In this short period of time you and I have experienced many changes in our profession. To name a few: The evolution of the Future of Nursing Campaign The implementation of the Affordable Care Act (ACA) The inception of collaborative alliances with allied health professionals The growth and influence of social media. Your Board of Directors has been and continues to be hard at work guiding you in this time of change and of great opportunity. Leadership skills, credibility, analytical skills and a sense of urgency have been manifested as it became apparent the future is going to be different from the past. Understanding and accepting these changes will require patience, correct and current information and a forum whereby one can participate in open dialog. Building alliances among our nursing associations and professional allied healthcare colleagues will lead to environments more amenable to meeting the needs of our patients. Your Board is participating in multiple alliances looking for ways to best address these changes in the healthcare system. The changes are coming quickly and the sense of urgency at finding the correct solutions NOW is present. Whether the issue is scope of practice, legislative or in the education sector, ANA\C is in the forefront representing you. I, as your president, am most pleased at the feedback I receive from you the members. We are a big state with multiple individual personalities that need to be taken into account when looking for solutions. ANA\C is working diligently to develop strategies which will ultimately result in a vision of practice barriers being removed and the empowerment of nurses a reality. Seems like a big order. Yes it is but if one takes small steps to begin with and experience short term wins nothing is impossible. Here are a few examples I want to share with you. Your Ballot Committee is hard at work looking for future leaders. I also invite you to support your Ballot Committee by running for an office. The consent form is on the website www.anacalifornia.org I am confident the Ballot Committee will find those leaders willing to run for office. I know these future leaders will commit to continuing the work of ANA\C. The ANA\C General Assembly has moved to an annual event to provide a more frequent way for members to gather for networking. The Quad Council, formed with the Association of California Nurse Leaders (ACNL), brings the Tri-Council concept down to the State level and Melanie Krupa, RN your Education Director represents you. Through the California Action Coalition and Gayle Sarlatte, RN, ANA\C is addressing new nursing roles— Care Coordinator role and Informatics Technology (IT) specialist are two examples. At the national level the National Council of State Boards of Nursing (NCSBN) deals with the Consensus Model and regulatory issues of the model. Workplace issues are dealt with through the California Institute for Nursing and Healthcare (The Institute) of which ANA\C participates at the advisory board level. The Institute deals with issues within the state educational system and job placement. ANA\C was a major sponsor for the Health, Heathcare and Economic Development (AHEAD) conference and Elissa Brown, RN, Director of Practice represented you as a presenter at the first ever two day conference. Dr. Dianne Moore, Vice President represented you at Certification Day at Cedars- Sinai Medical Center. Visibility at these venues is opening doors for ANA\C and increasing our credibility as The State Nursing Association. I personally, along with my Board, want to thank you for your continued support as we move through this maze of change into a world of opportunity for all nurses. OnMarch23,2010,PresidentObamasignedtheAffordableCareAct.Thelawputsin place comprehensive health insurance reforms thatwill roll out over four years andbeyond.

OVERVIEW OF THE HEALTH CARE LAW2010: A new Patient’s Bill of Rights goes into effect, protecting consumers from theworst abuses of the insurance industry. Cost-free preventive services begin for manyAmericans.2011:PeoplewithMedicarecangetkeypreventiveservicesforfree,andalsoreceivea50%discountonbrand-namedrugsintheMedicare“donuthole.”2012:AccountableCareOrganizationsandotherprogramshelpdoctorsandhealthcareprovidersworktogethertodeliverbettercare.2013:OpenenrollmentintheHealthInsuranceMarketplacebeginsonOctober1st.2014: All Americans will have access to affordable health insurance options. As the first open enrollment period came to a close March 31, 2014, the overalloutcomewaspositiveandthePresidenthadthefollowingmessage;

end.Andthisafternoon,weannouncedthat7.1millionAmericanshavenowsignedupfor

privateinsuranceplansthroughthenewHealthInsuranceMarketplaces.7.1millionThatdoesn’tcountthemorethan3millionyoungadultswhohavegainedinsurance

under this law by staying on their families’ plans. It doesn’t count the millions morewhohavegottencovered through theexpansionofMedicaidand theChildren’sHealthInsurance Program. It doesn’t include themore than 100million folkswho now havebettercare--whoarereceivingadditionalbenefits,likemammogramsandcontraceptivecare,atnoextracost.

Now, millions of our fellow Americans have the comfort and peace of mind thatcomeswithknowingthey’renolongerleavingtheirhealthandwell-beingtochance.Formanyofthem,qualityhealthinsurancewasn’tanoptionuntilthisyear--maybebecausethey couldn’t afford it, or because a pre-existing condition kept them locked out of adiscriminatorysystem.

Today, that’s changed. And while our long-broken health care system may not becompletelyfixed,it’swithoutquestionalotbetter.That’ssomethingtobeproudof--andthere’snogoodreasontogoback.

Regardlessofyourpolitics,oryourfeelingsabouttheAffordableCareAct,millionsmoreAmericanswithhealthcoverageissomethingthat’sgoodforoureconomyandourcountry.

At the end of the day, that iswhat this law -- and the other reformswe’re fightingfor,froma21st-centuryimmigrationsystemtoafairerwageforeveryAmericanwho’swillingtoworkforit--areallabout:

TheAmericanNursesAssociationCaliforniahas joined abroad and bipartisan coalition of nurses, doctors, communityhealth centers, hospitals, local governments, public safety,business and labor unions to fight a November 2014 ballotproposition drafted by trial lawyers out to profit frommalpractice lawsuits. This ballot measure was carelesslythrowntogetherwithnoinputfromthehealthcarecommunity.Ifpassed,itwillincreasecosts,threatentheprivacyofpersonalprescription drug information, and will jeopardize people’sabilitytoseetheirtrusteddoctors.

The measure’s main provision will quadruple the non-economic damages cap on California’s successful MedicalInjury Compensation Reform Act (MICRA), the law thatgoverns legalproceedings if someone is injured in amedicalprocedure. This single change will triple trial lawyers’ legalfeesinthenon-economicdamagesportionofmedicallawsuitsfiledagainsthealthcareprovidersandhospitals.(Note: Under MICRA, economic damages for past and future lost wages, past and future medical costs, and punitive damages are unlimited.)

Iftriallawyersgettheirway,malpracticelawsuitsandjuryawardswillskyrocket.Someonewillhave topay thosecosts.Andthatsomeone…isyou.

The measure contains two other unrelated provisionsdealing with drug testing and prescription drug databaseswhichwereintentionallyincludedbythebackersinanattempttomisleadvotersbytakingthefocusoffthelawsuitprovisions.The prescription drug database poses serious privacy risksfor California patients. And the drug testing provision wasincluded for political, not policy reasons. In fact, one of themainsupportersofthepropositionadmittedtotheLos Angeles Times thatthedrugrulesareintheinitiativebecausetheypollwell,callingtheseprovisions“theultimatesweetener.”

Here’s why the Lawyers’ Health Care Lawsuit Ballot Measureshouldberejected:Costly for Consumers

According to a study by California’s former LegislativeAnalyst, the new lawsuits and massive payouts under thispropositionwillincreasehealthcarecostsacrossallsectorsby$9.9billionannually.Thatamountstomorethan$1,000ayearinhigherhealthcostsfortheaverageCaliforniafamily.

AndCalifornia’s current independentLegislativeAnalyst’sOffice (LAO)warns the proposition could increase state andlocal government medical liability and health care costs by“hundredsofmillionsofdollarsannually,”placingtheburdenofthisadditionalcostonalltaxpayers.

Threatens People’s Personal PrivacyThis measure forces doctors and pharmacists to use a

Anacquaintance toldmerecently thatshe teachesreligionandhasastudentwhoisa nursing student.During class the student pondered, did her teacher haveParkinson’sdisease?Howwould you recognize Parkinson’s disease (PD)?Would you use the age-oldcriteriaofaslowshufflinggait,cogwheelrigidity,blankfacialexpressionortremor?Our older colleagues may have learned these criteria along with the pathophysiology,manifestations, and treatment of PD. In fact, much has been discovered in the pastseveraldecades,muchofwhichdeviatesfromwhatwewerepreviously taught.But…ifyouweretoadmitapatientwith,perhaps,abrokenhiporanupperrespiratoryorurinarytractinfection,andyounoticedintheHistorythatyourpatienthadPD,wouldyoumakecertainnecessaryconnections?HowwillPDaffecttheperson’shospitalization?

AnursecaringforonewithPDmustbecognizantofthecomplexitiesofthediseasewhichaffectactivitiesofdailyliving.Thecarecanbecomplicatedrelatedtomotorandnon-motoraswellasneuropsychiatricdiseasemanifestations,andmedicationsandtheirsideeffects.

PD is the second most common neurodegenerative disorder of adults yet there islittletonocontentinnursingschoolcurriculumdevotedtoPD.Oneofthemorerecentfindings is that the decrease or lack of dopamine producing cells is not limited to aspecific area, the substantia nigra, aswas previously thought. The pathophysiology ofPDisfarmorecomplex, involvingLewybodies, theproteinalpha-synuclein(themajorcomponentoftheLewybody).Alpha-synucleinispresentthroughout the central nervous system. There seems to be a protein misfolding problem or a change in its bindingcapacities leading eventually to neuron death. Therefore, as the disease progresses,we first seebrainstemeffects, thenmidbrain, and finally the cortexwhereweobservechangesphysically,cognitively,emotionally,andbehaviorally.

Patientswill usually presentwith slowness or tremor.However,more common nonmotor or prodromal symptoms are constipation, fatigue, apathy, depression, pain,stiffness. Sounding similar to several othermaladies such as aging, thyroid problems,osteoarthritis,MultipleSclerosis,ChronicFatigueSyndrome,Lymedisease,thecommoncold or flu?Other early signs of PD have been identified, such as decreased sense ofsmell.Evenaslighttremorofalittlefingercanbeoverlooked.

Yet, itmaybe that early diagnosis of PD is extremely important.Wedo know thatthe non-pharmacological treatments such as PT,OT aremore effective if begun early.Withthebesttreatmentonecanfunctionforyears,decades.ThemostcriticalreasontoidentifyPDand carefully chose treatments in the hospital pertains to physical therapy(PT)occupationaltherapy(OT)andmedicationadministrationandsideeffects.

Fallingmaybe themost threateningconsequenceofPD. It is best tobeginPTandOT before the person begins falling. More advanced PD persons experience posturalinstability including retropulsion (backward motion), imbalance, and festinating gait(increased walking speed in an effort to catch up with a displaced center of gravity).Exercise and stretching, standing/stepping/walking/taichi are effective in preventingfalls. Earlier treatment is preferred because falls are notorious for leading to fearof falling and eventually a feeling of “total helplessness.” OT improves the homeenvironment,forexample,byremovingarearugs,changingtheheightofthebathtubandtoilet.

PDdoesnotsuddenlyworsen.However,thesymptomsmaybeexacerbatedbyillness,infection,dehydration,orachangeinmedication.

There are a variety of medication classes used to treat PD. Carbidopa/Levodopa(Sinemet®)combinationof theabove twocompoundswith levodopabeingaprecursorof dopamine. This medication essentially replaces dopamine in the brain. Since theSinemet dosages so dramatically affect function, they must be given precisely on theindividual schedule for each patient. It may be given 14 to 18 times a day! And themultipleadditionaldrugsforPDpatientsaswellasmedicationsforspecificsideeffectsand/ormanifestationsofPDareintricatelyinvolved.Ofcourse,theadministrationofallofthesemedicationsdependsondiet,sleep,activity,psychologicalandemotionalfactors.

A plethora of side effects must be faced. For example, dopamine agonists leadto excessive daytime sleeping, dopaminergic drugs can cause compulsions such asgambling,eating,shopping,orhypersexuality.Levodopacanleadtoseveralsymptomsfrom nausea and dizziness to the “on/off” phenomenon and dyskinesias (jerky orwrithing movements). MAO-B inhibitors can cause nausea and vomiting, orthostatichypotensionandinsomnia.Sinemeteffectsoccuroverawindowoftimeandwidth.Overthe course of the disease, this Sinemet window narrows andmore time is spent withdyskinesiasordiseasemanifestations.

Too little Sinemet does not have enough (therapeutic) effect on the manifestationsof PD; toomuch causes very uncomfortable side effects. The interesting thing here isthatmostPDpatientspreferthedyskinesiastothedisease’smanifestationsandthereforeprefer the larger dose of Sinemet. Remember Michael J. Fox’s body undulations(dyskinesias)?Thisishispreference.

FundamentaltoadvancedPDisthe“on-offphenomenon.”ThisreferstothecomplexprocessbywhichSinemetactsforshorterandshorterperiodsoftime,ordoesn’tkickinatall,orinsomecases,veryerratically.WhentheSinemetdosehasprovidedabenefitand improvement in symptoms the patient is considered “on.”When the symptoms oftremor, stiffness or slowness re-emerge he is “off.” I was fortunate to witness thisphenomenon.WhenthepatientcametotheVAforhisregularvisithewasbarelyabletostand,walk,move,ortalk.HewasthengivenSinemetand,after20minutes,hestoodup,donnedhishatandcoat,clearlybidus“goodbye,”anddancedoutthedooranddownthehall.

As a result of the escalating unpredictable response to carbidopa/levodopa, manyof the other medications utilized help smooth out the response to dosages. Whenhospitalized, every effort must be given to getting the patient his medications at thescheduled time. Diet should be managed in relation to medication; protein should beavoidedforonehourbeforeandafterSinemetadministrationbecauseproteinscompetewithdopaminefortransport.

Referralforsurgicalmanagement-deepbrainstimulation(DBS)-isalsoanoptionforthosewhose symptomsareuncontrolledbypharmacotherapy. If thepatienthasaclear

response to carbidopa/levodopa in their “on” state the surgery may be possible. Thissurgery ismore safeandeffective thanpreviousPDsurgeries suchaspallidotomyandthalamotomy.Briefly, the surgeons place one or two thin flexible electrodes deep intothe frontal lobe(s)of thebrain into theglobuspallaidus internaand/or thesubthalamicnucleus (both in the basal ganglia). Then, a wire is threaded behind the ear (s) andconnected to thepulsegenerator implanted into theupper chest.Thereafter, thepowersourcewhichdeliverselectricalimpulsesismanagedexternallybyapractitioner(oftenaNursePractitioner).PDpatientshaveexperiencedremarkablerelieffromDBSformanyyears.Thepost-opmanagementofDBSsurgeryisfascinating.DBSsurgerycouldopencuttingedgemeaningfulnursingcareers.

PDpersonssufferinmanyways.Theyareembarrassedbytheirbehavior,theget-up-and-gonatureoftheirmovements,especiallybythedyskinesias.Neck,shoulder,andlowbackpainarecommon,causedbydystonia(impairedmuscletone)andrigidity.Researchshowsthatpatientsandcaregiversaremostbotheredbythenon-motorsymptoms.

Wehaveknownfor longthatPDismanifestedbyauniversalslowinginwalking,anarrowinggait,“freezing,”andslowedspeech.Wenowrecognizetheriskofaspirationpneumoniaduetoslowedeatingandswallowing,anddrooling.Eliminationisaffected.Dementiaanddepressionarerelativelycommon.Sleeppatternsareaffected.Thediseaseand/orthemedicationscancausetoolittleortoomuchsleep.

Lessfamiliaristherelativelycommon“sleepbehaviordisorder.”PDpersons’dreamscanbesovividthatthepersonwillactthemoutbyswingingarms,etc.Theymayinjuretheirsleeppartnersbychokingorhittingthemorinjurethemselvesbyhittingawallorfurniture (often in thehospital).Thisbehavior couldbe superimposedonPTSD.Theircaregiversareoftensaints.YoumightwanttoasktoobserveaPDsupportgrouptohearthesesaints’stories.

ItisstronglyrecommendedthateachPDindividualhaveprepareda“HospitalizationKit”totaketothehospitalifadmitted.Anexampleofakitisonthefollowingwebsite.

An excellent source for PD is http://www.parkinsons.va.gov/patients.asp. These areoutstandingvideosfor thepublicpreparedbytheSanFranciscoVAPADRECCoffice.ThereisahospitalspecifickitfreeforpatientsfromTheNationalParkinsonfoundationcalledthe“awareincarekit.”

Nurses are entrusted by the Board of Registered Nursing to “ensure the safety,comfort, personal hygiene, and protection of patients.”When you next admit a patientwithaHistoryofPD,takespecialcaretofamiliarizeyourselfwiththedisease.Youwillthen give the vital comfort, attention, patience, and educationwhich these remarkablepatientsandtheircaregiversrequire.

RN Day - A Day at the Capitol, April 7th, 2014

On a Friday night at my local chapter meeting ofthe California Nursing Student Association (CNSA) Ifirst heard about the Nursing Student in SacramentoInternship(NSSI)andtheopportunityitwouldprovidetobeapartofathreedayinternshipwithAmericanNursesAssociation (ANA). The event startswith the attendanceof RNDay- A day at the Capitol and would follow twomore days attending committee meetings and hearingsas well as meeting with representatives. I was intriguedby the announcement and made further inquiries aboutwhattheapplicationprocesswasandwhenitneededtobesubmitted.Iquicklyrealizedthattheduedatewasaroundthe corner, the nextWednesdaymorning to be exact.Asa first semester nursing student I am still adjusting tothe heavy workload that is required by California StateUniversity Sacramento (CSUS) BSN program and tothrowanotherduedateonmyplateat the timewasabitunsettling,butIfeltadeterminationtogoforit.Ispenttheweekend preparingmy submission and somehow fit it inwith all theotherpressingduedates. I am soglad that Idid.Iwasoneofthetworecipientschosentotakepartinthisamazingexperiencetogainadeeperunderstandingofourverypowerfuldemocraticprocess.

My lifeprevious to scrubsand IVmedicationswasasa legal clerk for theLegislativeCounselBureau. Iwas apart of the legislative process in a truly physical way.Iwasoneof themanywhodidn’tgohomeat theendofan eight hour shift. Instead we spent the night at workbecause language in a bill needed to be amended orbecause publication changes had to be made that nightbefore it could be released to the bill room. With thiswork I came to understand the process in general aboutbills and how they need to be moved through variouscommittees or appropriations, but I did not realize howan individual citizen could play an essential role in thisprocess. By taking part in this internship my eyes wereopen to new possibilities. Despite growing up in theheartofourstatecapital Ihadnever thoughtof trying tomake an appointment with my district representative inthe Senate or Assembly, or know that I may play a roleineducatingthesemembersonaparticularbill thatI feltstronglyabout.ThisexperiencechangedthatformeandInowseeanewopportunity tobe involved inpolicy fromtheperspectiveofaconcernedconstitute,futureregisterednurse,andcareproviderforourmostvulnerablecitizens.

ThefirstdayoftheinternshipstartedoffwithaspecialacknowledgementbythemanymembersofANA\C.ThenaveryinformativelecturewasprovidedbytheHonorableTriciaHunter, ExecutiveDirector ofANA\C, and former

Assemblywoman, which gave personal insight to thelegislative process and the different pieces of the puzzleof how a bill ismade and supported to becoming a law.The presentationswere relative to us all even though theaudiencewas comprised of various levels of nurses fromadvancedpractitioners,newlylicensed,andstudentnurses.Regardless of our experience levels we could stand asone voice involving issues that impact our professionandthepatientswecarefor.Iwasinspiredbythewealthof knowledge that each member possessed and theirwillingness to share their expertise. The contacts thatI made that day will stay with me and only add to myresourcesforinsightonnursingissues.Wewereinstructedonprotocolformeetingwithourlocalrepresentativesandwereencouraged tomakeourownappointments.Wenotonly were able to meet with legislative staff members,but were able to have a meeting with my co-intern’srepresentativeSenatorHannah-Beth Jackson,who said tous“nursesaremypeeps.”ThisexperiencefilledmewithasenseofpridefortheprofessionIhavechosentopursueandaconnectionwithhowimportantourvoiceis.

The next two days were packed full of attendinghearingcommitteesandvisitstootherrepresentativeswithour guides from ANA\C.We also had the great fortuneto accompany the Honorable Tricia Hunter on some ofhermeetings.These includedobservingaboard thatwasinterviewing new candidates forAssemblyMember seatsin the upcoming elections. This was a highlight for meto see the process of how support from a professionalorganization can play an active role in aligning withcandidateswhoalsoshareconcernsoncurrentlegislation.Through these kind of alignments a candidate is ableto voice to the people that they are going to fight forbills that nurses are standingbehind.Wealso attended apublicmeetingat theboardof registerednurses. Itwasapublic forumon a proposed pilot project for paramedics.Therewererepresentativesfromthemedicalfield,nurse’sorganizations, and fire chiefs of the counties it wouldeffect. The debate was very compelling on all sides andreminded me how there really are no black and whiteissues. I also witnessed how important the process ofdebate is.Onlywith the consideration of themany sidesofanissuecanmemberscometoadecisionthatwillbestserveourpublic.

The experience of participating in the NSSI left mefeelingdeeplymovedandstimulated tobemore involvedinourprocessofgovernmentandpolicymaking.Idecidedto run for Legislative Director of our CNSA chapter atCSUS,andwaselected.Partof theresponsibility for thisposition is to form a committee and draft a resolutionthis summer thatwouldbepresented toour statechapterofCNSA this fall. If chosen itwould bemoved forwardand presented at the national conference next year. Ihope to use the awareness that I gained over these threedays to helpmoldmywords into a tool of expansion forourpractice as studentnurses and to impact the liveswetouch. Iamso thankful tomyfaculty forallowingme toaltermyscheduledmidtermsandclinicaltimetoallowmetopursue thisopportunity.Myadvice toanyonewhohasatingeofinterestinthisexperienceistogoforwardwiththat feeling.Dowhatyoucan tomake thisopportunityapartofyourhistory.Ihaveincreasedmyunderstandingofhowimportantitistofollowourintuitionandtodowhatis within our power to make this world a more humanesocietygovernedbylawsthatsupportustocareandthrive.

that pops into their head when they hear the words,“Registered Nurse,” I imagine that some would conjureup images ofwomen in old-fashionedwhite dresseswithwhite hats selflessly caring for patients at the bedside.Others, who did not read Nurse Cherry Ames booksquiteasavidlyasachildas Idid,mightenvisionamoremodern scene with nurses wearing scrubs, competentlyusing both technology and therapeutic human interactionto meet their patients’ needs. I doubt that the words,“Registered Nurse,” would cause many to picture RNsin professional business attire meeting with legislatorsto advocate for the nursing profession and high-qualitypatientcare.Whilethismightnotbeatraditionalviewofwhatnursesdo,duringmyNursingStudentinSacramentoInternship (NSSI) I learned that legislation significantlyimpacts the way that nurses’ are able to provide patientcare.Consequently,itisimportantfornursestounderstandthe legislativeprocess,andhowwecanuseourvoices tohelp legislators make informed decisions when it comesto issues that impact the nursing profession and patientcare.AsanNSSIinternIhadthewonderfulopportunitytolearnfromnurseswhoaredoingexactlythis.

Currently a student in California State University,Northridge’s second degree Accelerated-BSN program,I took the scenic route tonursing, first earningaB.A. inpolitical science. While at one time my career dreamsincluded potentially going into politics, I ultimatelychose to pursue a career in nursing so that I couldmoredirectly serve people. Nurses have the incredible honorand opportunity to support and care for patients andtheir families during some of their most vulnerable,difficult, and beautiful life moments. When I chose topursuenursingIthoughtIwasleavingthepoliticalrealmbehind. Then I learned about NSSI, and the chance toexplore the intersection between legislation, policy, andnursing. During my NSSI experience I met nurses whoarenotonlyprovidingexcellentpatientcareintheclinicalsetting,butarealsousingtheirvoicestoimpactpolicyandlegislation.Ilearnedthattherealmsofnursingandpoliticsare not separate realms, but instead are often intricatelyinterconnected.

NSSI is a collaboration between theAmericanNursesAssociation\California (ANA\C), the Association ofCalifornia Nurse Leaders (ACNL), and the CaliforniaNursing Student Association (CNSA). This year Iwas one of two CNSA member applicants selectedfor the internship. Over the course of three days myfellow intern and I attendedRNDay at theCapital,metwith our local legislators and their staff, and attendedlegislative hearings. A highlight of the experience wasobserving Tricia Hunter, Executive Director of ANA\C,testify at a hearing about paramedics’ scope of practice.ThroughobservationanddiscussionwithTriciaabout thehearing Iwas able to better understand howprofessionalorganizations like ANA\C examine complex legislativeand policy issues, and make decisions about whetherto take a neutral, supportive, or oppositional stance onan issue. Another highlight was meeting with my locallegislators’ offices. In addition to meeting with stafffromAssemblyman JeffGorell’s office,my fellow internand Iwere able tomeetwithmy state Senator,Hannah-Beth Jackson and her staff. It was encouraging to seehow highly Senator Jackson, and all of the legislativestaffmemberswemetwithregardnursesandthenursingprofession.

Iwould like to thankANA\C,ACNL, andCNSA forthe opportunity to participate in NSSI. The experiencehelped me better understand how legislation and policyimpact nursing and patient care, and how we as nursesand nursing students can impact legislation and policy.I am inspired to find ways to use my voice to advancethe nursingprofession, andto encouragemy peers to dothesame.

The3rdNCSBNScientificSymposiumwasheldApril10th2014 inArlington,VA.TheprogramisbasedontheresearchgrantsthattheNCSBNprovideseveryyear(over2 million) for research possibly leading to new regulations. The keynote speaker wasDianaMason,PhD,RN,FAANwhose topicwas ‘FromScience toPolicy:ContinuingtheNightingaleLegacy.’Akeypointofherpresentationwas thatdata isnecessarybutnotsufficientforchange.SheisworkingonastudycalledtheIntersectionofMediaandPolicy:AcasestudywhichfocusesontheBRNdebaclewithdiscipline.Thefindingsofthestudywillbereadyforpublicationsometimeattheendoftheyear.

The program divided into three groups presenting research that could impactregulation. The three areas were Education, Practice and Regulatory. The Educationtrack included virtual nursing care for school childrenwith diabetes: a transformativemodelcreatingadatarepositoryfortrackingstudentnursingerrorsandnear-misses;useof a validation study to analyze entry-level nursing practice between triennial practiceanalysis cycles; and a multisite, mixed method examination of student and facultyexperiences and interactions in clinical practice. The Regulatory track was looking atunlicensedcarebyCNA’s.

IattendthePracticetrackwhichincluded:JeriL.Bigbee,PhD,RN,FNP-BC,FAANpresentedastudyonNursesandPopulation

Health:AMulti-factorialStudypaid forby theMooreFoundation.The research is thefirstonratiosandexperienceandeducationeffectsoncommunityhealth.Countieswerethe unit of analysis. Data was used from the NCSBNNurseyDatabase and 33 Stateswere part of the sample. BSN and experience (defined by the number of years fromgraduation)hadthegreatestimpact.Mammographyscreeningrates,teenagebirthrates,andpercentageofadultsreportingfairorpoorhealth.

Laurie Badzek, JD, LLM, MS, RN, FAAN presented on a study ExpandingRegisteredNursesScopeofPractice:AMethodforIntroducingaNewCompetencyintoNursingPractice.Thisisastudybeingdonewith21MagnetHospitals.Thestudyusesgenomicsas theexemplar and is ayearlonggenomiceducation initiativeand2controlMagnetHospitals.

ChristineSzweda,MS,RN,NE-BCpresentedEvaluating theUseofHumanPatientSimulationtoImproveCriticalThinkingCompetenciesandPerceivedSelf-ConfidenceofNewGraduateNursesintheICU.Theexperimentalgroupofparticipants’perceptionsoflearningandapplicationwerepositive.ImprovementsofparticipantsintheinterventiongrouponPBDSreassessmentratingswerenotassignificantashoped.

Alison Trinkoff, ScD, RN, FAAN spoke on Long-TermCare Regulation: Training,Leadership,andQualityOutcomes.Thestudyexaminedtheassociationofeducationandcertificationamongnursinghomeadministratorsanddirectorsofnursingwith residentoutcomes.Thefindingsindicatethatinvestmentinthesetwothingsimprovepatientcare.

Nancy Lerner,DNP,RN presentedNursingHomeQualityDeficiencies Increase inFacilities with High Nursing Staff Turnover. The objective was to examine CNA andlicensednurse turnover in relation todeficiencies of care in nursinghomes.The studydeterminedthatturnoverisrelatedtoqualityproblemsasmeasuredasdeficiencies.

Kihye Han, PhD, RN presented on Do Training Regulations for Certified NursingAssistants Influence Job Satisfaction. The study investigated associations amongstate regulations,CNA training and job satisfaction.The study determined to increaseretentionofCNA’sandimproveworkquality,regulationsregardingtraininghoursneedtobeexamined.Additionalfocusonworklifeskillsshouldalsobeconsidered.

The process of rewriting andmodernizing the Nursing Handbookhas taken a number of years, withhope that it will soon be adopted.The new language corresponds tothe Institute of Medicine (IOM) and

Future of Nursing Initiatives, particularlytherecommendationthatnursesbeauthorized

topracticetothefullextentoftheireducationandtraining.Thelanguagepromotesimprovingthequality

ofcare,assuringpatientsafety;andincreasingaccesstocare–includingprimarycare,mental health care and other specialty care, surgical and diagnostic services requiringanesthesia,andpainmanagement.ItwouldalsostandardizeAPRNpracticesandpoliciesacross theVAHealthcare system, andwouldbe congruentwith current policies in theU.S.ArmedForcesandIndianHealthservices.

ANA\California (ANA\C), alongwith the other supporters of adopting the NursingHandbook,believes thathealthcareservicesforVeteranswhohaveservedourcountrysowell,isanumberonepriority.AkeyelementinthemodifiedVHANursingHandbookisthepermittingofAPRNstohavefullpracticeauthority.ThiswillmeanthatAPRNsintheVHAsystemwillbeabletopracticemoreindependently,withouttherequirementofphysiciansupervisionorcollaborativeagreements.Thiswillhelpmeetthegrowingneedforhealthcareservices.

TheCaliforniaActionCoalition(CAAC)hasjoinedwithothersacrossthenationinpromoting adoption of theVHANursingHandbook.Work on the 8 future of nursingrecommendations continues through the CA AC work groups and regions, includingmanynursesaround the state.Workgroup#1– focusesonbreakingdownbarriers tonursingpractice.TheCAACalsohousestheCaliforniaAPRNCoalition,whichconsistsof representatives from the four CaliforniaAPRN organizations,ANA\California, andtheBRN.TheCaliforniaAPRNCoalition specificallydealswith removingbarriers toAdvanced PracticeNursing.With these structures in place, andmany nurses involvedinwork groups, coalitions andmultidisciplinary efforts,California iswell prepared toaddressAPRNandothernursingissues.
