• Project Description- Briefly describe your project.• Goals/Desired Outcomes- Include a bulleted list of your goals.• Evidence of Impact- Include the evidence you have that your

actions affected your intended goals (or unanticipated goals).• Results- Describe what happened. Connect these results to

the evidence. • Effect- What was the affect on you as a person and teacher.
