KCR KIT

1. INTRODUCTION:

1.1 About KCR KIT:In a move to help the poor, Government of Telangana introduced a scheme “KCRKIT” to the pregnant women’s, which envisages taking complete care of women atevery stage of pregnancy. Apart from this, the ‘Amma Vodi’ scheme, conceptualizedon the lines of Janani Sishu Suraksha Karyakram (JSSK) of National Health Mission(NHM), also entails providing free transport facilities in an ambulance to diagnosticservices for regular check-ups and tests. The vehicle will then drop the pregnantwomen back to their homes after the two-day stay in the hospital.

This scheme also provides financial incentives to the women who gave birth to thechild in the government hospitals/PHC Centre’s. Pregnant women will get benefitedby this scheme has to registered herself by going to the nearby PHC center wherethe respective ANM will login. The main objective of the KCR Kit scheme is toreduce the infant mortality rate and encourage institutional deliveries in the state.

1.2 Amount Dispersal Procedure:

4. PROCEDURE FOR REGISTRATION IN KCR KIT:

ANM`s need to perform the tasks specified in this section. ANM`s are provided a provision to capture the pregnant women details. In PHC Center`s DEO`s will register the pregnant women details.

4.1 KCR KIT URL:ANM’S are directed to capture all the pregnant women details with help of Asha Workersat Village Level . In this Process all the Collected details need to be Update in KCR kit


portal .All the ANM are provided with User ID’s to update the details ANM’s need tofollow below Steps 1. Browse the Url below mentioned

“http://kcrkit.telangana.gov.in/THD/Login.aspx”

4.2 KCR KIT HOME PAGE:The KCR KIT Home Page as shown in the following figure:

4.3 ANM LOGIN:Enter User Name and Password and click Login as shown in the following figure:

ANM Login with valid credentials for registering pregnant women details, if any

corrections/additions ANM can modify under KCR KIT. After Entering the User Name, Password, ANM Click “Sign In” Button as mentioned

in above Screen.

4.3.1 FORGOT PASSWORD:ANM Click on ‘Forget Password’ link as shown in the following figure:

4.3.2 CHANGE PASSWORD: (Verify)ANM enter the User Name for verification to change of password as shown in thefollowing figure:

ANM enter the User name of her for changing of password. Once User Name has been entered, ANM click on ‘Verify’.ANM enter the OTP (One Time Password) for change of password as shown in thefollowing figure:

Once User Name has been verified, OTP (One Time Password) will be sent to ANMregistered mobile Number.

ANM to Capture the OTP, which was received to registered mobile Number. If OTP has not been received, there is an option to re-send it, by click on ‘Re-Send’.

4.3.3 VALIDATE OTP FOR CHANGE PASSWORD:ANM to Validate OTP for change of password as shown in the following figure:

Enter New Password and confirm Password Click Submit to update new Password A Confirmation message will be displayed as show below

4.4 KCR KIT HOME PAGE: (ANM Login)Home Page of ANM login in KCR KIT as shown in the following figure:

After Successful login, ANM can view the ‘list of contents’ under KCR KIT homepage.

ANM`s can view details of – Pregnant Women Registered, ANC Checkup, High RiskCauses Identified, Expected Deliveries, Deliveries, Maternal Death, Infant Death,No. of Children Registered, Zero Dose Immunization, Full Immunization.


4.5 MOTHER/ CHILD REGISTRATION: (Selection) Select ‘Mother/ Child Registration’ link in home page as shown in the following figure:

By Click on ‘Mother/ Child Registration’ a new pop up will be populated for‘Verification/ New Registration’ of pregnant women details.

4.6 NEW REGISTRATION/ VERIFY: (Selection)Select ‘New Registration/ Verify’ for verification or registration of pregnant womendetails as shown in the following figure:

4.6.1 VERIFY DETAILS:ANM to verify details based on ‘Mother ID/ Aadhaar ID’ as shown in the followingfigure:

If pregnant women details have been already registered, ANM can verify thedetails based on ‘Mother ID/ Aadhaar ID’.

Mother ID will be generated at the time of registration. Once ‘Mother ID/ Aadhaar ID’ is entered, ANM click on ‘Verify’.

4.6.2 MOTHER REGISTRATION DETAILS:ANM can view the registered mother details as shown in the following figure:

ANM can view the Registered Mother details based on ‘Aadhaar ID/ Mother ID’. If the Women is registered, Expecting Mother Details, Previous Delivery Details,

ANC Details will be displayed (which are captured earlier).

4.7 NEW REGISTRATION DETAILS: Select ‘New Registration’ for registering pregnant women details (new) as shown in thefollowing figure:

Step 1: Click on “Mother/ Child Registration” under list of Modules in ANM HomePage.

Step 2: A New Popup will be populated either to ‘Verify/ New Register’ Pregnantwomen details.

Step 3: If a pregnant woman is already registered in KCR KIT, she can verify detailsthrough “Mother ID”.

Step 4: If women (pregnant women) is new for registration in KCR KIT, ANM clickon “New Registration” for registration purpose.

4.8 REGISTRATION DETAILS: (Home Page)‘New Registration’ details for pregnant women’s as shown in the following figure:

The above Screen explains ANM to capture the pregnant women details as follows:

4.9 EXPECTING MOTHER DETAILS: (Mother Registration Form) ANM to Capture ‘Expecting Mother Details’ as shown in the following figure:

Mandatory fields (*) to be Captured.


By Selection of LMP Date, Expected Date of Delivery will be calculated andautomatically date will be fetched from server.

Based on ASHA Name, ASHA Mobile Number will be fetched from Server. Once Expecting Mother Details are captured, ANM to Capture Previous Delivery

Details as shown in below.

4.10 PREVIOUS DELIVERY DETAILS: Previous Delivery Details Home Screen as shown in the following figure:

4.11 PREVIOUS DELIVERY DETAILS: (Selection/ Capture)ANM to Select Previous Delivery Details as shown in the following figure:

For Example: If ‘Gravida’ is G0 (First Pregnancy), then

‘Parity’ must be P0 (Previous Delivery), Abortions must be A0 (Any Abortions), Live must be L0 (Live Children) Death must be D0 (Death Children)

If Gravida is G2, ANM to Capture details of Parity (P1) In Previous Delivery Details, If Live is L1, One Child details to be captured.


If it is L2 (Live), Two Children details to be captured.

ANM to Capture the Live Children Details as shown in the following figure:

Based on Selection on Live (of Child), the Grid will be populated, if Live is L1 (1grid), if L2 (2 grids) will be populated.

Date of Delivery of the Live Child to be Selected, by Selecting the Date, Age will beautomatically calculated.

Place of Delivery of the Previous Child (Live) to be selected with either with‘Private Institution/ Govt. Institution/ Home’.

ANM to Capture the Place of Delivery of Live Children as shown in the following figure:

If the place of Delivery is Private Institute, there is no provision to select SubInstitutions.

If the place of Delivery is Govt. Institute, Drop down of Sub Institutions will befetched and ANM to Select the Respective Institute.

ANM to select the ‘Mode of Delivery’ as shown in the following figure:

Select Mode of Delivery with ‘Normal or C- Section’.

4.12 SUBMISSION OF DETAILS:

ANM click on ‘SUBMIT’ as shown in the following figure:

Mother Registration details will be submitted after capturing the ‘PreviousDelivery Details’.

Without Capturing Previous Delivery Details, the details can`t be submitted.

By Click on ‘SUBMIT’ notification popup will be populated as shown in the followingfigure:

ANM Click on ‘OK’ for Submission of Mother registration Details. Upon click on ‘OK’ a new Notification popup will be fetched from Server which

generated Mother ID & the same ID will be forwarded to the women registeredmobile number.

4.13 ACKNOWLEDGEMENT NUMBER: (Successfully Submitted)

Once data is submitted an acknowledgment number will be generated as shown in thefollowing figure:

As shown in the above screen, the Mother ID will be generated. The generated Mother ID will be useful for further purpose. ID will be in Numerical

format. By Click on ‘OK’ a new Pop up will be populated with ‘Maternal and Child

Protection Card’.

4.14 MATERNAL AND CHILD PROTECTION CARD:

Once Mother ID is generated, Maternal and Child Protection Card will also be generatedas shown in the following figure:

4.15 ANC DETAILS: (Home Page)ANM to Capture/ Select the ANC details as shown in the following figure:

Once the Mother Registration (Expecting Mother Details) & Previous DeliveryDetails are Captured, Successfully submitted message and an acknowledgmentnumber will be generated.

Pregnant women can visit Govt. Institutions for ANM Check up`s along with theacknowledgement number which is generated at the time of registration.

4.16 SELECTION OF ANC DETAILS:ANM to Select ANC Place details as shown in the following figure:

Minimum 2 ANC`s to be done in Government institutions for availing this benefitsof this scheme.

ANM selects Blood Group of the Pregnant Women. ANM selects the ANC Place either with ‘Private Institute’ or ‘Govt. Institute’.

ANM to Select Sub Institute (ANC Place) details as shown in the following figure:

If ANC place selected as ‘Govt. Institutions’, Sub Institutions will be fetched fromServer.

Sub institutions will be fetched only for Govt. Institutions.

ANM to Capture the Blood report of the pregnant women as shown in the followingfigure:

ANM to Capture: Doctor Name, Blood Pressure (mm/Hg), Hemoglobin (gm/dl),Blood Sugar, Weight (in Kgs), TT Given, and IFA Tablets (YES/NO), USG Scan(YES/NO).

4.17 ADD NEW ROW: (Under ANC`s)ANM to select ‘Add New Row’ for addition of New ANC details of pregnant women asshown in the following figure:

If ANM want to add a New Row of ANC Details, Click on ‘Add New Row’. If ANM want to delete the Added Row, Select the respective row with radio button

and click on ‘Delete’ There is a provision to select multiple rows, by selecting Radio Button.


4.18 HIGH RISK IDENTIFIED:ANM to select whether High risk identified (YES/NO) to pregnant women as shown in thefollowing figure:

ANM to Select whether High Risk Identified to Pregnant Women with YES/ NO.

If High Risk Identified is ‘YES’, select the type of Risk which is identified to PregnantWomen as shown in the following figure:

If High Risk identified is YES, Select the type of High Risk from the list as mentionedabove.

There is an option to select Multiple Types of High Risk. If High Risk Identified is NO, Types of High Risk will not display.

Once ANC details are captured/ updated, ANM click on ‘Update’ as shown in the followingfigure:

If there are any modifications/ additions in the Expecting women Details, PreviousDelivery Details, ANC Details, ANM is having a provision to update the details andclick on ‘Update’.

Whenever ANC is done to the pregnant women, ANM should capture the details, ifany corrections, ANM can modify and update the details.

4.19 DELIVERY DETAILS: (Home Page) Delivery details Home page for capturing details as shown in the following figure:

4.20 DELIVERY DETAILS: (Selection Place of Delivery) ANM to Capture the Place of Delivery details of pregnant women as shown in thefollowing figure:

Once delivery is done, ANM to Capture Date of Delivery of Child, Place of delivery(Private/ Govt. Home/ Transit).

If Place of Delivery is Private, there is no provision to capture Delivery Institution. If Place of Delivery is Govt. Institution, ANM to Capture the Delivery Institution.

ANM to Capture the Delivery Institution details as shown in the following figure:

ANM to Capture the Delivery done by (either in Private/ Govt. Home/ Transit) andDoctor Name.


ANM to Select Delivery Outcome of the Pregnant Women as shown in the followingfigure:

Once the delivery is done, the delivery details to be captured. ANM to Select the Delivery Outcome with any one of the following:

Live, Still Birth, Abortion, Twin Birth.

4.20.1 DELIVERY OUTCOME: - LIVEANM to Capture the Delivery Outcome Details (Live) as shown in the following figure:

Here Delivery Outcome is selected as ‘Live’.

ANM to Capture Mode of Delivery details as shown in the following figure:


ANM to select Mode of Delivery with ‘Normal’ or ‘C-Section’. If C-Section is Selected, ANM to Capture ‘Indication of C-Section’.

ANM to Capture Maternal Outcome details as shown in the following figure:

ANM to select Maternal Outcome with ‘Discharge’ or ‘Death’. If Discharge is Selected, ANM to Capture ‘Date of Discharge’.


Maternal Outcome Details (as Death) as shown in the following figure:

If Maternal Outcome is ‘Death’, capture the details with ‘Reason for Death,Date of Death, and Place of Death’.

Select Baby Kit details and Mother Details as shown in the following figure:

Select whether Institution has provided Baby Kit with (YES/ NO), if YES uploadthe Image of Baby Kit.


Once delivery has done, capture Mother & Baby Image & upload the imageunder Delivery Details.

4.20.4 DELIVERY OUTCOME: - TWIN BIRTHANM to Capture the Delivery Outcome details as shown in the following figure:

ANM Selects Delivery Outcome as ‘Twin Birth’.











Select Baby Kit details and Mother Details as shown in the following figure:

Select whether Institution has provided Baby Kit with (YES/ NO), if YES uploadthe Image of Baby Kit.

Once delivery has done, capture Mother& Baby Image & upload under DeliveryDetails.

4.21 SUBMISSION OF DETAILS:ANM click ‘Submit’ for submission of details as shown in the following figure:

Once the details are captured by ANM, click on ‘UPDATE’. If there are modifications, ANM click on ‘CANCEL’.

4.22 LOGOUT:ANM click ‘LOGOUT’ once pregnant women details are submitted as shown in thefollowing figure:

Once details are modified/ added, ANM can logout from KCR KIT Portal. Whenever ANM required login again, ANM can login with their credentials.


5. CHANGE MANAGEMENT PROCEDURES:

This document is meant for usage by the KCR KIT team and shall be the basis forpreparing the test case, incident, log and the final report. Any changes made to therequirements in future shall have to go through a formal change approval process.Wherever necessary and shall not make any alterations without the permission of theclient and the development team.
