– Vessel listed 20° to port– Water intruded into the bilge– 100 gallons of motor oil in

the engine

• Significance:– Threat of sinking or capsizing– No power to the bilge pumps– OSLTF opened

– Hazmat release of Parachlorobenzotrifluoride from a container

– 2 people received chemical burns– Cause was unsatisfactory blocking

and bracing

• Significance:– Release of hazardous material– Personnel injured– Unsatisfactory blocking and bracing

found in 6 containers

– Vessel grounded and sank as the tide came in

– Vessel started to discharge– OSLTF opened for dive ops and

fuel removal

• Significance:– Submerged for duration of

response– 150 gal potential– Owner financially unable to pay

for response– OSLTF opened

agency took responsibility– Vessel had 30° list during defueling,

ops halted– Vessel refloated with the tide

• Significance:– 4800 gallon potential spill on a

State Park– 4 contractors hired with 5 vessels

on scene– Ballast concerns during defueling
