Team/Player Information

Team Investment: $400 (minimum 10 players, maximum 16) No less than 3 girls playing in the field at all times. Investment includes team t-shirts, game balls.

Payment Options:

1) Mail completed team registration form along with TOTAL team registration fee to:

2) Print, Scan and email registration form to [email protected], and receive an invoice to pay via PayPal.

Registration and t-shirt pick up begin at 7am and will continue throughout the first round until 12 noon. Play will be all day- championship game will be at 8pm. Shirts will only be given for those who have signed off on their waiver forms.

PLEASE NOTE: IT IS THE CAPTAINS RESPONSIBILITY TO ENSURE ALL WAIVERS AND FORMS ARE HANDED IN ON TIME.

Schedules will be emailed to the team captains one week prior to the event, and posted online via the Suzy Blacks website at www.suzyblacks.com.
