On behalf of all of us here at Extended Care we wish our consulted facility staff a happy, healthy and safe
new year.
2019 will be another year of changes for Nursing Homes – don’t we say that every year? Two new
programs started in October 2018 for CMS that affects our facilities operations and one new program
starts in October 2019. All three programs need our staff ’s commitment in training and helping each
facility be successful.
These programs are: • Quality Reporting Program (QRP) - QRP started in October 2018 and if a facility is not performing
well (or close to National Rates) the facility’s Medicare payment rate may be reduced up to 2%.
These Quality Reports are on care issues that we have been focusing on heavily over the last few years
like: Pressure Sores developing or worsening in a facility and Falls with Major Injuries. So our work
in these areas must continue and everyone needs to be involved.
• Value-Based Purchasing Program (VBP) - VBP started in October 2018 and focuses on better
outcomes to the residents with Medicare (hospital stay) particularly focusing now on reducing
hospital readmissions. This is also something we have been working with our facilities on adding
Nurse Practitioner programs to some facilities; working with the residents’ Physicians are not
immediately giving discharge orders but assessing concerns and working with facility staff on
keeping the residents out of the hospital; create a “hot list” of those residents in their first 30 day
window of admission to the facility to monitor even more closely to avoid having to send to the
hospital.
• Patient Driven Patient Model (PDPM) – PDPM starts October 2019 and shifts a facility’s Medicare
Part A stay in a nursing home away from therapy minutes as the driver of the stay and creates a
model of therapy linked to clinical characteristics of the resident and creates a separate payment
piece for non-therapy ancillary services. Additional training will be conducted over the next several
months so we are all ready for the October change.
So what can our consulted facility staff do to help in these new programs? Educate yourself on these
changes, listen to leadership in changing the way we do things and practice good, safe care of our
residents.
It is not too late to get your FLU Vaccine. Influenza (flu) is a potentially serious disease that can lead to hospitalization and sometimes even
death. Every flu season is different, and influenza infection can affect people differently, but millions of
people get the flu every year, hundreds of thousands of people are hospitalized and thousands or tens of
thousands of people die from flu-related causes every year.

HAPPYNEW YEAR!
ChateauC H E C K O U T the event at
The Staff at Chateau felt it was very important to give back to their
community. In November, several staff members volunteered at People’s
Resource Center, the local food pantry for DuPage residents.
This December Chateau held a collection for People’s Resource Center.
They collected non-perishable food, household items, baby items and
children’s books. Chateau was very elated by the amount of donations that
they received from staff and resident’s families alike. They were able to
deliver two very large boxes of donations right before
Christmas. Neighbors helping neighbors.
Chateau held a Breakfast with Santa for
residents and staff families. It was a magical day
for all the children that attended. The residents
loved seeing their family member’s excitement
to see Santa. We are pretty sure a few of the
adults slipped Santa their wish list as well!
A New Year Is Here!

As we wrap up 2018 and enter into 2019 the
words “A New Year is here” can be heard in the
halls at Sebos Nursing and Rehabilitation. This
is the fresh start we have been waiting for! It is
time to begin anew and with renewed vigor. We
are looking forward to making and keeping those
new year’s resolutions, visualizing new goals
and making them reality and moving forward
toward success, growth and change! Here at
Sebos we are committing to making this our
BEST YEAR YET, a year that will bring all of us
closer to achieving our personal and professional
goals. We look forward to renewed hope, new
opportunities, new adventures and new ways to
give and love our residents. As Maya Angelou said
“We may encounter many defeats but we must
not be defeated!” 2018 was filled with lots of
love and loss and we were able to meet and care
for many new residents and families. In 2018 we
welcomed several new faces to Team Sebos. We
welcomed, Jillian, our new Administrator, who
joined us late December. We are excited to work
with her! Christina, our Director of Nursing who
has over 13 years hospital experience. Florence
and Milissa, who joined our Admissions and
Marketing team, have a combined total of
over 30 years of experience and Charlene, our
new Human Resource Director who is always
willing to help with a smile! We are looking
forward to see what 2019 has in store for Team

While facility staff enjoyed their holiday
parties and raffles at their facilities, the ECC
employees and support staff, Propay HR, Hunter
Management, and facility Administrator’s
enjoyed a festive holiday party at Kings Dining
& Entertainment in Rosemont, IL on December
12th. The various activities included a delicious
buffet dinner, bowling, and karaoke. Mr. Rothner
thanked everyone for their hard work and success
in 2018 and conveyed his best wishes for the
holidays and upcoming year. Ron Nunziato, CEO
of ECC and Ephraim Vilenski of Propay HR,
raffled off several prizes, including flat screen
tv’s, Echo dots, tablets, gaming systems, and
various other fantastic giveaways. From all of us
at Extended Care, we wish our ECC extended
families a healthy and prosperous 2019!
Grasmere employees enjoyed a retro-themed holiday party.
Employees enjoyed soul train entertainment, a costume contest
and took part in an epic raffle, where they won tv’s, jewelry, gift
cards, an Echo Dot, cologne and blankets. One of our long
standing employees, Malika Griffin, was awarded with the best
costume award and Employee of the Year! Congrats Malika and
wishing all of our employees happy holidays!
