ProceduresThere are three procedures we offer patients at the Surgical Weight Loss Program at Clinch Valley Medical Center:

Laparoscopic Sleeve Gastrectomy (VSG) The sleeve gastrectomy is a restrictive procedure in which two-thirds of the stomach is removed, which provides a decreased appetite and quickly feeling full. The remaining stomach looks like a long tube, hence the name sleeve, which restricts food intake by allowing only a small amount of food to be consumed at one time. Weight loss is achieved by both a reduction in stomach capacity and the production of the hormone ghrelin, which is associated with hunger and appetite. This procedure is also performed laparoscopically, which means less scarring, less pain and a faster healing time. This procedure works by restricting food intake without involving bypass of the intestinal track of a foreign object being placed around the stomach.

Laparoscopic Roux-en-Y Gastric Bypass (RYGBP)A small stomach pouch is created which restricts food intake resulting in a sense of fullness following even small meals. Therefore, as gastric bypass implies, food is re-routed past most of the stomach and the first part of the small intestine. RYGBP achieves weight loss through both gastric restriction and malabsorption.

Laparoscopic Adjustable Gastric Banding (LAGB) The LAP-BAND® System is an adjustable silicone band thatisinsertedlaparoscopicallyandplaced(likeabelt)aroundtheupperpartofthestomach.Anaccessport allows the surgeon to adjust the inner surface by adding or removing saline. This creates a new, smaller upper stomach pouch and leaves the larger part of the stomach below. This new upper pouch can only hold a small amount of food, so patients will feel full sooner and are not hungry between meals.

Weight loss surgery, or bariatric surgery, is a serious surgical procedure and requires an extreme change in lifestyle, eating and nutritional habits. Because of the lifelong commitment, the Surgical Weight Loss Program at Clinch Valley Medical Center requires an in-depth evaluation and screening prior to considering a candidate for weight loss surgery. This evaluation includes a thorough medical history and examination, as well as an evaluation of lifestyle and motivation.

Once you and your doctor have agreed that bariatric surgery is a good choice for you, there are several steps that will occur before surgery is scheduled. Some of these steps include: patient education classes, comprehensive blood testing, cardiology, pulmonary and gastroenterology evaluations (if needed) and appointments with dietitians who will assist you in achieving the mandatory preoperative weight loss.

Most patients have surgery three to six months after their initial inquiry (depending on your specific health problems and your ability to meet preoperative goals). You must meet all criteria prior to receiving a surgery date.

Dr. Ariel Rodriguez, our Bariatric Surgical Medical Director, completed his fellowship in Advanced Laparoscopic and Bariatric Surgery at the University of South Florida, Tampa. He has completed residencies in General Surgery at Harbor-UCLA Medical Center in Torrance, CA and Huntington Memorial Hospital in Pasadena, CA. During his residency and fellowship, Dr. Rodriguez completed over 300 bariatric surgeries including Lap-Band, Gastric By-Pass and Gastric Sleeve. He also completed complex revisional bariatric cases and spent time doing research in bariatric and metabolic surgery.
