CONTRACT TO LEASE

This agreement was entered into by two contracting parties the LESSOR which is the TEAM

MISSION PHILIPPINES, INC. and the LESSEE, ___________________________________

It is agreed that the LESSOR will provide the repairs and maintenance of the apartment or room

like: roofing/ceiling, walling, flooring, electricity and water system, installed lighting fittings for

fluorescent including ballast and starter except when the damage is caused by the Lessee(s) or

his/her family.

Apartment of size _______sq.m. for __________ pesos. Electricity and water will be paid by the

Lessee(s) according to the meter reading. If the Lessee wants to have their own meter for water

they will shoulder the installation cost. In case of vacating the place they can bring the meter

with them.

Smoking and drinking liquor are strictly prohibited. Proper garbage disposal is expected.

Payment for above share will be paid at the end of every month to Team Mission Finance Office,

Lambakin, Marilao, Bulacan.
