Aims and Objectives

Cherry Holt Care Home (Cherry Holt) is registered to accommodate for 52 persons who require residential or nursing care for the elderly. Cherry Holt Care Home treats service users with disease, disorder or injury. Cherry Holt also carries registration for 3 physical disability beds under pensionable age. The home also holds an additional registration for three adults with physical disabilities. Cherry Holt Care Home welcomes couples of the same and different gender to be residents in the Home and equality and diversity of residents in the home is fully promoted and welcomed as detailed in the Home’s Admission Policy. The care home aims to deliver a high standard of professional care at all times to the service users in our care. The care provision at Cherry Holt endorses a holistic approach which is sensitive to the needs of every service user and it encompasses the following essential aspects of the person:

Our aims in relation to the above aspects of care are outlined below: Privacy: Preserve every service user’s right to privacy at all times by: • Respecting and keeping confidential all information relating to a service

user and their relatives. • Respecting a service user’s preference to be alone and undisturbed. • Discussing personal matters with staff and relatives in private and

without violating confidentiality. • Provision of the facility for a service user to make and receive telephone

calls in private. • Handing over all correspondences to a service user in their original

sealed state. Dignity: • Treat every service user with respect and allow each person to live in a

dignified manner. • Maintain every service user’s autonomy by empowering each person to

consider choices and make their own decisions as far as possible. • Know each service user, develop a good rapport and treat each service

user with respect. • To deliver person-centred care helping to put the needs and aspirations

of the individual service user at the centre of care planning. • An extra effort has been made at Cherry Holt to promote dignity in the

Care Home All staff are trained in Dignity in Care to ensure the following ten dignity challenges are met as standard practice.

kept well hydrated. • Offer choice and support allowing service users to enjoy their diet. Choice: • Ensure every service user has options over their activities of daily living

in accordance with their capabilities. Rights: • Preserve all basic human rights of each service user including consent,

confidentiality, safety, equality and autonomy. • Encourage freedom of expression, participation and decision making. • Encourage right of access to entitlement of services and benefits. • Presume that every service user has the mental capacity to make decisions unless this has been proved otherwise through assessment. Fulfilment: • Enable the service users to realise their own aims and create

opportunities for them to achieve their goals in all aspects of daily living.

Philosophy of Care

Cherry Holt aims to provide its service users with a secure, relaxed, and homely environment in which their care, well - being and comfort are of prime importance. Carers will strive to preserve and maintain the dignity, individuality and privacy of all service users within a warm and caring atmosphere, and in so doing will be sensitive to the service users’ ever-changing needs.

Such needs may be medical/therapeutic (physical and mental welfare), cultural, psychological, spiritual, emotional and social. Service users are encouraged to participate in the development of their individualised Care Plans in which the involvement of family and friends may be appropriate and is greatly valued. This will be achieved through programmes of activities designed to encourage mental alertness, self-esteem, social interaction with other service users and with recognition of the following core values of care which are fundamental to the philosophy of our Home:

All Care Staff within the Home are appropriately qualified and trained to deliver the highest standards of care. Details of Staff Numbers and Staff Training

The home employs 50 members of staff. There are 7 members of staff on each shift, one nurse and 6 skilled mix staff. On the night shift there are five staff on duty, one nurse and 4 carers. The staff are selected for their qualities of reliability, integrity, friendliness, professionalism, interpersonal and caring skills. They are carefully screened and references are checked thoroughly. During induction all staff are trained in-house and supervised by experienced qualified senior staff in the following critical standards: • Understand the principles of care • Understand the organisation and role of the carer • Maintain Health and Safety • Understand the individual experiences and needs of the service user • Understand the effects of care home setting on service provisions Cherry Holt encourages all care assistants to hold a minimum of NVQ level 2 in Health and Social Care. The Quality and Training Manager ensures that all staff receive mandatory training, for example, Food Hygiene, Manual Handling, Safeguarding, Dignity in Care, Nutrition and Hydration. External trainers are brought in the home to train the staff for their professional development. Internal training is also offered to all staff on an ongoing basis by the Staff Development Manager; ensuring staff receive up-to-date information and advice on changes within the care sector.

Accommodation

Cherry Holt has 52 beds which are divided as follows: Ground Floor: 30 single rooms with en suite First Floor: 22 single rooms with en suite Social Rooms: There are three lounges (one designated smoking lounge) and three dining rooms on the ground floor and two lounges and dining rooms on the first floor, all centrally heated. Service users are encouraged to socialise in these communal rooms; however, service users are free to choose to stay in their own rooms if they so wish.

Admission

Service users interested in coming to Cherry Holt are encouraged to visit the home and sample the atmosphere and level of service. A trial period of four to six weeks is always offered before taking permanent residency. In the event of an emergency admission, as much information as possible is obtained in order for an objective assessment to be made. From this information, the service user will then be assessed on admission and a care plan is devised immediately. All the criteria of admission are considered, such as age, category and dependency levels in line with the registration status of the Home.

Financial Arrangements and Fees

We are committed to providing quality care at Cherry Holt Care Home. The fees charged are dependent on: 1. The type of facility. 2. The type of care package and needs of the individual service user. Depending on the personal financial situation, a service user can either pay the fees privately or have part or full of the fees paid by the contracting Social Services. Specific advice is available from the Home Manager they may prefer to contact Age Concern for representation.

Privacy and Dignity

It is emphasised to every member of staff that he/she must preserve and maintain the dignity, autonomy, individuality and privacy of all service users within a warm and caring atmosphere; and in so doing will be sensitive to the service users ever changing needs. An extra effort has been made at Cherry Holt to promote dignity in the Care Home and it is the service user’s right to be treated with respect and dignity. All staff have been issued dignity cards to ensure the following ten dignity challenges are met as standard practice.

Smoking and Alcohol

The Home has two designated smoking areas. With regard to alcohol, service users will normally make their own arrangements, however, as with smoking, they may require to be supervised.

Fire Safety

• The home has a Fire Alarm System fitted, with “Fire Exit Notices” and

“Fire Emergency Instruction Notices” displayed at strategic points throughout the home.

• New staff have an induction training which includes: Fire Prevention and

fire drills Policy, use of the fire appliances, evacuation, muster points, raising the alarm, and so on. Service users are informed of the emergency procedure during admission.

• A fire exercise is carried out once a week, this ensures all staff and

service users have a comprehensive understanding of their responsibilities. A full fire drill is conducted monthly which involves evacuation of the home.

• All fire systems and alarms are tested weekly by staff of the home and 6

monthly by the local Fire Officer. Records are kept of all such testing as part of the Proprietor/Managers responsibilities.

• All fire fighting equipment will be checked annually by a qualified fire

extinguisher maintenance engineer. • Where possible, furniture, fixtures and fittings must be made of fire-

resistant of fire-retardant fabrics and materials. • A fire risk assessment is carried out and reviewed periodically and this

is available for inspection at anytime.

Religion (Worship/Attendance at Religious Services)

Service users may attend religious services either within or outside the home as they so desire. Service users have the opportunity to attend Holy Communion every month. The home arranges the transport for such services and an escort, either with staff, friends or relatives. Service users have the right to meet clergy of their chosen denomination at any time. If required, a private room will be made available for such meetings.

Contact with Family and Friends

Service user’s family, relatives and friends are encouraged to visit the service user regularly and maintain contact by letter or telephone when visiting is not possible. In these cases, staff will offer to assist the service user to respond where help may be needed. Visitors are welcomed at all reasonable times, and are asked to inform the staff of their arrival and departure from the home. For Security and Fire Safety reasons, visitors must sign the visitor’s book on each occasion.

The service user has the right to refuse to see any visitor, and this right will be respected and up-held by the Person-In-Charge who will, if necessary, inform the visitors of the service users wishes.

Care Plan Review

Once developed the Care Plan is reviewed every four weeks at a minimum to ensure that the service user is responding in a satisfactory manner. Families and relatives will be encouraged to participate in the monthly formal reviews of care plans. Their relatives are always welcome to discuss their progress, level of support and care being provided. The Care Plan is reviewed at three levels: • Daily on a shift-to-shift basis. During staff shift changeover the service

user’s daily care notes are handed over by the out-going and in-coming shift and the reporting is discussed of the service users’ responses and activity patterns.

• At the end of every four week period. • Any amendments to the care plan are fully recorded.

Bereavement

In the unfortunate event of bereavement, the family can expect every possible support from staff. Whereas funeral arrangements are usually made by the next of kin, the Home Staff can be relied upon to assist and explain what is required. Where there is no next of kin, the staff will attend to the necessary arrangements. The Home will comply with any planned funeral arrangements specifically in respect of their chosen faith and religious beliefs.

Social / Therapeutic Needs

The home’s policy on “Therapeutic and Social Activities” takes into account the service user’s interests, skills, experiences, personalities and medical condition. Initially, information on the health and social needs are obtained during a pre-admission assessment. The home offers a wide range of activities designed to encourage the service user to keep mobile, and most importantly take an interest in life. Staff encourage and in certain instances help service users to pursue their hobbies and interests. Current and past activities that the home offers are listed below, however these are reviewed and can change depending on the likes and interests of the service users in the home. Service users are invited to make their own suggestions.

Outings

All outings are geared to service users’ needs and capabilities and due to this a limited number of service users can go on any one outing. Examples of outings are listed below: a. A drive around the countryside. b. Visit to garden Centres. c. Visit to pantomimes or plays. d. Shopping / Excursions Trips are planned in accordance that facilities for the disabled are in place.

Leaving or Temporarily Vacating

If a person wishes to be discharged from the Home, then 4 weeks notice must be given of this intention, or 4 weeks fees paid in lieu of notice. These conditions are waived during the 4 week trial period. If a service user temporarily moves out of the Home (e.g. to receive hospital treatment) the bed is retained for a period of eight weeks, provided 80% of the normal fee is paid. In the case of Social Services funded service users, this retention period would be reviewed by the Home Manager. In the unlikely event that the Home has to give notice to leave to a service user, a 4 weeks notice is equally applicable.

Monitoring and Quality

A variety of quality monitoring and auditing tools are utilised to ensure the quality of the service we provide is fully compliant to the Care Quality Commission, such as care planning, audit, risk assessments of the environment and a maintenance audit of equipment in the Home. An important part of our quality programme is to involve the service users and their relatives. We regularly ask for comments on the Home, the staff and services we provide using informal methods as well as more formal medium like the service users’ Satisfaction Survey forms.

Pets

Whilst we acknowledge the fact that many people have pets for company during their lifetime and that they may wish to bring an animal with them when they move, the management has a responsibility to all the service users with regard to Health and Safety, also as to their choice to have animals within the home. The Manager will treat each case on its own merit.

Medication

If a service user prefers to self-medicate and is deemed safe to do so following a risk assessment, the Home will support them with advice and guidance. A lockable facility is available accordingly. Otherwise all drugs will be managed by the staff, dispensed and ordered on their behalf under the instructions of their General Practitioner. Any service user may request to see a doctor in private if they so wish.

Telephone

The home has a cordless telephone, which the service users can use for incoming/outgoing calls. A nominal fee is charged for the outgoing calls. Service users may have their own private line should they wish.

Meals

Menus are varied and favourite dishes and special diets are catered for. Service users are encouraged to eat in the dining room but may choose to eat in their own room if they so wish. Tea, coffee and other hot drinks are served and are available 24 hours a day. Visitors are also catered for.

Complaints

At Cherry Holt we aim to maintain the highest standards of care but appreciate that from time to time there may be areas of concern, which need to be discussed. The Service user, their relatives, friends or representatives are invited to raise any concerns that they have over actual standards of care, or any other matter, in the following manner. If you are pleased with the level of services provided and you think the procedures of the home and staff have made a difference, we want you to tell us. You can make comments or compliments either verbally or in writing to our staff who you have direct contact with, or any member of the management team. Cherry Holt strives to get things right first time. However, in the event that a service user or relative feels the need to complain, the procedures below should be followed.

You should first discuss the matter with the person in charge. If the matter in your opinion is a serious one, or if you remain dissatisfied, you can record the complaint in the Complaints Book, which is available from the Care Home Manager or from the person in charge. Alternatively, you may write to the Manager directly expressing your concerns in confidence. A full investigation will be made into the complaint, and you will be advised of the outcome within a time-scale of 28 days, or earlier. If after this investigation, you are still not satisfied, or if you feel that the complaint is of a serious nature and you wish to speak to a registration officer first, then you should contact: CQC East Midlands, Citygate, Gallowgate, Newcastle Upon Tyne, NE1 4PA. APPENDIX A: Contract of Residence

Agreement Between ………………………………… (Proprietor) and ……………………………… (Service user or Nearest Relative) relating to acceptance of Accommodation in room …………………………

1. Upon payment of the agreed weekly charge (see 3 below) the proprietor undertakes to provide full board, personal care and services as required by the service user.

2. The service user may be expected to provide from his or her own

resources and or personal allowances, medical requisites (other than prescribed medication) hairdressing, newspapers, clothing and items required of luxury nature.

3. The weekly charge shall be £………………. Which is agreed shall cover the

provision of all services enumerated in 1 above and there shall be no “extras”. The fees mentioned should remain unchanged unless the proprietor gives 28 days written notice due to any clinical change and this agreement will be jointly amended. The fees will be reviewed annually.

4. The proprietor undertakes to maintain a standard of care consistent

with the requirements of the Health and Social Care Act 2008, and regulations made there under, and any requirements of the registration authority (Care Quality Commission).

5. Following the initial trial period of 28 days of the service user being

accepted on a permanent basis, this agreement shall remain in force until terminated by circumstances or by written notice given by either party for not less than 28 days before the date of termination. When there is a dispute about a proposed termination the registration authority shall be notified in writing of any such action.

6. The proprietor undertakes to try and arrange the health services as

necessary. Should the service user at any time require hospital treatment, the proprietor undertakes to retain the accommodation for the service user on payment of full fee (see 3 above) for a period of six weeks. Thereafter, the proprietor and the clinical manager will review the individual circumstances to decide on an extension of the arrangement or a termination of the placement.

7. The proprietor has to maintain certain records about the service user in

accordance with the registration authority. These details will be treated by the proprietor and member of staff as confidential.

8. The facilities of the home shall include unrestricted use of the service

users’ bedroom, lounge, other communal rooms, bathrooms, toilets and so on.

9. Smoking is only permitted in designated areas. Under no circumstances

should any service user be allowed to smoke in non-designated areas such as their own bedroom, lounge and toilets and so on.

10. The following operational details are accepted by the service user, although flexibility is afforded at the request of the service user.

Morning Drinks 10.30 - 11.00am

Lunch 12.00 - 1.00pm

Afternoon Drinks 2.00 - 2.30pm

Tea 5.00 - 6.00pm

Supper 7.00 - 8.00pm

11. If the service user has any complaint they should ideally be taken up

with the proprietor. The Home is registered with the Care Quality Commission (CQC) East Midlands, Citygate, Gallowgate, Newcastle Upon Tyne, NE1 4PA. Email: [email protected] . The service user will have free access to this department and its inspecting officers, who will visit the home and the service user from time to time.

12. The proprietor undertakes to ensure that at all times the home is

registered in pursuance of the Health and Social Care Act 2008 with CQC (Care Quality Commission).
