Pharmacokinetics and tissue distribution of
amoxicillin plus clavulanic acid after oral
administration in man
Abstract
• Augmentin (875 amoxicillin and 125 mg potassium
clavulanate) was administered orally to patients with
chronic bronchitis. Samples of lung, tonsil, middle chronic bronchitis. Samples of lung, tonsil, middle
ear mucosa and prostate were obtained and tissue
concentrations of both compounds measured. Peak
levels of amoxicillin ranged from 0.87 micrograms/g
(tonsil) to 2.56 micrograms/g (lung) and of clavulanic
acid from 0.20 micrograms/g (prostate) to 0.56
micrograms/g (lung) between 3 and 4 hours after

• Because acute diarrhea is most often self-limited and caused by viruses, routine antibiotic use is not recommended for most adults with non-severe, watery diarrhea.

Rehydration Therapy
• The first step to treating acute diarrhea is
rehydration, preferably oral rehydration.

• The reduced osmolarity ORS decreases stool
outputs, episodes of emesis, and the need for
intravenous rehydration, without increasing
hyponatremia, compared with the standard hyponatremia, compared with the standard
ORS.

duplicated by mixing 1/2 teaspoon of salt, 6
teaspoons of sugar, and 1 liter of water.

ไม่ควรใช้ยาปฏิชีวนะในแผลสดจากอบุัตเิหตุต่อไปนี�
• ไมใ่ช่แผลจากสตัว์กดั/คนกดั
• แผลขอบเรียบ ทําความสะอาดงา่ย
• แผลไมล่กึถงึกล้ามเนื �อ เอน็ หรือกระดกู แผลไมล่กึถงึกล้ามเนื �อ เอน็ หรือกระดกู
• ไมม่ีเนื �อตาย
• ไมม่ีสิ)งสกปรกที)แผลหรือมีแตล่้างออกงา่ย
• ไมป่นเปื�อนสิ)งสกปรกที)มีแบคทีเรียมาก (เช่น อจุจาระ ปัสสาวะ นํ �าสกปรก เศษอาหาร)
• เป็นผู้ มีภมูิต้านทานโรคปกติ
ยาปฏชิีวนะที�ควรใช้
• ไดคลอ็กซาซิลลิน (Dicloxacillin) –เดก็ : 25-50 มก./กก./วนั แบ่งใหว้นัละ 4 ครั, ง
ก่อนอาหาร นาน 2 วนั ก่อนอาหาร นาน 2 วนั
–วยัรุ่นและผูใ้หญ่ : 250-500 มก. วนัละ 4 ครั, ง ก่อนอาหาร นาน 2 วนั
ยาปฏชิีวนะที�ควรใช้ • หากผูป้่วยแพ ้เพนิซิลลิน ควรใช ้อีรีโทรมยัซิน ชนิดนํ,า
หรือ ร็อกซิโทรมยัซิน
–เดก็เลก็ : อีรีโทรมยัซิน ชนิดนํ,า 30-50 มก./กก./วนั แบ่งใหว้นัละ 4 ครั, ง ขณะทอ้งวา่ง นาน 2 วนั แบ่งใหว้นัละ 4 ครั, ง ขณะทอ้งวา่ง นาน 2 วนั
–เดก็โต : ร็อกซิโทรมยัซิน 5-8 มก./กก./วนั แบ่งใหว้นัละ 2 ครั, ง ขณะทอ้งวา่ง นาน 2 วนั
–ผูใ้หญ่ : ร็อกซิโทรมยัซิน 150 มก. วนัละ 2 ครั, ง ขณะทอ้งวา่ง นาน 2 วนั
ควรใช้ยาปฏิชีวนะในแผลสดจากอบุัตเิหตุชนิดซับซ้อน ดงันี�
• สตัว์กดั/คนกดั
• มีเนื �อตายบริเวณกว้าง มีเนื �อตายบริเวณกว้าง
• มีสิ)งสกปรกติดอยูใ่นแผลล้างออกไมห่มด
• ปนเปื�อนสิ)งสกปรกที)มีแบคทีเรียมาก (เชน่ อจุจาระ ปัสสาวะ นํ �าสกปรก เศษอาหาร)
ยาปฏชิีวนะในแผลสด
ยาปฏชิีวนะที�ควรใช้
• โคอะม็อกซีคลาฟ (Co-amoxiclav)
• เดก็ (คํานวณจาก Amoxicillin) : 25-50 มก./กก./วนั แบง่• เดก็ (คํานวณจาก Amoxicillin) : 25-50 มก./กก./วนั แบง่ให้วนัละ 3 ครั �ง นาน 2 วนั
• วยัรุ่นและผู้ใหญ่ : 375 มก. วนัละ 3 ครั �ง หรือ 625 มก. วนัละ 2 ครั �ง นาน 2 วนั
ยาปฏชิีวนะในแผลสด
• หากไม่มี โคอะมอ็กซีคลาฟ และจาํเป็นตอ้งรักษาผูป้่วยทีC รพ.สต. อาจใช ้โอฟลอ็กซาซิน หรือ โคไทรมอ็ก ซาโซล (Cotrimoxazole) หรือ ไดคลอ็กซาซิลลิน ร่วมกบั โอฟลอ็กซาซิน นาน 2 วนั (Cotrimoxazole) หรือ ไดคลอ็กซาซิลลิน ร่วมกบั โอฟลอ็กซาซิน นาน 2 วนั

• หมายถงึ การใช้ยาปฏิชีวนะเพื)อป้องกนัการติดเชื �อในผู้คลอดทารกครบกําหนดทางช่องคลอดด้วยวิธีปกติ ซึ)งผู้คลอดเช่นนี �สว่นมากไม่ต้องใช้ยาปฏิชีวนะป้องกนัการติดเชื �อหลงัคลอดสว่นมากไม่ต้องใช้ยาปฏิชีวนะป้องกนัการติดเชื �อหลงัคลอด

• หญิงคลอดปกติครบกาํหนดทางช่องคลอดดว้ยวธิีปกติทีCโรงพยาบาลจาํนวนมากเกือบทุกรายไดร้ับยาปฏิชีวนะหลงัคลอด ขณะทีCยงัไม่หลกัฐานเชิงวชิาการระบุวา่การใชย้าปฏิชีวนะในภาวะนี, มีประโยชน์ปฏิชีวนะในภาวะนี, มีประโยชน์
• ยาทีCนิยมใช ้คือ amoxicillin กินหลงัคลอดนาน 5 ถึง 7 วนั หากการใชย้าดงักล่าวเป็นการป้องกนัการติดเชื,อหลงัหตัถการ กไ็ม่สอดคลอ้งกบัหลกัการใชต้า้นจุลชีพเพืCอป้องกนัการติดเชื,อ

• 1) การใชย้าปฏิชีวนะเพืCอป้องกนัการติดเชื,อ ควรใหย้าก่อนทาํหตัถการ
• 2) ระยะเวลาของการใชย้าปฏิชีวนะเพืCอป้องกนัการติดเชื,อไม่ควรนานกวา่ 24 ชัCวโมง ควรนานกวา่ 24 ชัCวโมง
• 3) amoxicillin ไม่น่ามีฤทธิT ต่อเชื,อทีCเป็นสาเหตุของการติดเชื,อทีCสมัพนัธ์กบัการคลอดซึC งมกัเป็นแบคทีเรียกรัมลบและ anaerobes ทีCสาํคญัคือการใชย้าดงักล่าวมีผลเสีย ไดแ้ก่ ผลขา้งเคียงของยา การชกันาํใหแ้บคทีเรียดื,อยา ค่าใชจ้่ายของยา และทารกไดร้ับยาปฏิชีวนะจากนํ,านมมารดา

• ไม่ควรใชย้าปฏิชีวนะในหญิงคลอดปกติครบกาํหนดทางช่องคลอดดว้ยวธิีปกติ
� กระบวนการทาํหตัถการเกีCยวกบัการคลอดและการดูแล� กระบวนการทาํหตัถการเกีCยวกบัการคลอดและการดูแลบาดแผลอยา่งเหมาะสมมีความสาํคญัทีCสุด
• ยาปฏิชีวนะอาจมีประโยชนใ์นรายทีCมีการฉีกขาดของฝีเยบ็ระดบัทีC 3 หรือ 4

• ยาปฏิชีวนะทีCอาจเลือกใช ้คือ cefazolin 1-2 กรัม ฉีดเขา้หลอดเลือดดาํ ครั, งเดียว ก่อนเยบ็แผลทีCฉีกขาดระดบัทีC 3 หรือ 4 หากแพ ้penicillin ควรใช ้clindamycin 600-900 มก. ฉีดเขา้หลอดเลือดดาํ ครั, งเดียวหรือ 4 หากแพ ้penicillin ควรใช ้clindamycin 600-900 มก. ฉีดเขา้หลอดเลือดดาํ ครั, งเดียว
• หากหญิงหลงัคลอดมีแผลฝีเยบ็อกัเสบติดเชื,อหรือติดเชื,อในอุง้เชิงกราน ควรแจง้หรือกลบัมาพบผูร้ักษา
