Executive Director

Contents

Families play many roles in our daily lives. Whether traditional, extended, or blended, family can be a place where many find stability and security. Family also is a place where its members’ dreams can be shared, supported and launched.

The end of 2013 marked the beginning of a new family—The ThedaCare Family of Foundations, Inc. Our new familyis comprised of seven already established member Foundations, including the Appleton Medical Center Foundation, Community Hospice Foundation, and the Theda Clark Medical Center Foundation. You are invited to read more about our new family on page 21. Once again, we believe that combining the stories of these three Foundations in one report is a prudent use of financial, environmental and staff resources.

This 2013 Report to the Community highlights the many ways you have helped us touch the lives of patients and families at a time of vital need. Collectively, generous donors helped us provide the gift of stability in supporting a home-like environment at the Appleton Medical Center Guest House. Contributions helped us bring a sense of security for families heading toward an uncharted, personal journey through hospice. And the community supported clinical education so our nursing team could be even more responsive to patients’ hopes and dreams. Your family’s care. Our family’s mission.

At age 65, Barb Kromholz and her family have been a part of the ThedaCare family for 29 years. They moved in right down the street from Appleton Medical Center (AMC), where Barb and her husband Steve raised a daughter and a son. Their new neighbor recommended a ThedaCare family physician.

Like most families, the Kromholz clan made their share of visits to the Emergency Department over the years, with a particularly memorable run to Theda Clark Medical Center when their son Jay broke his collarbone.

Steve also required emergency attention when their new puppy scratched his cornea. He also had a more serious trip to the ER, as well as follow up care at the Heart Institute at AMC, when they suspected a heart attack. In the end, he was fine after actually suffering a panic attack.

Barb herself had joint replacement surgery at AMC and years later joint fusion surgery at Theda Clark Medical Center.

Beautiful Inside and OutThedaCare Cancer Care cosmetologist Lisa Kellnhauser receives multiple thank you notes every year for helping those who lose their hair from chemotherapy. The following is justone example:

I found myself dreaming of my hair every night, and it was not pretty. I went to talk to someone because it had been falling out for some time now. It is a good thing that I have thick hair! It won’t be long and I will have no hair.

The hats, scarves, wig and service that [High Hopes Salon] can provide for us is great! I do not dream about my hair anymore. I know that I will be fine and look good now because of the things that you have given to me!

477Beautiful PatientsCosmetologist Lisa Kellnhauser met with 477 patients at the High Hopes Salon in 2013, an increase of more than 20 percent! A grant to update the salon and move it to a more private location within the Martha Siekman Cancer Center at AMC has helped raise awareness and given more patients the courage to use this supportive service.

GrantsAMC Foundation

iPads for Patient Education—CardiologyIt is crucial for patients and families to understand their cardiac condition, to know what tests are being ordered and why, and to be prepared for any necessary procedures and subsequent recovery. However, physicians have limited time with each patient to answer questions and adequately explain at a level the patient can understand.

In recognition of this issue, the American College of Cardiology created a patient education application—an app—that uses video to show exactly what happens in a variety of different exams and procedures. The video tutorials are interactive and easy to understand.

The AMC Foundation provided a $15,000 grant to purchase iPads for improved patient education at the Heart Institute at AMC. While the iPad apps aren’t intended to replace the conversations our physicians have with their cardiology patients, providers report that this feature has significantly enhanced our patients’ comfort level by allowing them to explore their condition in more depth at a slower pace.

Through your generosity, this project was funded by donationsdesignated for cardiac care.

AMC Guest House Renovation & UpgradeHome is where your family is. The AMC Guest House provides temporary housing for families of patients receiving care at Appleton Medical Center, particularly patients who are in critical condition with family who don’t live nearby. The AMC Guest House can also provide housing for patients who live more than 30 miles from the Martha Siekman Cancer Center and need to receive care daily during active treatment.

With the help of a $29,000 grant from the Appleton Medical Center Foundation, the Guest House was able to complete much-needed renovations, including carpeting throughout the house, updated appliances, new furniture and bedding, additional kitchen supplies, and televisions in all four bedrooms.

Through your generosity, this project was funded by proceedsfrom the Laughter is the Best Medicine event.

Lung Cancer Screening—Research ProjectLung cancer has a deservedly bad reputation. The second most diagnosed cancer in both men and women, lung cancer is the leading cause of cancer death in the United States and throughout the world. In fact, more people die of lung cancer each year than from colon, breast, and prostate cancers combined. One of the biggest issues with lung cancer survival is that it rarely causes any symptoms until the very late stages,once treatment options dwindle toproviding comfort.

The ThedaCare Cancer Care program received a $50,000 grant from Appleton Medical Center Foundation to pursue a unique and creative approach to detecting lung cancer at an earlier stage. Radiologist Anthony Eclevea, M.D., and vascular medicine internist Glenn Huth, M.D., will research the efficacy of imaging a larger view during chest CTs conducted for other clinical reasons. By “over reading” a chest CT for a coronary artery calcium score, patients can be screened for lung cancer in its early stages without exposing patients to any additional radiation.

The research includes capturing a larger image, modifying the radiologic interpretation process, and collecting certain additional demographic information for 1,000 patients to provide a statistically relevant and comprehensive database for comparison. Initial results of the first 750 subjects indicate that 185, or 25 percent, have been recommended for follow-up studies.

A sampling of additional grants funded in 2013 includes:

Build-a-Bear Voice RecordingsA patient diagnosed with Stage 4 breast cancer was able to record voice messages to each of her 10 grandchildren that were inserted in personalized Build-a-Bears. She gave the stuffed animals to her grandchildren as Christmas gifts to provide a lasting memory.

National Cancer Survivors Day CelebrationThedaCare Cancer Care sent cards to celebrate survival to former patients and supported healthy living through activities during the week leading up to the national day of celebration. Daily giveaways, drawings for gift baskets, a healthy cooking class, and other events helped us reach more than 1,500 local cancer survivors.

Continuing Education for Cancer CareBehavioral Health CounselorsTwo therapists who work exclusively with ThedaCare cancer patients were able to attend the American Psychological Oncology Conference. The conference is uniquely designed to improve the translation of research to practice in psychosocial oncology. As a result of what they learned at the conference, our therapists createda new group program called Living Fully to support both patients and caregivers.

The overall financial picture for the Appleton Medical Center Foundation is strong. The generosity of our charitable sponsorsand individual donors once again contributed to our financial stability. And thanks to the successful recovery of our

investment portfolio, net assets grew by more than 10 percent this year. This growth and stability enabled the AMC Foundationto grant nearly $750,000 this year in support of improved health for area families and local communities.

This number represents the funds raised by Appleton Medical Center Foundation through new contributions. This number does

not reflect pledge payments on any capital campaigns.

Our professionally managed investment portfolio continues to

make significant gains, successfully recovering from losses experienced

during the global economic recession in 2011.

The Foundation’s Board of Directors uses approximately five percent of the total unrestricted assets to fund programs, new equipment, and initiatives outside of capital

campaigns each year.

The Appleton Medical Center Foundation was able to improve

our net assets thanks to a strongermarket providing interest on

investments in 2013 and through the commitment of our generous

donors, as well as responsible fiscal management of available funds.

GrantsCommunity Hospice Foundation

Pet Therapy in Waupaca AreaA grant from Community Hospice Foundation helped expand the pet therapy program to Waupaca and the surrounding communities. This successful program provides animal companionship to hospice patients, who can often be quite lonely or who perhaps simply miss their own pets since moving into full-time hospice. One such relationship formed between pet therapy dog Hodge and his friend at the Wisconsin Veterans Home in King. They visited every Wednesday morning for two months. On the last Wednesday they spent together, Hodge licked his friend’s neck, ear, and hand to let him know Hodge was there to visit. As he lay quietly on his side, eyes closed as though sleeping, Hodge’s friend crackeda smile in acknowledgement. He passed away soon after.

Dining Room Table at Cherry MeadowsFamilies are nurtured around the dinner table. So the addition of a dining room table to Cherry Meadows was the obvious choice for two donors who wanted to “give back” to their hospice family. Susan Clapper and Keith Zimmerman both decided to direct the memorial gifts of their late spouses to Cherry Meadows in the form of this dining room table and chairs.

Patient Room Renovations at Cherry MeadowsThere’s no place like home. But sometimes at the end of life, we need a little extra help. It may be necessary to find a safer place to feel at home. Located within ThedaCare’s Peabody Manor in Appleton, Cherry Meadows is a home-like hospice setting featuring 10 private rooms, shared living areas, 24-hour staffing, and unlimited visiting hours. In addition to residential care, Cherry Meadows offers short-term inpatient care for pain or symptom management and respite care to temporarily relieve home caregivers.

An $80,000 grant from Community Hospice Foundation is making it possible to renovate and upgrade outdated patient rooms.Planned changes will provide a safer and more comfortable home away from home:• Security enhanced medicine cabinets• Wall-mounted digital televisions with wireless headphones for the

hearing impaired• Wheelchair friendly wood laminate flooring to replace worn

carpeting• New patient dressers with locks for securing valuables• Updated window treatments• New flame-retardant bed comforters

Hospice Hopes & Dreams Lobster Dinner

The Hospice Hopes & Dreams program was developed in an effort to help people at the end of life fulfill one last wish or even a lifelong hope or dream. Often the requests are simple, including things like recreating a special memory, attending an event, or travelling to see family or helping a relative travel to visit the hospice patient.

Rosemary, a patient at Cherry Meadows, loves lobster and remembers many special times shared over lobster dinners. Hospice Hopes & Dreams was able to cater a lobster dinner to Rosemary’s room to share with a friend, creating new memories to mingle with the past.

Through your generosity, this project was funded by donations designated for Hopes & Dreams.

Additional programs supported by Community Hospice Foundation include:

Forever in Our Hearts Photography Program This program offers the opportunity to capture a final portrait of the family as a celebration of their life together. The family receives one 11x13 portrait ready to frame.

Massage Therapy Massage has been shown to not only reduce physical discomfort, pain and nausea, but also decrease emotional distress and anxiety.

Volunteer ActivityVolunteers provide support to our patients and families each day. The volunteers may assist with meals, listen and read to patients, share hobbies, and more. Grant money from the Community Hospice Foundation offsets costs incurred by volunteers, allowing our volunteers to focus on what’s important: our patients.

59,600 Days of CareIn 2013, your contributions gave the gift of nearly 60,000 days of care to more than 1,300 patients.That averages out to almost 45 days per patient.

$213,000 in SavingsVolunteers provided approximately

The overall financial picture for the Community Hospice Foundation improved in 2013. Thanks to successful fundraising events,positive ongoing relationships with our charitable sponsors, and generous individual contributions, our total revenue has continued togrow for the second year in a row. In addition, our professionally managed investment portfolio almost doubled in gains this year. In all,

the net assets for the Community Hospice Foundation increased by nearly 10 percent, enabling CHF to provide close to $100,000 in grants.

This number represents the funds raised by Community

Hospice Foundation through new contributions. This number does not reflect pledge payments on

any capital campaigns.

Our professionally managed investment portfolio continues to

make significant gains, successfully recovering from losses experienced

during the global economic recession in 2011.

The Foundation’s Board of Directors uses approximately five percent of the total unrestricted assets to fund programs, new equipment, and initiatives outside of capital

campaigns each year.

ActionP.A.R.T.Y. at the PAC has reached more than 30,000 teens since it began in 1998. But this year, we reached out to parents, as well.

“Friends obviously have a big impact on a teen’s behavior,” shares Kathi Hegranes, ThedaCare Injury Prevention Coordinator. “But studies show that parents actually have more influence on children’s decisions and behavior than their friends.”

The new parent component not only raises awareness with parents about the importance of reinforcing good choices, it also reminds them to model positive behaviors themselves. Some of the parent feedback included:

GrantsTCMC Foundation

Birth Center Blanket WarmersWhen our family grows through a new addition, it’s important to keep both mother and baby the right temperature after childbirth. One of the ways the Theda Clark Birth Center helps babies maintain proper body temperature is by providing a warmed blanket. We’re able to provide blankets that are not too warm or too cold using Steris blanket warming machines. Unfortunately, two of our machines were so outdated that they could no longer maintain the correct temperature, and manufacturers could no longer service the old parts. A TCMC Foundation grant for $14,000 enabled the birthing center to purchase two replacement blanket warmers, keeping our littlest patients as safe and comfortable in their mother’s arms as they were in the protection of her womb.

ThedaCare Child Learning Center PlaygroundThedaCare makes it a priority to take care of our employee family’s real families at the ThedaCare Child Learning Center daycare. We practice what we preach through a safety project supported jointly by both Theda Clark Medical Center Foundation and Appleton Medical Center Foundation. The $19,000 grant helped build a safe playground for the daycare, so ThedaCareemployees can focus on patient care.

Through your generosity, this project was jointly funded by donationsdesignated as general/unrestricted.

Insty Med Prescription DispenserA full 93 percent of patients who come into the ER receive a prescription for treatment. Unfortunately, Neenah has no pharmacies that stay open 24 hours a day, and Appleton only has two near the hospital. Physicians not only want their patients to begin treatment as soon as possible, but patients want relief, too. Theda Clark Medical Center Foundation and Appleton Medical Center Foundation jointly supported a grant for Insty Med prescription dispensers to be purchased and installed in the ER waiting rooms for each hospital. Approximately 50-70 percent of ER prescriptions can be filled using these vending machine-style dispensers. Physicians can e-prescribe directly to the machine. And security is located nearby for safety.

A sampling of additional grants funded in 2013 includes:

iPads for The Stroke CenterThe iPads are used for therapy “drills” to address speech, language, voice and cognition. They also allow patients to observe the dynamic process of normal swallowing as well as disordered swallowing (known as dysphagia). Being able to view these processes helps patients to understand and accept the rationale when speech language pathologists recommend a modification to the patient’s diet or the use of a swallowing strategy.

ToughPad for ThedaStarAn in-flight computer for the ThedaStar helicopter—called a ToughPad—enables flight staff to monitor patient care reporting and easily consult treatment protocols. Compact, versatile and extremely

resilient, the ToughPad allows ThedaStar providers to quickly and efficiently document treatment under sometimes less-than-ideal conditions.

Continuing Education for Anesthesia NursesA grant from Theda Clark Medical Center Foundation made it possible for three registered nurses to attend the American Society of Peri-Anesthesia Nurses conference in 2013. As a result of what they learned at the conference, we have begun implementing an Obstructive Sleep Apnea questionnaire. Approximately 80 percent of patients experience this condition post-surgery. The new questionnaire accurately predicts which patients will experience Obstructive Sleep Apnea, reducing the need for emergency intervention.

The 2013 High Flyin’ Golf Outing raised more than $80,000 this year, an impressive increase of nearly

18 percent! The popular annual event was heldJuly 15, 2013, at North Shore Golf Club in Menasha.

144 people enjoyed 18 holes of golf, while another 40 participated in a wine tasting event hosted by

wine specialist Amber Mihna, CS, CSS, CSW.Both groups started the day with lunch and

ended with an hors d’oeuvre reception.

raising more than $58,000 to support improvements to Appleton Medical Center’s Guest House and the

hospice rooms at Cherry Meadows.

On March 8, 2013, Chicago’sfamed comedy troupe

brought their “Laughing Matters” tour to the Fox Cities Performing

Arts Center. To add to the fun,193 guests joined us for dinner and

a reception before the show.

An increase of nearly 20 percent over the previous year, event

proceeds are distributed equally between AMC Foundation and

Community Hospice Foundation.

The 6th year for the Light Up the Night for Hospice event was truly a magical evening. Guests gathered at the Waupaca Ale House White Tent on August 20, 2013, to raise more than $14,000 in support of the Pet Therapy Program.

Pet therapy volunteers logged over 200 volunteer hours last year at nursing homes and for hospice patients throughout Waupaca and the surrounding communities. Event attendees enjoyed meeting several of the six registered canine therapy dogs who bring comfort to patients through visits and friendship.

This year, we took “light up the night” literally by concluding the evening with a release of paper lanterns into the night sky in honor of friends and loved ones. It was an incredibly touching and heartfelt moment.

“Pet therapy is a ‘remember when’ moment for many of our patients,” said Christine Faulks, pet therapy coordinator. “They see our dogs, pet them, and memories of happier times seem to revisit their thoughts. It seems that our therapy dogs

rekindle fond memories of the patient’s own pets and often times a name or story about their beloved pet is shared.”

It will never happen to me. Isn’t that what all teens think when told about the dangers of speeding, drunk driving, not wearing a seatbelt, or distracted driving—such as texting? It will never happen to me.

But it must be happening to someone, as motor vehicle accidents among teens have reached epidemic proportions. Our community program, P.A.R.T.Y. @ the P.A.C., goes beyond mere preaching to help teens make a personal connection and think about real-life situations they’re faced with every day. Pairing real stories shared by actual survivors with a dramatic, realistic accident simulation, the P.A.R.T.Y. program helps teens grasp the potential consequences of risk-taking behaviors.

In November 2013, nearly 5,000 sophomores from 35 area high schools attended the educational program at the Fox Cities Performing Arts Center (P.A.C.). Two new additions this year were a partnership with AT&T to bring in national speakers from their “It Can Wait” campaign, Sontiana Brandt and Merry Dye, as well as a parent component put on by the Winnebago County Health Department.

The first program of its kind in the United States, P.A.R.T.Y. @ the P.A.C. is funded by Theda Clark Medical Center Foundation along with many generous community sponsors. Since ThedaCare began offering the P.A.R.T.Y. program in 1998, it has reached more than 30,000 teens.

The program itself is a collaboration among the Trauma Center at Theda Clark Medical Center, area fire fighters, law enforcement officers, independent physician specialty groups (including trauma surgeons and neurosurgeons), emergency medical technicians, and the wonderful survivors who volunteer to share their stories and put a face to the statistics.

We continue to work toward preventing these accidents rather than responding to them by addressing the reasons behind teen motor vehicle accidents and encouraging young drivers to make wise choices.

Some of the memories Barb recalls with a chuckle, but others were more difficult. Her mother Flo passed away in December at age 96 after spending a year and a half living in the ThedaCare Hospice Residence.

“My mom moved up here to Wisconsin after my dad died to be close to us. She was in great health at 90 years old, still playing golf and bridge regularly,” Barb says.

When Flo complained of back pain, her primary care physician ordered an MRI. They found breast cancer that had metastasized into her pelvic region. After an initial consultation with ThedaCare Cancer Care, Flo insisted on no treatment. She met with Dr. Jack Swanson to discuss palliative care options and moved into an apartment at ThedaCare Hospice Residence.

“It’s such a caring and comfortable place,” Barb confirms. “My mom was able to live life to the fullest extent all the way ‘til the end.”

At the Hospice Residence, Flo enjoyed visits from certified pet therapy dogs and their friendly owners. Steve accompanied his mother-in-law to the “Senior Prom,” where high school seniors decorated and dressed up as if going to the prom and danced to big band music with the hospice “seniors.” The hospice volunteers helped Flo create a video message where she shared her thoughts about what great moms her daughter and granddaughter had become and talked about how much she loved being a great-grandma. Toward the end, Community Hospice Foundation helped Flo organize a family photo depicting four generations together through a special grant program, the Hospice Hopes and Dreams fund.

“There were so many specialmoments—special memories—during the family photo shoot,” Barb recalls. “At the time, I was still going through the trauma of knowing my mom was dying and figuring out her prescriptions and everything else. I wasn’t thinking about getting a family picture taken! But the significance of that photo now and the memory of that day… it’s priceless.”

ThedaCare touched the Kromholz’ lives in other ways, too. Steve spent the last 5 years of his career leading the ThedaCare Behavioral Health department. And Barb and Steve’s daughter Kelly is a registered nurse who did her clinicals in school at ThedaCare.

In so many ways, ThedaCare is part of the fabric of theirdaily lives.

“I think the fact that we never changed out of the ThedaCare system for 29 years says something in and of itself,” Barb concludes. “They have the best doctors and the equipment and facilities to treat just about

anything! And now a connection with Mayo Clinic…you can’t ask for anything better than that. It’s all those little extras that make the difference between good and exceptional.”

In the summer of 2012, a representative group from ThedaCare’s seven foundations began meeting to create a future of improved collaboration, shared resources and gains in operational efficiencies. This new group, named The Radix Rugland ad hoc committee after co-chairs Harry Radix and Walter Rugland, engaged in working and planning sessions for more than a year, culminating in the creation of the ThedaCare Family of Foundations, Inc.

The ThedaCare Family of Foundations and its member Foundations, who have designated funds within the Family (see list to the right), sole purpose is to continue to maximize the value of each gift received to improve the healthcare experience for patients and their families. While all one Family, the member foundations play a vital role in bringing gifts to life in your community. All current gift arrangements and future gifts received will be used in exactly the way expressed by the donor. The underpinning of the work of the Radix Rugland group was to ensure that all gifts given locally would stay local if desired by the donor. The Family of Foundations’ work will be governed by two representatives from each member Foundation.

Whether gifts support a local initiative or a regional program, such as hospice services, our commitment remains to extend the value of each gift to its fullest and best use. The creation of the ThedaCare Family of Foundations means that we will become more efficient with the “back of house work” and more importantly, ThedaCare will make a renewed investment in the operations of the Foundation Family so your gift is usedto its fullest to improve patient care.
