Your Welligent Home Screen

Log in to Welligent using your Single Sign on (SSO) and password.

You will be taken to your WelligentHome Screen.

Locate your caseload.

1. Find the Well Provider tab to the left of your screen.

2. Select Daily/Weekly Services Data Entry.

Service Tracking

1. Date: When the Daily/Weekly Services Data Entry screen opens, it will always default to the current date. You can change dates by clicking on the calendar icon. It is highly recommended that if you cannot document services on a daily basis, you enter no more than 5 days at a time.

2. You can document for any student at your location. When you are documenting services for your own student, select the box marked, My Events.

3. Then click on Search to refresh the screen.

For every active service record assigned to you, there will be two service record lines. You will document all services that were provided in the Primary Service Record (i.e. The top line). You will document any Student Absent minutes in the Secondary Service Record (i.e. the second line)

(If any information appears to be incorrect on this screen, notify the student’s Case Manager so that it can be corrected.)

Scenario 1: Student is present for all prescribed minutes

When the student is present for all prescribed minutes, record the number of minutes service was provided in the box marked Minutes for the day you are documenting service. Leave the Cancel Code blank.

For instance, here, service was provided to the student for 360 minutes on Monday, September 12, 2016.

Scenario 2: Student is absent for all prescribed minutes.

All student absent minutes are to be documented on the Secondary Service Record (i.e. the second line).

Document the total number of minutes absent plus the appropriate absence code.

For instance, here, the student was absent (code “SA”) from service for for 360 minutes on Thursday, September 15, 2016.

Scenario 3: Student is absent for a part of prescribed minutes due to a student-related issue (i.e. the student arrived late or left early).

1. Service Minutes are documented on the Primary Service Record (i.e. top line).2. Absent Minutes are documented with the appropriate absence code on the Secondary Service Record (i.e. bottom line)

For instance, here, the student was present for 300 minutes and absent for 60 minutes on Tuesday, September 13, 2016.

Note:

The total sum of both lines (for each day) must equal the total number of prescribed minutes.

As you can see here, each day totals 360 minutes, whether the student was present for the entire service, present for only part of the service, or absent for the entire service.

When you have completed your Service Tracking session, click Save.

(Failure to click Save may result in losing your work. Always save your work.)

You will receive a confirmation message that the student’s record has been updated.

Click Ok.
