The second plot (gated on real cells)
is a side scatter vs CD45 plot. This
plot allows one to distinguish major
cell populations and obtain a "flow
differential". Lymphocytes express
low SS and bright CD45.
Granuolocytes express high SS and
bright CD45. Monocytes express
moderate SS and bright CD45.
Apoptotic cells, erythroid precursors
and plasma cells are negative CD45
and variable SS. Blasts (if present)
are dim to negative CD45 and
variable SS.
Each population as defined by CD45
and SS is painted a distinct color
which is carried throughout the entire
analysis layout for ease of
recognition.
The first plot is a forward scatter (size) vs
sidescatter (granularity/complexity) plot. A
gate is drawn to identify and exclude the
"debris" which could be nonviable or
fragmented cells (low forward scatter),
platelets or other small particles. All
events except debris is denoted as "real
cells".

The next five plots are forward
scatter (size) vs each individual
marker (fluorescence). This coupled
with the specific populations in
defined colors can provide valuable
information about the proportions
and contents of the sample tube.
Notice the red lymphocyte
population expresses some CD19
(B cell), no CD10 and
predominantly CD5 (T cell)
expression. This along with the
small size identifies a normal
lymphoid population. The
granulocyte and monocyte
populations are larger (high FS) and
negative for CD19 and not CD5.
These serve as internal negative
and positive controls for the
antibodies and control that gating is
set correctly.
The flow differential shows a
significant population of CD45
dim/negative, low side scatter
cells. This predominant
population corresponds with
the blasts seen on the
smear.
There is a decrease in the
granulocyte component.

in the lymphocyte gate. CD19 is a pan B
cell marker, CD5 is a pan T cell marker
and CD10 is a marker present on some
malignancies. CD19 and CD5 expression
in normal lymphocyte populations show
mutually exclusive populations.
Co-expression of CD19 and CD5 (if
present) is diagnostic for chronic
lymphocytic leukemia (CLL).
The summary of markers in this
tube are gated on the lymphocyte
population. The lymphocytes
express some CD5 and minimal
CD19. CD10 is not expressed.
It is presumed these represent the
residual normal lymphocytes in the
sample.

negative/dim populaton expresses
CD19+CD20+, without kappa or lambda
light chain expression. There is a
portion of the lymphocytes (red) that
express CD19+CD20+ (pan B cell
marker). There is a significant
lymphocyte cluster that is negative for
CD19+CD20+. It is presumed that
these are the T lymphocytes in the
sample. In addition, a population of
lymphocytes express kappa and
lambda expression.
These serve as internal negative and
positive controls for the antibodies.

The plots can identify a clonal excess

surface of B lymphocytes which
indicates malignancy.
The residual lymphoid population shows
a subset of CD19+CD20+ lymphocytes
which express a normal kappa lambda
ratio.

In normal lymphocytes, one would expect the

lymphocytes. These markers are helpful in
assessing T and NK cell malignancies.

CD56 is a natural killer cell marker.
Please note CD3 is a lineage specific

marker CD56 is predominantly
negative. These represent the

tube. In this tube, different T cell
subset (CD4 and CD8) and the
activation marker CD38 are used In
normal lymphocytes, one would expect
the CD4 and CD8 expression of
lymphocytes to mark mutually
exclusive populations. The sum of the
CD4 + CD8 expression should add up
to the total CD3 population (prior tube).
These markers are helpful in assessing
T cell cell malignancies and identifying
activated cells.

In the FS vs CD38 plot one can
estimate the plasma cell population.
If more than 0.5-1% of the population
is CD38 bright, additional studies
such as CD138, CD56, CDD117,
cytoplasmic Kappa and cytoplasmic
Lambda may be added to further
characterize the plasma cells.
The next three plots show only the
cells in the lymphocyte gate. The
lymphocyte subset markers CD4
and CD8. The activation cell marker
CD38 is assessed on lymphocytes
and the respective lymphocyte
subset.
Plasma cells, if present in the
sample, will express very bright
CD38. In the last plot, we screen for
plasma cells by gating on only
those CD38+ cells that fall into the
fourth decade of fluorescent
intensity.
The gated lymphocytes
express the mutually exclusive
CD4 and CD8 populations The
CD4/CD8 ratio is normal .
These represent the residual
normal T lymphocytes in the
sample.
The activation marker CD38 is
expressed on the lymphocytes.
Plasma cells (bright CD38) are
below 1%.

The next six plots show only the cells in
the dim CD45 gate and their respective
marking for the cytoplasmic TdT, CD10
and CD19. TdT marks B lymphoblasts
(and some myeloblasts). CD10 marks the
CALLA (common acute lymphoblastic
leukemia antigen)
If blasts are present, these markers can
help assess and differentiate
lymphoblastic leukemia. .
The blasts express TdT, CD10
and CD19. TdT is coexpressed
on both the CD19 and CD10
cells.

DNA ploidy and S-phase fraction can
be a useful tool in assessing
hematologic malignancies.
The first plot is the ratio of peak vs
area, of the FL3 linear cell
fluorescence as it traverses the laser
spot. We have gated out any doublets
that may interfere with the analysis.
Doublets are cells that stick together
and give a false signal of twice the DNA
content.
The second plot allows us to select
different populations to assess the DNA
content. DNA content of the singlicate
gat includes both the normal
lymphocytes (that serve as a diploid
control) and the abnormal population.
The subsequent single parameter
histograms assess the DNA histogram
of the selected populations. In this case
all population show a normal diploid
histogram. Accurate S-phase can only
be assessed in cases with > 20,000
events.
The "flow differential" in this
specimen shows a bright CD45
(normal ymphoid population) and a
dim CD45 (blast) population.
Additional DNA studies using an
appropriate antibody in FITC (CD19
or CD20) vs DNA can provide more
specific data.
In this case the normal
lymphocytes serve as the diploid
control. The blasts are aneuploid
(hypodiploid). The S-phase fraction
appear high. Further analysis is
need to determine DNA statistics.

Summary of Statistics
This page is a summary of the
statistics on this case.
A dim/negative CD45, low SSC
population expresses the B
lymphoid markers CD19 (dim)
and CD10. HLA Dr, CD34 and
CD38 are also expressed. CD20,
CD23, FMC-7 nor kappa/lambda
light chains are expressed.
Myeloid, monocytoid and T
lymphoid markers are not
expressed.
Additional studies showed
cytoplasmic TdT, cytoplasmic
CD79a are also expressed on the
blasts.
This is most consistent with a
Pre-B Acute lymphoblastic
leukemia (ALL)
DNA content of the blasts is
aneuploid (hypodiploid) with a
high S-phase fraction.
The lymphocyte population
expresses a normal phenotype,
predominantly T cells with a
normal CD4/CD8 and B cells with
a normal kappa/lambda ratio.The
granulocytes in the sample
express a normal phenotype. This
serves as an internal control.
The "flow differential" identifies a
predominant dim/negative CD45
low SSC population, and a
decreased granulocyte
population. The proportion of
lymphocytes is normal.
