Site Visit Report #041 DATE: July 26, 2016

PROJECT: Lakewood High School Project: 2014-05 .

OBSERVATIONS: 1. Allied workers are installing section of east-west water main along south side of the driveway

immediately south of the Lakewood HS office, working from 11th eastward. 2. Concrete slab at Locker Rooms, west Commons Alcove, and toilet rooms at Athletics / Performing

Arts entry has been watered. Allied workers assembling hoses to bring water from wall hydrant near outside door to female locker room to water the slab.

3. North Classroom Wing, 2nd Floor, east, concrete was poured today; pour completed before 9:30. Workers using walk-behind power trowel machines were finishing slab

4. MTC representative was performing Ff and Fl testing in the Locker Rooms 5. Formwork construction underway at north side of Theater. 6. Workers picking up cut-off bits of rebar at east end of SCW – sections cut out for control joints. 7. The +/-80’ long truss at the roof-height change in Commons was erected today, beginning shortly

after 10 am. Earlier, columns along the west side of Commons were hoisted into place and bolted. 8. Noticed workers welding at various braced frames at west end of NCW, first floor. Also saw welding

in progress at SE corner of Ticket Booth. 9. Plumbers setting Locker Room shower drains (areas blocked-out during yesterday’s pour. 10. Electricians are laying conduit to Gyms and installing conduit which will pass up into CMU walls. 11. Center-West Parking: additional curbs have been formed, some more poured. 12. Stadium turf installation continues. “Field” of turf has been placed up to 20-yard line (from the

west), with most of seam at 25-yards complete; workers will soon be pulling turf over to reach 30 yards.

Concrete placement at NCW, 2nd Floor, east portion is complete.

Slab placed yesterday has been watered.
