Urbanization, increasing obesity, and lack of exercise are all contributingto a global increase in diabetes. Accompanying this will be a rise in thecomplications of heart disease, blindness, strokes, and kidney failure.This plenary session will demonstrate diabetes to be an exemplar ofchronic-disease management, which with concerted effort andcollaboration among the individual, healthcare system and society, canimprove outcomes for patients. The Diabetes Audit and ResearchSystem (DARTS) in Tayside, Scotland will be demonstrated with adetailed description of how this is supporting National Diabetes Care.

Cancellations and Substitutions

All cancellations and substitutions must be submitted inwriting by mail or fax to the Institute for HealthcareImprovement. There will be no charge for cancellations madeby March 14, 2005 or for substituting one person for anotherat any time. Conference Registration Why is enrollment for the Office Practice Summit onlyavailable online this year?Enrollment for all of IHI’s events will soon take place viawww.ihi.org. The Office Practice Summit has a completelyweb-based enrollment process that will allow customers tomanage their enrollments themselves, including makingchanges to session selections, editing their name badges, andconfirming receipt of payment, all via IHI’s new onlineenrollment system.

I’m attending as part of a group. Can I still use theonline enrollment system?Yes, absolutely. IHI’s online enrollment system was built towork with our customers’ needs, and many groups attendour events. You will simply need to select the GroupDiscount enrollment rate during your enrollment process.Individual members of a group can enroll themselves, or athird party may enroll a group of individuals.

I’m enrolling someone else. Can I still use the onlineenrollment system?Yes, you will be given the option to indicate whether youare enrolling yourself or someone else, and the system willaccurately process either type of enrollment.

What if I am not permitted to (or don’t want to) payonline with a credit card?A large number of IHI’s customers either must pay bycheck, or receive an invoice in order to submit a paymentrequest. For this reason, we’ve made a “pay later” optionavailable during the payment stage of the online enrollmentprocess. A confirmation and invoice showing that paymentis due will be issued to you.

How does online enrollment work?IHI’s online enrollment system consists of a number ofsteps that will walk you through the enrollment process forthe 6th Annual International Summit on Redesigning theClinical-Office Practice

You will be asked to:• Indicate whether you are enrolling yourself or someone else.• Select an enrollment rate.• Indicate which Minicourse and/or General Conference

Learning Lab or workshop sessions you would like to attend.

• Format name badges for the conference.• Select which of several payment methods you would like

Continuing Medical Education

The Institute for Healthcare Improvement is accredited by theAccreditation Council for Continuing Medical Education(ACCME) to sponsor continuing medical education forphysicians.

Continuing Education for Physicians

The Institute for Healthcare Improvement designates thiseducational activity (the 6th Annual Office Practice Summit)for a maximum of 17.75 hours in category 1 credit toward theAMA Physician's Recognition Award. The Institute for Healthcare Improvement will be applying tothe Massachusetts Association of Registered Nurses, Inc., anaccredited approver by the American Nurses CredentialingCenter's Commission on Accreditation, for approval toprovide continuing nursing education. This continuingeducation activity carries up to 21.3 Contact Hours. American Academy of Family Physicians(AAFP) Credit Hours
