WELCOME The Practical Nursing Faculty welcome you to the Practical Nursing (PN) Program at FLORIDA STATE COLLEGE @ JACKSONVILLE. This manual has been designed specifically for PN students to provide some important information about the PN program. We hope that you find the content informative and helpful. The Practical Nursing Program Student Policy & Procedure Manual is intended to supplement the FLORIDA STATE COLLEGE @ JACKSONVILLE Catalog which contain material related to student rights, academic policies, registration, financial aid, campus facilities, and course offerings. We urge you to take the time to familiarize yourself with these documents. Although your studies will be rigorous and demanding at times, please know that the faculty and staff are here to support and mentor you, answer your questions, and facilitate the achievement of your goal to become a Practical Nurse (PN). The material contained in this manual will provide answers to many of your questions about the nursing program including its clinical requirements. However, you should also communicate with your course instructors on a regular basis about your progress in the program and any concerns that might develop. We encourage you to seek our assistance. The Practical Nursing faculty and staff are committed to supporting your accomplishments as you prepare for a career as a Practical Nurse. Practical Nursing Curriculum and General Policies

The following courses and grading criteria will apply to all students in the Practical Nurse Program at FLORIDA STATE COLLEGE @ JACKSONVILLE. They are designed to direct and guide the student through the program and to enhance the student’s opportunity for successful completion of the curriculum.

1. Practical nursing courses which must be completed each term are:

• NOTE Clinical hours may vary according to clinical facility availability

• Must meet or exceed 850 benchmark on the Comprehensive HESI to complete PNIII and participate in completion ceremony activities.

2. Continuous enrollment will be required of students admitted to practical nursing unless exceptional circumstances exist. Exceptional circumstances will be determined by the Dean of Nursing.

3. CPR certification acquired through an approved “American Heart Association”

course that includes “hands on” simulated experiences with infants, children, and adults is required for continuance. CPR certification must remain current throughout enrollment in the nursing program.

4. Eight hours lab practice outside of classroom time is required in PN I.

5. Students will adhere to Professional Guidelines established by the FSCJ

Department of Nursing in Appendix A, the PN Code of Ethics, the PN Standards for Professional Nursing Practice, the Rules and Regulations of Florida Board of

Nursing, and the requirements of each agency where assigned for clinical experiences.

6. Criminal Background:

All students are required by the clinical sites to have a criminal background check and drug screening completed prior to going to their facility. It is due on the first day of class so the clinical sites can review and determine if the student can attend clinical experiences at their facility. Data provided therein may be a basis for denial of admission or denial for continuation in the Nursing Program. Students prohibited from completing the required clinical hours and in the areas mandated by the profession and the state of Florida will be exited from the program. Background checks and drug screens may also be periodically requested by the College. Students are responsible for the cost of the drug screening and background check. In addition, there will be a second background check that is required by the specific clinical facilities and AHCA. This is mandatory and must be completed prior to clinical experience beginning. Failure to comply will result in the student being prohibited from clinical experiences. Upon completion of all program requirements, a student with a positive background check may write or appear before the Florida Board of Nursing to gain approval to take the NCLEX Exam. NOTE: program completion with a + background does not mean the Florida Board of Nursing will allow the completer to sit for the NCLEX-PN.

See Appendix B and please complete the required procedure with the Dean of Nursing before the end of the term. Effective July 1, 2009, pursuant to Section 456.0635, Florida Statutes, health care review boards shall refuse to renew a license, certificate or registration, issue a license, certificate or registration and shall refuse to admit a candidate for examination if the applicant has been convicted or plead guilty or nolo contendre, regardless of adjudication, to a felony violation of: chapters 409, 817, or 893, Florida Statutes; or 21 U.S.C. ss. 801-970 or 42 U.S.C. ss 1395-1396, unless the sentence and any probation or pleas ended more than 15 years prior to the application, terminated for cause from Florida Medicaid Program (unless the applicant has been in good standing for the most recent 5 years) or terminated for cause by any other State Medicaid Program or the Medicare Program (unless the termination was at least 20 years prior to the date of the application and the applicant has been in good standing with the program for the most recent 5 years). The Florida nursing or allied health licensure or certification application requires disclosure of any criminal history and the disposition of all cases prior to board review. Being convicted of, or having entered a plea of guilty or nolo contendere to, any of the listed felonies in Florida Statute 456.0635 or termination for cause from the statutorily-described Medicaid programs will preclude an applicant from obtaining a nursing or allied health license or certification in the state of Florida. Entry and acceptance into the nursing program does not guarantee licensure or certification in the State of Florida.

English As A Second Language:

Students with English as a second language are encouraged to obtain assistance from the course instructor and/or the Learning Commons Tutor when pronunciation or understanding medical terminology is an issue impacting the achievement of satisfactory grades. Get help early! All course documents including, but not limited to, syllabi, lecture notes, handouts, quizzes, exams, etc. are written in English. Dictionaries of English-to-other-language are prohibited during test-taking. The NCLEX-PN is written in English.

Student Policies and Procedures

Attendance:

1. Classroom and clinical hours will vary depending upon the admission of the students to the day or evening cohort.

2. Students are required to report to their assigned area as scheduled. Students are required to sign-in and out in the classroom and clinical site.

3. Punctuality is expected in both class and clinical. A tardy is considered absence in

excess of 15 minutes. After 2 tardies, 4 hours will be made up. Habitual tardiness will indicate a need for counseling and appropriate action by instructor. The PN Program hours are aligned with FBON, Florida Board of Nursing.

4. Students are required to attend all scheduled clinical hours, including clinical hours

on campus. Students who fail to meet the mandated 1,350 hours of instruction will not graduate. If the absence occurs on the day of the scheduled clinical experience, the instructor must be notified no later than one hour prior to the scheduled start time (e-mails are not acceptable). Note: Voice mail messages are not acceptable; student must make real-time voice contact with the instructor. Documentation, including medical clearance, validating the reason for the absence must be provided if requested. The student will assume full responsibility for all content covered during the absence and will be held to the same level of performance as that of other students. An incomplete or failing grade may result from a clinical absence. In order to successfully complete a clinical course, students must meet all clinical objectives and demonstrate satisfactory clinical performance. In the event a course is failed, both components (class and clinical) must be repeated in their entirety.

5. Clinical tardiness (such as arriving late for the start of clinical and leaving clinical

early, etc.) is determined by the instructor and may result in dismissal for the day. Three episodes of tardiness will equal one clinical absence. Clinical tardiness is defined as arriving late for the start of clinical/pre-conference or leaving before the end of post-conference.

6. Any student reporting for a clinical experience who appears ill or injured may be sent home at the discretion of the clinical instructor, and this will count as a clinical absence.

7. Excessive absences will jeopardize the student’s progress in the program.

Therefore, the maximum number of absences is limited to 24 hours per classroom or 24 hours per clinical maximum per semester. (10% of the course class and clinical combined for FA purposes only).

8. All absences, both class and clinical will be made up to successfully to complete the

term. A student may be allowed to make up clinical absences at the discretion of the faculty member and only during the days and hours designated. Students who exceed the number of absences will receive an unsatisfactory grade and will not advance to the next term. Any student who must leave classroom, lab skills, or clinical for any reason is required to notify the assigned instructor.

9. Routine medical and dental appointments must be scheduled after school hours. 10. No visitors are permitted in classroom, lab skills, or clinical areas. This includes

spouse, infants, children and significant other.

No Call/No Show

When the student is absent from clinical for any reason and has not called the

clinical unit previous to the start of the shift, this will be considered a no call no

show and is a program violation which will result in the student being placed on

program probation. Students who accumulate more than two no call/no shows in

the program will be permanently exited from the program.

Grading: 1. The grading scale in the Practical Nurse Program is:

A = 93-100 B = 86-92 C= 80-85 F=<80 A passing grade will be received when the student meets the objectives of each nursing course and performs satisfactorily in the clinical setting.

2. Students are required to achieve a minimum overall average of 80% for all courses

(Basic Health Care Worker, Articulated Nursing Assistant, Practical Nursing 1,2, and 3 on examinations, including the final examination (without any rounding) to successfully complete the course. If at any point it is clear that an 80% on all tests is not obtainable, the student will be exited.

3. Course faculty has the discretion not to include written work (projects, quizzes, etc.)

in the calculation of the final course grade. Clinical Performance will be weighed and is equivalent to 20% of the Final Course Grade.

4. Students who fail to meet all other course requirements will receive one (1) of three (2) instructor assigned grades. These grades are:

F = Failure (below 80 in Basic Health Care Worker Below 80 ANA, PN 1, PN 2, PN 3) FN = Failure for non-attendance These grades will be assigned in accordance with the colleges and individual course policies.

Grade Disputes:

After a student has sufficiently and appropriately discussed the issue with the nursing faculty, they may set up an appointment to meet with the Dean of Nursing, if further consideration is desired, the student may pursue a formal grade appeal. 1. Examinations: Students are required to complete examinations as scheduled. To take an

examination at an unscheduled time requires approval by the faculty member prior to the occurrence. Students must provide documentation validating the reason to take the examination at another time, if requested. Students who are allowed to make up examinations must take them the date, time, and place specified by the faculty member. Make-up examinations may be in a modified format than the original multiple choice examination. A failing grade may result from unsatisfactory performance. Opportunities to remediate will be provided based on the needs of the student. Students are expected to request a faculty conference after each examination failure.

2. Written Assignments:

Each course instructor will set the course policies regarding written assignments, missed assignments, late assignments, etc. Students will be required to sign a statement indicating that they have read and understood the course policies and course requirements for the courses in which they are enrolled.

Electronic Devices:

The use of electronic devices that may record audio or visual data is prohibited in all classes and clinical components of the Nursing program. These include, but are not limited to: Cell phones (including camera phones and smart phones), pagers, programmable calculators, digital watches, electronic pens or hand-held computers. They are not allowed in any testing situation (class, clinical or in the nursing skills laboratory) with or without instructor permission.

Possessing and/or using these unauthorized devices during an exam or academic exercise constitutes a violation of the nursing program’s academic integrity policy. The student‘s device will be confiscated by the faculty-proctor; the student will receive a zero (0) for the exam; and the student will be disallowed from attending classes, pending review by the Dean of Nursing for determination of the appropriate action to be taken.

Clinical Performance, Evaluation And Conferences:

1. Clinical performance is documented on the clinical performance rubric and will count as 20% of the course grade. Should a student receive an unsatisfactory evaluation (<2 points) for two attributes, the performance will be considered unsatisfactory and will constitute a clinical failure. A clinical failure results in a course failure. Refer to Appendix J

2. There will be an end-of-course conference to discuss student self-evaluation and

the instructor’s evaluation of the student’s clinical performance. In the event that the student has had clinical experiences with more than one instructor during the term, all instructors will provide a clinical evaluation rubric and findings will be averaged. Should a problem arise regarding the clinical evaluation, the student has the recourse of seeking a joint conference with the instructor and the Dean of Nursing.

3. Instructors will have periodic individual conferences with students as needed to

discuss clinical and academic performance.

Medication Safety Examination:

Medication Administration Safety is a program requirement. One way students demonstrate the knowledge to safely administer medications is to successfully complete a drug safety test at the start of each course in each term of the program. 1. Students must successfully complete a medication safety test at various times

throughout the curriculum.

2. The test must be passed before the student enters the clinical setting. 3. The passing grade for the medication safety test is 80%. 4. Three attempts will be permitted to successfully complete this test. If a student is unsuccessful on the first attempt, the test will be reviewed with the student and the test will be retaken.

5. If a student is unsuccessful on his or her second attempt, the student will be referred

for the necessary assistance/tutoring. 6. Use of basic calculators that have been approved by the Dean of Nursing on

medication safety tests is permitted after successful completion of the Articulated Nursing Assistant course. Programmable calculators and cell phone calculators are not permitted.

E-Mail Address:

Students are required to obtain a College e-mail address. This address is the only one that will be used for electronic communications with students. Additionally, this is the only address that will be used to provide students with access to “on line” resources. It is a unique, individualized email address that ends: [email protected]

Evolve Student Accounts:

All students in the PN Program have access to an account from EVOLVE that provides access to supplemental training and remediation activities to enhance NCLEX success. This account is linked to all program textbooks that are published under the EVOLVE label and are used in each term of the program. The account is also linked to HESI Case Studies which are required per syllabus throughout the program. The user name and password from EVOLVE are unique to each student and are used to access the supplements. Students need to use this user/password information to access course supplements, required and recommended assignments, end-of-course HESI exam, and post HESI Exam Remediation packets, etc.. Please make a note of this information in the notebook you use for each course in the program so you have this readily available. Elsevier suggests the use of Google Chrome or Firefox for better performance when accessing your Evolve account.

NCLEX Preparation Exams:

In order to prepare for NCLEX, students are required to complete 10 (ten) NCLEX review questions daily or 50 (fifty) per week from the NCLEX-PN review book and turn in verification of completion each week to the Lead Instructor. Standardized Examinations are given throughout the program at the North Campus Testing Center in preparation for the National Council Licensure Examination (NCLEX). The proctored examinations will be counted as a test score in each content area. These exams give students and faculty an indication of predicted performance on the NCLEX Exam for PN licensure. Failure to complete and remediate these required examinations by the conclusion of the course will result in a grade of Failure (“F”) and the inability to move forward to the next course An NCLEX Review and Comprehensive exit exam is also given at the end of the program to promote optimum performance on the National Council Licensure Examination. Students are required to take the HESI comprehensive exit exam #1 before completing Practical Nursing 3 (approximately one month prior to pinning). If the 850 benchmark score is met, the Comprehensive HESI exit #2 is not required. If the benchmark is not achieved the student will receive a grade for the class but will not be able to participate in the pinning ceremony. These students will need to create their remediation packets by pulling and printing information from the areas where their low scores occurred. Once the remediation is complete and approved by the designated faculty person, the student’s name will be sent to the testing center and the student will be allowed to schedule the Comprehensive Exit HESI #3. If a third attempt is required, students will be referred for further remediation and the HESI test taking strategies. Once the prescription is completed and approved by the instructor, the student will be allowed to take the comprehensive exit exam a third time. Students are required to complete the HESI case studies, practice quizzes, and exams in the Elsevier software each semester. Failure to do so will result in an “F” grade for the

course and will prevent progression to the next course. Please remember that an “F” can adversely affect financial aid. Students will be permitted to take the practice quizzes and exams a maximum of 2 times after receiving their 1st HESI score. Therefore, practice quizzes and tests should be used as intended involving determining the correct answer and learning why it is correct. Any student who has not obtained the 90% benchmark after 2 remediation’s completed during the program will be exited from the course in which the remediation benchmark was not achieved, receive a failing grade for the course and be forced to repeat the course, if that is an option.

Pinning Ceremony:

This is a traditional ceremony in the nursing profession recognizing successful completion of the program. Students who participate in the pinning ceremony are required to wear a professional white uniform – not the school uniform (females can wear a white dress or pants) school nursing caps are optional. Shoes must be white leather and nurse appropriate – heels/open toes are not permitted. The official school PN pin can be purchased in the North Campus book store. No other pins will be utilized during the pinning ceremony. Participation is optional and is dependent upon the student meeting all program requirements.

Complaint Process and Chain of Command:

Please adhere to the established protocol for handling problems/ issues in the Practical Nursing Program. First, the student must consult with the course faculty member. Second, if no resolution in the issue is achieved, the student may make a written request and schedule a meeting with the Associate Dean of Nursing. Third, if no resolution in the issue is achieved, the student may schedule a meeting with the Dean of Nursing and the faculty member.

Confidentiality:

All client information, verbal or written, is confidential. Failure to protect the “patient bill of rights” exposes the student, faculty, the PN program and the College to litigation by the patient, his or her family, and/or the clinical facility. Clinical agencies are required by federal law and HIPAA rules to ensure that each student signs confidentiality contracts before entering the clinical setting. Any breach of confidentiality on campus or in the clinical setting may be grounds for dismissal from the practical nursing program. Students must:

• Protect any client information that comes to them

• Refrain from leaving any information in client rooms, on desks, in classrooms, in the cafeteria, or out for any public viewing

• Avoid taking anything with a client’s name affixed in any manner off the unit

• Abstain from identifying clients, nursing staff, doctors, or other persons by name in care plans, notes, or other exercises for learning purposes. Use initials if identifying information is needed

• Avoid discussing clients, staff or care issues in public (i.e. with friends or family in public places such as cafeterias, elevators, lounges, bars, restaurants, etc.)

• Never discuss client problems with other clients or patients’ family members

• Never discuss student’s own personal problems with clients

• Cell phones must be on silent or vibrate and not to be used in the patient area.

• Do not witness legal documents during your role as a student nurse

• Do not take any pictures and/or post anything to social media

Violation of Policies/Procedures:

Florida State College at Jacksonville is dedicated to maintaining an environment which fosters student success. The College endeavors to protect the rights of students and also expects all students to act responsibly. Attendance at the College is a privilege, and in order to maintain the College ideals of scholarship, character, and personality, the right is reserved by the College to establish rules and regulations to foster these ideals and protect the interest of the College. Each student is subject to federal and state laws, respective county and municipal ordinances, and all rules and regulations of the College. Violation of the policies/procedures of our clinical partners and/or the FLORIDA STATE COLLEGE at JACKSONVILLE Practical Nursing Program including, but not limited to parking, facility access, cell phone use (talking, texting, photographing, etc.), professional appearance, professional demeanor and conduct, may result in removal from the clinical facility by the facility and/or the College. Should such removal occur, conference with the student, full time faculty member responsible for that course and the Associate Dean of Nursing will be required to determine whether or not a recommendation will be made to the North Campus President that the student not be allowed to proceed in the Practical Nursing program. Student Injuries/Incidents: (Refer to Appendix F)

Students who sustain any injury/incident during their program of study must notify their course instructor and the Associate Dean of Nursing. Both the student and the instructor must complete a “Student Notification of Injury Form” (Appendix F). The original copy of the form goes to Campus Security; a copy goes to the student; a copy goes to the instructor; a copy goes to the Associate Dean of Nursing, and a copy gets faxed to FSCJ Risk Management Office in the Martin Center for College Services.

LIBRARY AND LEARNING COMMONS

The college’s Library and Learning Commons (LLC) are committed to providing access to high-quality, relevant library resources and services and tutoring support. The North Campus Library, because of its unique health programs, is very strong in the health area. Comparable collections and services are also available at the Deerwood Center and Nassau Center. Tutors are available college-wide for: English, Math, Science, computers, and foreign language. North Campus LLC also offers nursing tutors. Many online resources, including professional nursing journal databases and e-textbooks, are available to students at all locations. Interlibrary loans are also available. Pregnancy: (Refer to Appendix H)

Pregnancy is not an illness and does not necessitate withdrawal from the Practical Nursing program. Students must:

Formally notify the Practical Nursing department of the pregnancy in writing.This notification is to ensure that the Practical Nursing program expectations are not detrimental to the health of the pregnant student or her unborn child. For form and instructions

Provide a note from the physician/nurse practitioner/midwife stating that the student may participate in all activities of a student nurse without any moving/lifting/standing /sitting restrictions. This will be placed in the student’s file in the Nursing Office. Notify her classroom and clinical instructors.

Illness:

1. Students who have been hospitalized and are returning to class and clinical experiences are required to bring a doctor’s statement verifying clearance to resume all school activities.

2. Students who have been absent from clinical experiences due to injury or illness (INCLUDING HOSPITALIZATIONS) must provide a doctor’s written statement verifying clearance to return to the clinical experience. The form should state that “student is able to move, stand, sit, lift, drive with no limitations to required class or clinical performance requirements”.

3. Any student reporting for a clinical experience who appears ill or otherwise

compromised may be sent home at the discretion of the clinical instructor.

POTENTIAL INFECTIOUS AND ENVIRONMENTAL

HAZARDS IN NURSING PRACTICE

It is the responsibility of each nursing student to adhere to hospital policies and the guidelines on “universal standards/precautions” of the Centers for Disease Control (CDC) and Prevention (United States Public Health Service) during clinical experiences.

HEALTH REQUIREMENTS:

Students are responsible for complying with the health requirements of the College, Department of Nursing and clinical agencies. Students are required to sign a statement indicating that they have read and understood the policies and procedures related to student health (Refer to the Health Clearance Policy and Procedure Sign-off in Appendix D). Failure to comply may result in administrative withdrawal. A physical examination by a licensed physician, Physician’s Assistant or Nurse Practitioner in the State of Florida must be completed and submitted on the Physical Examination Form (See Appendix D) by the first day of class.

IMMUNIZATIONS AND TB TESTING: (Refer to Appendix D)

1. All immunizations, testing, and titers should be completed prior to admission to the program and results submitted to the Nursing Department by the first day of class or designated date.

2. Annual Flu shots are required. If you choose not to receive an annual

immunization then you are required to wear a “Mask” at all clinical experiences. Students are advised not to receive the nasal flu vaccine (Flu Mist). Since this vaccine is a live, attenuated virus, it can be passed on to contacts because of the shedding from the nares as it replicates. If there are extenuating circumstances, you must provide documentation from your medical doctor and meet with the Associate Dean of Nursing All immunizations, testing, and titers must be completed and lab results submitted prior to the first day of class. The only exception is being in the process of completing an immunization series. In this case, the student must provide a copy of the office slip generated at the time each injection in the series is given as proof of a “good faith” effort to complete the series. In all cases, a student may not attend the clinical component of a course without all requirements being satisfied. This is not only Nursing program policy but a mandate from the clinical agencies.

must be submitted prior to the first day of class. If either titer is negative, the

student must receive immunizations (another booster) and a titer (blood work to prove student has a positive titer). drawn at the appropriate time. Lab slip showing result of the positive titer must be submitted prior to the first day of class. In the event that titers are not documented, the student will be exited from the program by the Associate Dean of Nursing Hepatitis B series (3 injections) is strongly suggested (See Section on Blood Borne Exposure) and a positive titer is desirable. However, students may elect to sign a Hepatitis B declination form. In all cases, the CDC recommends all health care personnel who may come in contact with blood products (eg. needles) to complete the Hepatitis B series and seek a positive titer. In the event that the Hepatitis series was completed more than 5 years ago, a titer to show immunity must be submitted.

3. A Mantoux or PPD tuberculin skin test is required prior to the first day of class

and every year thereafter. If the result was positive, a chest x-ray is required every 5 years. This is a professional expectation and failure to keep this up to date will result in being removed from the program.

BLOOD AND BODY FLUIDS EXPOSURE:

A student who is exposed to blood or other body fluids directly or indirectly while in the clinical area will be advised by the instructor to follow the CDC guidelines for exposure, as well as those of the assigned clinical agency. Treatment of an Exposure Site (from CDC Guidelines). Wounds and skin sites that have been in contact with blood or body fluids should be washed with soap and water; mucous membranes should be flushed with water. No evidence exists that using antiseptics for wound care or expressing fluid by squeezing the wound further reduces the risk of blood borne pathogen transmission; however, the use of antiseptics is not contraindicated.

EXIT INTERVIEWS: (Refer to Appendix E)

Each student exiting a course for any reason must have an exit interview with the instructor. This is required in order to be considered for reinstatement to the Practical Nursing program. Exit Prescriptions are required for re-instatement in the Practical Nursing program. Eligible students are those who: (1) do not achieve a passing grade or (2) withdraw from a course or (3) withdraw from the program. Exit Prescriptions provide written documentation of the reason for withdrawal and state exactly what remediation (e.g. test-taking program, stress management video viewing, review course objectives, practice certain skills, etc.) the student needs to do to be considered for reinstatement. The goal of the prescription is to assist the student’s efforts to be successful on re-admission. (See Appendix E for details)

It is in the student’s best interest to complete these prescriptions ASAP since re-instatement status depends on the date the completed prescription was submitted to the Associate Dean of Nursing and should be completed within 1 year of exit.

REINSTATEMENTS: (Refer to Appendix E)

A student may take a course in the Practical Nursing Program a maximum of two times. After each unsuccessful attempt the student will be exited from the program and provided with a prescription from the exiting instructor. The prescription must be successfully completed by the student and graded by the exiting instructor or Associate Dean of Nursing. A reinstatement application must also be submitted and a seat must be available for reinstatement to be considered. After the second unsuccessful attempt to successfully complete any portion of the program the student will be denied re-entry into the practical nursing program and referred to the campus career counselors to consider another career. All reinstatements must take and pass the A2 PN entrance exam with a minimum score of 75% in all subtests.

STUDENT ACADEMIC DISHONESTY:

ACADEMIC DISHONESTY IS CONSIDERED A SERIOUS OFFENSE AND IS EXPRESSLY PROHIBITED. THIS POLICY INCLUDES ALL COURSE ASSESSMENTS, RELATED ASSIGNMENTS, AND ALL STANDARDIZED TESTS (E.G. PERT, CLAST, CAT). SUCH DISHONESTY INCLUDES:

1. Any act or behavior on the part of the student which tends to interfere with or

otherwise disturb the orderly conduct, processes, functions and/or interests of the class is expressly prohibited. Such acts or behaviors include but are not limited to academic dishonesty.

2. Academic dishonesty, in any form, is expressly prohibited by the rules of the District

Board of Trustees of FLORIDA STATE COLLEGE at JACKSONVILLE. 3. As used herein, academic dishonesty incorporates the following:

A. Cheating 1. The giving or taking of any information or material with the intent of

wrongfully aiding oneself, or another in academic work considered in the determination of a course grade or the outcome of a standardized assessment.

2. Failure to report cheating when he/she observes cheating as defined above

or has reasonable knowledge that a quiz, test, or examination has been compromised.

B. Plagiarism

1. The act of stealing or passing off as one’s own work the words, ideas, or conclusions of another as if the work submitted were the product of one’s own thinking rather than an idea or product derived from another source.

2. Any other form of inappropriate behavior which may include but is not limited to falsifying records or data, lying, unauthorized copying, tampering, abusing or otherwise unethically using computer or other stored information; and any other act or misconduct which may reasonably be deemed to be a part of this heading.

4. IF A STUDENT IS DETERMINED BY A PROFESSOR TO BE LYING, STEALING

OR CHEATING, THE STUDENT MAY BE RECOMMENDED FOR DISMISSAL FROM THE PRACTICAL NURSING PROGRAM PERMANENTLY.

DUE PROCESS:

Any student alleged to have committed any act of academic dishonesty as defined herein, shall be entitled to due process as defined in District Board of Trustees’ Rule 6Hx7-2.18, prior to the administration of disciplinary action including suspension and dismissal. PLEASE REFER TO THE COLLEGE CATALOG STATEMENT REGARDING DUE

PROCESS PROCEDURES.

In the Practical Nursing Program, if a faculty member/test proctor suspect’s behavior that may represent academic dishonesty, the student(s) involved will:

1. have the tests/ assignments confiscated 2. be escorted out of the classroom 3. be required to meet with the faculty-proctor, and the Instructional

Associate Dean of Nursing and Dean of Nursing prior to re-entry into that course.

Lying, stealing or cheating may subject a student to a recommendation for permanent dismissal from the Florida State College at Jacksonville PN program. These policies span the duration of the program, and students are responsible for reviewing these policies at the beginning of each term. Students will be required to read and “sign-off” on additional requirements specific to each course. These policies are in effect for the duration of the program unless modified by the Department of Nursing. Students are responsible for reviewing these policies at the beginning of each term. Non-compliance with the policies as presented may result in an “F” for a final grade.

APPENDIX A

Practical Nursing Program

PROFESSIONAL GUIDELINES

PROFESSIONAL BEHAVIOR AND GUIDELINES

Students in the Practical Nursing Program will comply with the academic regulations and requirements as stated in the catalog and program policies of the FLORIDA STATE COLLEGE at JACKSONVILLE. Whenever a student is assigned to a clinical activity, whether in an acute care facility, long-term care facility, community agency, or outpatient facility, the student is representing FLORIDA STATE COLLEGE at JACKSONVILLE, the Nursing Department, its faculty, and its student body. The student is expected to behave with professional demeanor and adhere to the dress code at all times.

DRESS CODE

The dress code is designed to provide guidelines on appropriate dress for the professional nurse. Students are expected to arrive for clinical activities on time and in the appropriate attire. Students not adhering to the guidelines will not be permitted to remain in the clinical setting. If a clinical agency has a dress code that is more restrictive than the following, the stricter dress code will apply. 1. FLORIDA STATE COLLEGE at JACKSONVILLE nametag and photo ID

FLORIDA STATE COLLEGE at JACKSONVILLE nametag and photo ID must be clearly visible at all times during clinical activities. If a lab coat or sweater is worn, the nametag must be attached to the lab coat or sweater. The nametag is to be worn on the collar or lapel of the uniform or lab coat.

2. Uniforms

School uniforms only. Must be consistent with the program policy. Students should maintain clean (stain-free and pressed) and professional attire.

3. Jewelry Watch: A watch that notes minutes and seconds is required. Earrings: One small stud in the ear lobe is permitted (no hoops, dangling earring or other facial jewelry is permitted including the tongue). Rings: Only a wedding band with no stone is permitted. Ornate jewelry is not

allowed.

4. Hygiene Students must maintain good personal hygiene.

5. Hair Hair must be neatly contained, of a natural color, and appropriate for safe client care. Hair longer than shoulder length must be pulled back and neatly contained to keep out of face and off the collar. Hair should not be unruly or unkempt.

6. Nails

Nails must be well-groomed and an appropriate length not visible from the palmar surface of the hand to perform safe nursing care. Polish, if worn, should be in good repair and of a neutral tone. Artificial nails, overlays, or wraps are not permitted by the Centers for Disease Control.

7. Stockings

Socks or stockings must be white with the uniform; flesh tones or match slack color if not wearing uniform.

8. Shoes

In clinical facilities, students must wear shoes that are made of leather/leather-like (white) material. All shoes must be kept clean at all times. White shoes must also be worn with the community service uniform. No heels/clogs/open-toed shoes are allowed.

9. Make-Up Clean, natural look. No excessive make-up. No fragrance (perfume or aftershave) may be used.

10. The following are not permitted for any clinical activity: Open toe or high heeled shoes; sweats, jeans, shorts, T-shirts, halter tops, crop tops, or tank tops (the midriff should not be exposed), ill-fitting clothing, hip hugger or low slacks.

11. Lab Coats/Sweaters When students are required to go into a clinical facility to obtain assignments and gather information to prepare for a clinical laboratory experience, the school’s uniform or professional attire with a white lab coat and appropriate identification must be worn. During clinical experiences, lab coats should be worn in accordance with the policies of the assigned clinical agency. Lab coats may not be worn while giving patient care. Navy blue or white sweaters may be worn with the uniform. Sweaters may not be worn while giving patient care.

12. Students participating in off-site community service activities as a requirement of

their courses will wear the green polo shirt with the college logo, khaki pants or trousers and shoes that are made of leather/leather-like (white) material. All shoes must be kept clean at all times.

PROFESSIONALISM

1. Orientation to Clinical Agencies

Orientation to clinical agencies is mandatory. Students who fail to attend orientation sessions and provide course instructors with documentation of attendance and completion of in-services and other requirements will not be given clinical assignments. This could result in a clinical absence or clinical failure.

2. Clinical preparation Clinical experiences are designed to meet specific learning objectives. The student should be aware of these objectives and be prepared to fully participate in all activities. This includes the completion of assigned readings and/or written work. Failure to comply will result in the student being sent home and incurring a clinical absence. See course syllabi for specific policies.

3. Punctuality The student is expected to be punctual for all clinical-related experiences. If the student is going to be tardy or absent, the instructor must be contacted no later than one hour prior to the beginning of clinical time.

The student should come equipped for the clinical activity. Only bring books,

worksheets, and equipment needed for that day’s experience. Purses and other valuables should be left at home or in the car.

Students may not work on other assignments during the clinical experience. Students are not to use agency computers or copy machines for personal or other school

work. Gum chewing and tobacco use are not permitted in any clinical experience.

Florida State College at Jacksonville is a “Smoke Free” Institution Pagers and cellular phones are not permitted in clinical areas. They should be left at

home or in the car. Students are expected to remain in the clinical facility for the duration of the experience.

If the student desires to leave the clinical area for any reason, the instructor’s approval must be obtained and the instructor must be informed before leaving and upon return.

While in a clinical facility, students must remain on the unit assigned. Students must maintain confidentiality of client data. Use only initials on papers referring

to clients.

4. Illnesses and Injuries Students are not permitted to participate in clinical activities if they are ill or injured. At the discretion of the faculty member, students with infectious processes or injuries requiring bandages or prosthetic or assistive devices will be evaluated for continuance in the program. Clearance from a physician must be supplied if requested.

APPENDIX B

FLORIDA STATE COLLEGE at JACKSONVILLE

Practical Nursing Program

ARREST RECORD CLEARANCE

Introduction The purpose of this procedure is for students to obtain clearance from the State Board of Nursing in order to take the NCLEX-PN. If this clearance is not obtained, you will be denied a seat to take the NCLEX (even if you have graduated from a nursing program at FLORIDA STATE COLLEGE at JACKSONVILLE). Caveat: Students should obtain clearance in the term in which they are admitted.

Students who do not obtain this timely clearance will be “wait” listed and processed after all new student and graduating student business is completed.

Process Prepare a folder labeled with your name and address, program, term in which you are presently enrolled, and telephone number(s) (home and/or cell). Once you have completed I and II (below), all the documentation should be placed in this folder. The folder should then be brought to the Associate Dean of Nursing and Dean of Nursing. Keep a copy of all documents you submit. Your material will be processed by the Associate Dean of Nursing and Dean of Nursing and you will receive a call when it is ready for you to pick up, upon completion of the program. Checklist Once I, II, III, IV on your checklist below are complete, you will have all of the required materials to bring to the Associate Dean of Nursing and Dean of Nursing for processing and to send to the Florida State Board of Nursing. I. Gather the following information and place in a folder labeled with your name and

address; program and term; and telephone numbers (cell and home).

APPENDIX C

FLORIDA STATE COLLEGE at JACKSONVILLE

Practical Nursing Program

SUBSTANCE USE/ABUSE POLICY

FLORIDA STATE COLLEGE at JACKSONVILLE is committed to maintaining high standards of nursing education and practice. Safe practice includes unimpaired performance in the clinical setting and, as such, students are expected to perform nursing activities without the use of any chemical substance that could alter or impair thinking. These chemical substances may be alcohol or illicitly obtained drugs.

If unsafe behavior is documented, the following process will be implemented:

It is the responsibility of nursing faculty to observe student behaviors which could threaten the safety and well-being of patients, other students, or faculty. If a faculty member or a licensed or certified professional at the clinical facility where the student is assigned has reasonable suspicion that a student is impaired, the observation will be documented and corroborated:

1. The student will be relieved of patient care responsibilities and be given an opportunity to

discuss the behavior with the person(s) who witnessed the behavior.

2. The faculty member will inform the student that a college-financed physical and/or mental examination may be required.

3. The faculty member shall advise and consult with the Associate Dean of Nursing and Dean

of Nursing or the designated alternate by telephone about the situation. 4. The Associate Dean of Nursing and Dean of Nursing (or the designated alternate) may

schedule an appointment for the student with the independent medical facility used by The College to have a physical and/or mental examination within six hours of the student’s removal from the clinical setting.

5. A signed, informed consent for release of information regarding results of the examination(s) to Florida State College at Jacksonville will be obtained from the student.

6. The faculty member shall assist the student to arrange for transportation home or to the independent medical facility, as appropriate. The student will be advised not to operate a vehicle.

7. If substance use is confirmed through the physical and/or mental examination, the student will be recommended for suspension and/or such other disciplinary action that may be appropriate. Such other action may include referral to the Intervention Project for Nurses (IPN) for evaluation and the institution of appropriate therapeutic measures. The cost of such therapeutic measures will be borne by the student.

8. Students who are accepted by the Intervention Project for Nurses must immediately advise

the Associate Dean of Nursing and Dean of Nursing of their participation in this program

and comply with all IPN rules and regulations. Students who fail to comply with these requirements may be recommended for permanent dismissal from the Nursing Program.

Practical Nursing Program

EXIT INTERVIEW AND REINSTATEMENT GUIDELINES

REQUIREMENTS:

1. The student who does not achieve a passing grade or who withdraws for any reason may

be considered for reinstatement according to the PN program criteria for reinstatement.

2. The student who exits or withdraws from a course may be reinstated to that course one time only, within one calendar year.

A. The student who withdraws from a course for reasons other than academic failure must submit documentation of resolution of the problem with letter of request for reinstatement.

B. A student who withdraws for personal reasons must be passing the course he or she is currently enrolled in. Passing is defined as maintaining a minimum average of 80% for the class work and a minimum of 80% for the clinical portion. (Note: the 80% must be obtained without any rounding of unit examinations.) Failure to maintain the 80% average will be considered a withdrawal for academic jeopardy/failure. Failure in course theory or in clinical practice will require that the student repeat both the theoretical and clinical components for that course, if reinstated.

C. The student who fails a course or withdraws for academic jeopardy/failure the first time will:

have an exit interview with the instructor who will provide a prescription for the student to complete prior to reinstatement. This exit interview is a requirement in order to be considered for reinstatement at a later date.

1. provide written documentation concerning the reason for withdrawal at the time of the exit interview. Provide a written statement pertaining to a plan for resolving the problem that made withdrawal necessary.

2. complete the prescription prior to application for reinstatement.

3. submit written request for reinstatement to the Instructional Associate Dean of Nursing and Dean of Nursing with written documentation that the prescription has been met.

D. Failure in course theory or in clinical practice will require that the student repeat both the theoretical and clinical components for that course, if reinstated.

REINSTATEMENT CRITERIA:

Reinstatements to the Practical Nursing program are based on the following criteria:

1. Satisfactory compliance with all of the requirements listed above as well as current admission requirements.

2. Availability of space in the course for which reinstatement is requested.

3. County of residence (counties served by FLORIDA STATE COLLEGE at JACKSONVILLE will have priority).

4. Previous performance in Practical Nursing courses.

5. Satisfactory demonstration of skill proficiency, if requested by the faculty or Instructional Associate Dean of Nursing and Dean of Nursing Ability to meet all of the Practical Nursing Programs’ technical requirements.

6. Recommendation of the faculty.

PRIORITY FOR REINSTATEMENT (Based on the following groupings)

Group 1 - Military Duty

Proof of all military leaves will be honored and given first priority.

Group 2 - Medical Leave

(Short term emergency medical leave, including pregnancy)

STUDENT ACCIDENT/INJURY PROCEDURES

If the injury appears life threatening, dial 9 then 911 and notify North Campus Security (766-6608/6609, 633-8200). If in the hospital, life-threatening injuries may be referred to the Emergency Department. The student should contact his/her primary insurance carrier to receive authorization for medical treatment. The student completes a Notification of Injury form. If the injury occurs on campus, Campus Security has forms. If the injury occurs at a clinical site, the instructor will have the form. Students with medical insurance choose where to receive treatment. Students without medical insurance who have non-emergent injuries will be contacted by FSCJ Risk Management following submission of the Notification of Injury form and notified where to seek treatment.
