POSITION DESCRIPTION
Position Title: Team Leader, Aged Care
Award/classification: SACS Award -Dependent upon qualifications and levels of
experience
Employment type (full time, EFT): 0.6 EFT Fixed term until 30th June 2017
Additionally Dianella has an extensive salary packaging
program details of which are available on request.
Position No:
Location: Team Leader is expected to move across three primary sites;
Broadmeadows, Meadow Heights and Craigieburn however
staff may be transferred to other location in Northern Melbourne
depending on space and service demand.

POSITION SUMMARY
The Aged Care program assists older people, people with disabilities and carers living in the local area to;
increase and/or maintain independence and well-being
provide respite
through the provision of culturally appropriate state and federally funded programs.
The primary aim of the program is to support client directed care by providing opportunities for clients and
carers to live as valued and participating members of the community.
The Team Leader plays a pivotal role is overseeing and supporting staff, clients and carers in the planning,
delivery and evaluation of an engaging, meaningful aged care program.
DUTIES AND RESPONSIBILITIES Client management
o Support staff in the planning, implementation and evaluation of a client directed aged care program
o Assess new and existing clients and carers as required
o Triage and manage clients/carers according to client/carer goals
o Review client attendance, client/carer needs and management on a quarterly basis
o Conduct team meetings on a monthly basis
o Perform duties within the scope of practice of the role, and according to the clinical privileges
confirmed through the verification of credentials (qualifications, registration and professional
competencies).
Staff management
o Supervision and management of all staff, including organising rosters and approving leave
o Build capacity of staff, including access to formal and informal training and professional development
activities
o Conduct annual performance appraisals
Program management
o Coordinate the aged care program to ensure the delivery of services in line with service agreements,
legislation, Dianella policies and procedures and accreditation requirements
o Support the effective financial management of the program including purchasing of food and
resources and management of petty cash
o Assist with reporting related to funding for the program

o Support staff to ensure accurate statistical reporting and maintenance of client files
o Ensure relevant food handling regulations are adhered to and Occupational Health and Safety (OH
and S) standards are met
o Flexibility to carry out all staff duties to cover staff duties, as required
o Lead and support staff in the delivery of quality improvement activities. This may include participation in
clinical audits, conducting client satisfaction surveys and achievement of client outcomes.
o Report incidents, risks, compliments and complaints as per Dianella Community Health policy.
KEY POINTS TO NOTE
o Occasional after hours and weekend work may be required.
o This position requires outreach work to be undertaken within the Dianella catchment.
o The position is subject to available funding
o Travel between sites, clients’ homes, and other community locations is required
RELATIONSHIP
Internal
Promote and integrate the Aged Care programs within Dianella Community Health
Work closely with the Volunteer and Transport Coordinators to ensure the effective and coordinated
delivery of services
Promote and maintain a multidisciplinary team approach in the provision of services to clients and
carers
Liaise with the Branch Manager, Aged Care regarding client, carer and staff needs
External
Provide professional and timely advice and assistance to clients, carers and external stakeholders
Maintain an understanding of the health needs of the community and the role of Dianella in meeting
those needs
Promote and develop relationships with relevant associated Service agencies and staff
MAINTAIN PROFESSIONAL SKILLS AND QUALIFICATIONS
o Maintain professional accreditation as required and fulfil the requirements of the Association including
maintaining their code of ethics or tailor appropriately
o Continually upgrade skills and knowledge through attending seminars and workshops and reading
professional journals and newsletters
AUTHORITY
The occupant of this position has the authority to Team Leader – refer to Delegation Manual.
Other relevant information o Current Victorian Driver’s licence
o Valid Working with Children’s Check (valid for 5 years from the date of issue)
o Satisfactory Police Check (valid for 3 years from the date of issue)

KEY SELECTION CRITERIA
Essential Qualifications and Registrations: Certificate IV in Leisure and Health
Minimum of 4 years experience planning, implementing and evaluating aged care and/or disability
programs
Posses computer skills necessary to use windows based programs
First Aid level 2
Food handling certificate ( or willingness to undertake same)
Experience managing teams
Desirable Qualifications and Registrations:
Cert III in Aged or Community Care
Additional training in dementia management
Experience in leading/ monitoring quality and safety improvement activities
Essential Qualities/Experience
Commitment to creating fun and laughter in your programs
Strong commitment to client and carer enablement and well-being
Experience in managing staff and/or teams
Experience working with a range of health professionals
Well developed verbal and written communication skills
Desirable Qualities/Experience Understanding of client directed care
Experience working with clients and carers from culturally diverse backgrounds
Experience working with clients with dementia
