    FORM 1

    DESCRIPTION:

    The application for permission to construct, extend or

    take into use any building as a factory.

    Form 1

    contains flowchart of the manufacturing process at its various stages.

    Be submitted in triplicate.

    mentions the names of the chemicals used (if any).

    Safety measures to remove dust, fumes and gas.

    Measures to effectively dispose of wastes and effluents.

    Presents blueprints of the Factory, its surroundings (i.e. roads, drains, ventilation)

    and also the position of the machinery within the plant.

    Be accompanied along with a certificate from the TNPCB, regarding the

    arrangements made for the disposal of Trade waste and Industrial effluents.

    DESCRIPTION:

    For the Application for the registration and

    grant/renewal of License.

    Form 2

    Be accompanied by a treasury receipt or a crossed demand draft drawn from aNationalized bank in favor of the Dy. Chief Inspector of Factories (DCIF)

    Be submitted in triplicate.

    Every application received by DCIF be registered in Form 3.

    Satisfied, the DCIF will grant license in Form 4.

    Occupier may remit the license/ initial license or renewal of license (yearly) in

    favor of doing so for a lump sum period of 5 consecutive years .

    Every license/ renewal of license granted shall remain in force up to 31st

    December of the same year for which the license was granted/renewed.

    The licensing fee is to be paid by October 31st of the same year.

    PENALTY

    In the event that the fee is not paid on time then a fine shall be imposed along

    with the fee.

    Fines range from

    10% of the Fee amount along with the Fee, if paid in the month of NOVEMBER.

    20% of the Fee amount along with the Fee, if paid in the month of DECEMBER.

    30% of the Fee amount along with the Fee, if paid after the expiry of the license.

    NOTE: If the original license is lost or has been destroyed, a duplicate license canbe obtained by paying an amount of Rs. 50/-

    Be issued by the Certified Surgeon.

    The foil and counterfoil be filled.

    Contain the left thumbprint of the person in whose name it is being issued.

    The counterfoils of all issued certificates be kept with the Certified Surgeon.

    The Certified Surgeon shall visit the Factory. During visit, issues the Certificate of Fitness

    in FORM 27.

    The records of all examinations done during visit be kept with the Factory Manager.

    The results of the tests entered by the Certified Surgeon in the HEALTH REGISTER in

    FORM 17.

    REPORT OF EXAMINATIONOF PRESSURE VESSEL OR PLANT

    Every Pressure Vessel in service be thoroughly examined by Notified person.

    Conduct External Test Once every 6 months.

    Conduct Internal Test Once every 12 months.

    Internal Test can be replaced by conducting a Hydrostatic Test Once in 2 years.

    Incase of hard to open Pressure Vessel, period of External Test extended - once

    every 4 years.

    EXAMINATION REPORT sent to Inspector within 7 days of it being held
