1. Provide a sample VOID cheque from your chequing account with your name and address on it. 2. Bank-stamped completed form, which can be faxed directly to our office from your financial
institution if they provide this fax service. 3. If an account-holder fills out their own direct deposit form via their personal online bank profile, the
original/online direct deposit form with original signature of account-holder must be provided. This form can be either dropped off in person at the Band Office or mailed in.
Youth turning 18 – Banking information In order to receive your settlements on your Birthday, please ensure your banking information is
provided to FWFN Finance in the format described above at least one-week in advance of your Birthday.
Outstanding Distributions Members who have not yet provided updated banking information for EFT relating to outstanding
distributions such as interest, land claims or other payables, will be processed during the last week of each month to be paid by the end of the month. during the last week of each month to be paid by the end of the month.

BINGO BALLS YOUTH RECREATION PROGRAM
NOTICE OF CHANGE TO POLICY
(EFFECTIVE for all forms submitted after January 10, 2018)

Section: Child & Youth
Subject: Child & Youth Recreation Support
1.1 Child & Youth Recreation Support Purpose: To Provide assistance with the cost of recreational activities for children and youth who are registered members of Fort William First Nation, both on and off reserve. To assist families in providing their children with healthy active programs, physical activities, and maintaining a healthy family bond through time spent together. Policy: The child or youth is eligible for assistance in the amount of $500.00 once per calendar year. Assistance is limited to registration fees and equipment related to recreational and/or educational activities only (music, art, dance, sports). ** Children and Youth are not required to be registered in a formal recreational or educational activity to utilize the bingo ball youth recreation program for recreational/educational equipment. Fort William First Nation registered members must meet the following criteria: 1. Custodial parent(s) or legal guardian for the child or youth may submit under this program. 2. Children and youth must be between the ages of birth to 17 years old at the time the expense was incurred. 3. Receipts submitted must be dated within the last 12 months. 4. No private sale receipts will be eligible. 5. Cheques will be issued directly to supplier.
Procedure:
1. Contact the Bingo Manager at the Community Centre office or Receptionist at the FWFN Administration office for a request form. ALL FORMS MUST BE FILLED OUT IN FULL OR PAYMENT DELAYS MAY OCCUR (i.e. all birth dates, full names, band membership numbers, and receipts attached)
2. Requests are received and approved by the Bingo Manager on a first come first serve basis, and as fundraising dollars allow.
3. Cheques will be issued directly to the recreation organization. 4. If payment has already been submitted by the custodial parent(s) or legal guardian, then original proof of
payment must be attached to request form and a cheque will be issued back to the custodial parent(s) or legal guardian.
5. Handwritten receipts from a business vendor (ie. Minor Hockey Teams) must be accompanied by proof of registration or verified by the Bingo Manager (notes made on application).
6. Final approved files are submitted to the finance department for processing on Mondays by 4pm in which the payment will be ready the Friday of that week.

Supercom Industries LP is currently exploring the provision of hot lunches daily for the duration of its’s TIER 1
Training Program (11 WEEKS) to be held in various locations. This is in anticipation of being able to lower the
frequency of lateness of students who will have to go offsite and to be able to provide a better-quality lunch
that what a daily lunch rate can provide for individually through meal allowance. Lunches are to consist of any
combination of soup, sandwiches, pasta, or grain.
Training locations vary and are listed below with the number of individuals at each site.
Bidders are to provide a price per individual served and this price is to be all inclusive of all costs eg, food costs,
prep, transportation, administration. Bids can be for any location, individually or grouped at the discretion of
the bidder.
