other events in a system.

Useful Referencesh A Fault Tree analysis identifies and ranks combinations of events represented on a
Fault Tree, that cause system failure and provides estimates of system's failure probability.
i Using this detailed information, efforts to improve system safety and reliability can be highly focused and tailored to individual system by using quantitative results from data input. Additionally, Fault Tree analysis can help prevent failures from occurring by providing with data showing, how and under what circumstances, failure could occur, allowing for alternative measures to prevent (or design out) catastrophic failures or hazards.
j Boolean methodology and equations are used to construct and simplify Fault Tree. k As Fault trees are constructed, Boolean equations are used to evaluate Quantitative
and Qualitative characteristic of a critical system. l Qualitative analysis of Fault Tree determines:
i. Probability of system failure (top event) based on a single failure (basic event) cause or common cause potential using minimal cut sets,
ii. Combination of component failures (minimal cut sets), iii. Importance ranking of contributors to system failure.
m Quantitative analysis of Fault Tree focuses on probabilities of system and cut set failure or occurrence of top event based on probabilities of failure of basic events.)
4 T hazop- Hazop software, T risk- QRA tool, a (Software incorporates features and facilities that:b Serve as a framework, within which preparation for review can be structured.c Ease task of recording meeting minutes and help to maintain team's focus of attention
and interest.d Give speedy access to material useful to study team, such as previously, identified
problems, failure rate data and other such historical information. e Allow professionally, formatted reports to be produced with minimum of effort.f Permit additional management information to be extracted from study records.g Provide a comprehensive and easy to use system for effective action follow up and
close out, without significant administrative burden that this usually entails)
