RISK MANAGEMENT AND SAFETY POLICY SKYLINE STEEL ERECTORS

TO: All Managers, Managers and Employees RE: Company Policy As a member of our organization, you automatically accept a moral obligation to your fellow employees and an economic obligation to Skyline Steel Erectors to see that operations under your care, custody and control are carried out in an efficient and safe manner. Along with other responsibilities, safety consciousness must always exist in your thinking and planning. Because of this obligation, you must prevent obvious unsafe acts on the part of those you work with, and you must also anticipate potential hazards. After an accident occurs, it is too late to prevent it. All employees must recognize that working in an unsafe manner is counter‐productive. Most important, each employee is encouraged to demonstrate leadership ability by setting a good example. To make our approach to safety more effective and uniform throughout the organization, you are receiving this manual outlining and formalizing our Loss Prevention Program. We feel this will be a useful tool to help in understanding and discharging our mutual responsibilities. It is the objective of this company to conduct all operations as safely and efficiently as possible. To accomplish this, we are assigning the responsibility, authority and accountability for Loss Prevention to all management. Tyson Siebertz is the Loss Prevention / Safety Coordinator. It is his responsibility to administer a total Loss Prevention effort at all levels and to coordinate these efforts all involved ensuring that the Loss Prevention standards are met throughout the organization. MANAGEMENT RESPONSIBILITIES

Managers of Skyline Steel Erectors have the responsibility to enforce the Safety Program. To do this, safety must become a part of the job function along with the following responsibilities:

1. Familiarize themselves with the safety program, policies, procedures, and training and

ensure its effective implementation.

2. Be aware of all safety considerations when introducing a new process, procedure,

machine or material to the workplace.

3. Give maximum support to all safety and health programs.

4. Actively participate in safety goals and program betterment.

5. Consistently and fairly remind and enforce all company safety rules.

6. Provide complete safety training to employees prior to the assignment of duties.

7. Inspect work areas often to detect unsafe conditions and work practices.

8. See that all injuries, no matter how minor, are treated immediately and referred to the

managers and Safety coordinator(s) to ensure prompt and proper reporting

9. Review all accidents and near misses to ensure that proper reports are completed

10. Suggest and support measures taken to prevent accident reoccurrence.

11. Correct safety hazards in a timely manner.

12. Maintain a safe and healthful workplace.

EMPLOYEE RESPONSIBILITIES

Employee responsibilities for safety include the following:

1. Adhere to all safety rules and regulations.

2. Wear appropriate safety equipment as required.

4. Maintain equipment in good condition, with all safety guards in place when in

operation. Inspect prior to use.

COMMUNICATION

Skyline Steel Erectors encourages communication on safety and health topics by everyone in the organization. In order to achieve open lines of readily understandable communication, Skyline Steel Erectors does the following:

TRAINING

The Injury and Illness Prevention Program requires extensive training, including:

• Initial training for new hires, current employees and supervisory employees

• Changes in the IIPP Program

• Training for any new employee or supervisor with respect to the general hazards for the operation, those specific to their assignment and how these hazards are controlled.

• Training for employees and supervisors assigned to new responsibilities for which they have not received safety training.

• Training for any current employee or supervisor potentially affected by the identification of a new hazard introduced by a process or raw materials change or any previously unrecognized hazard.

During the training, employees must be advised of:

• Potential occupational hazards identified in their workplace generally and those specifically related to their job assignment.

• Means of minimizing potential hazards, including work conditions, safe work practices and personal protective equipment (the code of safe practices for the job safety class should effectively cover this information).

• Any new hazards introduced by a change in equipment processes, raw materials, etc.

Documentation of training must be specific and convincing to satisfy Cal/OSHA’s inspectors. The Safety Coordinator will support managers by supplying documents meeting the following minimum requirements: date of training, name of trainer, subject matter of training, training aids and information used, and attendees. An acknowledged training record (signed by employee) shows the employee actually participated. If non‐English‐speaking employees are involved, the documentation of training must address how these employees were trained so they understood the material.

INSPECTIONS

The Injury and Illness Prevention Program requires periodic scheduled inspections of all workplaces subject to the standard. It also requires inspections when the employer introduces new chemicals or changes its operations, or becomes aware of a new or previously unknown hazard. Some employers consider injury/illness investigations another form of inspection as well.

Inspections perform several roles in injury and illness prevention:

• To verify ongoing compliance with controls and safe practices designed to prevent previously identified occupational hazards.

• To identify potential hazards not previously recognized (when the program is first developed, when an occupational injury or illness occurs, and when new substances, processes, procedures, or equipment are introduced that present new occupational safety and health hazards or when the employer is made aware of a new or previously unrecognized hazard).

The standard also amplifies the statutory requirements for abatement of hazards identified during inspection. Any dangerous conditions must be acted upon immediately.

• Imminent danger. If there is an immediate danger of serious harm, the inspection should result in an immediate correction of the problem, or taking the piece of equipment or workstation out of service. This must be done in an obvious manner including physical tag‐out and lockout with full knowledge of the area employees, supervisors and managers.

• Less serious or immediate hazards. Correction is required in a timely manner consistent with the severity of the hazard. Although the standard is more flexible regarding less serious hazards, any problem that can be corrected immediately should be.

Walk through safety inspections to identify hazards, unsafe work conditions, employee actions are completed, documented and logged site specific binders by the safety coordinator. Other inspections that are conducted may include fire extinguishers, air compressors, propane dispenser/tank and equipment and crane inspections.

ALL MANAGEMENT SAFETY MEETING

Management and the Safety Coordinator will meet on a quarterly basis to review the following:

1) Roll Call.

2) Minutes of the previous meeting

3) Unfinished business of the previous meeting.

4) Monthly inspection reports.

5) Discussion of accidents and corrective action taken.

6) Accident trends.

7) New and outstanding recommendations submitted by outside agencies (insurance

carrier, fire department Cal/OSHA, etc.)

8) New business.

All meetings will be documented.

SAFETY MEETINGS

Safety meetings are to be held weekly or when an individual is new to a jobsite. Meetings can be held all together in some situations, or for each individual. The Safety Coordinator will present the information at each meeting. Role is taken at each meeting and employees who are absent from the meeting are still required to be given a meeting or talk. The Safety Coordinator will ensure that make ups are completed and that all meetings are documented. Documents are kept in the site specific binder. All documentation is kept for five years.

Additionally, Skyline Steel Erectors takes employee suggestions and input very seriously. Suggestions can be made verbally or submitted in writing (or anonymously) to the safety coordinator and without fear of reprisal. The safety coordinator is equipped with a phone to be able to communication with employees, management and other agencies.

SAFETY RULES All employees are responsible for their own safety, as well as that of others in the workplace. For the protection and safety of all employees, Skyline Steel Erectors has established the following rules designed to prevent accidents and injuries. Compliance with these rules is mandatory. Receipt of these rules by new employees will be documented. 1) Report to work in good physical condition. Notify your supervisor if your ability or

alertness is impaired or if you are suffering from illness or injury. 2) Use or possession of drugs and/or alcohol is prohibited. Possession/ under influence of

drugs and/or alcohol will result in immediate dismissal. 3) All employees are to have a clear understanding of his/her job duties. 4) All employees are to be informed of the company’s accident procedure, emergency

exits, first aid kit locations, fire extinguishers, name and numbers of important contacts and job postings. Always know the closest first aid kit, fire extinguisher, and emergency exit closest to where you are working.

5) Proper footwear and clothing will be worn at all times. 6) Do not wear loose clothing, jewelry or keep long hair hanging down where there is a

danger of catching such articles in moving machinery. 7) Use appropriate gloves when handling materials with sharp or jagged edges, which may

result in lacerations. 8) Eye protection is required when performing any task that could produce flying particles

or when entering an area where welding and grinding are ongoing. 9) Horseplay, running, fighting or any activity that may result in injury or waste will not be

tolerated. 10) Operate machinery with all guards in place. Tampering with safety devices is cause for

immediate disciplinary action. 11) Do not operate any machine with which you are not familiar. 12) Machines must never be cleaned, adjusted or repaired until after the machine is turned

off, the circuit is broken at the power source (including lock‐out) and a lock and warning tag is placed at the controls. Each person involved in maintenance must have his/her own personal padlock to ensure total lack of power until all work has been completed.

13) Any defects in materials, machinery, tools and equipment must be reported immediately to a supervisor.

14) Do not leave tools, materials or other objects on the floor, which might cause other to trip and fall – practice safe housekeeping.

15) Do not block exits, fire doors, aisles, fire extinguishers, gas meters, electrical panels or traffic lanes.

16) Materials, tools, or other objects shall not be thrown from buildings or structures to prevent others from falling objects. Secure tools and materials when working over others or aisle ways.

17) Avoid risk of rupture, internal injury or back injury in attempting to lift or push excessive loads. If an object is too heavy to move without strain, ask for help.

18) Observe the correct position for lifting. Stand with your feet slightly apart, assume a squatting position with knees bent and tuck your chin. Tilt head forward, grasp the load with both hands and gradually push up with your legs, keeping your back straight and avoiding any abrupt movement.

19) Do not distract others while working. When approaching a machine operator for any purpose, do so from the front or the side in a way that he or she will see you coming and will not be shocked or surprised. If conversation is necessary, first make sure the machine is turned off.

20) Do not allow oil, wax, water or any other materials to remain on the floor where you or others may slip. Report any spills to your supervisor.

21) Keep a solid footing at all times. Pay special attention to freshly oiled wood or steel forms.

22) Be attentive near floor openings and edges of slab. Remove any loose material near them. Immediately replace any guardrails removed to land material. Do not access an unsafe work area.

23) When handling hazardous materials insure you follow prescribed safety procedures and use required safety equipment. When using secondary containers filled by others, insure that they are labeled as to their contents and hazards.

24) Do not attempt to operate machinery for which you are not trained. 25) Unnecessary and excessive haste is the cause of many accidents. Exercise caution at all

times. Walk, do not run. 26) The use of hot production equipment or materials for the purpose of cooking or heating

food is strictly prohibited. 27) All work related injuries and accidents must be reported immediately to your lead or

manager. 28) All tools and equipment shall be maintained in good condition. 29) Be aware of impalement hazards. Bend down protruding nails and wire ends. Do not

work over unprotected impalement hazards. Hazards must be protected with approved troughs and protective covers.

30) Only appropriate tools shall be used for the job. 31) Any damage to scaffolds, false work, or other supporting structures shall be immediately

reported to the foreman and repaired before use.

32) Gasoline shall not be used for cleaning purposes. Additionally, when transferring or refueling equipment engines must be stopped. No smoking or open flame is allowed within 25ft or operation. Keep containers closed when not in use.

33) Scaffolding shall be inspected prior to use by a competent person. All platforms or scaffolds shall be secured with proper planking and handrails.

34) Riders are not allowed on forklifts, crane hooks, or loads. Additionally, scaffolds are never to be moved with a person aboard.

35) Only approved positioning belts, body harnesses and lanyards with double locking snap hooks shall be used. Do not use leather belts for positioning, as they will rot and become weak with age.

36) Do not enter into or work in an area without sufficient lighting. Be assured that the area is free of hazards such as items that may cause you to trip and stumble, unsafe ladders, scaffolding equipment, partially framed walls and pits, holes, trenches, etc. Do not enter a confined space without the required training, precautions, and authorization.

37) Employees working around heavy equipment, or working on bridges or roadways, must wear orange or highly visible colored clothing or vests to increase their visibility. Always know where heavy equipment is when working and make sure the operator knows your position before starting work.

38) Never enter a trench or excavation five (5) feet or deeper unless properly sloped or shored. Ladders are required in trenches over four (4) feet deep and are extended from the floor of the excavation to at least three (3) feet above the top of the excavation. Ladders are to be located so as to provide means of exit without more than twenty‐five (25) feet of lateral travel.

39) Make sure all back‐up alarms, horns, lights, hook safety latches, dead‐man, triggers, and similar safety devices are operative at all times.

40) Alert machine operator when workers are in trenches to avoid backfilling on workers. 41) Do not crawl on, under, or squeeze by equipment or machinery. Walk around. 42) Work shall be so arranged that employees are able to face ladder and use both hands

while climbing

It is imperative that all employees become thoroughly familiar with the above safety rules. Failure to comply with safety rules or procedures, or failure to wear the appropriate safety equipment will result in disciplinary action up to and including termination.

DISCIPLINARY PROCEDURES

Skyline Steel Erectors strives to make safety a number one priority every day therefore management and the safety coordinator are responsible for ensuring the safety policies and procedures in place are understood and practiced by all employees. Employees who fail to comply with safety rules and/or code of safe practices will be subject to disciplinary action up to and including termination. Supervisors will follow the normal disciplinary procedures as follows:

1. Verbal counseling ‐‐ the first step. Must be documented in the employee’s personnel file.

2. Written warning ‐‐ outlining nature of offense and necessary corrective action. 3. Suspension without pay ‐‐ the third step or a separate disciplinary action resulting from

a serious violation. 4. Termination ‐‐ if an employee is to be terminated, specific and documented

communication between the manager and the employee, as outlined, must have occurred.

Managers/Lead men will be subject to disciplinary action for the following reasons:

1. Repeated safety rule violation by employees under their supervision. 2. Failure to provide adequate training prior to job assignment. 3. Failure to report accidents and provide medical attention to employees injured at work. 4. Failure to control unsafe conditions or work practices. 5. Failure to maintain good housekeeping standards and cleanliness.

Supervisors who fail to maintain high standards of safety will be demoted or terminated after three documented warnings have been levied during any calendar year.

HEAT INJURY AND ILLNESS PREVENTION

In accordance with Title 8 of the California Code of Regulations, Section 3395, every employer shall have a Heat Illness Prevention Program in place where employees are working outdoors. Heat Illness is a serious medical condition resulting from the body's inability to cope with a particular heat load, and includes heat cramps, heat exhaustion, heat syncope and heat stroke. Skyline Steel Erectors provides training, written procedures and documentation for the prevention of heat illness. While on jobsites, it is the responsibility of Skyline Steel Erectors to provide access to shade and water, first aid and emergency response. Training Each Skyline Steel Erectors employee will receive our Heat Injury and Illness Prevention Procedures for Outdoor Workers and Management Training handout, which they will be required to read, and then sign, acknowledging that they have read, and understand, the procedures. Additionally, supervisors will review heat illness prevention and procedures with employees in periodic safety meetings. The Safety coordinator(s) and supervisor shall stay informed of the weather for extreme heat days and humidity, and provide additional review to employees during these times. In addition to the Heat Injury and Illness Prevention Procedures for Outdoor Workers and Management Training handout, supervisors will also be required to review and acknowledge the Heat Injury and Illness Prevention Procedures for Outdoor Workers Additional Supervisory Training handout. Supervisors will also receive training verbally from the Skyline Steel Erectors Safety Coordinator(s), and will be re‐trained periodically to ensure compliance with changing laws and regulations. Access to Shade & Water Skyline Steel Erectors encourages employees to notify supervisors if they need to rest due to the heat. Employees are allowed access to shade at all times if experiencing heat illness. This may be achieved through canopies and air conditioned construction trailers for no less than five minutes at a time. Skyline Steel Erectors provides water and cups to the employees in the field at all times. Employees are encouraged to drink water continually throughout the day in order to stay hydrated. Water containers will be monitored throughout the day to ensure an adequate supply at all times.

HEAT INJURY AND ILLNESS PREVENTION PROCEDURES FOR OUTDOOR WORKERS

AND MANAGEMENT TRAINING

1. All employees and supervisors must be trained in Heat Illness Prevention prior to working outdoors.

2. Environmental risk factors attributed to heat illness include: temperature, humidity, radiant heat from the sun and other sources such as machinery, personal protective equipment and duration.

3. Personal risk factors that are attributed with heat illness include: age, weight, acclimatization, health, water consumption, alcohol consumption, medication use.

4. Acclimatization means the body’s adjustment to work in the heat peaks in most individuals within four and fourteen days of regular work for at least two hours per day in the heat. SSE understands that employees may need to build up tolerance to the heat over a period of time.

5. Come to work hydrated. Remember, alcohol and caffeine deplete water in your body. A good habit is to drink a large glass of water right when you wake up in the morning.

6. Be prepared to drink 2 gallons of water a day‐that’s 1 quart each hour! Bring a refillable container to work.

7. When you’re on break, you need to be drinking water. Drink even when you’re not thirsty.

8. If you’re urine is yellow and smells bad, you’re not getting anywhere near enough water to drink. DRINK UP!!

9. Take rest periods in the shade/air conditioning and notify supervisor if you need a break.

10. Look out for the warning signs of heat stroke: high body temperature (103 degrees), red‐hot and dry skin (no sweating), rapid‐strong pulse, throbbing headache, dizziness, nausea, confusion.

11. Warning signs of heat exhaustion, a milder form of heat stroke, are: heavy sweating, paleness, muscle cramps, tiredness, weakness, dizziness, headache, nausea or vomiting, fainting.

12. People at higher risk of heat illness are: overweight, sick and/or taking certain medications, children up to 4 years of age, or adults age 65 or older. If you are at risk or care for someone who is, take extra special care to stay cool and hydrated.

13. If you notice signs or symptoms of heat illness in yourself or a co‐worker, notify your supervisor IMMEDIATELY.

14. Skyline Steel Erectors encourages employees to use a “buddy system” to monitor for signs of heat illness. Partner up with a co‐worker to monitor each other.

15. IF YOU SUSPECT HEAT ILLNESS: Get victim to shady area and take measures to cool the victim rapidly: sit in air conditioned room, wrap in cool/wet sheet, submerge in cool water or take a cool shower, drink water.

16. If condition doesn’t improve, seek medical attention as soon as possible. See emergency/accident information and fill out appropriate documentation. Location and contact information for the nearest medical facility will be located in Section 4 of the Site Specific Project Binder, which the project foreman maintains at every job site. Call 911 for emergency medical service.

17. Clear and precise directions to the job site are provided in the Site Specific Project Binder. The project foreman will review these directions with all employees to ensure that all employees can give directions to emergency response personnel if necessary.

HEAT INJURY AND ILLNESS PREVENTION PROCEDURES FOR OUTDOOR WORKERS

ADDITIONAL SUPERVISORY TRAINING

1. All supervisors will receive heat illness prevention training prior to being assigned to

supervise outdoor workers. In addition to being knowledgeable in heat illness prevention guidelines, supervisors will be trained in how to implement these guidelines and follow through with medical attention as needed.

2. It is the supervisor’s responsibility to ensure that adequate amounts of drinking water and cups are kept on site at all times, and that employees are informed where drinking water is located.

3. It is the supervisor’s responsibility to ensure that adequate areas of shade or air

conditioning are available to employees, and that employees are informed of these locations.

4. On hot days, or during a heat wave, supervisors will hold a tailgate meeting to review

heat illness prevention information.

5. Supervisors should frequently remind employees of the importance of drinking adequate amounts of water, up to one quart per hour on hot days, and encourage the consumption of water at regular intervals.

6. Supervisors will monitor weather conditions and forecasts, and be prepared to make

necessary adjustments to prevent heat illness.

7. The Site Specific Project Binder, Section 4, shall contain directions and contact information for the nearest medical facility to the job site. It shall also contain a page stating the locations of water and shade on the job site. It is the supervisor’s responsibility to review this information, to communicate this information to all employees, and to inform employees where this information can be found.

8. Supervisor shall ensure that all employees can give clear and precise directions to the job site, in case of an emergency.

9. Supervisors shall stay vigilant of employee whereabouts throughout the work shift, and

shall monitor them for signs and symptoms of heat illness.

10. When an employee exhibits symptoms of heat illness, move the employee to a shaded area immediately, and begin taking steps to cool the employee, as outlined in the Skyline Steel Erectors Heat Illness Prevention Procedures. If necessary, call emergency medical responders (911), and be prepared to give clear directions to the employee’s location.

11. Be prepared to fill out all accident reports and necessary paperwork, as required by law, and by Skyline Steel Erectors management.

12. In the event of an incident of heat illness, report the details as soon as possible to Skyline Steel Erectors management.

ACCIDENT INVESTIGATIONS AND REPORTS Skyline Steel Erectors Managers and Supervisors carry out a thorough program of accident investigation. Supervisory personnel will be primarily responsible for investigating all accidents in their areas of responsibility. Accidents involving fire, death, serious injury, or extensive property damage will be investigated jointly by the Supervisor, General Manager, and Loss Prevention Manager. The primary goal of the accident investigation program is the prevention of future similar accidents through the use of knowledge derived from the investigations. Additionally, the investigation will be used to prepare reports required by Federal and State law as well as the Worker’s Compensation Insurance Carrier. These reports are critical in establishing the Company’s and the Supervisor’s liability under the law. When an employee is injured at work, the supervisor is responsible for taking emergency action to have first aid administered, to obtain professional medical attention as soon as possible and protect other employees and equipment. Skyline Steel Erectors requires whenever an accident occurs the individual be sent to a drug screen/physical, this will be scheduled by the safety coordinator. Accident forms are to be completed by the injured employee, witnesses and supervisor. The supervisor must then begin to investigate the accident. The following procedures have been found to be effective when investigating accidents:

1. GO to the scene of the accident at once. 2. TALK with the injured person, if possible. Talk to witnesses. Stress getting the facts, not

placing blame or responsibility. Ask open‐ended questions. 3. LISTEN for clues in the conversations around you. Unsolicited comments often have

merit. 4. ENCOURAGE people to give their ideas for preventing a similar accident. 5. STUDY possible causes ‐‐ unsafe conditions, unsafe practices. 6. CONFER with management representatives about possible solutions. 7. WRITE your accident report giving a complete, accurate account of the accident. 8. FOLLOW‐UP to make sure conditions are corrected. If they cannot be corrected

immediately, report this to your supervisor. 9. PUBLICIZE corrective action taken so that all may benefit from the experience.

In order for the Supervisor’s Report to be effective it should contain, as a minimum, detailed answers to the following questions:

A. What Was the Employee Doing? Explain in detail the activity of the employee at the time of the accident.

B. What Happened? Indicate in detail what took place, describe the accident, the type of injury, the part or parts of the body affected and whether the employee was wearing appropriate safety equipment.

C. What Caused the Accident? Explain in detail the condition, act malfunction, etc., that caused the accident. Remember there will be more than one reason or cause for an accident.

D. What Can Be Done to Prevent a Similar Accident? Indicate corrective action to prevent recurrence.

The Supervisor’s Report, along with the Employee Report, must be submitted to the Loss Prevention Manager no later than 24 hours after the accident. Each supervisor must maintain an adequate supply of the Supervisor’s Report and the Employee’s Report forms, which may be obtained from the Loss Prevention or Personnel Manager.

RETURN TO WORK

Skyline Steel Erectors is committed to returning injured employees to modified or alternative work as soon after an injury as possible. Temporarily modifying the employee’s job or providing the employee with an alternative position will do this. The employee’s medical condition along with any limitations or restrictions given by the attending physician will be considered as a priority when identifying the modified/alternative position. PURPOSE This program is intended to provide our employees with an opportunity to continue as valuable members of our team while recovering from a work related injury. We want to minimize any adverse effects of an ongoing disability on our employees. This program is intended to promote speedy recoveries, while keeping the employees’ work patterns and income consistent. At the same time, we benefit from having our employees provide a service and contribute to the overall productivity of our business.

SCOPE This program applies to ALL employees of Skyline Steel Erectors RESPONSIBILITIES All injuries and the duration of the disability will be handled by the Supervisor/Manager and Safety Coordinator. The Safety Coordinator will act as a liaison between Skyline Steel Erectors, the injured worker, the attending physician and Employers Direct Insurance Company. The Safety Coordinator will make sure the appropriate paperwork and forms have been properly handled and submitted to the appropriate parties. The Supervisor/Manager will monitor the modified/alternative work and gather any additional information that may be needed to properly handle the return to work efforts. All Supervisors/ Managers In the event of an injury, the supervisor/manager will make sure that our employee receives first aid or proper medical evaluation at our selected network medical clinic. If possible, the supervisor/manager or safety coordinator will accompany the employee to the medical clinic. The attending physician shall be notified on the first visit that Skyline Steel Erectors has a return to work program and that modified/alternative work will be provided. The supervisor/manager will work closely with Safety Coordinator to coordinate the return to work efforts and will be responsible for introducing the employee back into the work place in the modified/alternative position. Supervisor/manager will make sure that the injured employee receives necessary assistance from co‐workers and that the employee does NOT work outside of his/her restrictions. Monitoring for transition into full duty work will be the supervisors/managers responsibility. Employees If an injury occurs on the job, the employee is required to report it to their supervisor/manager immediately. If the injury requires more attention than first aid, the employee must proceed to our selected network provider, Enloe Occupational Health for occupational injury. If available,

an employer representative will accompany the employee to the medical clinic. Together with the physician, the employee’s physical restrictions and limitations shall be discussed. All employees are expected to return to the worksite if medically feasible the very SAME day to report the physician’s findings and to discuss modified or alternative work. This will enable all parties to be kept abreast of the employee’s condition. Employees that have an injury shall report to the worksite if medically feasible after each visit to discuss his/her recovery. If not medically feasible they are to call in. Once an employee has returned to work, it is his/her responsibility to work within the physical limitations that the physician has given. The employee shall perform only those duties that are assigned to him/her. An employee shall immediately notify his/her supervisor of any difficulty in performing the duties. The employee must also notify his/her supervisor in advance of any medical appointments. The employee is to consider their work schedule when making their Doctor appointments. The employee shall keep his/her supervisor/manager informed of the recovery process and the ability to perform modified/alternative work.

NEAR MISS – NO INJURY Accident Procedure

All Employees:

• Assess situation—correct hazard if you can do so safely. • Report incident to your supervisor or tell someone to go get your supervisor. • Clear area of personnel to preserve accident scene • Be available to discuss details with supervisor and/or safety coordinator • Don’t discuss accident unnecessarily among uninvolved employees and don’t recreate

what happened—you might not be so lucky! Supervisor:

• All of the above as appropriate, and: • Red tag or lockout damaged or malfunctioning equipment • Listen to accounts of incidents to develop understanding of cause • Determine action needed to prevent reoccurrence • Complete Hazard ID & Correction Log and/or Safety Maintenance Form (IIPP‐Hazard ID

& Correction Tab, Safety Maintenance) and make a copy for Safety Coordinator

Safety Coordinator:

• Review facts with involved employee(s) and reporting supervisor • Develop an understanding of all factors to determine appropriate correction /

modifications were made to prevent reoccurrence • Schedule training if needed or administer disciplinary action • Review documentation to ensure completion or assist lead in completing appropriate

documentation • Provide copy of completed forms to log in master safety binder.

REPORTING A DEATH OR SERIOUS INJURY

The Safety Coordinator will comply with California regulations which require an employer submit a report to the nearest Cal/OSHA district office if any of the following occurs:

• An employee is seriously injured on the job or in connection with the job. • An employee suffers a serious job‐related illness. • An employee dies on the job or in connection with it

The employer must make the report as soon as practically possible, but no longer than 8 hours after the employer knows or, with diligent inquiry, could have known, of the death, serious illness or injury. The following information must be included in the report:

• Name of business • Address • Type of business • Location of accident • Name of injured • Age • Occupation • Injury • Insurance carrier

EMERGENCY ACTION PLAN Major disasters must be anticipated and procedures must be developed and mastered if the well‐being of our employees is to be protected and if we are ready to serve our community. The following pages detail the organizational structure of our plan outlines for emergency measures to be taken in the event of fire or other emergency. Remember, your conduct and actions during the first few minutes of any emergency may not only save your life, but the lives of your coworkers and other members of the community as well. GENERAL INFORMATION Two important telephone calls need to be made if the facility is to be evacuated for any of the following reasons:

• A fire or disaster within the facility. • An external hazardous condition threatening the facility.

If either of these two situations occur, notify these agencies:

• Fire Department: 911 • Oroville Police Department: 530‐538‐2444

Upon order of management or other person(s) in charge to totally evacuate the facility, the following actions will be taken:

1. Initiate evacuation center receiving plan. (It may be necessary to transport company personnel to a local evacuation center).

2. Priority of evacuation may be necessary if there are handicapped employees. 3. Materials and supplies to be evacuated:

RESPONSIBILITIES The Safety Coordinator will:

1. Coordinate the Emergency Evacuation Plan throughout the facility. 2. Make certain the Program is familiar to all employees and that all new employees are

promptly oriented. 3. Schedule fire safety classes as necessary.

4. Arrange and execute fire drills within the facility. 5. Maintain a log of fire drills conducted. The log shall include the date and time of each

drill, the time required to evacuate the building, and the initials of the person making the recording.

6. Report and correct any deficiencies noted during the fire drill. 7. Maintain a file of committee meetings and activities, including committee minutes. All

documents are to be signed by the committee chairperson. The Safety Coordinator will be aided by Managers who will:

1. Facilitate the Emergency Evacuation Plan. 2. Keep constant check on all personnel to be sure that they are completely familiar with

all phases of the Plan, which they are required to know. 3. See that all employees participate in ALL fire drills, fire classes, and other practice

sessions. 4. Be certain that all employees are familiar with, and make thorough fire prevention

inspections when they are assigned to do so. 5. Take the necessary steps required to correct any discovered fire hazards.

It is the duty of every employee to:

1. Be completely familiar with the Emergency Evacuation Plan and his or her duties and responsibilities in the program.

2. Participate in all fire drills and practice sessions. 3. Attend all fire training classes when assigned. 4. Learn the location of and how to operate fire alarm systems and all fire extinguishing

equipment. 5. Report any fire and/or safety hazard located anywhere on Company property.

MEDICAL SERVICES AND INFORMATION

Skyline Steel Erectors takes employee safety very importantly. Additionally, it is imperative that employees are educated on the nearest medical locations and the first aid supplies that are available at the 2738 Feather River Blvd. location and any jobsite. This information is kept in the site specific binder at the job sites and in the accident/emergency information binder at the SSE main office. Skyline Steel Erectors requires:

Safety meeting addressing accident and medical information along with a handout for each employee

First Aid/CPR certified person present at all times Listing of emergency medical providers posted Spill Clean Up Kit First Aid Kit at every jobsite

o Examples of first aid supplies: adhesive dressings, eye wash/care, gauze, scissors, tweezers, tape, cold pack, CPR Kit, emergency blanket, antibiotic ointment, antiseptic wipes, etc.

o Medication is prohibited in first aid kits The first aid kits are checked weekly after the safety meeting by the job site foreman. An

inventory sheet will be filled out and turned into the Safety Coordinator by mail or fax.

OFFICE SAFETY PROGRAM

• Regular Office Safety Meetings shall be held. The Safety Coordinator will review,

evaluate, and correct hazards. • At the end of each day the following will be checked:

o Night lights on o Doors and windows securely fastened o Fans and machinery off o Safe and file cabinets locked o No cash left lying out o Unplug/turn off coffee pot o When in doubt ‐ Recheck o Lights turned off

• There will be a person trained in First Aid and CPR in the office at all times. A fully‐equipped, approved, first aid kit will be maintained at all times in the office in a location known to all employees.

• Telephone numbers of the local fire department, police, ambulance and hospital will be posted.

• All employees will be instructed in emergency procedures in the event of fire, earthquakes or other emergency situations with drills at least annually.

SAFETY SUGGESTIONS FOR OFFICE WORKERS

• Adjust your chair so that you sit with your feet flat on the floor. Keep your back straight and your buttocks against the back of the chair. Your knees should be slightly above or even with your hips.

• Hold reading materials at eye level, supporting your elbows on a desk or chair arms. Use a vertical stand for typing notes to prevent bending your neck.

• Avoid holding the phone receiver between your head and shoulder. Use a speakerphone or headset if you need your hands free while you talk on the phone.

• When lifting heavy items, keep your back straight; bend from the knees rather than the waist. Use your leg muscles ‐ not your back ‐ to help you return to a standing position. Carry items close to your body.

• Assess your work area and work habits. Keep frequently needed materials in drawers or on bookshelves that can be reached without stooping down or reaching overhead. Arrange desk items so that you do not need to twist to reach them.

• Take occasional breaks to stand up and walk around to relieve pressure on the lower back from sitting.

• Take a few minutes every hour to do some simple exercises and stretches to release tension in the shoulder, neck and hands.

OFFICE CODE OF SAFE PRACTICES All employees shall follow the company’s code of safe practices, company policies and procedures, and report unsafe conditions or practices to their supervisor or Safety Coordinator. Injuries and near miss incidents shall be reported immediately.

• Workplaces shall be kept free of debris and improper floor storage. Worktables, desks, and other work surfaces shall be kept clean, orderly and dry.

• Combustible waste shall be stored and properly disposed of daily. • Adequate isle space shall be maintained for proper access and egress. • Be alert and aware of all possible hazards. • Horseplay, scuffling, or physical aggressive contact is prohibited. • Improper use of office equipment that may cause harm to you, others, or the

equipment, is prohibited. • Electrical cords shall be in good condition. Frayed or worn cords shall be replaced. The

proper grounding shall be used. • An appropriate, well maintained First Aid Kit shall be located in every office. The same

shall be secured in all Company vehicles. • Use of alcohol and/or illegal drugs is prohibited. Employees under the influence while

on the job will be dismissed immediately and shall be subject to disciplinary action up to and including termination.

• No employee shall knowingly be permitted or required to work when impaired by illness, fatigue, etc.

• Employees shall work only in areas with proper lighting to minimize glare and eyestrain. Rest periods shall be used to relax eyes and reduce eye fatigue.

• File cabinet drawers shall be opened one at a time, and closed when the task is completed. Caution shall be used to avoid pinched fingers.

• Exit doors shall be clearly marked and free of obstructions. Exit doors shall remain unlocked from the inside during work hours.

• Stairways shall be meet regulatory code, be clearly marked, have skid resistant tread, and be kept clear of obstructions.

• When needed, sturdy step stools shall be used. Standing on chairs, tables, or unsafe objects is permitted.

• Employees shall know the location of fire extinguishers, and know the proper use. • Employees shall be instructed in Emergency Evacuation Procedures. • Employees shall use the proper lifting techniques. Handcarts shall be used to move

heavy objects. • Violent acts, intimidation and harassment of employees will not be tolerated. • Use of tools shall be by personnel knowledgeable in the use of such tools. • Administrative and Professional Personnel visiting field job sites shall be instructed in

the hazards of construction. They shall be instructed in the field Code of Safe Practices and abide by the following safety guidelines:

o Company Vehicle Policies and Procedures shall be followed. o The appropriate clothing, such as heavy fabric to reduce the risk of abrasion and

contact with hazardous substances and environmental hazards, shall be worn. o Hard hats and the appropriate Personal Protective Equipment (outlined in the

field IIPP) shall be used on all construction sites. o Walkman type radio/tape/CD players with earphones are not permitted while at

the job site. o Safety vests shall be worn when working in traffic and on all construction sites.

HEARING CONSERVATION PROGRAM

Skyline Steel Erectors is dedicated to providing a safe and healthful working environment for our employees. We believe that safety in all operations and activities is of primary importance. However, it is the employee's responsibility to seek assistance when required, and to carry out the job in a safe manner. Skyline Steel Erectors will administer a continuing, effective hearing conservation program, as described in the following paragraphs, whenever employee noise exposures equal or exceed an 8 hour time weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent. For purposes of the hearing conservation program, employee noise exposures will be computed without regard to any reduction provided by the use of personal protective equipment.

• When excessive noise exposure is suspected, the noise level shall be measured with a combination dositer and Class 52A sound level meter.

• The Company shall notify any employee who is exposed to an 8‐hour time weighted average above 85 decibels.

• An audiometric testing program shall be established and administered to any employee whose exposure equals or exceeds the 8‐hour time weighted average of 85 decibels. It is provided at no cost to the employee. Audiograms shall be reviewed by a certified audiometric technician. Employees in this category will be tested annually, and any significant shifts shall be reviewed by an audiologist or otolaryngologist as required, at no expense to the employee.

• Employees who are laid off or terminated, and then are re‐hired , shall be re‐tested within 30 days of their re‐hire, if their annual exam came due during the period they were away from work.

• Employees shall receive initial and annual training as outlined below: o Effects of noise on hearing; o Purpose of hearing protection, advantaged, disadvantages, attenuation of

various types, and instructions on selection and proper fitting, use, and care. o The purpose of audiometric testing, and an explanation of the testing procedure.

• Any questions concerning the Hearing Conservation Program may be directed to the Company Safety Manager.

The Information contained in this Injury and Illness Prevention Program is the minimum requirement for Hearing Conservation. Project specific noise exposure, monitoring, and testing requirements will be in accordance with the Company’s Hearing Conservation Program and OSHA requirements. The Company’s Hearing Conservation Program may be reviewed at the Company’s office.

RESPIRATORY PROTECTION

There are many materials in the working environment that can harm employees through their respiratory systems. Skyline Steel Erectors’ Respiratory Protection Program is intended to reduce or eliminate this hazard for our employees. Certain hazards may be reduced or eliminated with engineering methods. These methods should be used whenever feasible. However, some hazards require Personal Protective Equipment in the form of respirators to protect employees. Respirators will be provided for employees when other engineering control measures fail to remove the hazard. There are minimum standards for the use of respiratory protection equipment in certain operations. Only respiratory equipment meeting or exceeding those standards shall be used. This determination and assignment of respiratory protection shall be conducted by a qualified person.

MINIMUM RESPIRATORY EQUIPMENT STANDARDS

However, there may be circumstances that necessitate special requirements. After careful evaluation, a change in the level or protection may be specified by the Foreman, Superintendent, Safety Manager, Field Safety Officer, or the Project Manager. When it is determined that respiratory protection is required, Skyline Steel Erectors will provide the appropriate equipment, and employee training in the following:

• Respirator hazard recognition, • Hazard evaluation, • Oxygen deficiency recognition, • Equipment types and limitations, and • Equipment inspection and maintenance.

The information contained within this Injury and Illness Prevention Program is the minimum requirement for respiratory protection. Project specific hazard assessment, respiratory protection evaluation, and employee training will be in accordance with the Company’s Respiratory Protection Program and OSHA guidelines. The Company’s Respiratory Protection Program may be reviewed at Skyline Steel Erectors’ office.

FIRE PREVENTION PLAN

• All potential fire hazards will be identified at each work site and facility. • The proper method of handling and storing these potential hazardous ignition sources

(such as welding, smoking, etc.), control procedures, type of fire protection equipment and/or systems which can control a fire involving them will be explained to all employees.

• The job superintendent, foreman and/or supervisor shall be responsible for the maintenance of equipment and systems used to prevent or control ignition or fires. They shall also be responsible for the control of accumulation of flammable or combustible waste materials.

• The job superintendent, foreman and/or crew leader shall control accumulations of flammable and combustible waste materials and residues so they do not contribute to a fire emergency. Each employee, upon initial assignment, shall be instructed those sections of the fire prevention plan which pertains to their assignment.

• This plan shall be kept on the job site and made available for employee review as requested.

• All equipment and systems installed at the work place to prevent unintentional ignition of combustible materials, shall be regularly and properly maintained, according to established procedures.

• This section applies to all fire prevention plans. The fire prevention plan shall be in writing, except as provided in the last paragraph of this section.

• The following elements, at a minimum, shall be included in the fire prevention plan: o Potential fire hazards and their proper handling and storage procedures,

potential ignition sources (such as welding, smoking and others) and their control procedures, and the type of fire protection equipment or systems which can control a fire involving them.

o Names or regular job titles of those responsible for maintenance of equipment and systems installed to prevent or control ignitions or fires.

o Names or regular job titles of those responsible for the control of accumulation of flammable or combustible waste materials.

HOUSEKEEPING Skyline Steel Erectors shall control accumulations of flammable and combustible waste materials and residues so that they do not contribute to a fire emergency. The housekeeping procedures shall be included in the written fire prevention plan.

TRAINING

Skyline Steel Erectors shall inform employees of the fire hazards of the materials and processes to which they are exposed. Skyline Steel Erectors shall review with each employee upon initial assignment those parts of the fire prevention plan, which the employee must know to protect the employee in the event of an emergency. The written plan shall be kept in the workplace and made available for employee review. MAINTENANCE

Skyline Steel Erectors shall regularly and properly maintain, according to established procedures, equipment and systems installed in the workplace to prevent accident ignition of combustible materials.

HAZARD COMMUNICATION PROGRAM

To ensure that information about the dangers of all hazardous chemicals used by Skyline Steel Erectors is known by all affected employees, the following hazardous information program has been established. Under this program, you will be informed of the contents of the OSHA Hazard Communications standard, the hazardous properties of chemicals with which you work, safe handling procedures and measures to take to protect yourself from these chemicals. This program applies to all work operations in our company where you may be exposed to hazardous chemicals under normal working conditions or during an emergency situation. All work units of this company will participate in the Hazard Communication Program. Copies of the Hazard Communication Program are available in the IIPP Program and the MSDS binders for review by any interested employee. The Safety Coordinator is the program coordinator, with overall responsibility for the program, including reviewing and updating as necessary. LABELING We will make certain that containers are adequately labeled to identify the hazardous chemicals contained therein, and will show hazard warnings appropriate for employee protection. The warnings will utilize a combination of words, pictures and symbols, which will convey the hazards of the chemical(s) in the container. The labels will be legible and prominently displayed. Exceptions to this rule are as follows:

• We are permitted to post signs which convey the hazard information if there are a number of stationary containers in a given area which have similar contents and hazards.

• Operating procedures, process sheets, batch tickets, blend tickets and similar written materials can be substituted for container labels on stationary process equipment if they contain the same information and are readily available to employees.

• We are not required to label portable containers, as long as the transferred chemical is for immediate use by the employee who made the transfer.

• We are not required to label pipes or piping systems. MATERIAL SAFETY DATA SHEETS (MSDS) Chemical manufacturers and importers are required to review the available scientific evidence concerning the hazards of the chemicals they produce then report that information to employers who purchase their product. In most cases, Skyline Steel Erectors will choose to rely on the evaluations performed by our suppliers. If, for some reason, we do not trust the evaluation of the manufacturer, we will arrange for additional testing.

We will consider any chemicals listed in one of the following sources to be hazardous:

• 29 CFR 1910, Subpart Z, Toxic and Hazardous Substances, Occupational Safety and Health Administration (OSHA),

• Threshold Limit Values for Chemical Substances and Physical Agents in the Work Environment American Conference of Governmental Industrial Hygienists (ACGIH), or

• Hazardous substances prepared pursuant to Labor Code Section 6382. The Safety Coordinator is responsible for obtaining or developing a MSDS for each chemical used in the workplace. Each MSDS will include the specific chemical identity of the chemical involved and the common names. MSDS for Skyline Steel Erectors can be found in the main office. Each data sheet will provide information on the physical and chemical characteristics of the chemical; known acute and chronic health effects and related health information; exposure limits; whether the chemical is considered to be a carcinogen; precautionary measures; emergency and first aid procedures; and the identification of the organization responsible for preparing the sheet. Each department supervisor will be responsible for maintaining the MSDS’s describing chemicals used in his/her department and for keeping them readily available to employees. The Safety Coordinator will maintain a master file for all departments. PROP 65 The State of California has determined that certain substances have the potential to cause cancer or harm the human reproductive system. The law is intended to prevent the knowing discharge of these chemicals or products into water or land where they may pass into any source of drinking water, and to give warning to individuals who may be exposed to such chemicals. Since the average job may have several of these chemicals in use or storage, each location shall take the following steps:

• Post the Prop. 65 warning sign (for chemicals known to cause cancer or birth defects). • Employees shall control their work so no employee is knowingly and intentionally

exposed to a Prop. 65 labeled substance without “clear and reasonable” warning (read the labels, heed precautions, follow handling instructions and refer to MSDS as needed).

• Employees shall control their work so that no employee shall knowingly discharge or release a Prop. 65 labeled chemical which could enter any source of drinking water (follow handling and disposal instructions).

TRAINING Our employee training program will include instruction on how to read and interpret information on a MSDS, and how employees can obtain and use the available hazard information. It is our goal to provide hazard communication training during the first 30 days of employment and whenever a new chemical is introduced to the work area. Training will be done in a classroom setting and will be conducted by the Safety Coordinator or another person who has been properly trained. The training program will consist of:

• Overview of OSHA hazard communication standard • Overview of Prop 65 • How the hazard communication program is implemented, how to read and interpret

information on labels and MSDS, and how employees can obtain and use the available hazard information.

• The hazards of the chemicals in the work area. • Measures employees can take to protect themselves from the hazards. • Specific procedures put into effect by the company to provide protection, such as

personal protective equipment. • Methods and observations, such as visual appearance or smell, workers can use to

detect the presence of a hazardous chemical to which they may be exposed. • Identify locations where hazardous chemicals are stored, handled, dispensed or

transported, and the location of each process and operation that uses them. • Locations of MSDS and written hazard communication

STEEL ERECTION FALL PROTECTION/PREVENTION PLAN

The purpose of this Plan is to:

• Supplement our standard safety policies and procedures by providing safety standards specifically designed to cover fall protection and fall prevention.

• To ensure that employees are trained and made aware of the safety provisions which are to be implemented by this Plan prior to the start of steel erection procedures.

• This fall protection/prevention plan addresses the use of conventional fall protection/prevention at a number of areas on the project, as well as identifying specific activities that may require non‐conventional means of fall protection/prevention.

• This Plan is designed to enable Skyline Steel Erectors employees to recognize fall hazards and to establish the procedures that are to be followed in order to prevent falls to lower levels or through openings in walking/working surfaces. Each employee will adhere to these procedures except when doing so will expose the employee to a greater hazard. When this is the case, the employee shall notify management of the exposure, which shall be addressed before proceeding further.

RESPONSIBILITIES

It is the responsibility of the on‐site safety supervisor to implement this fall protection/ prevention plan. The Safety Coordinator is responsible for continual observational safety checks of the work area and enforcement of safety policies and procedures. Superintendents and/or Foremen shall be responsible for correcting unsafe acts or conditions immediately. Employees are responsible for understanding and adhering to the procedures of this plan, following the instructions of the Superintendent/Foreman, and notifying management of unsafe conditions or acts. FALL PREVENTION/PROTECTION PLAN

1. PERIMETER SAFETY CABLE

• On multi‐story structures, perimeter safety cables shall be installed at the final interior and exterior perimeters of the floors as soon as the metal decking has been installed.

• Pendant lines, catenary lines and other lines used for tie‐off shall be designed and installed by a qualified person.

2. CUSTODY OF PERIMETER SAFETY CABLE‐CONTROLLING CONTRACTOR

• Fall protection (perimeter cable system) provided by Skyline Steel Erectors shall remain in the area where steel erection activity has been completed, to be used by other trades, only if the controlling contractor or its authorized representative has:

o Directed the steel erector to leave the fall protection in place. o Inspected and accepted control and responsibility of the fall protection prior to

authorizing persons other than steel erectors to work in the area. o Signed Skyline Steel Erectors Safety Cable Acceptance form as required.

3. CONNECTING

• Connectors shall be protected from fall hazards of more than 30 feet above a lower level and have completed connector training in accordance with the training requirements of this Program.

• When connecting beams or other structural members at the periphery or interior of a building or structure where the fall distance is greater than two stories or 30 feet, whichever is less, iron workers shall be provided with and use a personal fall protection system as described in this Program, tied‐off to either columns, pendant lines secured at the tops of columns, catenary lines, or other secure anchorage points.

• At heights over 15 and up to 30 feet above a lower level, connectors shall be provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or be provided with other means of protection from fall hazards.

• When moving from work point to work point or releasing slings, and the fall distance is greater than 30 feet or two stories, whichever is less, connectors:

o Shall coon or walk the bottom flange (inside flange of peripheral beams) o May walk the top surface of securely landed decking bundles o May walk the top flange if they are tied‐off to catenary lines or use other fall

protection in accordance with “Criteria for Fall Protection Equipment” of this Plan.

4. WORK OTHER THAN CONNECTING

• When performing any other work than connecting at a work point, iron workers shall be provided with and use personal fall protection where the fall distance is greater than 15 feet.

• When moving from work point to work point or releasing slings, and the fall distance is greater than 15, Ironworkers other than connectors:

o Shall coon or walk the bottom flange (inside flange of peripheral beams);

o May walk the top surface of securely landed decking bundles; or o May walk the top flange if they are tied‐off to catenary lines or use other fall

protection in accordance with “Criteria for Fall Protection Equipment” of this Plan.

5. SHINNING (CLIMBING) OF COLUMNS

• When connecting beams or other structural members at columns the practice of shinning (vertically climbing up or down) columns to access workpoints shall be permitted where the fall distance does not exceed two stories or 30 feet, whichever is less.

• Where the fall distance exceeds two stories or 30 feet, whichever is less, iron workers shall be provided with and use a personal fall protection system as described in Article 24 tied‐off to either columns, pendant lines secured at the tops of columns, catenary lines, or other secure anchorage points.

6. TRAINING REQUIREMENTS • Training Personnel: Training required by this section shall be provided by a qualified

person(s).

• Fall Hazard Training: Skyline Steel Erectors shall provide a training program for all employees exposed to fall hazards. The program shall include training and instruction in the following areas:

o The recognition and identification of fall hazards in the work area. o The use and operation of guardrail systems (including perimeter safety cable

systems), personal fall arrest systems, positioning device systems, fall restraint systems, safety net systems, and other protection to be used.

o The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used;

o The procedures to be followed to prevent falls to lower levels and through or into holes and openings on walking/working surfaces and walls.

o The fall protection requirements of this Program.

• Connector Procedures: Skyline Steel Erectors shall ensure that each connector has been provided training in the following areas:

o The nature of the hazards associated with connecting.

o The establishment, access, proper connecting techniques and work practices associated with connecting.

• Controlled Decking Zone Procedures: Skyline Steel Erectors shall ensure that each

employee working within the Controlled Decking Zone (CDZ) has been provided training in the following areas:

o The nature of the hazards associated with work within a controlled decking zone. o The establishment of CDZs, access, proper installation techniques, and work

practices required by subsection (n) of Cal‐OSHA §1710.

7. CRITERIA FOR FALL PROTECTION EQUIPMENT

• Guardrail systems, safety net systems, personal fall arrest systems, positioning device systems and their components shall conform to the criteria in Cal‐OSHA Section 1670.

• Fall arrest system components shall be used in fall restraint systems and shall conform to the criteria in Cal‐OSHA Section 1670. Either body belts or body harnesses shall be used in fall restraint systems.

• Perimeter safety cables shall meet the criteria for guardrail systems in Cal‐OSHA Section 1670.

8. IMPALEMENT HAZARDS

• Employees shall not work above impalement hazards. The controlling contractor shall provide the required covers or troughs for impalement hazards such as, rebar, stakes, conduit, etc., before employees access the work area. Employees working at grade or at the same surface as exposed protruding reinforcing steel or other similar projections shall be protected against the hazard of impalement by guarding all exposed ends that extend up to 6 feet above grade or other work surface, with protective covers, or troughs.

9. TEMPORARY FLOORS‐HOLE COVERS

• The derrick or working floor of every building shall be solidly decked over its entire surface except for access openings.

• There shall be a tight and substantial temporary floor within two floors below and directly under that portion of each tier of beams on which erection, riveting, bolting, welding or painting is being done. For operations of short duration of exposure to falling, fall protection shall meet the requirements of this Plan.

• Floor planks or metal decking that are temporarily removed for any reason whatsoever, shall be replaced as soon as work requiring their removal is completed or the open area shall be properly guarded.

TRAINING REQUIREMENTS

Training Personnel. Prior to removal of temporary floor planks or metal decking, employees shall be instructed by assigned supervision the steps to be taken to perform the work safely and in proper sequence as follows:

• When gathering and stacking temporary floor planks on a lower floor, in preparation for transferring such planks for use on an upper working floor, employees shall remove such planks successively, working toward the last panel of such floor, so that the work is always being done from the planked floor.

• When gathering and stacking temporary floor planks from the last panel, employees assigned to such work shall be protected by a personal fall protection system as required by this Plan.

• No person shall proceed with any work unless the planking and metal decking, or nets required by this article are in place.

• All floor opening covers shall bear a sign stating, "OPENING‐DO NOT REMOVE", in 2 inch high, black bold letters on a yellow background

10. LADDERS

• Ladders shall meet the requirements of Cal‐OSHA Section 1675. • Ladders shall be inspected before each shift. • Ladders shall be secured at the top and shall extend 36” above the landing. • Hand lines shall be used to pull up tools and materials. Employees shall not carry items

while climbing a ladder. • Damaged or malfunctioning ladders shall be removed from service. 11. AERIAL LIFTS AND ELEVATED WORK PLATFORMS/SCAFFOLDS

• Aerial lifts and their use shall meet the requirements of Cal‐OSHA Section 3642. • Pre‐shift inspections shall be conducted and documented. • Only trained and authorized employees shall access aerial lift. • Damaged or malfunctioning equipment shall be repaired or removed from service.

• Scaffolds shall meet the requirements of Cal‐OSHA Section 1637. • Employees shall be trained in the safe use of scaffold. • Pre‐shift inspections of scaffolds shall be conducted and documented. • Damaged or faulty scaffold shall be tagged and repaired before employee access.

12. FALL RESCUE PLAN

• If a fall occurs, any employee hanging from the fall‐arrest system must be rescued safely and quickly. Planning shall take place before there is any risk of a fall. Select one or more of the following methods that will be used for fall rescue procedures. Note your selection by marking or circling the appropriate methods:

o Equipment that lets the victim rescue himself, such as a self rescue ladder. o A system for rescue by co‐workers if available such as:

Crane with an approved man basket. Ropes or litter to lower the victim. Ladder to assist in support for the victim. Call the emergency services number listed at the project.

• The methods for rescue, as listed above, should only be utilized if the rescue can be accomplished safely. Move the victim carefully, as injuries may exist.

• All employees shall be aware of the plan and all employees shall know the availability of the fall rescue equipment.

• Employees shall be instructed in self‐rescue and the rescue of others.

THE FOLLOWING SHOULD BE DONE AFTER A FALL AND DURING THE RESCUE

• Provide fall protection for the victim and the rescuers. • Communicate with the victim and monitor him constantly. • Call any additional rescue service needed. • If the worker has fallen but they are not injured or unconscious, and self rescue is

impossible, get them to use their leg muscles (raising legs up and down with use of a rope, hose, etc.) to keep the blood pumping back into the torso.

• If none of the above is available, try to push against any foothold if possible. • Rescue the suspended worker as quickly as possible. Time in suspension should not

exceed five to ten minutes. • Try to keep the suspended worker as close to horizontal during the rescue to lessen the

pressure on the legs.

• Continuously monitor the suspended worker for signs and symptoms of suspension trauma.

• If needed, ensure that the worker receives the appropriate First Aid/CPR or emergency medical service, once rescued.

• Avoid laying the rescued worker flat after the rescue. Keep the upper body raised in a sitting position for approximately thirty minutes.

• If emergency medical services are assisting with the rescue, insist that they follow these instructions.

• The suspended worker, who has experienced suspension trauma, should be evaluated by a health‐care professional. Possible delayed affects are not unusual.

LOCKOUT PROCEDURES PROGRAM

PURPOSE • This procedure establishes the minimum requirements for lockout of energy sources

that could cause injury or death to personnel. All employees shall comply with the procedure.

RESPONSIBILITY

• The responsibility for seeing that this procedure is followed is required of all employees. All employees shall be instructed in the safety significance of the lockout procedure by the designated Training Supervisor. Each new or transferred affected employee shall be instructed by the designated Training Supervisor in the purpose and use of the lockout procedures.

PREPARATION FOR LOCKOUT

• Employees authorized to perform lockout shall be certain as to which switch, valve, or other energy isolating devices apply to the equipment being locked out. More than one energy source (electrical, mechanical, or others) may be involved. Any questionable identification of sources shall be cleared by the employees with their supervisors. Before lockout commences, job authorization should be obtained.

SEQUENCE OF LOCKOUT PROCEDURE

• Notify all affected employees that a lockout is required and the reason therefore. • If the equipment is operating, shut it down by the normal stopping procedure (such as:

depress stop button, open toggle switch). • Operate the switch, valve, or other energy isolating devices so that the energy source(s)

(electrical, mechanical, hydraulic, and other) is disconnected or isolated from the equipment. Stored energy, such as that in capacitors, springs, elevated machine members, rotating fly wheels, hydraulic systems, and air, gas, steam or water pressure, must also be dissipated or restrained by methods such as grounding, repositioning, blocking, bleeding down.

• Lockout energy isolating devices with an assigned individual lock. • After ensuring that no personnel are exposed and as a check on having disconnected the

energy sources, operate the push button or other normal operating controls to make certain the equipment will not operate. CAUTION: Return operating controls to neutral position after the test.

• The equipment is now locked out.

RESTORING EQUIPMENT TO SERVICE • When the job is complete and equipment is ready for testing or normal service, check

the equipment area to see that on one is exposed. • When equipment is clear, remove all locks. The energy isolating devices may be

operated to restore energy to equipment. PROCEDURE INVOLVING MORE THAN ONE PERSON

• In the preceding steps, if more than one individual is required to lock out equipment, each shall place his/her own personal lock on the energy isolating device(s). One designated individual of a work crew or a supervisor, with the knowledge of the crew, may lock out equipment for the whole crew. In such cases, it may be the responsibility of the individual to carry out all steps of the lockout procedure and inform the crew when it is safe to work on the equipment. Additionally, the designated individual shall not remove a crew lock until it has been verified that all individuals are clear.

RULES FOR USING LOCKOUT PROCEDURE

• All equipment shall be locked out to protect against accidental or inadvertent operation when such operation could cause injury to personnel. Do not attempt to operate any switch, valve, or other energy isolating device bearing a lock.

• The Information contained in this Injury and Illness Prevention Program is the minimum requirement for Lockout Procedures. Specific requirements and procedures will be in accordance with the Company’s Lockout Program and OSHA requirements. Skyline Steel Erectors’ Lockout Program may be reviewed at the main office.

TEMPORARY ELECTRIC FOR CONSTRUCTION

The hazards associated with Temporary Electric sources are some of the most overlooked, yet most dangerous, hazards on a construction site. The following covers the bodily effects that electrocution can cause, as well as some of the different operation safety precautions that should be taken to avoid electrocution.

MILLIAMP ERE CURRENT VALUES AND THEIR AFFECT ON THE HUMAN BODY 01 or less Causes no noticeable sensation. No sensation is felt because the

current is not strong enough to register with the nerves 01 to 04 Slight shock sensation. Not painful. Individual maintains muscular

control. 5mA is accepted as maximum harmless current intensity. 05 to 14 Painful shock, yet individual maintains muscular control. 15 to 19 Painful shock, muscular control is lost. 20 to 49 Painful shock. Severe muscular contractions. Breathing is difficult.

Discomfort and breathing difficulties may persist hours after current has been interrupted.

50 to 250 Ventricular fibrillation occurs. Ventricular fibrillation is a condition caused when one or both of the lower chambers of the heart stop pumping while one or both of the upper chambers continue to function.

250+ Severe burns. Also, the thoracic (area between neck and diaphragm) muscles) muscles contract, causing cardiac arrest and preventing ventricular fibrillation. If the current is interrupted within a fraction of a second of when it started, the heart may begin to function. However, if the current is not quickly interrupted, death results.

THE HAZARDS • Temporary electrical serves as a power source for hand tools and equipment when

permanent electrical resources are unavailable. There are many dangers when using temporary electrical sources, because temporary electrical requires electricity to pass through temporary devices to equipment. Depending on the equipment being used, if power is interrupted or a power surge occurs, the equipment may cause injury to an employee, or damage to equipment, or the product.

• Power interruption can be caused by many different things. The following is a list of some items to look for to prevent power interruption:

o Homemade outlets o Frayed and damaged cords or tools o Overloaded circuits o Connections exposed to water

o Exposed electrical conductors o Cord assemblies with improperly connected terminals

• The unfortunate results of these exposures include electric shock and in some instances, serious injury or death.

THE REGULATIONS • OSHA’s federal standards for construction electrical requirements are located in 29 CFR

part 1926, Subpart K. Cal‐OSHA’s construction electrical requirements are found in Section 2405, Title 8 California Code of Regulations. Requirements for Ground Fault Circuit Interrupters or Assured Equipment Grounding programs were adopted in July, 1986. The basis for the rules is to provide protection for construction employees from the most common electrical shock hazard, the ground fault.

• The California Regulations state, “An employer shall use either or both ground‐fault circuit interrupters or an assured equipment grounding conductor program. These requirements are in addition to any other requirements for equipment grounding conductors.”

GFCI SYSTEM

• A Ground Fault Circuit Interrupter is a fast acting circuit breaker that senses small imbalances in the circuit caused by current leakage to the ground and, in a fraction of a second, shuts off the electricity. The GFCI continually matches the amount of current going to an electrical device with the amount of current returning from the device along the normal electrical path. Whenever the amount “going” differs from the amount “returning” by approximately 5 milliamps, the GFCI interrupts the electric power within as little as 1/40 of a second. Keep in mind that the that the GFCI will not protect employees from line to line (hot wire to hot wire) shocks.

• The following are specific requirements taken from the California Code of Regulations: o All 120 volt, AC, single‐phase, 15‐ and 20‐ampere receptacle outlets on

construction sites, which are not a part of the permanent wiring of the building or structure and are in use by employees, shall have approved ground‐fault circuit interrupters for employee protection.

o Receptacles on a two‐wire, single‐phase portable or vehicle‐mounted generator rated not more than 5 kW, where the circuit conductors of the generator are insulated from the generator frame and all their grounded surfaces, need not be protected with ground‐fault circuit interrupters.

o Feeders supplying 15‐ and 20‐ampere receptacle branch circuits shall be permitted to be protected by a ground‐fault circuit interrupter approved for the purpose in lieu of the above provisions.

ASSURED EQUIPMENT GROUNDING PROGRAM

• California Regulations state, “An employer shall establish and implement an assured equipment grounding conductor program on construction sites, covering all 120‐volt, AC, single‐phase, cord sets, receptacles, which are not a part of the permanent wiring of the building or structure and equipment connected by cord and plug, which are used by employees.” The following are the minimum requirement:

a. Written Program. A written description of the program, including the specific procedure adopted by the employer shall be available at the job site for OSHA and any affected employee.

b. Qualified Person. The employer shall designate one or more qualified persons to implement the program.

c. Daily Inspections. Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug including those which are not required to be grounded, except cord sets and receptacles which are4 fixed and not exposed to damage, shall be visually inspected before each day’s use for external defects, such as deformed or missing pins or insulation damage, and for indication of possible internal damage. Equipment found damaged or defective shall not be used until repaired.

d. Testing. The following tests shall be performed on all cord sets and receptacles, which are not a part of the permanent wiring of the building or structure and cord‐and‐plug connected equipment required to be grounded:

i. Continuity. All equipment grounding conductors shall be tested for continuity and shall be electrically continuous.

ii. Grounding. Each receptacle and attachment cap or plug shall be tested for correct attachment of the equipment‐grounding conductor. The equipment‐grounding conductor shall be connected to its proper terminal

NOTE: Double‐insulated tools or other similar equipment are not required to be grounded.

e. Testing Frequency. All tests shall be performed:

i. Before all newly acquired equipment is used ii. Before equipment is used following any repairs

iii. Before equipment is used after any incident which can be reasonable suspected have caused damage (for example, when a cord set is run over)

iv. At least every three months f. Red Tag/Out of Service Equipment. Equipment, which has not met

these requirements, shall not be made available or permitted to be used by employees.

g. Marking/Color Coding Receptacles. Cord sets and cord‐ and plug connected equipment passing the tests shall be identified. Color‐ coding is most common, but logs may also be maintained as long as employees are informed and an interval, usually 3 months, is maintained for testing and updating all equipment.

In conclusion, temporary electrical is a necessary, but dangerous, operation. Special care must be taken to ensure the safety or all employees and product. By understanding the effects of electrocutions and how to prevent them, a safer jobsite is created.

FORKLIFT SAFETY PROGRAM

BASIS • Accidents resulting from powered industrial truck operation can result in severe

personal injury or death, major property damage and major damage to company products. This poses a serious problem for workers and their employer. The OSHA Powered Industrial Trucks Standard establishes uniform requirements to make sure that hazards associated with the use of Powered Industrial Trucks are evaluated, and that this hazard information and training is transmitted to all affected workers.

GENERAL • Skyline Steel Erectors will ensure that the requirements of the OSHA Standard for

powered industrial trucks will be adhered to. This standard practice instruction is intended to address comprehensively the issues of; employee training, authorization, safety requirements, fire protection, new purchase designs, maintenance, and general operation of fork trucks, tractors, platform lift trucks, motorized hand trucks, and other specialized industrial trucks used within our facility.

• Contents of the Powered Industrial Trucks Program

a. Written Program. Development and maintenance of a written powered industrial trucks program.

b. Training Program. Development and implementation of the employee training program regarding; authorization, use, operator maintenance, and associated hazards.

c. Operations Program. Development and implementation of the operations program regarding; authorization, use, operator maintenance, and associated hazards.

d. Configuration Program. Development and implementation of the vehicle configuration program regarding; modifications or additions to vehicles which affect capacity, and safe operation.

Powered Industrial Trucks Program

• Written Program. Skyline Steel Erectors will review and evaluate this standard practice instruction when any of the following occurs:

o Every three years. o When changes occur to governing regulatory sources that require revision. o When changes occur to related company procedures that require a revision. o When facility operational changes occur that requires a revision.

o When there is an accident or a near‐miss that relates to this area of safety. o Anytime the procedures fail.

• Effective implementation of this program requires support from all levels of management. This written program will be communicated to all personnel that are affected by it. It encompasses the total workplace, regardless of the number of workers employed or the number of work shifts. It is designed to establish clear goals and objectives.

• Training program. Operator training. Only trained and authorized operators shall be permitted to operate a powered industrial truck. All operator training and evaluation shall be conducted by persons who have the knowledge, training, and experience to train powered industrial truck operators and evaluate their competence.

• The Information contained in this Injury and Illness Prevention Program is the minimum requirement for Forklift Safety Training and Procedures. Specific requirements and procedures will be in accordance with the Company’s Forklift Safety Program and OSHA requirements. Skyline Steel Erectors complete Forklift Safety Program may be reviewed at the main office.

Statement of Fleet Safety Policy Accidents, whether they occur on the streets and highways or in any facility, profit no one. They may result in damage to the goods, products or equipment in our care. They contribute toward delays in our daily business activities. When personal injuries occur to our employees, they can cause unnecessary misery as well as possible loss of income. It is well recognized that most accidents can be prevented, thus avoiding their many undesirable consequences. It is equally well recognized that accident‐free operations are not achieved by chance. Motor Vehicle Driving Record Evaluation Guide An individual driver is considered unacceptable (unless there are reasonable extenuating circumstances) if his/her driving record for the most recent three to five year period exceeds the following standards: Age 16‐25: two or more moving violations within the last three years Any age: a major violation within the last five years Major violations: In physical control while under the influence of alcohol or other mind‐altering substances, homicide or assault with a motor vehicle, leaving the scene of an accident, license revocation/suspension; operating a vehicle while license is suspended, reckless driving, acceleration or speed contest, negligent driving, permitting an unlicensed person to drive, operating a vehicle without owner’s permission, using a motor vehicle for the commission of a felony, etc. Skyline Steel Erectors realizes that your driving habits will be no different whether operating company owned equipment or personal vehicles on company business. Therefore, as part of your overall annual job performance review, we will request, at our expense, updated information by obtaining current information as supplied from the Department of Motor Vehicles and Licensing. You may be requested to approve the release of an updated driver report based on State requirements.

If new information is negative in nature and exceeds our evaluation criteria, a meeting will be arranged between you and your supervisor. Options at such a point include transfer to a non‐driving position, prescription of an advanced driver training course or termination of employment. Driving records obtained will be kept confidential. Road Tests Prior to operating a company vehicle, in addition to providing a valid driver’s license and meeting the motor‐vehicle record criteria, drivers must complete a Road Test provided by an authorized member of management. This is generally a 20‐30 minute observation of your driving skills with documentation with the vehicle you are expected to be operating. This also includes pre‐operational check, securing a load and documentation. Undue concerns may result in continued driver‐training being required prior to driving a company vehicle. Securing a Load Material and equipment falling from vehicles can result in fatalities to pedestrians or other drivers. Prior to each trip in a company vehicle, a final walk‐around of the vehicle “by the driver” should be completed to ensure that all material and equipment is adequately secured. In cases of questionable security, management or veteran drivers should be contacted for advice. Experienced Driver Ongoing Training In support of preventing employee injury, preventing damage to company vehicles and personal vehicles used for company business and protecting our customers and the public, Skyline Steel Erectors prescribes periodic (every 3 years) experienced driver training and a ride along by management. Drivers of Skyline Steel Erectors vehicles or those frequently driving their personal vehicles on company business will be requested to view the Driving Safely PowerPoint within the first 60 days of employment. This presentation will be coordinated by the safety coordinator. Accident Involvement and Reporting Any Accident

We require that whenever an accident results in injury or death to any person or property damage of any kind, regardless of amount, the driver of a motor vehicle (when it is a company vehicle or when you are operating a non‐company vehicle for business purposes in the course of your employment) must:

1. Stop without delay! Pull off the road, if possible. Position the vehicle to minimize any obstruction to traffic and set the parking brake. In a serious accident, wait for law enforcement personnel before moving the vehicle.

2. Activate the four way flashers.

3. If in imminent danger to loss of life (such as fire or smoke being present or if on an active

railroad track), evacuate vehicle occupants to a safe location by utilizing recognized first aid techniques (if known) such as the “clothes drag” or “blanket drag”, recognized/taught by the American Red Cross. Otherwise, do not move occupants.

4. Summon aid for those injured (911 in most areas).

5. Set up the reflective warning triangles. Placement of the reflective triangles may be

delayed for up to ten minutes to attend to life threatening emergencies.

6. Provide comfort and solace as the incident requires, make no statements relating to fault or responsibility for the accident.

7. Notify law enforcement and your company. Keep discussions with law enforcement and

others to a minimum. Restrain the desire to discuss the accident with anyone other than your company representative.

8. Exchange information with others involved in the accident. Acquire the names,

addresses, phone numbers, makes of vehicles and license numbers of all drivers, passengers and witnesses. Obtain badge number of police officer. Complete accident report form.

9. While maintaining a safe distance from traffic and emergency crews, photograph the

damage to all vehicles and/or property. Photograph the relationship of the debris fields and skid marks to the vehicles. Photograph the license plates of all vehicles, including those of witnesses.

Even in instances when the driver strikes a parked vehicle that is unattended, the driver is required to:

1. Stop immediately

2. Try to locate the owner or custodian of the unattended vehicle.

3. If the owner or person responsible for the vehicle cannot be located, the driver must place his/her name and address in a conspicuous place on the vehicle.

4. Report all details of the accident to your supervisor as soon as possible.

An accident reporting kit has been placed in the glove compartment of all company vehicles. The camera with this kit should be used to document the damage as noted in 9 above. Emergency Equipment/Accident Kits With Cameras The following equipment is provided in the following company vehicles: 2001 & 2003 Chevrolet Pick Ups.

• A fire extinguisher • An emergency blanket • Reflective triangles to be placed properly at an accident scene • A disposable camera to photo‐document an accident scene • A first aid kit • A company accident report form

Accident Investigation An investigation of the accident will be conducted to see if the accident was preventable or non preventable. They will also be reviewed for evidence of defensive driving techniques. All documentation of the accident will be put in the master safety binder as well as in the driver’s personnel file. If necessary, disciplinary actions will be at the discretion of management. These actions may include, but not be limited to the following: Verbal and/or written warning Attendance at a driver’s training seminar

Temporary assignment to a non‐driving position Any combination of 1, 2, and 3 above. Probation period ranging from 1‐3 weeks Suspension without pay Termination of employment Unauthorized Riders and Drivers Skyline Steel Erectors does not permit unauthorized drivers or riders in company owned vehicles. An unauthorized rider or driver shall be defined as: Any person other than the assigned operator of the unit, or other employee not authorized by a member of Skyline Steel Erectors management or driver who is not listed on Skyline Steel Erectors authorized driver list. Riders are permitted when they have a business relationship with Skyline Steel Erectors and the authorized driver of the vehicle. Seat belts are required to be worn by all company car occupants. It is the responsibility of the driver to ensure this. Vehicle Inspections All company vehicles are required to have a minimum of semi‐annual inspections to ensure proper preventative maintenance (tire rotations, oil changes, etc.), drivability, care and record keeping. Pre‐trip inspections, also known as walk‐arounds are required to be completed by the driver utilizing that vehicle. No written documentation on this inspection for passenger vehicles is required unless there is an issue found. In the case there is an issue, a maintenance form shall be completed and turned into management or the safety coordinator and under no circumstances shall the vehicle be operated. Vehicle Maintenance Drivers of company vehicles are required to properly maintain the vehicles at all times. Vehicles should not be operated with any defect that would inhibit safe operation during current and foreseeable weather and lighting conditions. Preventive maintenance such as tire rotations, regular oil changes, lubrication and tire pressure/fluid checks determine to a large extent whether you will have a reliable, safe vehicle to drive and support your work activities. maintenance activities conducted on company vehicles will be coordinated by Skyline Steel Erectors Management. In the case that a vehicle has issues while on the road, each vehicle has a list of repair services. Authorized Personal Use of Company Vehicles Skyline Steel Erectors prohibits the use of company vehicles for personal use. Use of Personal Vehicles for Company Business Skyline Steel Erectors has a limited number of employees who drive personal vehicles for business purposes for more than once a week local errands. These employees are notified by management to do so. Employees are required to submit a certificate of insurance prior to driving for company business and are required to submit one annually. All documentation is kept in the employee’s personnel file. Company Vehicles and Cell Phones Skyline Steel Erectors strives to provide a safe place of employment for its employees. This “safe place” extends to employees who use cellular phones in the course of completing their business responsibilities when driving. Skyline Steel Erectors will provide hands‐free equipment to employees who have a business need for cellular phones. “Hands‐free” in this instance refers to not having to hold the phone while carrying on a discussion; however, placing calls and hanging up may require handling the phone. Drivers using a hands‐free system should find a place to safely pull off of the road should they find themselves in conversations which can hinder safe driving or situations where they must refer to or take notes. Tickets will be the responsibility of the cell phone user, not the company. As of right now, tickets do not result as a moving violation nor have any points associated your driving record. Drugs and Alcohol Policy In order to comply with our intent to provide a safe work place, Skyline Steel Erectors forbids:

1. Consumption of an intoxicating beverage prior to operating a company vehicle or operating a personal vehicle on company business

2. The consumption or possession of an intoxicating beverage or controlled substance

while (i) on duty, (ii) operating or in physical control of any company vehicle or (iii) on company business in a non‐company vehicle.

3. Operating any vehicle, or being in physical control of such vehicle, on company business

while under the influence of mind‐altering substances (prescription medication, over the counter medication, hallucinogenics, etc.)

You may be asked to submit to a blood test by any Supervisor, Manager or Company Officer as they have cause to believe that you may be under the influence. If you test positive for alcohol, mind altering and/or controlled substances or are found to be in possession of such, as outlined above, you will be subject to disciplinary action up to and including termination of employment.

Confined Space Entry Program

This company and its associated work places have confined spaces that, due to various chemical and physical properties, may cause death or serious injury to employees who may enter them. This Confined Space Entry Program is developed and established to identify, evaluate, and control such spaces, and more important, to detail procedures and responsibilities for entering and working within confined spaces. Adherence to the policies and directives contained in this program is mandatory for all supervisors and employees of this company. Supervisors and employees failing to follow this program are subject to disciplinary action and/or dismissal. DEFINITIONS Confined Space‐ A space that:

• Is large enough and so configured that an employee can bodily enter and perform assigned work; and

• Has limited or restricted means for entry and exit; and • Is not designed for continuous employee occupancy.

Permit Required Confined Space‐ A confined space that:

• Contains or has a potential to contain a hazardous atmosphere; or • Contains a material that has the potential for engulfing an entrant; or • Has an internal configuration such that an entrant could be trapped or asphyxiated by

inwardly converging walls or by a floor which slopes downward and tapers to a smaller cross section; or

• Contains any other recognized serious safety or health hazard. Acceptable Entry Conditions‐ The conditions that must exist in a permit space to allow entry and to ensure employees can safely enter into and safely work within a permit required confined space. Entry‐ The action by which a person passes through an opening into a permit required confined space. Entry is considered to occur as soon as any part of the entrant’s body breaks the plane of

an opening into a space. NOTE: For entry to occur, there must be intent to bodily enter the confined space. May reach into a space, and not bodily enter (say to adjust a valve), and do so without an entry permit being required. Entry Permit‐ The written or printed document provided by this company to allow and control entry into a permit space. Entry Supervisor‐ The person responsible for determining if acceptable entry conditions are present at a permit space where entry is planned, for authorizing entry and overseeing entry operations, and for terminating entry. Hazardous Atmosphere‐ An atmosphere that may expose employees to the risk of death, incapacitation, impairment of ability to self‐rescue, injury, or acute illness from one or more of the following conditions:

• Flammable gas, vapor, or mist in excess of 10 percent of its lower flammable limit (LFL); • Airborne combustible dust at a concentration that meets or exceeds its LFL. (this may be

approximated as a condition in which the dust obscures vision at a distance of 5 feet or less);

• Atmospheric oxygen concentration below 19.5 percent or above 23.5 percent; • Atmospheric concentration of any substance for which a dose of permissible limit is

published (in Subpart G Occupational Health and Environmental Control; or Subpart Z Hazardous and Toxic Substances); or

• Any other atmospheric condition that is immediately dangerous to life or health. Non‐Permit Confined Space‐ A confined space that does not contain, or with the respect to atmospheric hazards, have the potential to contain any hazard capable of causing death or serious physical harm. Prohibited Condition‐ Any condition in a permit space that is not allowed by the permit during the time when entry is authorized. Testing‐ The process by which the hazards are identified and evaluated. Testing includes specifying the tests that are to be performed in the permit space. CONFINED SPACE LOCATIONS Workplaces within this company have been surveyed to identify any confined spaces that may be present. Such locations are listed below.

as outlined in this and OSHA 1910.146 at no cost to the employees, maintain that equipment properly, and ensure that employees use that equipment properly.

g. When acting as a host employer for a contractor performing permit space entry work, shall:

1. Inform contractor of permit space entry program; 2. Apprise contractor of hazards of particular permit spaces and precautions and

procedures implemented for protection of employees in or near permit spaces;

3. Coordinate entry operations with contractor when both will be working in or near permit spaces and debrief contractor after entries.

4. Obtain information from contractor of permit program to be followed and coordinate multiple entry operations.

5. Debrief contractors of the hazards encountered or created. h. If necessary, reclassify a non‐permit confined space as a permit space when there are

changes in use or configuration. Employees:

a. Will not enter any permit required confined space unless specifically authorized by an entry supervisor and only in full accordance with this program and the OSHA standard.

b. Attend and complete any scheduled training required by his/her supervisor and this program.

c. When selected as an entrant, attendant, or entry supervisor, perform those duties as outlined in this program.

PERMIT REQUIRED CONFINED SPACE ENTRY PROGRAM AND PROCEDURES All permit required confined spaces will be identified by the safety manager. Exposed employees will be informed of such spaces through posting with warning signs or other equally effective means, such as facility maps or training. Only trained and qualified employees will be authorized as permit space entrants, attendant, or entry supervisor. No employee shall enter a permit space without having a properly completed entry permit signed by an entry supervisor. Entry permit procedures are as outlined below:

a. Entrants will obtain an entry permit from the entry supervisor prior to entry of the space.

b. The entrant will accomplish all pre‐permit actions required for entering the space, such as atmospheric testing, hazards control/elimination actions, have all required equipment on hand, provide for attendant and rescue services, etc.

c. Complete all items on the permit. d. The entry will be authorized and the permit will be signed only by an authorized entry

supervisor. If any item on the permit is checked as “NO” (meaning not yet completed or available), the permit will not be signed.

e. Entry may proceed. A copy of the entry permit will be places outside the confined space until the permit has been cancelled by appropriate personnel.

Testing and Monitoring

a. Test the space as necessary to determine if acceptable entry conditions exist before beginning entry operations. Initial testing of the atmosphere must be done from outside the confined space prior to any entry. If isolation of the space is infeasible because the space is large or part of a continuous system (such as a sewer), entry conditions will be continuously monitored where entrants are working.

b. Test or monitor the permit space as necessary to determine if acceptable entry conditions are being maintained during the course of entry operations.

c. When testing for atmospheric hazards, test first for oxygen, then for combustible gases and vapors, and then for toxic gases and vapors. Parameters for non‐hazardous atmosphere are:

1. Oxygen between 19.5 and 23.5 percent; 2. Flammability less than ten percent of the lower flammability limit (LFL); 3. Toxicity less than the permissible exposure limit (PEL).

An authorized attendant must be present and monitoring the entry at all times. The attendant will not be assigned any other duties that may interfere with his attendant duties. Attendant duties are outlined below. Equipment required for permit required confined space entry includes that equipment required for testing and monitoring; ventilating; communications between the entrant and attendant, and for summoning rescue; personal protection; lighting; barriers/shields for openings; means of ingress and egress; and any other equipment necessary for safe entry and rescue. Rescue and Emergency Services:

a. Non‐entry rescue is the preferred method for rescue of personnel from a permit required space. Employees will not enter a permit space for rescue unless they have been specifically trained and equipped for such rescue.

b. To facilitate non‐entry rescue, retrieval systems or methods shall be used whenever an authorized entrant enters a permit space, unless the retrieval equipment would increase overall risk of entry or would not be of value to any rescue. Retrieval system requirements are:

1. Each entrant shall use a chest or full body harness, with a retrieval line attached at the center of the back near shoulder level, or other appropriate point.

2. Other end of retrieval line shall be attached to a mechanical device or fixed point outside of permit space enabling immediate use. A mechanical device will be used to retrieve personnel from vertical type permit spaces more than five feet deep.

3. If injured entrant is exposed to any substance with a required MSDS or similar document, that MSDS or document will be made available to the medical facility treating entrant.

c. If rescue should become necessary, the attendant will: 1. Notify and summon the rescue team/service; 2. Attempt non‐entry rescue procedures to the extent possible by the

circumstances. 3. Monitor the situation and be ready to give rescuers information on how many

victims and their status, what hazards, chemical types, concentrations, etc. are present.

d. Only designated personnel will enter permit spaces for rescue purposes. Each designated rescue team member will be trained on:

1. Use of personal protective and rescue equipment necessary for making the rescue from the permit space;

2. Performance of assigned rescue duties and also that training required of authorized entrants;

3. Basic first‐aid and cardiopulmonary resuscitation (CPR). At least one member of the rescue team will hold current certification in first aid and CPR.

Each rescue team member will practice making permit space rescues at least once every 12 months, by means of simulated rescue operations and in spaces representative of the types of permit spaces from which rescue is to be performed.

Permits will be cancelled by the entry supervisor upon completion of the work, or when any prohibited condition arises. Permits cannot just be let to expire. Cancelled permits must be kept for the annual review. Program Review: Cancelled entry permits will be retained on file for at least one year. The Permit Space Program will be reviewed within one year of each entry using these cancelled permits to revise the program as necessary to ensure employees are protected from permit space hazards. A single review coverings all entries in the preceding year may be conducted. DUTIES OF THE ENTRANT, ATENDANT, AND ENTRY SUPERVISOR

Entrant a. Know the hazards that may be faced, including the mode, signs or symptoms, and

consequences of the exposure; b. Properly use equipment as required; c. Communicate with the attendant as necessary to enable the attendant to monitor

entrant status and to alert entrants of the need to evacuate the space. d. Alert the attendant whenever the entrant recognizes any warning sign or symptom of

exposure to a dangerous situation, or detects a prohibited condition. Attendant

a. Know the hazards that may be faced during entry, including the mode, signs or symptoms, and consequences of the exposure.

b. Is aware of possible behavioral effects of the hazardous exposure. c. Continuously maintain an accurate count and identity of authorized entrants. d. Remain outside the permit space during entry operations until relieved by another

attendant. e. Communicate with entrants as necessary to monitor entrant status and to alert entrants

of the need to evacuate. f. Monitor activities inside and outside space to determine if safe for entrants to remain in

space and orders evacuation when necessary. g. Summon rescue and emergency services when assistance for emergency exit from

permit space is necessary. h. Take the following actions when unauthorized persons approach or enter a permit

space while entry is underway. 1. Warn them to stay away, or exit immediately if they have entered. 2. Inform the entrants and entry supervisor if unauthorized persons enter the

permit space. i. Perform non‐entry rescues as specified by company procedure. j. Perform no duties that might interfere with their primary duty to monitor and protect

authorized entrants. Entry Supervisor

a. Know the hazards that may be faced during entry, including the mode, signs or symptoms, and the consequence of the exposure.

b. Verify that acceptable conditions for entry exist before endorsing the permit and allowing entry to begin.

c. Terminate the entry and cancel the permit when entry operations are complete or a prohibited condition arises.

d. Verify that rescue services are available and the means for summoning them are operable.

e. Remove unauthorized individuals who enter or who attempt to enter the permit space. f. Determine, whenever possible and at appropriate intervals, that acceptable entry

conditions are maintained TRAINING Only trained and qualified employees may be authorized as entrant, attendant, entry supervisor, or in‐house rescue team members. The training will establish proficiency in the duties required by this program so that the employee acquires the understanding, knowledge, and skill necessary for the safe performance of his/her duties. Training must be completed before employee is assigned duties under this program, before there is a change in assigned duties and, whenever a supervisor has reason to believe either that there are deviations from permit space entry procedures or inadequacies in the employee’s knowledge or use of this program. Supervisors will certify that this training has been accomplished. The certification will contain the employee’s name, signatures or initials of the trainers, and the dates of training. The certification will be kept on file. ALTERNATE ENTRY PROCEDURES Alternate entry procedures may only be used when the only hazard is an actual or potential hazardous atmosphere. If alternate entry procedures are used, no permits are needed, no attendant or supervisor is required, and rescue provisions need not be used. Training and a written certification are required. Conditions To Be Met To Qualify For Alternate Procedures:

a. The only hazard posed by permit space is an actual or potential hazardous atmosphere. (See Note after the Permit Space Reclassification section.)

b. Continuous forced air ventilation alone is sufficient to maintain safe permit space. c. Monitoring and inspection data that supports above demonstrations have been

developed and documented. d. If initial entry is necessary to obtain above date, it shall be performed in accordance

with this program.

e. Documented determinations and supporting data will be made available to entrants. Entry must be in accordance with the following requirements:

a. Any condition making it unsafe to remove an entrance cover shall be eliminated before removing the cover. When entrance covers are removed, the opening shall be promptly and effectively guarded.

b. Before entry, the internal atmosphere shall be tested with a calibrated direct‐reading instrument, for the following conditions in the order given:

1. Oxygen content: 19.5 – 23.5% 2. Flammable gases and vapors: <= 10% of LEL 3. Potential toxic air contaminants: < PEL

c. There may be no hazardous atmosphere within the space whenever any employee is inside the space.

d. Continuous forced air ventilation shall be used as follows: 1. Entry not permitted until hazardous atmosphere is eliminated. 2. Ventilation shall be directed to immediate areas where employees are or will be

present and will continue until all employees have left the space. 3. Air supply shall be from a clean source and may not increase hazards in space.

e. Atmosphere within space shall be periodically tested as necessary to ensure that ventilation is adequate. If hazardous atmosphere is detected during entry:

1. Each employee shall leave space immediately; 2. Space shall be evaluated to determine how hazardous atmosphere developed;

and 3. Measures must be taken to protect employees from hazardous atmosphere

before any subsequent entry. f. The entry supervisor will verify that the space is safe for entry and that all of the above

requirements have been met. Such verification will be in writing to include the date, location of the space, and the signature of the person providing the certification, and shall be made available to each employee before entry.

PERMIT SPACE RECLASSIFICATION A permit space may be reclassified as a non‐permit space:

a. If there are no actual or potential atmospheric hazards and if all hazards within permit space are eliminated without entry, space may be reclassified for as long as the non‐atmospheric hazards remain eliminated.

b. Hazards may be eliminated by such actions as purging or inerting tank/vessels of contaminants, emptying material from hoppers/bins, use of company lockout/tag

procedures for electrical/mechanical hazards. The control of atmospheric hazards through forced air ventilation does not constitute elimination of that hazard (it only controls the hazard: the preceding Alternate Entry Procedures must be used in such cases).

c. If entry is required to eliminate hazards, it shall be according to regulations and the space may be reclassified for as long as the hazards remain eliminated.

d. Entry supervisors will certify in writing that all hazards in permit space have been eliminated and make this document available to each entrant.

e. If hazards arise in declassified permit space, employee(s) shall exit and the employer shall determine whether to reclassify space.

NOTE: A combination of reclassification procedures and alternate entry Procedures (e.g. using lockout/tagout to eliminate a physical hazard, then continuous forced air to control an atmospheric hazard) may not be used together. Situations as such must be entered under the permit program.

WRITTEN PERMIT

The following information must be included in the written permit. The permit must be a standardized format for each entry. 1. The permit space to be entered 2. The purpose of the entry 3. The date and the authorized duration of the entry permit 4. The authorized entrants within the permit space, by name or by such other means 5. The personnel, by name, currently serving as attendants 6. The individual, by name, currently serving as entry supervisor, with space for signature or

initials 7. The hazards of the permit space to be entered 8. The measure used to isolate the permit space and to eliminate or control permit space

hazards before entry 9. The acceptable entry conditions 10. The results of initial and periodic tests, with the names or initials of the testers and when

the tests were done 11. The rescue and emergency services that can be summoned and the means for summoning

them 12. The communications procedures used by authorized entrants and attendants to maintain

contact during the entry 13. Equipment (such as personal protective equipment, testing, communications, alarm system,

and rescue equipment) to be provided for compliance with this section 14. Any other information whose inclusion is necessary in order to ensure employee safety (Attached to permit) Any additional permits, such as for hot work, that have been issued for work in the permit space.
