What does the core technology look like
and who owns it?
Many people in this room own the core technology needed to delivery digital health services.

Integrated point of care monitoring using an iPad
Patient at home.
Red Cross Volunteers = eyes and ears of the system.
Specialist Geriatric Nurse in a hospital with consultant back up. Also has contact with social services and patient’s GP.
Depending on what the patient’s data shows up their needs are assessed and s/he may get a primary care intervention or attention from a Geriatrician e.g. an urgent out-patient appointment or an urgent / emergency admission to hospital.
Two volunteers visit a frail man who lives with his frail wife. They carry out standard tests e.g. BP, weight, temperature , blood oxygen , blood sugar etc. these tests are with the supervising nurse within seconds and are recorded in a care plan which hangs off the back of the hospital EPR.
