Clever Care NOW provides mobile home nursing,social support and patient transport services forthe growing needs of the community.

We also offer business and community healthchecks, in home mobile physiotherapy and preand post operative nursing care.

Our connections across the community servicessector allows us to refer to other community andallied health services including nutrition andmeals, community transport, home modificationsand more.

PATIENT TRANSPORT

We strive to ensure our community has themobility and freedom to connect, stay social andparticipate in everyday activities or experiences.

Our non emergency patient transport serviceassists people who are frail, aged, wheelchair orbed bound to attend appointments, outings andfamily events such as weddings, funerals or end oflife experiences.

The fully equipped, airconditioned and speciallymodified vehicles and equipment offercomfortable door to door travel across Sydneyand interstate.

Mobility aids and equipment are available toensure the most comfortable and tailored care isdelivered during transportation.

Our thoughtful and caring qualified nursingprofessionals are on-hand during each trip forassistance if needed.

OUR MISSION

HOSPITAL TRANSPORT

Clever Care NOW provides a reliable, safe and flexible serviceto transport people to and from hospital pre admissions andfollowing discharge.

If needed, our home nursing team is available to providecontinuity of care at home during recovery.

Upon discharge we can offer six weeks post acute nursingcare without charge. We also offer nursing care throughapproved Aged Care Package providers and to privatepatients.

MEDICAL APPOINTMENTS

Our nurses can liaise with doctors, specialists and healthprovider clinics to ensure ample time is scheduled to arrive atappointments on time.

The door to door service allows travel to and from home,aged care facilities or nursing homes to the chosen healthprofessionals.

There are many things that make a weddingspecial and none more so than having agrandparent, elderly or wheelchair bound loved onewith you on the day.

We can transport grandparents or elderly loved onesto weddings in our specially modified vehiclesaccompanied by professional and qualified nurses tooffer extra assistance if required.

We plan the assisted travel with the bride, groom orfamily to ensure the utmost care is taken for theloved one’s travel, access to the venue and ongoingcare throughout the celebration if required.

We can travel across Sydney, New South Wales andinterstate.

Weddings

Farewelling family or friends at funerals isan emotional time.

We help families ensure their loved ones can be partof that time, remembering and honouring thosemost dear to them.

Our caring staff provide transport and offersupport during the time of bereavement.

Social Outings Being socially active and participating in thecommunity is vital for overall health and wellbeing.

Our patient transport service is available forshopping trips, lunch, dinner and other socialoutings.

Ask us about our ad hoc social outing ‘stand by’ ratesif time slots become available.

Funerals

We offer transport for those nearing the endof their life to visit family or a special place.

We can help make their final wish, one to becherished by all.
