Lymphomes = diversité histologique et clinique
Lymphomes
• Environ 12 000 nouveaux cas par an en France
• Les cellules tumorales ressemblent à leur contrepartie normale : importance de l’architecture, morphologie, phénotype, expression génique, profil mutationnel
• 25 à 30% sont des lymphomes extra-ganglionnaires
• La chirurgie est réservée à l’étape diagnostique
• Le traitement repose essentiellement sur de la chimiothérapie +/- immunothérapie
• Les rechutes sont fréquentes et parfois tardives
Incidence (1)
• En France, les LNH représentent la 6ème cause de cancers (www.dep-iarc.fr), et l’augmentation constante des taux d’incidence (3-4% par an dans les années 70 et 80, 1-2% dans les années 90), en grande partie inexpliquée, a abouti à un doublement du nombre de cas nouvellement diagnostiqués sur les trente dernières années.
• Dans les pays industrialisés, cette augmentation est plus
marquée pour les lymphomes agressifs et chez les sujets de plus de 65 ans. L’incidence des LNH croît de façon exponentielle entre 20 et 79 ans
Incidence (2)
• L’augmentation de l’espérance de vie (aujourd’hui estimée en France à 77 ans pour les hommes et 84 ans pour les femmes (www.insee.fr) laisse prévoir une augmentation constante dans les années à venir de la fréquence des LNH chez les sujets âgés.
• On peut estimer qu’un tiers environ des nouveaux diagnostics de LNH sont portés aujourd’hui chez des sujets de plus de 70 ans.
