1. Incident, Accident & Near miss

What is Incident?

Event that give rise to an accident or has the potential to lead to an accident.

Examples:

All Accidents as described below

All near misses as described below

What is Accident?

Unplanned/Undesired event giving rise to death, ill health, injury, damage or other losses to personnel or property.

Examples:

Trips or falls causing injury. Explosion or implosion of a pressurized vessel. Chemical spills. Injuries occurring while machining. Activities or equipment failure causing fire or

property damage.

Accident: Suspended load falling on

person(s)

What is Near Miss? An event which could have resulted in injury or loss

but did not in fact do so this time.

All Near Misses involve people & some type of action.

Examples:

Tripping over loose wire but managed to get

balanced

Fall of material near persons while hoisting

Slipping on wet floor but managed to get balanced

Job fixed in Chuck jaw of lathe flew off when

started

Near miss: Suspended load falling near persons

2. Safety – a matter of life - On Accidents and prevention - Sirdesai N R

Do not be lost in over confidence or complacency during the work that you are doing Do not say that you have tons of experience and regarding safety you have to fear or learn nothing Do not be absent minded and go to the wrong equipment or bay for working Do not take short cuts, defeat interlocks or hurry up to catch your bus in the evening Always follow the check lists so that you do not miss anything Interrupting work and taking phone calls during critical activities would be highly unwise Please take utmost care of safety as it is a matter of your life

Ignoring safety will lead to an accident which is terrible The condition of those electrocuted or suffer accidents is horrible Months are spent in hospitals getting treatments unbearable Then also they are extremely lucky if they somehow survive Please take utmost care of safety as it is a matter of your life

You are empowered to STOP unsafe work, an authority which you can anytime exercise Do not take any sort of calculated risks, life isn’t a game of dice Follow the safety systems correctly and if in doubt take your senior’s advice Remember always that for a calamity a small error, misunderstanding or slip can also suffice Please take utmost care of safety as it is a matter of your life

Come, let us together bring about a new safety culture Where things are anticipated in advance and untoward incidents do not occur This is the best gift we can offer each other, in our daily working life Please take utmost care of safety as it is a matter of your life

bungalow, Union holiday home & Inspection bungalow at Lonavala, Mulshi, Male guest

Conducted training on confined space, LOTO & Electrical safety to contact workers at

TPSDI Ambernath.

Fire Safety Audit survey was carried out at Trombay colony, Carnac, Dharavi, Parel,

Mahalaxmi, and Backbay.

Meeting was held with GDF Suez at MEPL Nellore for developing site specific procedures to

considering best procedures

6. Security Highlights

I. Trombay Training of permanent/MGB/Solaris ESM guards carried out regularly on behaviour/technical

aspects/ISPS /Drill etc in Trombay.

On 26 Aug, Mock Drill on evacuation was conducted by jetty Security staff under ISPS.

On 27 Aug, CEO & MD visits Trombay PH to inaugurate the new TPSDI facility.

I-Day parade celebrated with pomp & show.And tree planting carried out the saw day. Also

training briefing of Sec pers by head sec Jojobera.

A five KM walk was organised by Security to make the employees aware of fitness by

walking
