1 MISSIONSTATEMENTThemissionoftheOlaGrimsbyInstituteistoprovidethehighestqualityintegratedmanualtherapyeducationtopracticingorthopedicoutpatientmanualtherapistsand physicians. The Institute's programs aim to develop a competent andautonomous professional capable of diagnosing and caring for patients withneuro-musculosceletal dysfunction. The Institute serves to support, encourageand facilitate the pursuit of excellence in orthopedic patient care by advancingthe skills and intellectual competency of the clinician and by encouraging andrequiring clinically relevant research The instructors exemplify the manualtherapy clinician as well as enable the fellowship student to understand,synthesizeandapplyscientificknowledge,manualskills,andcriticalthinkingtopatientassessmentand intervention.Wedo thisbyenablingourstudents,whoare practicing manual physical therapists, to design scientifically-basedtreatments for the health of each individual patient, resulting in a reducednumberoftreatmentsandimprovedoutcomes.

2 PHILOSOPHYOFTHEOLAGRIMSBYINSTITUTE2.1 PEOPLEHELPINGPEOPLE

ThefacultyandadministrationoftheOlaGrimsbyInstitutebelieveinthepowerofpeoplehelpingpeople,andwestrivetoapplythisbelieftothestudents,faculty,staff,therapistsandpatientsassociatedwiththeInstitute.Webelievethatwhenonepersonishelped,everyoneisalittlebetterforit.Wearethereforededicatedtohelpingasmanypeopleaspossiblebyexpandingthepracticeoforthopaedicmanual physical therapy based on clinical expertise and the most currentunderstandingofthehumanneuro-muscular-skeletalsystems.

Asan internationally-focused,professional,educationconsortium,wedesignallofourprogramswith respect to the social, cultural, andprofessional implicationsunique to each region in which our programs are offered. This means thatelementssuchascurricula,presentation,andpricingstructuresaredesignedtoservethestudentsandtheirpatientsineachuniquepartoftheworldwheretheOlaGrimsbyInstituteoperates.

2.2 KNOWLEDGEANDPRACTICE TheInstitutebelieves thataphysical therapist’scompetence isdevelopedthroughthepursuitofactivelearningintwodomains:

•Knowledge-Integratingathoroughunderstandingofanatomy,physiologyand biomechanicalfunctionwiththeprocessofclinicalreasoning,analysis,diagnosisanddecision-making.

• Practice - Extensive hands-on experiencewith patients to achieve the advancedmanualskillsforassessmentandinterventionofmovementdysfunction.

2.3 ON-GOINGEDUCATION TheInstitutebelievesthattheprofessionaldevelopmentofthephysicaltherapistisanongoingandcontinuouspath;therefore,oneoftheInstitute’sresponsibilitiesis to provide practicing physical therapists and other healthcare professionalswith instructionalprograms inoutpatientorthopaedicmanual therapy that arecurrentandinclusiveofevolvingresearch.

Theprogressionofourongoingprofessional educationprograms includesClinicalCertification, followed by ourDoctoral Degree residency, both ofwhich can beachieved in the course of one year. Our second-year residency results in aFellowship Certification; both of these programs are credentialed by theAmericanPhysicalTherapyAssociation(APTA).

Followingthethirdyearofclinicalspecialization,studentsmaybeawardedaPh.D.inOrthopaedicManualTherapy.

As a degree granting institution, the broad goal of our research program is toprovidetheprofessionwithevidence-basedinformationnecessarytosupportthefuturedevelopmentofphysicalandmanualtherapy

2.4 QUALITYPATIENTCAREATLESSCOSTThe Institute places quality of orthopaedic manual therapy above all else in thetraining of our students and the care of our patients. We have supportingevidence that by providing physical therapists with advanced training, inorthopaedicmanualphysical therapy, theybecomemoreeffectiveandefficient.Theachievementthroughthisphilosophyandpracticeisbetterpatientoutcomesinlesstime,resultinginthedecreasedcostofservices.

Furthermore,thereisevidencetosuggestthatphysicaltherapistswhocompleteourresidency programs have a higher level of personal satisfaction in theperformance of their work compared with physical therapists without ourresidencyeducation.

Researchalsoshowsaradicalincreaseintheperceptionoftheirclinicalcompetence,aswellasareductioninthenumberoftreatmentsnecessaryinordertogainfullrecoveryfromspinalaswellasextremityinterventions.

OlaGrimsby,PT,DMT,FFAAOMP HonoraryFellowbytheAAOMPT,“TeacheroftheYearAward”BytheKaltenbornTeachIMustAwardbytheAAOMPT.DearStudent:ItakegreatprideinwelcomingyoutotheOlaGrimsbyInstituteConsortium!Our programs are the result of a thirty-five year dream of mine to create anacademicandclinicalresidencycurriculumforPhysicalTherapistsintheUnitedStates. The process has included teaching and administration of the NationalProgram for Orthopaedic Manual Therapy in Norway, studies of the multi-national“schoolsofthought”,andlecturesthroughouttheworld.Ourfacultyarehighly qualified clinicians with broad experience in the many aspects of ourprofession.In2007,weweregrantedtherighttoissuetheworld’sfirstPh.D.inOrthopaedicManualTherapy.

Through an extensive curriculum, we are among the leaders of postgraduatetraining for Physical Therapists. For studentswhowant to earn aDoctorate ofManual Therapy degree, become a Fellow of the American Academy ofOrthopaedic Manual Physical Therapy (AAOMPT), or a attain a Ph.D. inOrthopaedicManualTherapy,inadditiontotheCertificateofCompletion,currentenrollment is available through the Institute’s required registration under theUtahPostsecondaryProprietarySchoolAct.

Althoughweseeka scientific rationale forourclinical skill,wewill alwaysputanemphasisontheartofourperformance.Inrecognitionoftheneedforscientificverification,itisimportanttorememberthatresearchhaslittlevaluetousunlessit is clinically applicable.Consequently,wedonotoffer youa finishedproduct,butanopportunitytoparticipateinaprocessofconstantdevelopment,fosteringthedevelopmentofalifelonglearner.

The Ola Grimsby Institute is recognized as a provider of the Manual Therapyfellowship training by the American Academy of Orthopaedic Manual PhysicalTherapy and the American Physical Therapy Association. As of the summer of1992,theAPTAisa fullmemberoftheInternationalFederationofOrthopaedicManipulative Therapy, and the programs of the Institute will also meet theinternational standard. As of 2003, we are credentialed through the AmericanPhysicalTherapyAssociation.Furthermore,theOlaGrimsbyInstituteisaffiliatedwithnumerousacademicorganizationsthroughoutEuropeandAsia.

In spite of our academic and professional achievements, we do not have all theanswers.Inhumbleadmirationfortheenormouspotentialofourprofession,wechallengeyoutoparticipate,toquestion,tolearnandtoshareforthebenefitofpersonal and professional growth and optimal patient care. 3 ADMISSIONREQUIREMENTS3.1 YearIOrthopaedicResidency(On-Site/IndependentStudy)

1.Graduateofaphysicaltherapyprogramaccreditedandrecognizedbythenationalparentbody.

2. Proof of licensure to practice physical therapy within their current practicesetting.

3.DemonstratedinterestinfurtherprofessionaldevelopmentasdocumentedintheOGI

OrthopaedicResidencyApplication.4.DemonstrationthattheResidentisactivelyworkinginanorthopaedicsetting. 5. Demonstrate that they are eligible for membership within the national parentbody.

6.Applicantsmustprovideallhistoryofprofessionalacademictraining7.Applicantsarerequiredtosubmitanautobiographicalstatement.8. Applicants are required to provide references from two employers orprofessionalassociates.

9.Selfphotograph–canbeemailedelectronically10.OfficialtranscriptssenttotheOGIbusinessoffice.11.TheOperationsmanagerandCAOwillreviewallapplications,requestingclarificationoradditionalinformation.Ifdeterminednecessary,theCAOwillscheduleinterviewswillbescheduledforapplicantstoassistwiththedecisiontoacceptorrejectanapplicant.

12.Ifinterviewsareperformed,eachapplicanttobeinterviewedwillbenotifiedinwriting(email)aminimumoffourweekspriortotheirinterview.OncetheinterviewscheduleisconfirmedbetweentheapplicantandtheCAO,eachapplicantwillbeinformedofthespecifictime,date,andplacefortheirinterview.Interviewsmaybeconductedface-to-face,Skypeorphone,dependingupongeographiclocationsofapplicantsandinterviewers.

3.Demonstrationthatthestudentisactivelyworkinginanorthopaedicsetting.4.Proofoflicensuretopracticephysicaltherapywithintheircurrentpracticesetting.5.Demonstratethattheyareeligibleformembershipwithinthenationalparentbody.6.TheFellowshipprogramdoesnotrecognizetransfercreditsfromanyotheruniversityorResidencyprogramtomeetrequirementsforfellowshipapplication.

7.TheOperationsmanagerandCAOwillreviewallapplications,requestingclarificationoradditional information. Ifdeterminednecessary, theCAOwill schedule interviewswillbescheduledforapplicantstoassistwiththedecisiontoacceptorrejectanapplicant.

3.5 CurricularComponent:

For studentswho have completed the previous Part IManual Therapy asDPT orMOMT:

Youwillneedtocompletethesecondyear(Fellowship)andthethirdyear(ClinicalSpecializationinManualTherapy).

4 NEWRESIDENTRECRUITMENTNew Resident recruitment occurs throughout the world and takes place atworkshopsandcontinuingeducationcoursessponsoredbytheInstitute.Atthesemeetings the facultymembersdescribe the Institute’s activitiesandenrollmentmaterials are distributed to interested people. The Institute does not utilize“agents”, employees or independent contractors who are hired to recruit newresidents. All enrollment agreements for admission are submitted to theInstitute’scentraladministrativeoffice.TheInstitutedoesnotgrantpriorcreditsuchas reducedhours in theprogramorany tuitiondiscount foranypreviouseducationor training completed. 5 ACADEMICCALENDAR•The academic calendarbegins everyFebruary, endingwith examinations in SaltLakeCity,Utah,thefollowingJanuary.

• For the Clinical Certification Program, there are 6 seminar weekends of classincludinga7thweekendforreviewandexaminations.

•DMTPrograms,whencompletedon-site, involve10classweekendsthroughouttheyear.Inaddition,3weekendseminarsarerequired(STEP1,STEP4,MT6).Allattemptsaremadetoscheduletheseseminarsinthecityofanon-siteresidency,buttravelmaybenecessarytocompletethesecourses.A4thseminarisrequired,theMT5ClinicalProblemSolvingcourse,whichispresentedinJanuarytheweekofexaminations.

• Independent study residents must maintain weekly contact with assignedfacultymembersfor8or12monthsdependentonresident’senrollment,aswellas attend six mandatory short-term courses (MT2, MT3, STEP1, STEP4, MT5,MT6). Students are required to schedule these courses based on the coursesoffered nationally or internationally, listed on the OGI website. Inaddition,allDMTresidentsarerequiredtocomplete150hoursof fulltimeclinicalsupervisionthefirstyear

• Transition form COMT: For students transitioning from Clinical CertificationProgram(CCPorCOMT)completionofonlinemodulesisrequires,muchliketheIndependent Study Residency program listed above. 9 STUDENTTUITIONRECOVERYFUNDCalifornia law requires that, upon enrollment, a fee be assessed in relation to thecostof tuition.ThesefeessupporttheStudentTuitionRecoveryFund(STRF),aspecialfundestablishedbytheCaliforniaLegislaturetoreimbursestudentswhomightotherwiseexperienceafinanciallossasaresultofuntimelyschoolclosure.StudentsmaybereimbursedbySTRFonlyforprepaidbutunusedtuitionmoneys.Institutionalparticipationismandatory.(EducationCode:Section94342)

Itismandatorythatenrolleeskeepacopyofanyenrollmentagreement,contractorapplication to document enrollment; tuition receipts or canceled checks todocument the total amount of tuition paid; and records which will show thepercentage of the course which has been completed. 10.2 Cancellation:

D. Assume youcancel after completing 50 hours of instruction and before completing theremaining37.5hoursofthetotal87.5hours(perquarter).

13 FINANCIALASSISTANCETheInstitutedoesnotofferfinancialassistanceatthistime.

14 ATTENDANCEREQUIREMENTS Excused and Unexcused Absences: students are required to attend every class inorder to successfully complete their program (Certification, Residency,Fellowship and PhD). We recognize that extenuating circumstances do occur.Absencesarethereforeconsideredexcusedorunexcusedasfollows:

Excused - Illness, death, birth, accidents and extraordinary circumstances. Thesemust be substantiated in writing and are excused at the discretion of theChairmanoftheBoard.

Unexcused-Allotherabsences,includingclasscuts.When absences occur notify the school before the missed class. Residents areresponsibleforcourseworkoutlinedintheirresidentsyllabus.Itistheresident’sresponsibilitytomakeupanyquizzesand/orexamsifapplicable.

Maximumabsences:Amaximumofthreeclasssessionsmaybemissed.Tomaintainsatisfactory progress, these classesmust bemade upwithin the specified timeperiod. If more than three class sessions are missed for whatever reason, theresidentwill be placed on academic probation. Further absenceswill result ininterruptionof the trainingprogram.The residentmaybe required to repeatacourse and be subject to additional tuition charges if the absences wereunexcused.

Interruption for Unsatisfactory Attendance: Residents with three unexcusedabsencesinanyclasswillreceivewrittennotificationofacademicprobationforaone-monthperiod.Furtherabsencesduringthisperiod,forwhateverreason,willresultininterruptionofthetrainingprogram.(Seemaximumabsences).

Make-up Work: All work must be made up within 10 days of the missed class.Makeupworkcannotbesubstitutedforhoursofclassattendance.

LeaveofAbsence:WrittenrequestsforaleaveofabsencewillbeconsideredbytheChairman of the Board. If granted, residents have two years to resume theprogram, when and if it is offered again. The Resident will be responsible forpayingalltuitionfeesrelatedtoanyremainingprogramrequirements.Ifaleaveof absence exceeds two (2) years, the Resident will be required to retake theentirecurriculum.

Lateness: As a courtesy to the faculty members and your fellow classmates, it isexpectedthatallresidentswillbeontimetoclass.Tardiness,withoutlegitimatereason,onthreeoccasionswillbetreatedasanunexcusedabsence.

15 PROGRESS/GRADINGSYSTEM A. TheInstitute’sgradingsystem: Excellent 90-100 A AboveAverage 80-89 B Average 70-79 C BelowAverage 60-69 D Unsatisfactory 59orbelow FAveragerequiredforcertifiedcompletionofthecourseisaminimumpassinggradeof80%.To complete theoverall course students are also required to completethe following: all quizzes, assignments/projects, surveys, 1:1mentoring hours,mid-terms,clinicalpracticalexamsandfinalwrittenandpracticalexaminations.TheDMTprogram requires completion of a research project, otherwise only acertificateofcompletionisawarded,ratherthantheDMT.ThealsoPhDprogramrequirescompletionanddefenseofyourdissertation,otherwiseonlyacertificateofcompletionisawarded.

ExaminationAttendanceIndividualsarerequiredtoattendallscheduledexams.Failuretodosowillresultinagradeofzeroforthatspecificexam.Individualsmaybeabletodemonstrateanextenuatingcircumstance, inwhichcase, theywillbeofferedanopportunity tosit foraretakeexam.Theexammaybe thesameas theexamtakenby the other residents or it may follow a different format. That decision will be made by the Fellowship Program Coordinator and provided to the individual a minimum of three days prior to the date of thescheduled retake.

ExaminationPolicyExamination policies are the same for the Residency and Fellowship programs.Quizzes, written exams and practical exams are an integral component of thetrainingforstudentstodemonstrateaccountabilityforabilitytomeetobjectivesand criteria for passing. Therefore, the assessment of the student’s abilitywillcontinue throughout the course of the 1-year program. The grading of theprogramwillbecomposedofthefollowingelements:

MonthlyQuizzesandAssignmentsEachcoursewillhave1ormorequizzesandassignmentsassociatedwithspecificdidactic content within the curriculum. These electronic quizzes can be takenseveral time,with thehighestscorecounting.Theaveragegrade forallquizzesand course assignments combined will be worth 25% of the individual’s finalgrade.Theaveragegrade forquizzes andassignmentsmustbe greater thanorequalto80%.Individualswhocannotachieveatleast80%quizmayberequired

toperformindependentstudyactivitiestodemonstratecompetencywithinthatarea. This will be formulated by the Program Coordinator. If the individual’saverage for the quizzes, assignments and exams falls below a 70% for twoconsecutive months they will be placed on academic probation. A formalremediation planwill be establishedwith scores reaching 80%or better to beremovedfromacademicprobation.

ProgramWrittenandPracticalPatientExamsTwowrittenexaminationsshallbecompleted,at theendofeachof thetwoterms(~5monthsand10monthsintotheprogram).Inaddition,2practicalexamsand6treatmenttechniqueexamswillbecompletedoverthecourseoftheprogram.The average grade for all thesepractical examinations combinedwill beworth25%oftheindividual’sfinalgrade.

FinalComprehensiveWrittenExamOnefinalwrittenexamwillbeadministeredattheendoftheprogram.Thegradeforthe finalwrittenexaminationwillbeworth25%of the individual’s final grade.Individuals must achieve a grade of 70% or higher on this exam in order tocompletetheprogram.IFagradeoflessthan80%isachievedthentheindividualwillbeofferedanopportunitytositforare-takeexam.Ifthegradeachievedonthere-takeexamislessthan70%theywillbeplacedonprobationandaformalremediation plan will be established. Studentswill not be eligible to complete the program with a score of less than 70% on this re-take exam. Individuals will be offered a maximum of three re-take opportunities to pass the final written exam. If it is not passed by 70% on the third re-take, the individual will be dismissed from the program.

Final Practical Patient Exams Each student will perform one final practical exam either involving patients, or patient

models at the discretion of each Examination Chair. Students must achieve a grade of 70% or higher in order to complete the program. If a grade of less than 70% is achieved the student will be offered an opportunity to perform a re-take second practical exam. If the grad achieved on the second exam is less than 70% they will be placed on probation and a formal remediation plan will be established. Individuals will not be eligible to complete the program with a score of less than 70%. Individuals will be offered a maximum of three opportunities to pass the final practical patient exam. If final practical patient exam is not passed by the third time the individual will be dismissed from the program. If a practical exam is failed secondary to “safety” issues/concerns the student must successfully pass a follow up practical or they will not be able to proceed within the program. Failure to successfully pass a re-take practical exam may result in termination from the program.

Program Completion Program completion is achieved with the completion of all assignments, quizzes and

exams. The final grade will be an average of the aggregate scores from the quizzes/assignments, program exams, final written and final practical. The aver must be 80% or higher to successfully complete the program.

B.UnsatisfactoryProgress:Residents,whoarenotperformingatasatisfactorylevel,duringthecourseoftheresidency,willbenotifiedinwritingbytheChairmanoftheBoardandgivenfeedbackastohowtheycanimprovetheirperformance.IftheresidentcontinueswithpoorperformancetheymaybeplacedonAcademicProbation.Ifthisoccurs,theResidentwillbegivenspecificguidelinesastowhatthedeficienciesare,guidelinesofwhatisexpectedtobecorrected,amechanismand/orsuggestionsofhowthedeficienciescanbecorrected,andatimetableforwhenthecorrectionswillbeexpectedtobecompleted.

C. Re-enrollment:Re-enrollmentwillbeapprovedonlyafterevidence is showntothe Chairman that conditionswhich caused the interruption for unsatisfactoryprogresshasbeenremedied.

D.SatisfactoryCompletionofanyprogramisdependentonsuccessfulcompletionoftheevaluationconductedbytheBoardofExaminersappointedbytheChairmanoftheBoardandtheBoardofDirectors.(SeePracticalExaminationPoliciesandProcedures). The Certificate of Completion document is to be issued uponsatisfactory completion of the Orthopaedic Manual Physical Therapy Program.The certificate of completion is certifying/recognizing the competency of thegraduate.

E. Graduation Rates: historical graduation rates are not available but the OGIcurrentlytracksgraduationratesofitsstudentsforaccreditationreporting.

16 RESIDENTCONDUCTPOLICYAt thediscretionof theChairman,aResidentmaybedismissed fromthe Instituteforaseriousorrepeatedincidentofanintoxicatedordruggedstateofbehavior,possessionofdrugs,alcoholorweapons,aswellas,behaviorscreatingasafetyhazard to other persons, disobedient or disrespectful behavior to anotherResident, administrator, faculty member, or any other stated or determinedinfractionofconduct.

17 RESIDENTCOMPLAINTPROCEDUREAnycomplaintsmaybedirectedeitherorallyor inwritingtoanyfacultymember,administrator,ordirectlytotheChairmanoftheBoard.Theresidentwillreceivenotificationinwritingthatthecomplainthasbeenreceivedwithintendays.

Upon receipt of the complaint, the Chairman, along with the Vice-President ofAcademicAffairs (seeAdministrativeChart)will then investigate thecomplaintthoroughly.If it isdeterminedthatthecomplaint is legitimate,theInstitutewillimplementresponsiblepoliciesorprocedurestoavoidsimilarcomplaintsinthefuture.FilingacomplaintwillinnowayaffectaResident’srightsorremedies.

18 PLACEMENTASSISTANCE&EMPLOYMENTRATESAsarule,residentsintheprogramarealreadyemployedasphysicaltherapistsandthereforetheInstituteoffersnoplacementassistancetoitsgraduates,butwillberendering the service of referrals of graduates to employers when regionalpositions are available. As students are gainfully employed physical therapist

prior to and during the Institute’s programs, 100% of graduating therapistsremaingainfullyemployedatthetimeofgraduation.

23 ORTHOPAEDICRESIDENCYPROGRAMS23.1 YearI:DoctorofManualTherapy(DMT)

TheDMTresidencyprogramisoneyear in lengthand it isofferedthroughouttheU.S.,EuropeandAsia.Theprogramconsistsof supervised instructions thatcanbe completed one of two ways (Independent Study Residency or On-SiteResidency).EitheraCertificateofCompletionoraDoctorateofManualTherapy(DMT) will be awarded upon successful completion from either of the twoprograms. The basic sciences and an evidence-based background in clinicalpractice form the curricular foundation of assessment and treatmentinterventions for spinal and extremity dysfunctions. The instruction includesmethodstoimproveclinicalreasoningandproblemsolvingwhichwillassistwiththe development of a patient diagnosis as well as the patient’s plan of care.Interventionstaughtwillbemanualtherapybasedwithemphasisonsofttissuetechniques,articulations(i.e.oscillations,mobilizations,andmanipulationsofthespine and extremities), and specific exercise dosing principles. Residents willparticipate inaclinicalsupervisionexperiencewithanOGIapprovedinstructorfor a minimum of 150 1:1 contact hours. The resident must receive approvalregarding his/her research topic and complete a scientific inquiry toward adissertation. Enrollment begins in September with an application deadline ofNovember1stforclassesbeginninginDecember.Duringtheprogram,residentsareresponsibleforalltravelandlodgingcoststhatmaybeincurred.

•DoctoralDissertation:Year3isthecompletionoftheDoctoralDissertationdevelopmentprocess and uses the students’ approved prior study as the point of departure forcollecting, analyzing, and interpreting the data and composing the Dissertationmanuscript. As was the case in the two previous years, completion of the DoctoralDissertations involves interactions with the Personal Research Mentors who wereassigned in year 1 and whose role continues to be leading the students through theactivitiesassociatedwithcollectionoftheirdataandthecompositionoftheDissertationmanuscript. The student will expand his/her research study and expand all areas to appropriately fit the format of a PhD dissertation. There is a step-by-step process that includes relevant “handouts” to assist the student in successful completion of his/her Dissertation Manuscript. Studentswho need assistance in analyzing and interpreting their data canobtaintheservicesofmethodologists/statisticiansforthispurpose.TheDSCChairsandMembers provide suggestions for improvement, after the students and their Mentorshave produced a complete copy of the Dissertation manuscript. Once the DSCs haveapproved theDissertationmanuscripts, students scheduleOralDefenses thatwill takeplace in mid-January in Salt Lake City, on days/times that are compatible with theschedulesoftheirDSCs.FollowingsuccessfulcompletionoftheirOralDefenses,studentsrespond to suggestions for improvement that may grow out of their Oral Defenses,producecleancopiesoftheirDissertations,obtainthesignaturesoftheirDSCs.
