Staff Wellness Challenge!
Staff Wellness Challenge Rules and Expectations
1. Each day a specific task is given. If completed, please check the box. Activities should be completed on the designated day.
2. Complete a minimum of 90% of the activities (25 out of 27) to be eligible for the drawing prize.
3. Submit your completed form by Friday, May 8th to your Wellness Champion. Forms may be submitted by email. The challenge has been provided in an editable Word format so that you can check the boxes of completed activities by clicking in each box, and you can fill-in your name and A# at the bottom. If you choose to print your form, you can also submit by scanning or taking a photo and emailing it to your Wellness Champion.
4. Every Saturday is a “Bonus” day for you to complete something for yourself, such as reading a book or magazine, doing a mind teaser or puzzle, etc.
5. Participants will be in charge of tracking their own progress.6. This is an honor system challenge, intended to be fun. Please be
honest throughout the challenge.
