Our Mission at Almost Home Kids is to provide transitional care in a home-like setting to children with complicated health needs, training for their families and respite care.

Dear Friends, Fiscal year 2012 has been an exciting time for Almost Home Kids. With the completion of its new facility at 211 E. Grand Avenue in Chicago’s Streeterville neighborhood, we saw our vision for the future take shape. We believe that all children who are medically fragile and their families should have a comfortable, home-like environment where they can grow, learn and heal together. Because of the generosity of our donors and partners, AHK can now extend this opportunity to more families in the Chicago area and beyond.
Our expansion into new communities was made possible by strategic and vital partnerships with institutions, such as Lurie Children’s Hospital and the Ronald McDonald House Charities, and longtime friends, such as Gary Lee Partners and Liska + Associates. Likewise in Naperville, we celebrated our 13th anniversary of providing transitional and respite care in partnership with the individuals, corporations, groups and foundations that have helped to make our house a home for over a decade. Additionally, our partnerships with state and local officials have allowed us to impact legislation affecting children and families with special needs. We truly would not be where we are today without the collaboration and dedication of our friends, neighbors and representatives.

Caring for Our Children There is a growing need for facilities dedicated to the care of children who are medically fragile. These children are in a stable condition, but dependent on life-sustaining medications, treatments and equipment and have need for assistance with daily-living activities.
For each of the past two years, Almost Home Kids has been forced to turn away more than 50 children and families due to lack of space at the Naperville facility. With the addition of our new facility in Chicago which opened this September, we can now help twice as many families in need of transitional and respite care.
How We Do ItTransitional Care — A bridge from hospital to home
AHK provides a 120-day Transitional Care Program for children who are ready to be discharged from a hospital but for whom preparations at home need to be made. In a comforting environment, children receive 24-hour medical and nursing support from skilled pediatric nurses. AHK trains families on how to both care for their children who are medically fragile and how to prepare the home.
Respite Care
The Respite Care Program at AHK offers children a comfortable place to stay with access to medical
and nursing support while their parents/caregivers tend to personal or urgent family issues. The Respite Transportation Program provides roundtrip transportation for children from their home to an AHK facility.
Our model for providing transitional and short-term respite care can be replicated throughout the country. Where the need exists, the organiza-tion will help facilitate development of other Almost Home Kids locations.
Community Support
In addition to training and care, AHK provides community resources and activities that become a support system the family can depend on for years to come.

Medical Home Model
Hospital is ready to discharge the child, but parents and other caregivers need to continue learning their child’s care and home support needs to be established.
Child is diagnosed with complicated health care needs during acute phase at hospital.
Transitional Care offers short-term care for the child, care coordination and training for caregivers.
Respite Care offers a safety net in the continuum of care.
Trained caregiver takes the child back home.
Parent needs to take a short break.

Our Fiscal ImpactEach day of care at Almost Home Kids costs $850 per child. Three out of four families we serve live at or below the federal poverty level, and most qualify for government assistance to help cover some of the costs of their care, including round-the-clock nursing, physician care, medication and medical equipment costs. Since insurance and government programs do not begin to cover all of the costs associated with our compre-hensive level of care, we rely on philanthropy to make up the difference. Your support allows us to provide transitional and respite care for children in need.
Did you know?■ Almost Home Kids saves taxpayers $3,265 a day per child, compared to what it costs the State of Illinois
for children receiving continuous clinical care at a hospital. That’s a 500% savings!
■ There are currently more than 6,000 children in Illinois who are medically fragile and dependant on technology.
■ Children with special healthcare needs represent an estimated 12 – 18% of children in Illinois.
■ Since opening their doors in 1999, Almost Home Kids has served more than 800 children.
■ Almost Home Kids expansion to Chicago is projected to provide an additional 3,500 days of care per year. This translates to an estimated $16 million in cost savings to Illinois taxpayers with the two locations.

Giving In-Kind Donations from individuals and organizations in our community make up a critical piece of our annual budget. While we rely on fundraising to underwrite our operating costs, we also could not function without the generous donations of goods and services from individuals, groups and corporations. We extend our deepest gratitude to our in-kind donors for bringing much needed supplies or gifts for the children, providing labor and materials to upkeep our home, contributing to our silent auctions and donating time and talent to advance our mission. Below are a few examples of the difference made at Almost Home Kids by in-kind donors this year:
■ Gary Lee Partners (GLP) donated interior design and architectural services for our new AHK Chicago facility at 211 E. Grand Avenue. GLP collaborated with Herman Miller, Design Within Reach and Desks Inc. on furnishings and fixtures for our urban facility.
■ LexisNexis partnered with Home Depot employees to fund and implement patio improvements at our Naperville home.
■ Hill-Rom partnered with us to help bring in their state-of-the-art beds for our new facility.
■ The DreamHome at the Merchandise Mart donated their space to host Parties for a Purpose benefitting Almost Home Kids.
■ Hundreds of businesses and service providers in our community donated items for our silent auctions, raising over $35,000 this year!

We Need Your Support
AHK relies on philanthropy to deliver critical care to children and their families. Support comes from individuals, civic groups, government, corporate and philanthropic partners. Together, we “share the care” by providing medical care and compassion for children with complicated medical needs.
While the funds have been raised to construct, furnish and equip the new facility in Chicago, operational costs for both locations and rent costs for Chicago are ever present.
Approximately $1.5 million is needed from the private sector to balance the annual budget.
