Quitline services include the following:o Free Nicotine Replacement Therapy (NRT) which includes patches, gum and lozenges
– NRT is delivered in 2 shipments of 4 weeks eacho Four free proactive coaching callso Unlimited reactive coaching callso Educational materials and personalized quit plan
Pregnant smokers and the Quitline:o All pregnant smokers are eligible for Free Quitline Services including NRT (with
Physician approval) regardless of insurance statuso All those living with pregnant smokers are eligible for Free Quitline Services
Fax to Quit:o Complete the fax to quit form and fax to the Quitlineo Quitline personnel will contact them within 24 hours to complete the intake process
