Complete Repair Order Form (Page 2) This form provides us with all the shipping, billing and repair information to help us provide you with better service. - Please ensure that all required fields are completed - Before shipping, please email or fax the completed Repair Order Form before to ensure the quickest turnaround 2. Complete Proforma Invoice (Page 3) This form provides Canadian Customs with information on the nature of the goods being returned. - In the quantity fields, please enter the number of individual units being returned. Calculations will be performed automatically. NOTE: If returning a single antenna or base, include it as one RMU - Complete the company information on the bottom of page 3. 3. Complete NAFTA Certificate (Page 4) The NAFTA certificate shows to Customs that the goods returning were made in Canada and should be returned duty free. - Please complete the company information on the top of the page. Once the above forms are complete, 3 copies of each must accompany the shipment. Please place them on the outside of the box to make them accessible by the courier and customs. 4. This form provides us with all the shipping, billing and repair information to help us provide you with better service.

- Please ensure that all required fields are completed - Before shipping, please email or fax the completed Repair Order Form before to ensure the quickest turnaround 2.
