    INTRODUCTION

    National provisional estimates of the basichealth variables collected in the 1970 Health h-terview Survey of the civilian, noninstitutionalpopulation are presented in this report. Whilethe detailed tables in this report contain data byage and sex categories, later reports will presentmore detailed analysis of similar data by addi-tional selected demographic variables. The texttables present data for 1968 and 1969 as well asfor 1970; however, the discussion is limited tochanges occurring in 1970 since the previous re-port (Current Estimates, Series 10, No. 63) com-pares the 1968 and 1969 data.

    HIGHLIGHTS FOR THE PERIOD

    Acute Conditions

    Approximately 203.4 acute conditions per 100persons per year were reported in 1970; the rateof 200.8 for 1969 was comparable (table A). Theserates were based on estimates of 406.5 millionacute conditions in 1970 and 396.5 million acuteconditions in 1969. Acute conditions are those ill-nesses and injuries which had their onset in the2 weeks prior to the interview week and for whichthe person either sought medical attention or had1 day or more Qfrestricted ac,tivi~. Although therewas no marked increase in the overall incidenceof acute conditions between the 2 years, the in-cidence of acute injuries increased from 25.5 in-

    juries per 100 persons in 1969 to 29.6 injuriesper 100 persons in 1970. Injuries among femaleswere primarily responsible for this increase(table 1).

    Generally, no change occurred in the rates ofrestricted activity and bed disability days asso-ciated with acute conditions from 1969 to 1970.However, work-loss days for the currently em-ployed population increased from 347.7 days per100 persons in 1969 to 378.0 days per personsin 1970. Most of this increase was among femaleworkers who lost time as a result of acute res-piratory conditions and injuries. Children 6-16years of age lost fewer days from school as theresult of acute conditions in 1970 than in 1969with the rate dropping from 490.6 days per 100children to 426.8 days per 100 children.

    The above-mentioned increase in the inci-dence of acute injuries was also reflected in a13.4 percent increase in the number of personsinjured with a rate of 28.0 persons injured per100 persons in 1970. This increase was causedprimarily by accidents occurring in places otherthan in the home, at work, or in moving motorvehicles. In general, for the “person injured”measure a person is counted only once for eachaccident regardless of the number of injuries sus-tained, whereas each separate injury is countedin the incidence of acute injuries. Detailed dataon acute conditions and associated disability daysmay be found in tables 1-8 and 10-12. Data onacute conditions are to be shown in more detailfor July 1969-June 1970 in Series 10, No. 77.

  • Disability

    Table B summarizes days of disability andlimitation of activity for 1968, 1969, and 1970.Disability refers to any temporary or long-termreduction of a person’s activity due to acute orchronic conditions. Restricted activity, bed dis-ability, work-loss days, and school-loss days arereported in the health interview in association withspecific acute and chronic conditions. Since it is

    possible for a particular day of disability to beattributed to more than one condition, the person-day measure counts each day of disability only

    once regardless of the number of conditions caus-ing disability on that day. A day of restricted ac-tivity is one on which a person substantially re-duces his normal activity for the whole day due toan illness or injury. Each day spent in bed for allor most of the day is also counted as a day ofrestricted activity, but the converse is not neces-sarily true. Similarly, each day lost from work orschool is a day of restricted activity.

    An estimated 14.6 days of restricted activityper person was reported in 1970. Each personhad an average of 6.1 bed days in 1970. Approxi-mately 5.4 days were lost from work for each

  • currently employed person 17 years of age andover. While the overall rate for restricted activity,bed days, and work loss are comparable to the1969 levels, females averaged more work lossin 1970 (5.9 days) than in 1969 (5.2 days). Chil-dren 6-16 years lost less time from schooldue to illness and injury in 1970 than in 1969;the rate of school-loss days for 1970 was 4.9days per child compared with 5.4 days in 1969.This decrease was primarily among females.Detailed data for person days of disability areshown in tables 16 and 17.

    During 1970 approximately 23.6 million per-sons representing 11.8 percent of the civilian,noninstitutional population had some degree oflimitation of activity as the result of one chroniccondition or more. This estimate included 8.9percent who were limited in their major activity(working, keeping house, or going to school). Limi-tation of activity is a measure of long-term re-duction in activity resulting from chronic diseaseor impairment and is defined as inability tocarry on the usual activity for one’s age-sexgroup, such as working, keeping house, or goingto school, restriction in the amount or kind ofusual activity, or restriction in relation to otheractivities (civic, church, or recreation). Theproportion of the population with limitationof activity was approximately the same in both1970 and 1969. Detailed information on activitylimitation is shown in table 9.

    Utilization of Medical Services

    Table C summarizes measures of healthservices collected during the latest 3 years ofthe Health Interview Survey for which data areavailable.

    The number of discharges from short-stayhospitals was 13.3 per 100 persons reported in1970. The rate of discharges in 1970 was aboutthe same as that in 1969, and there was littlechange within age groups in the rate of hospitali-zation. The average length of stay remained aboutthe same in 1970—an estimated 8.6 days for bothsexes, 10.0 days for males, and 7.6 days for fe-males. However, among persons 65 years and overthe average length of stay dropped from 14.9 daysin 1969 to 13.1 days in 1970.

    About 10.3 percent of the population had onehospital episode or more during the y&ar, the samerate as during the previous year. The majorityof these persons had only one hospitalization dur-ing the year. The average number of days in thehospital for persons with one episode or morewas 10.0 days. Females generally averaged fewerdays than did males. Detailed data on hospitali-zations may be found in tables 13-15.

    An estimated 303.6 million visits to dentistswere made by the civilian, noninstitutional popu-lation in 1970. This was an average of 1.5 visitsper person during the year. While the averagenumber of dental visits per person remained thesame as in 1969, the percent of the populationwho saw a dentist within a year prior to the inter-view increased from 45.0 percent to 46.8 percent in1970. About 12.7 percent of the population hadnever seen a dentist, with the figures ranging from34.5 for children under 17 to 0.9 for persons45-64 years of age (table 19).

    During 1970 an estimated 926.9 millionvisits were made to physicians. These includedvisits to doctors’ offices, hospital emergencyrooms or outpatient clinics, and industrial andschool clinics, home calls, and telephone callsbut excluded physicians seen by a’ hospital in-patient. This was an average of 4.6 “visits perperson during 1970 as compared with 4.3 visitsper person in the previous year. As in earlieryears, the number of visits per person generallyincreased with advancing age.

    Approximately 71.9 percent of the populationconsulted a physician during the 12 months priorto interview in 1970—a slight increase from1969.Proportionately more females saw physiciansduring the year preceding the time of interviewthan did males. Detailed physician visit data areshown in tables 20 and 21.

    Seasonal Variation

    Tables 22-24 and figures 1-3 show quarterlyestimates for incidence of acute conditions, per-sons injured, and disability days. The patterns byquarter for incidence of acute conditions andrates of disability days are essentially the samein 1970 as in the previous year. However, thenumber of persons injured per 100 persons by

  • quarter was slightly higher for 1970 than for thecorresponding quarter in 1969. This was due tothe higher rate each quarter for females.

    CONTENTS OF 1971 QUESTIONNAIRE

    1 limi -Data on incidence of acute conditions,tation of activity, persons injured, hospitalization,disability days, dental visits, and physician visitsare now collected yearly in the Health InterviewSurvey and are shown in this publication. A listof the publications containing detailed data on theseitems for previous years is shown at the end ofthe text of this publication. Periodic reports up-date data on these health topics and selectedunpublished data are also available upon request.

    Chronic condition data resulting in activitylimitation is collected in the survey each year.Procedure used to collect information on theprevalence of chronic respiratory conditions issimilar to that used for the digestive system in1968 and the musculoskeletal system in 1969(see question 16 of questionnaire in appendix III).

    Other new data collected on the 1970 ques-tionnaire included information on the smokingexperience of persons 17 years of age and over,data on health insurance and Medicare coverage(first and fourth quarters of 1970), and the volumeof medical and dental X-ray visits, including placeof visit and area of body exposure (second andthird quarter). Again, recent publications contain-ing detailed data on these supplementary itemsare shown at the end of the text of this publication.

    Demographic items collected during 1970, inaddition to age and sex shown in this publication,are race, family relationship, marital status,usual activity status, education, veteran status,current employment status, industry and occupa-tion, family income, and place of residence(metropolitan or nonmetropolitan area and geo-graphic region).

    1The present report is the second in this series to use the

    Eighth Revision of the International Classification of Diseases,

    Adapted (lCDA), in the classification of conditions resulting in

    illneis, hospitalization, physician visits, etc. This provides data

    comparable to that collected in 1969, and as long as broad

    conditions group are used the categories are generally

    comparable to earlier years. For a discussion of the major

    changes in the ICDA and the impact on Health Interview

    Survey data, see appendix 111, Series 10, No. 69.

    SOURCE AND LIMITATIONS

    OF THE DATA

    The information from the Health InterviewSurvey presented in this report is based on datacollected in a continuing nationwide survey con-ducted by household interview. Each week aprobability sample of households is interviewedby trained personnel of the U.S. Bureau of theCensus to obtain information about the health andother characteristics of each member of thehousehold in the civilian, noninstitutional popula-tion of the United States. During 1970 data werecollected for 45 of the 52-week period. The firstquarter contained a sample reduction of 7 weeksdue to budgetary restrictions, whereas the re-maining quarters each contained the full 13 weeks.During the 45 weeks in 1970 the sample was com-posed of approximately 37,000 households con-taining about 116,000 persons living at the time ofthe interview. Comparability with previous annualestimates are not affected by the reduced samplesince the weighting procedure employed by theHealth Interview Survey adjusts for the missingweeks, although the sampling errors are slightlyhigher in 1970 (see statement on adjustment ofrelative sampling error in appendix I).

    A description of the design of the survey,the methods used in estimation, and general quali-fications of the data obtained from surveys ispresented in appendix I. Since the estimatesshown in this report are based on a sample ofthe population, they are subject to sampling er-ror. Therefore particular attention should be paidto the section entitled “Reliability of Estimates.”Sampling errors for most of the estimates areof relatively low magnitude. However, where anestimated number or the numerator or denomi-nator of a rate or percentage is small, the samp-ling error may be high. Charts of relative samp-ling errors and instructions for their use areshown in appendix I.

    Certain terms used in this report are definedin appendix II. Some of the terms have specifiedmeanings for the purpose of the survey. Forexample, estimates of the incidence of acuteconditions include, with certain exceptions, thoseconditions which had started within 2 weeks andwhich involved either medical attention or re-stricted activity. The exceptions, which are listed

  • in appendix H, are certain conditions such asheart trouble and diabetes which are always con-sidered to be chronic regardless of duration oronset.

    Estimates of the number of disability daysassociated with acute conditions are derived fromthe number of days of disability experienced dur-ing the 2-week period prior to the week of inter-view and include all such days reported even ifthe acute condition causing the disability had itsonset prior to the 2-week period. Disability daysassociated wtih acute conditions are recordedon a condition basis. If an individual reports morethan one illness or injury on the same day, thecount of disability days will exceed the actualnumber of days disabled, i.e., person-days ofdisability.

    Appendix III contains the questionnaire usedin the interview. Also shown are the cards usedby the interviewer in asking certain questions.
