Chronic nonmalignant pain is an enigmatic condition,subject to various interpretations and theories withrespect to its etiology and persistence. Seriouscontroversies have arisen with regard to certaindiagnoses, the role of psychological factors, thepotential for addiction, and the efficacy of chronic opioidtreatment. Management of the chronic pain patientdependent on prescription opioids is perhaps the mostdifficult and controversial area of pain management. Bypresenting opposing viewpoints, this debate willelucidate the science and the art of the use and misuseof opioids for chronic pain.
