Illinois Coach Consent Form Directions for completing and to view team member and additional coaches’ form status

FIRST Illinois Robotics insurance carrier is now requiring that all team coaches and all team members parent/guardian complete a consent form specifically for Illinois.So coaches & team member guardian will now have 2 forms to complete on 2 separate websites:

Illinois form https://registration.firstillinoisrobotics.org If you are a returning coach, login.If you are a rookie coach with a new team or a new coach you will need to create an Illinois Registration account. Logon to the https://registration.firstillinoisrobotics.org website

Go to your team “Dashboard” and click on “Team #”

Click on the tab “Consent Forms”

From the “Consent Form Tab” you can: Complete your form by clicking on “Add a Consent Form” and completing Email parents a link to form with program and team number filled in.

When the Consent Form opens in a new browser page copy the link and email that link to guardians.Suggestion: Have a parent/guardian team meeting. Login to your team account and select “Add a Consent form” and have forms completed by a guardian during meeting.
