The Omaha System

Standardized taxonomy or language designed to document client care from admission to discharge.

Why is using a standardized language important?

Three Components:•Assessment component: Problem Classification Scheme (area of focus)•Intervention component: Intervention Scheme (your actions)•Outcomes component: Problem Rating Scale for Outcomes (outcomes)

What is a Pathway?

Pathways reflect customized care plans that are developed to represent a specific client group.

They can be used for:orientation of new cliniciansclient assessment templatessaving time by eliminating repetitious documentationdocumenting data consistently

Pathways articulate best practices for client assessment and care planning in Omaha System terms.

Pathways make documentation faster and more consistent.

What is the difference between a Care Plan and a Pathway?

Pathway:

Care Plan:

Pathways are designed for use with client populations and include the problems/interventions that are most often identified with that population.

Care plans are designed for an individual client within a client population. We use pathways as a starting point for an individual care plan, and then individualize it even further as needed.

Creating a Pathway

Three steps to creating a new pathway:

What is the 1st, 2nd and 3rd Step?

Choose only problems that are most commonly experienced by that population.

Add typical interventions related to those problems.

Identify the client population

How do I find already existing Pathways?

http://omahasystemguidelines.org/ - New project by Karen Monsen and other creating evidence-based guidelines

Network with other users through our Listserv, User Group meetings or with a neighboring agency

Network with neighboring state agencies to split the task of creating pathways/custom tabs/reports and then share them

Champ Software is in the process of creating a document that will allow access to view available pathways which will be housed in the Knowledgebase

Creating a PathwayHow to take a form style document and make it into a pathway?

Take each question and..

1.Find the problem: are you using an umbrella problem or specific problem (remember that more problems does not make better documentation)

2.What target are you going to use?

3.What targets do you do in which Categories?

4.Create your guide: your guide can be the exact wording from your document, you can build in your answer or you can shorten the phrase.

Example: Child’s Comprehensive PhysicalQuestion - Systems Review: Gastrointestinal (bowel function; bowel movement consistency; color; frequency)

Problem: Umbrella Problem of Growth/Development (could also use Bowel Function)Category: Assessment/Surveillance

Target: screening procedures; Category: Treatments and Procedures; Guide: bowel function within normal limits (describe consistency, color and frequency)

• Create a unique prefix that you use to pull that information only.

• Build the answer or range right into the pathway’s guide.

Intervention Info:

• Charting: all Problem details including problem notes

• Intervention: Interventions=guides; ifyou want a report to only include thoseitems you checked off or completed, choose completed; pathway name is available

How to Request a Pathway

You will need to get permission from the agency and forward that to [email protected] and include:

• Name of the pathway group and pathway name• Where do you want it transferred from and where do you

want it “pasted” to.

Example: I would like the Rabies pathway (group: human hazards) from Vanderburgh’s production database transferred to our training and production databases.
