“Population health” is a term of convenience. The root word of “population” means “multitude,” and yet whatever we may say or think or do, we all know that healthcare is a profoundly individual experience. The noble and difficult goal is to improve the overall health of our community by doing it one patient at a time. Each time a patient doesn’t come back to the hospital for an unnecessary readmission, the community gets a little better. Every time an algorithm identifies someone as a risk for complications, the community improves a bit more.

It is with that thought and spirit we welcome you to the 2015 HealthLeaders Media Population Health Exchange. Over the next two days, we will all have the good fortune to gather around the table and share ideas of how to make your healthcare organizations better.

We hope you will appreciate the unique design and format of the Exchange. The editors at HealthLeaders have gathered a multidisciplinary team of clinical, information technology, and executive leaders, one that likely mirrors the team responsible for driving improvement at your health system. We hope that you will get to know and network with peers at other health systems who have different responsibilities than your own, and that the fresh perspective will empower you with new ideas.

No health system can help a population of people without data and the analytics that will identify

those in greatest need and suggest strategies. This session will explore which analytics approaches

are working, issues such as interoperability and data ownership across the care continuum, and the

data essentials for population health management.

Do: Clinical Transformation Toward Value-Based Healthcare

Population health transformation starts with a sacrificially honest self-evaluation of the health

system’s capabilities, from identifying gaps in care to reducing variation across the system. This ses-

sion will explore clinical initiatives that aim to reduce variations in care, improve efficiency, quality,

and outcomes, and engage and align employed and affiliated physicians.

No provider-based system can do it alone, as meaningful transformation requires them to look

outward for community resources to support patient goals, and to work with payers to develop

Sameer Badlani, MD, FACPCHIEF HEALTH INFORMATION OFFICER Intermountain Healthcare System/Salt Lake City, Utah

Dr. Sameer Badlani is the chief health information officer for Intermountain Healthcare System. He is a member of the leadership team responsible for the enterprisewide implementation of the Cerner EHR and codeveloping the next phase of innovation in clinical informatics. Trained in clinical informatics from the University of Utah, Dr. Badlani received his MD from Delhi University and completed an internal medicine residency at the University of Oklahoma, Tulsa, where he also served as the chief resident. Recently he was nominated secretary on the board of AMDIS, the premier national association for CMIOs and medical directors in applied clinical informatics. In 2014 he moved from the University of Chicago Medicine (UCM), where he served as the CMIO for three years and as a faculty member in the department of medicine for eight years. At UCM he led the EHR implementation and developed the enterprise clinical informatics program with emphasis on big data analytics and processes within MDM and data management, in addition to codeveloping a certificate program on introduction to clinical informatics.

Frank C. Astor, MD, MBA, FACSCHIEF MEDICAL OFFICER NCH Healthcare System, Inc./Naples, Florida

Born and raised in San Juan, Puerto Rico, Dr. Frank Astor is bilingual and bicultural. In 1978, Dr. Astor received his MD from the University of Puerto Rico School of Medicine. He trained in otolaryngology and head and neck surgery at The Cleveland Clinic Foundation in Ohio and completed fellowships in oncology and reconstruction from the University of Cincinnati and the Oregon Health and Science University. In addition, Dr. Astor has an MBA in general management from Bowling Green State University, Ohio, and subsequently finished an AHIP executive leadership fellowship program.

Dr. Astor has practiced head and neck surgery in several academic settings, including San Juan Veteran’s Administration Hospital, Cleveland Clinic Florida, and the University of Miami. He has served as medical director for Blue Cross Blue Shield of Florida and as CMO for Triple S-Blue Cross Blue Shield of Puerto Rico. At Florida Blue he worked in the area of professional programs and developed an expertise in medical disparities in international health.

Dr. David Battinelli is responsible for the overall professional management of clinical, education, research, and operational issues related to medical and clinical affairs at North Shore-LIJ Health System. Previously, he served as the health system’s chief academic officer and senior vice president of academic affairs, where he was in charge of all undergraduate and graduate educational programs, continuing medical education, and academic affairs and institutional relationships. Dr. Battinelli is dean of education and the Betsy Cushing Whitney Professor of Medicine at the Hofstra North Shore-LIJ School of Medicine. A board-certified internist, he came to North Shore-LIJ from Boston Medical Center (BMC), where he served as vice chairman of education, program director, internal medicine residency program and professor of medicine at Boston University School of Medicine. He was also an active staff physician at BMC and the Boston Veterans Administration.

Pamela M. Beckwith, FACHESYSTEM VICE PRESIDENT, QUALITYCarolinas HealthCare System/Charlotte, North Carolina

As system vice president of quality for Carolinas HealthCare System (CHS), Pamela Beckwith heads the development and implementation of systemwide patient quality and safety initiatives. She joined CHS in 2001 as a director for CHS Management Company, an internal consulting arm, where she was responsible for clinical operational efficiencies and a variety of business development initiatives. Following a promotion in 2008, Beckwith spearheaded a turnaround of quality and patient safety operations for CHS, as well as numerous systemwide initiatives through the CHS Quality Division. One initiative led to an $11 million contract bundle with the U.S. Department of Health and Human Services’ Partnership for Patients (PfP). Under her leadership, CHS was named one of 26 PfP organizations that developed a nationwide program aimed at reducing preventable injuries and complications from healthcare-acquired infections through the Carolinas HealthCare System Hospital Engagement Network (HEN) and the Carolinas HealthCare System Leading Edge Advanced Practice Topics.

Dr. Joe Boyce is the chief information officer at Heartland Health, a nonprofit healthcare system and Malcolm Baldrige National Quality Award recipient located in St. Joseph, Missouri. Prior to joining Heartland in 2009, Dr. Boyce was responsible for physician workflow designs at Cerner Corporation in Kansas City, Missouri. His work history also includes service as a NASA flight surgeon in Houston from 1986 to 1991, followed by five years practicing occupational medicine and medical technology development at Sandia National Laboratories in Albuquerque, New Mexico. Dr. Boyce spent 11 years at Intermountain Healthcare, an internationally recognized, nonprofit system of 22 hospitals serving patients in Utah and southeastern Idaho. There, he was a regional informaticist, developing advanced software designs, mobile Picture Archiving and Communications System documentation, and other solutions.

Matt Chambers, CHCIOCHIEF INFORMATION OFFICERBaylor Scott & White Health/Dallas

Matthew Chambers is chief information officer of Baylor Scott & White Health, the largest not-for-profit healthcare system in Texas with 46 hospitals, over 500 patient care sites, and more than 6,000 affiliated physicians. He is responsible for designing and deploying the next generation clinical architecture to enable Baylor Scott & White to meet the triple aim of increased access, lower per-capita costs, and improved population health. Prior to the merger with Baylor Healthcare, he was the CIO of Scott & White and led the implementation of the fully integrated enterprise electronic medical record system, achieving HIMSS Analytics Stage 7 at four hospitals and 23 clinics at a nationally recognized rapid pace. Prior to joining Baylor Scott & White, Chambers held leadership positions at KPMG, BearingPoint, and James Martin & Co. Throughout his career, he led large-scale business and technology transformation projects with a focus on M&A, post-transaction IT transformation, and IT strategy and governance architecture.

John BoscoSENIOR VICE PRESIDENT AND CHIEF INFORMATION OFFICERNorth Shore-LIJ Health System/Great Neck, New York

John Bosco oversees all aspects of information technology and telecommunications for North Shore-LIJ Health System. He is responsible for aligning North Shore-LIJ’s technology deployment and business strategies; overseeing technology purchases; facilitating communication to effectively plan, budget, and implement technology solutions; establishing IT policies, standards, and procedures; ensuring security, privacy, and regulatory compliance of all technology systems; and providing leadership to the IT team. Bosco joined North Shore-LIJ in 2004 as vice president and chief technology officer; he was responsible for developing the strategic plan for technology throughout the health system and overseeing the daily operations of the IT department. He has more than 25 years’ experience in IT management, primarily in the healthcare industry. Before coming to North Shore-LIJ, Bosco served as senior information technology executive with Capgemini Ernst & Young, where he was responsible for directing the IT team that provided system development services, maintenance, and support to healthcare clients.

Kenneth Cole, MDCHIEF CLINICAL TRANSFORMATION OFFICERBaton Rouge General Health System/ Baton Rouge, LA

Dr. Kenny Cole serves as Chief Clinical Transformation Officer (CCTO) for Baton Rouge General/General Health System. He is an Infectious Disease Specialist with advanced degrees from Louisiana State University, Dartmouth and executive training from Harvard School of Business. He has served on Baton Rouge General’s medical staff from 2001-2013 in various roles such a Medical Director of Infectious Disease and member of critical care and pharmacy committees. In addition, Dr. Cole was one of Baton Rouge’s first physician Lean Six Sigma Green Belt graduates.

Over the last year of his career while pursuing a Masters in Healthcare Delivery Science at Dartmouth, Dr. Cole served as Vice President of Care Delivery and Associate Chief Medical Officer at Blue Cross Blue Shield of Louisiana. In this role, he was instrumental in building value-based, collaborative relationships with physicians and providers across the statewide network, focused on achieving better outcomes for its members.

Deborah Dahl, vice president of patient care innovation, brings together diverse teams who scan, select, and implement innovative clinical strategies at Banner Health. These programs include ambulatory intensive care, teleICU, clinical risk intervention for OB, and the nation’s largest simulation medical center, as well as the beta site for teleacute.

Dahl has held a variety of positions within Banner, including associate administrator for Banner Desert Medical Center, the largest (600 beds), most comprehensive hospital in Arizona. As vice president of technology and materials, she focused on technology assessment, coordinating the capital prioritization process, and monitoring, assessing, and coordinating adoption of technologies, as well as providing traditional biomedical engineering services for over 30 facilities in eight states. With an annual budget of $363 million, Banner’s materials management division encompasses the contracting, purchasing, and distribution for non-pharmaceutical materials.

James Frazier, MDVICE PRESIDENT, MEDICAL AFFAIRSNorton Healthcare/Louisville, Kentucky

Dr. James Frazier currently serves as the vice president of medical affairs at Norton Healthcare and is a member of the Norton Healthcare executive leadership team. Dr. Frazier is responsible for the functioning and improvement activities of the medical staff, peer review, and medical executive committee. He also has primary responsibility for the coordination of various performance improvement and quality-related activities in conjunction with the system vice president for quality and clinical effectiveness. Dr. Frazier is responsible for ensuring that the performance improvement, utilization management, and risk management plans are implemented effectively. He also is responsible for integrating quality and cost information through data-driven initiatives and for ensuring the appropriate response and follow-up required by various physician departments and committees. In addition, Dr. Frazier assists in ensuring positive interdisciplinary communication between the system and hospital departments, the formal medical staff, and Norton Healthcare’s clinical effectiveness department. Finally, Dr. Frazier is responsible for moving Norton Healthcare’s hospitalist agenda and driving standardization and utilization improvement across the system.

Camille Haycock, MS, APN, NEA-BCVICE PRESIDENT AND CHIEF QUALITY OFFICERCatholic Health Initiatives/Englewood, Colorado

Camille Haycock is vice president and chief quality officer for Catholic Health Initiatives, based at the national office in Englewood, Colorado. In this role, she provides leadership and oversight for enterprisewide clinical quality improvement, infection prevention, nursing research, evidence-based practice (EBP), quality regulatory reporting, and patient safety. She has over 20 years’ experience in leadership roles. She has guest-lectured at multiple national venues on EBP, patient and family engagement, care continuum, quality improvement, safety, service excellence, and quality monitoring. Prior to her executive leadership roles, her nursing contributions include nurse practitioner positions, flight nursing, and clinical nurse specialist roles in critical care and cardiology. Haycock holds a BSN and an MS, and has published in multiple peer-reviewed journals.

Shiraz Fagan was named chief executive of the Scripps Medical Foundation in May 2012. In this role, she is responsible for planning and managing the daily operations of Scripps Medical Foundation, which includes Scripps Clinic, Scripps Coastal Medical Center, and the Scripps Cardiovascular and Thoracic Surgery Group. The Scripps Medical Foundation operates 20 clinic locations throughout San Diego and is supported by more than 600 physicians and 2,500 employees.

Fagan joined the project management office (PMO) at Scripps in 2002 as a senior project manager to assist the organization with critical financial turnaround and process improvement priorities. Subsequent to her role in PMO, Fagan served as chief operating executive for Scripps Clinic and, most recently, vice president of physician services. As part of her role in the physician services division, Fagan completed eight key physician group acquisitions for Scripps Health.

Prior to Scripps, Fagan was a senior associate at Computer Sciences Corporation in San Francisco.

Dr. Lawrence Kosinski has been in the practice of gastroenterology since 1984. He is currently one of the managing partners of the Illinois Gastroenterology Group, the largest gastroenterology practice in Illinois. He is also president of the Elgin Gastroenterology Endoscopy Center. In addition to his medical practice, Dr. Kosinski is the clinical private practice councilor for the American Gastroenterological Association and serves on its governing board. Locally, he is a member of the Advocate Sherman Hospital board of directors. Dr. Kosinski received his MD from Loyola University Stritch School of Medicine and earned his Master of Management degree (MBA) from Northwestern University Kellogg School of Management in 1998.

Jean KrauseCHIEF QUALITY AND SAFETY OFFICERGundersen Health System/La Crosse, Wisconsin

Jean Krause is the chief quality and safety officer at Gundersen Health System. She is responsible for quality, patient safety, risk management, inpatient coding, clinical documentation improvement, and the implementation of the Gundersen Improvement System. Krause has more than 39 years of healthcare experience, with 23 of those focused on quality improvement. Her role as a member of the executive leadership team is development and execution of the strategic plan for quality and patient safety, providing resources to support this work. She leads the design and implementation of the Gundersen Improvement System. This work aligns the human side with the science of improvement, change, and the principles of Lean. She is highly skilled in improvement theory, having studied under Dr. Edward Deming in systems thinking, strategy, program design, and execution. As an internal quality consultant, coach, and mentor to the executive leadership team, physician leaders, managers, and staff, she is able to help Gundersen achieve its goal of world-class performance.

Dr. Anil Keswani is the corporate vice president of ambulatory care and population health management for ScrippsHealth in La Jolla, California. He also serves as the president of the medical group at the Scripps Center for Executive Health. His focus includes Lean management, primary care redesign, innovative access points (retail health and telehealth), care navigation programs, physician alignment strategies, and population health IT and analytics. Formerly, Dr. Keswani was a system vice president of medical management at Advocate Health Care as well as the chief medical officer for Dreyer Medical Clinic in Aurora, Illinois. Dr. Keswani attended medical school at Boston University and graduated from the honors accelerated BA/MD six-year medical program. He is also board certified by the American Board of Internal Medicine.

Scott Joslyn is the senior vice president and CIO for MemorialCare, a six-hospital system in Southern California with revenues of $2 billion, 1,500 inpatient beds, 2,500 affiliated physicians, and approximately 11,000 employees. He is responsible for IT, networking, and telecommunications. Joslyn oversees research for MemorialCare, which supports and participates in multisite clinical investigations. He is also on the board of the Summation Health Ventures, a venture capital fund partnership between MemorialCare and Cedars Sinai Medical Center. Dr. Brian Yeaman is the chief medical informatics officer for Norman Physician Hospital Organization. He has over 10 years of informatics experience in helping organizations transform the capture, integration, and delivery of clinical data, information, and knowledge. He is a nationally renowned expert in health information exchange (HIE) and health IT who has guided some of the largest healthcare companies in understanding their core competencies and helped bring new solutions to the healthcare market. Dr. Yeaman participated as the lead clinical collaborator in the establishment of one of the first HIEs in Oklahoma, which served over 3 million patients. This effort led to his leadership and present service as chief administrative officer for a subsequent HIE that has expanded to serve more than 4 million patients, receives data from facilities covering a five-state geographic area, and has become a model for sustainable HIE conducted under private funding and governance. Dr. Yeaman has also created and led an initiative to combine employed and private physicians to develop a shared services model selection of a single electronic health record to represent the Norman community and to develop one of the first continuity of care models in the United States.
