The following notes apply to the systems outlined herein:
1. The roof system evaluation herein pertains to above-deck roof components. Roof decks shall be in accordance with FBC requirements to thesatisfaction of the AHJ. Load resistance of the roof deck shall bedocumented through proper codified and/or FBCApproval documentation.
2. Unless otherwise noted,fastenersand stress platesfor insulation attachment shallbe as follows. 3. Unless otherwise noted, insulation maybe anyone layer or combination of polyisocyanurate, polystyrene, woodfiberboard, periite,GlasRoc Roof Board or gypsum-based roofboardthat meets the QA requirements
4. Minimum 2G0 psi, minimum 2-inch lightweight insulating concrete may besubstituted for rigid insulation board for System Type D(mechanically attached base sheet, bonded roof cover), whereby the base sheetfasteners are installedthrough the LWIC to engage the structural steel or concrete deck. Thestructural deckshall be of equal or greater configuration to the steel and concrete deck listings.
5. Unless otherwise noted, insulation adhesive application ratesare as follows. Ribbon or beadwidth isat the timeofapplication; the ribbons/beads shall expand as notedinthe manufacturer's published instructions.> Hot asphalt (HA): Full coverage at 25-30 ibs/square> Ashland Pliodeck (A-PD): Continuous 0.75inch wide ribbons, 12-inch o.c. Ribbons ofsubsequent layers shall beperpendicular tothose in the layer below.> Dow Insta-Stik (D-IS): Continuous 0.75 to 1 inch wide ribbons, 12-inch o.c.> Dow Spray-N-Grip (D-SG): Full coverage>
> MillenniumOne Step Foamable Adhesive (M-OSFA): Continuous 0.25 to 0.5-inch wide ribbons, 12-inch o.c.> Millennium PG-1 Pump Grade Adhesive (M-PG1): Continuous 0.5 to 0.75-inch wide ribbons, 12-inch o.c.> OMG OlyBond 500 or OlyBond Green (OB500): Continuous 0.75-inch wide ribbons,12-inch o.c. 6. Unless otherwise noted, all insulations are flat stockor taper board of the minimum thickness noted. Tapered polyisocyanurate at the following thickness limitations maybe substitutedwith the following MaximumDesign Pressure (MDP) limitations. In nocaseshall these values be used to 'increase' the MDP listings in the tables; ratherifMDP listing below meetsor exceeds that listed fora particular system inthe tables, thenthe thinner board listed below may be used as a drop-in for the equivalent thicker material listed in the table:
> Ashland Pliodeck (A-PD) @ 12-inch o.c.
> Ashland Pliodeck (A-PD) @ 6-inch o.c.
> Dow Insta-Stik (D-IS):

7. Bonded polyisocyanurate insulation boards shall be maximum 4 x 4 ft.
8. For mechanically attachedcomponents or partially bonded insulation, the maximum design pressure for the selected assembly shall meetor exceed the Zone 1 design pressure determined inaccordance with FBCChapter 16, and Zones 2 and 3 shall employ an attachment density designed by a qualified design professional to resist the elevated pressure criteria. Commonly used methods are RAS 117 and FM LPDS 1-29.Assemblies marked with an asterisk* carry the limitations set forth in Section 2.2.1.5.1(a) of FM LPDS 1-29 for Zone 2/3 enhancements.
9. Forfully bonded assemblies, the maximum design pressure forthe selected assembly shall meet or exceed critical design pressure determined inaccordance with FBC Chapter 16,and no rational analysis ispermitted.
10. For mechanically attached components over existing decks, fasteners shall be tested in the existing deck for withdrawal resistance. Aqualified design professional shali review the data for comparison to theminimum requirements for the system. Testing and analysis shall be in accordance with TAS105 or ANSI/SPRI FX-1.
11. For existing substrates ina bonded recover or re-roof installation, the existing roof surface or existing roof deck shall beexamined forcompatibility and bond performance with the selected adhesive, andthe existingroofsystem (for recover) shallbe capable of resisting project design pressures on itsownmeritto the satisfaction ofthe AHJ, as documented through field uplift testing inaccordance withASTM E907, FM LPDS 1-52,ANSI/SPRI IA-1 or TAS 124.
12. For RecoverApplications using System Type D,the insulation is optional; however, the existing roof system shall be suitable for a recover application.

The following notes apply to the systems outlined herein:
1. The roof system evaluation herein pertains to above-deck roof components. Roof decks and structural members shall be in accordance with FBC requirements to the satisfaction of the AHJ.Load resistance of the roof deck shall be documented through proper codified and/or FBCApproval documentation.
2. Base sheet fasteners shall be of sufficient length for minimum 0.75-inch penetration through the wood deck.
3. For mechanicaiiy attached components, the maximum design pressure for the selected assembly shaii meet or exceed the Zone 1 design pressure determined in accordance with FBC Chapter16, and Zones 2 and 3 snail employ an attachment density designed by a quaiifinH ripsign professional fo rpsisf the eievaipd pressure criteria. Commonly used methods are RAS117 ai\d FiVILPDS 1-29. Assemblies marked with an asterisk* carry the limitations set forth in Section 2.2.1.5.1(a) of FM LPDS 1-29 for Zone 2/3 enhancements.
4. For fully bonded assemblies, the maximum design pressure for the selected assembly shall meet or exceed critical design pressure determined in accordance with FBC Chapter 16, and norational analysis is permitted.
b. For mechanicaiiy attached components over existing decks, fasteners shaii be tested in the existing deck for withdrawal resistance. A qualified design professional shaii review the data forcomparison to the minimum requirements for the system. Testing and analysis shall be in accordance with TAS 105 or ANSI/SPRI FX-1.
6. For existing substrates in a bonded recover installation, the existing substrate shall be examined for compatibility and bond performance with the selected adhesive to the satisfaction of theAHJ.
7. Unless otherwise noted, refer to the following references for bonded base, ply or cap sheet applications.

members shall be in accordance with FBC requirements to the satisfaction of the AHJ. Load resistance of theroofdeckshall be documentedthrough proper codified and/or FBC Approval documentation.
For recover installations, the existing roof shall be examined in accordance with FBC 1510.
For mechanically attached components, the maximum design pressure for the selected assembly shall meet orexceed theZone 1 design pressure determined in accordance with FBC Chapter 16. Zones 2 and 3shall employan attachment density designed by a qualified design professional to resist the elevated pressure criteria.Commonly used methods are RAS 117 and FM LPDS 1-29. Assemblies marked with an asterisk* carry thelimitations set forth in Section 2.2.1.5.1(a) of FM LPDS 1-29 for Zone 2/3 enhancements.
5.7 For fully-adhered components, the maximum design pressure for the selected assembly shall meet or exceedcritical design pressure determined in accordance with FBC Chapter 16. No rational analysis is permitted forthese systems
5.8 For mechanically attached components over existing roof decks, fasteners shall be tested in the existing deckforwithdrawal resistance. Aqualified design professional shall review the data for comparison to the minimumrequirementsfor the system. Testing and analysis shall be in accordance with TAS 105or ANSI/SPRI FX-1.
5.9 For bonded components over existing substrates in a re-roof (tear off) installation, the existing deck shall beexamined for compatibility with the adhesive to be installed. If any surface conditions exist that bring systemperformance into question, field uplift testing in accordance with ASTM E907, FM LPDS 1-52 or TAS 124 shall beconducted on mock-ups of the proposed new roof assembly.
5.10 Metal edge attachment (except gutters), shall be designed and installed for wind loads in accordance with FBCChapter 16 and tested for resistance in accordance with ANSI/SPRI ES-1 or RAS 111, except the basic windspeed shall be determined from FBC Figure 1609.
5.11 All products in the roof assembly shall have quality assurance audit in accordance with the FBC and l-'.A.C. Rule61G20-3.
6. INSTALLATION:
6.1 RESISTO Modified Bitumen roof systems shall be installed in accordance with RESISTO published installationinstructions, subject to the Limitations / Conditions of Use noted herein.
6.2 System attachment requirements for wind load resistance are set forth in Appendix 1. "MDP" •• MaximumDesign Pressure is the result of testing for wind load resistance based on allowablewind loads, and reflects theultimate passing pressure divided by 2 (the 2 to 1 margin of safety per FBC 1504.9 has already been applied).Refer to FBC 1609 for determination of design wind loads.
7. BUILDING PERMIT REQUIREMENTS:
As required by the Building Official or Authority Having Jurisdiction in order to properly evaluate the installation ofthis product.

5.2 This Evaluation Report is not lor use in the HVHZ
5.3 Fire Classification is not part of this Evaluation Report; refer to current Approved Roofing MaterialsDirectory for fire ratings of this product.
5.4 Wind Classification:
5.4.1 All shingles noted herein are Classified in accordance with FBC Tables 1507.2.7.1 and R90S.2.6.1 to ASTM
D3161, Class F and/or ASTM 07153, Class H, indicating the shingles are acceptable for us in all wind zonesup to Vas(i = 150 mph (VU|. - 194 mph). Refer to Section 6 for installation requirements to meet this windrating.
5.4.2 Presidential Accessory, Accessory for Hatteras, Shangle Ridge, Shadow Ridge, Cedar Crest, NorthGate Ridgeand NorthGate Accessory hip & ridge shingles have been evaluated in accordance with ASTM D3161, ClassF. All except NorthGate Ridge and NorthGate Accessory require use of BASF Sonolastic N^ J. adhesive orHenkel PL' Polyurethane Roof &. Flashing Sealant, applied as specified in manufacturer's applicationinstructions, for use in wind zones up to Vi1sd -• 150 mph (Vl]lt = 194 mph).
5.4.3 Classification by ASTM D715S applies to exposure category B or C and a building height of 60 leet or less.Calculations by a qualified design professional are required for conditions outside these limitations.Contact the shingle manufacturer for data specific to each shingle.
5.4.3.1 Analysis in accordance with A.STM D7158 indicates the measured uplift resistance (Ri) lor ihe CertainTeedasphalt roofing shingles listed in Section 4.1 through 4.6 (except Presidential Solaris™) exceeds thecalculated uplift force (F,-) at a maximum design wind speed of Vil5C1 = 150 mph (Vlji: - :l94 mph) forresidential buildings located in Exposure D conditions with no topographical variations (fiat terrain) havinga mean roof height less than or equal to 60 feet.
The shingles are permissible under Code for installation in these conditions using v. he installationprocedures detailed in this Evaluation Report and CertainTeed minimum requirements, subject tominimum codified fastening requirements established within any local jurisdiction, which shall takeprecedence.
5.5 All products in the roof assembly shall have quality assurance audits in accordance with the Floiida BuildingCode and F.A.C. Rule 61G20-3.
Installation:
6.1 Roof deck, slope, underlayment and fasteners shall comply with FBC 1507.2 / R905.2 and the shinglemanufacturer's minimum requirements.

6.2 Installation of asphalt shingles shall comply with the manufacturer's current published instructions, usingminimum four (4) nails per shingle in accordance with FBC 1507.2.7 or Section R905.2.6 and the minimumrequirements herein.
6.2.1 Fasteners shall be in accordance with manufacturer's published requirements, but not less than FBC1507.2.6 or R905.2.5. Staples are not permitted.
6.2.2 Where the roof slope exceeds 21 units vertical in 12 units horizontal, use the "Steep Slope" directions.
6.3 CertainTeed asphalt shingles are acceptable for use in reroof (tear-off) or recover applications, subject to thelimitations set forth in FBCSection 1510 and CertainTeed published installation instructions.

Wind Classification:
All shingles noted herein are Classified in accordance with FBC Tables 1507.2.7.1 and R905.2.6.1 to ASTMD3161, Class Fand/or ASTM D7158, Class H, indicating the shingles are acceptable for us in all wind zonesup to Vasd = 150 mph (VLI|t = 194 mph). Refer to Section 6 for installation requirements to meet this windrating.
Presidential Accessory, Accessory for Hatteras, Shangle Ridge, Shadow Ridge, Cedar Crest, NorthGate Ridgeand NorthGate Accessory hip & ridge shingles have been evaluated in accordance with ASTM D3161, ClassF. All except NorthGate Ridge and NorthGate Accessory require use of BASF Sonolastic NP 1 adhesive orHenkel PL® Polyurethane Roof & Flashing Sealant, applied as specified in manufacturer's applicationinstructions, for use in wind zones up to Vasd = 150 mph (Vul! = 194 mph).
Classification by ASTM D7158 applies to exposure category Bor Cand a building height of 60 feet or less.Calculations by a qualified design professional are required for conditions outside these limitations.Contact the shingle manufacturer for data specific to each shingle.
Analysis in accordance with ASTM D7158 indicates the measured uplift resistance (RT) for the CertainTeedasphalt roofing shingles listed in Section 4.1 through 4.6 (except Presidential Solaris™) exceeds thecalculated uplift force (FT) at a maximum design wind speed of Vasd = 150 mph (Vuit = 194 mph) forresidential buildings located in Exposure Dconditions with no topographical variations (flat terrain) havinga mean roof height less than or equal to 60 feet.
The shingles are permissible under Code for installation in these conditions using the installationprocedures detailed in this Evaluation Report and CertainTeed minimum requirements, subject tominimum codified fastening requirements established within any local jurisdiction, which shall takeprecedence.
All products in the roof assembly shall have qualityassurance audits in accordance with the Florida BuildingCode and F.A.C. Rule 61G20-3.

Underlayment shall be acceptable to CertainTeed Corporation and shall hold current Florida StatewideProduct Approval, or be Locally Approved per Rule 61G20-3, per FBC Sections 1507.2.3, 1507.2.4 orR905.2.3.
Installation of asphalt shingles shall comply with the manufacturer's current published instructions, usingminimum four (4) nails per shingle in accordance with FBC 1507.2.7 or Section R905.2.6 and the minimumrequirements herein.
Fasteners shall be in accordance with manufacturer's published requirements, but not less than FBC1507.2.6 or R905.2.5. Staples are not permitted.
Where the roof slope exceeds 21 units vertical in 12 units horizontal, use the "Steep Slope" directions.
CertainTeed asphalt shingles are acceptable for use in reroof (tear-off) or recover applications, subject to thelimitations set forth in FBC Section 1510 and CertainTeed published installation instructions.
