INDICACIONES/CONTRAINDICACIONES

Indicaciones

Los pacientes que cumplen con los siguientes criterios deben ser considerados para este sistema:• Pacientes con amputaciones transtibiales.• Pacientes que mencionan falta de suspensión o inseguridad con su prótesis actual.• Pacientes que podrían beneficiarse de una mejor salud de la extremidad.• Pacientes que muestran una buena función cognitiva y que están "en sintonía" con el tema de ajuste de prótesis.• Pacientes con una destreza manual razonable.• Personas que acostumbran caminar en su barrio.• Pacientes con tejido excedente.

Contraindicaciones

Los pacientes con cualquiera de las siguientes condiciones no pueden ser considerados para este sistema:• Pacientes con amputaciones transfemorales.• Pacientes con una longitud del miembro residual menor a 5 pulgadas.• Pacientes con miembros residuales bulbosos.• Pacientes con heridas abiertas.• Pacientes que carecen de las habilidades para colocarse y quitarse una interfaz de gel, o de las habilidades para colocarse y

quitarse apropiadamente el sistema.• Pacientes que tienen una mala higiene del miembro residual o que no pueden o no quieren cumplir con las instrucciones de

2. Captura y modificación de la forma • Capture la forma sobre el liner y la calceta.• Reduzca el modelo positivo del 5 al 7 %.

COLOCACIÓN INICIAL

1. Dé la vuelta al liner Alpha Duo de forma que el logotipo quede en el interior. Coloque el extremo del liner al final del miembro. No importa la posición del logotipo. El logotipo puede quedar hacia el frente, hacia atrás o hacia un lado.

2. Enrolle cuidadosamente el liner hacia arriba sobre el miembro. Asegúrese de que no queden pliegues o burbujas de aire entre el liner y la extremidad. NO JALE el liner sobre la extremidad. Si lo hace, puede estirar el gel, provocar que se estire la piel y tener molestias.

3. Dé la vuelta a la calceta de gel One de forma que el lado de silicona quede hacia afuera. Coloque el extremo de la calceta volteada en el extremo del liner.

4. Enrolle la calceta sobre la extremidad de forma que el lado de silicona quede contra el liner. Asegúrese de que no queden pliegues o burbujas de aire entre la calceta de gel One y el liner.

CAPTURA Y MODIFICACIÓN DE LA FORMA

Capture la forma del miembro sobre el liner y la calceta de gel One con el sistema OMEGA® o mediante la creación manual del molde, de acuerdo con los criterios para el paciente y la evaluación del miembro residual.

Reduzca el modelo positivo del 5 al 7 % para que el peso se soporte en toda la superficie (la cantidad puede variar según la técnica de moldeado, la longitud del miembro y el tipo de tejido). Incluya una barra de rótula ligera, si lo desea, y un apoyo posterior. NO aumente el modelo en ningún lugar.

Después de determinar la alineación, fabrique un encaje según las instrucciones para la fabricación del sistema transtibial WillowWood One.

• Observe la trama de la calceta de gel One. El espaciado desigual de la trama indica presiones irregulares en el encaje.

• Revise si el encaje está apretado alrededor de los bordes de la rótula cuando la pierna está completamente extendida.

• El encaje ajusta correctamente si:

• El miembro entra en contacto con el extremo distal del encaje sin añadir calcetas sobre la calceta de gel One.

• Colocar una calceta de una capa sobre la calceta de gel One evita que el miembro entre en contacto con el extremo distal del encaje al soportar todo el peso.

3. Mientras el encaje flexible y la manga aún están húmedos, deslice la manga sobre el encaje flexible:

• El extremo más grande de la manga debe quedar hacia la parte superior y el extremo más pequeño con las pestañas de sellado debe quedar hacia la parte inferior.

• El borde distal de la manga debe estar aproximadamente 5 cm (2 pulg.) por debajo de la línea de recorte posterior del encaje.

5. Inserte el perno de brida a través del encaje flexible y el orificio de la conexión One.

Aplique adhesivo para tornillos Loctite® Blue 242® (o un producto equivalente) al perno de brida.

6. Con un encaje de 7 mm, instale la tuerca de la conexión One en el perno de brida. Apriete a 25 libras pulgada.

No apriete en exceso. Lije el perno de brida, si es necesario,

para que quede al ras con la tuerca de la conexión One.

7. Atornille la tapa de la conexión One con la junta tórica.

4. Inserte el encaje flexible con la manga One dentro de él en el encaje laminado. Alinee el orificio en el encaje flexible con el orificio de la conexión One en el encaje laminado. Rocíe ligeramente con alcohol las pestañas de la manga One o el interior del encaje si es necesario.

9. Si hay una bomba LimbLogic en el encaje, pídale al paciente que se ponga de pie en una posición en la que se esté soportando todo el peso, luego presione el interruptor de encendido/apagado para encender la bomba LimbLogic. Consulte las instrucciones de LimbLogic para obtener más detalles.

Uso de una loción, crema, polvo o jabón demasiado fuerte o irritante.

Indague si el paciente ha cambiado recientemente a un producto de limpieza dife-rente o si ha comenzado a usar alguna loción, crema o polvo que contenga aceites de hidrocarburos o grasas o aceites de origen animal (el jabón antibacterial ha causado irritaciones en la piel de algunas personas con amputaciones).

El liner o el encaje están sueltos, posi-blemente porque el miembro residual ha reducido su tamaño.

Ajuste el encaje o cambie a un liner de otra talla, estilo o grosor para corregir la holgura. Para confirmar que no se trata de una reacción alérgica, realice una “prue-ba de parche”: coloque una pieza del material del liner en otra parte del cuerpo y observe si hay reacción.

Irritación de la piel a lo largo del borde superior del liner.

El liner se está colocando sobre la extre-midad en lugar de enrollarse sobre esta, o el liner está demasiado ajustado.

Compruebe que el procedimiento de colocación sea el apropiado.

Heridas abiertas y piel dañada.

Pueden ser causadas por distintos pro-blemas, lo que incluye el ajuste del enca-je y variables específicas del paciente.

Atienda la causa específica de la situación y coloque una venda u otra envoltura apropiada para evitar el contacto directo entre la herida y el liner.

El sistema tiene fugas. Pestañas de la manga. Remplace la manga.

Los tornillos de la bomba están flojos o no están colocados.

Indications

Patients who meet the following criteria should be considered for this system:• Patients who are transtibial amputees.• Patients who comment about lack of suspension or feeling insecure in their current prosthesis.• Patients who potentially would benefit from better limb health.• Patients who exhibit high cognitive function and are “in tune” with prosthetic fit.• Patients with reasonable hand dexterity.• Community ambulators.• Patients with redundant tissue.

Contraindications

Patients with any of the following conditions may not be considered for this system:• Patients who are transfemoral amputees.• Patients with a residual limb length less than 5 inches.• Patients with bulbous residual limbs.• Patients with open wounds.• Patients who lack the ability to don and doff a gel interface or the ability to properly don and doff the system.• Patients who have poor residual limb hygiene or who are unwilling or unable to comply with the cleaning and disinfecting

instructions for the liner.• Patients with a combination of limb length and limb circumference that is unsuitable according to the chart below.

2. Shape Capture and Modification

• Capture the shape over the liner and the sock• Reduce the positive model 5-7%

• Follow the LimbLogic Prosthetist Instructions to attach the LimbLogic pump to the socket, charge the battery, and adjust system settings.

• Assemble the One Sleeve to the flexible socket

• Insert the flexible socket into the laminated socket

• Install the One Link

• Attach the Suction Pyramid.

INITIAL DONNING

1. Invert the Alpha Duo Liner so that the logo is on the inside. Place the end of the liner against the end of the limb. The position of the logo does not matter. The logo can face to the front, to the back, or to the side.

2. Carefully roll the liner up onto the limb. Make sure that there are no wrinkles or air pockets between the liner and the limb. DO NOT PULL the liner onto the limb. Pulling the liner may stretch the gel, causing pulling on the skin and discomfort.

3. Invert the One Gel Sock so that the silicone side is facing out. Place the end of the inverted sock against the end of the liner.

4. Roll the sock up onto the limb so that the silicone side is against the liner. Make sure that there are no wrinkles or air pockets between the One Gel Sock and the liner.

SHAPE CAPTURE AND MODIFICATION

Capture the shape of the limb over the liner and One Gel Sock using the OMEGA® System or hand casting, according to patient criteria and residual limb assessment.

Reduce the positive model 5-7% to obtain Total Surface Weight Bearing. (Amount may vary depending on casting technique, limb length and tissue type.) Include a slight patella bar if desired and a posterior shelf. Do NOT build up the model anywhere.

After determining the alignment, fabricate a socket according the WillowWood One Transtibial System Fabrication Instructions.

DEFINITIVE SOCKET

DIAGNOSTIC SOCKET

Fabricate a conventional diagnostic socket.

Apply the One Sleeve on the exterior of the socket as you would for a typical suspension sleeve.

Conduct static and diagnostic fittings, and modify the socket to correct any areas of discomfort or proper fit:

• Observe the weave of the One Gel Sock. Uneven spacing of the weave indicates uneven pressures in the socket.

• Check to see if the socket is tight around the edges of the patella when the leg is at full extension.

• The socket fits correctly if:

• the limb contacts the distal end of the socket without adding socks over the One Gel Sock.

• adding one single-ply sock over the One Gel Sock prevents the limb from contacting the distal end of the socket under full weight bearing.

3. While the flexible socket and sleeve are still damp, slide the sleeve onto the flexible socket:

• The larger end of the sleeve should be toward the top, and the smaller end with the sealing fins should be toward the bottom.

• The distal edge of the sleeve should be about 2” (5 cm) below the posterior trim line of the socket.

5. Insert the flange bolt through the flexible socket and One Link hole.

Apply Loctite® Threadlocker Blue 242® (or equivalent) to the flange bolt.

6. Using a 7 mm socket, install the One Link Nut onto the flange bolt. Tighten to 25 in lb.

Do not over-tighten.

Sand down the flange bolt if necessary so that it is flush with the One Link Nut.

7. Screw on the One Link Cap with O-Ring.

4. Insert the flexible socket with the One Sleeve on it into the laminated socket. Align the hole on the flexible socket with the One Link hole on the laminated socket. Lightly mist the One Sleeve fins or the inside of the socket with alcohol if necessary.

9. If there is a LimbLogic pump on the socket, have the patient stand up into a full weight-bearing position, then press the on/off switch to turn on the LimbLogic pump. Refer to the LimbLogic instructions for more details.

2. For ease of assembly, mist the outside of the flexible socket and the inside of the sleeve with alcohol.

8. Have the patient step into the socket and gently pull the sleeve up onto the residual limb. Make sure the patient does not tug on the sleeve, as tugging can damage the sleeve.

To prolong the life of the One Sleeve, avoid tugging on the silicone-only portion of the sleeve or poking that area of the sleeve with your fingers.

POSSIBLE ISSUES

New liner user is sweating excessively.

Body has not yet adjusted to the closed environment of the liner.

The perspiration should subside after several weeks of liner use.

Long-time liner wearer suddenly starts sweating.

Air is getting between the liner and the residual limb, possibly due to residual limb shrinkage.

Check the fit of the liner and socket. Tighten up the socket or change to a liner of a different size, style, or thickness.

Hole in the liner. Replace liner.

Skin irritation all over the residual limb.

Poor hygiene. Review proper cleaning, rinsing, and disinfecting procedures.

Use of a harsh or irritating lotion, cream, powder, or soap.

Check to see whether the patient has recently changed to a different cleaning product, or has started using any lotions, creams, or powders containing hydrocarbon oils or animal fats or oils. (For some amputees, antibacterial soap has caused skin irritations.)

The liner or the socket is loose, possibly due to residual limb shrinkage.

Tighten up the socket or change to a liner of a different size, style, or thickness to eliminate the looseness. To confirm that this is not an allergic reaction, perform a "patch test": apply a piece of liner material somewhere else on the body and look for a reaction.

Skin irritation along top edge of liner.

The liner is being pulled onto the limb instead of being rolled onto the limb, or the liner is too tight.

Review proper donning procedure.

Open wounds and non-intact skin.

Could be caused by a number of issues, including socket fit and specific patient variables.

Address the specific cause of the situation, and apply a bandage or other appropriate covering to prevent direct contact between the wound and the liner.

System leaks. Sleeve fins torn. Replace sleeve.

Holes in sleeve. Replace sleeve.

Pump screws loose or missing, Refer to instructions for screw installation.

ReturnsWillowWood will handle all One System returns. If you wish to return a One System product, follow these simple steps: 1. Be prepared to provide the following information:

• Account # or company name • Purchase order # and/or serial # • Quantity to be returned • Reason for return

2. Please call Customer Care at 1.800.848.4930 to get a Return Merchandise Authorization (RMA) number. This must be done before the product is returned.

3. When packing the product, place the RMA number prominently on the outside and inside of the package containing the product being returned.
