If a vendor has already returned a proposal and any of the information provided below changes that proposal, please submit the changes along with this amendment. You need not re-submit an entire proposal prior to the opening date and time.

1. Can you provide the prior pricing for the contractors who were awarded this contract? All of the documents can be downloaded from our website at http://purchasing.state.nv.us/Fire_Fuels/Fuels.htm Pricing is contained in the attachments to the contracts labeled Contractor’s Response.

2. RFP Section 1.1.3: There are 9 different scopes of work identified in section 1.1.2. If, for instance, vendor wants to bid on 3 different scopes of work, must vendor submit 3 separate packages, with each addressing a particular scope of work, and including the rest of the redundant information that will be the same in each master binder? Or, is only one package required (with correlating copies) for each vendor, regardless of the number of scopes of work on which the vendor bids? Section 1.1 of the RFP is Goals and Objectives, and is a summary of the scopes of work. The scopes of work are detailed in Section 3 of the RFP. Only one package is required from each vendor (with correlating copies). The individual scopes of work that the vendor is responding to should be separated by tabs in the appropriate section of the response (as per section 9.2.2.6 of the RFP).

3. What are the “minimum requirements that the successful bidder must meet” as referenced in NRS 333.335(3)(b) and NRS 333.311? Minimum requirements as are detailed in the RFP.

7 4. How can we obtain from you samples of what you consider to be “proper” proposals and/or winning

proposals that we could use to assist us in preparing our proposal? All statewide contracts are posted on our website at http://purchasing.state.nv.us/services_index.htm or you can submit a public records request for specific contract information.

5. Are there any other guidelines or written materials that the State would recommend in helping a vendor prepare the proposal? Please visit our website at http://purchasing.state.nv.us/pur_info/tipsrfp.htm for 10 Tips for a Successful RFP Response.

6. In section 3.3.2.2, the State describes Brush masticators as “light, medium and heavy duty”. Please give more guidance on what falls under each of those qualifications, i.e. what size equipment and/or horsepower range falls within each of those designations. A light masticator is defined as a wheeled or tracked vehicle having up to a 150 HP engine capable of supplying simultaneous power to the wheels/tracks and to all hydraulics. The light masticator can be equipped with a hydraulically or PTO operated rotary mastication head or a flail type power head. Both types of masticator must be capable of masticating brush and small trees up to 6 inches in diameter. A medium masticator is defined as a wheeled or tracked vehicle having between 150 and 350 HP engine capable of supplying simultaneous power to drive components, all hydraulics, and to the masticator head. The medium masticator can be equipped with a hydraulically operated rotary masticator head or flail type power head. Both types of power heads must be capable of masticating brush and trees up to 12 inches in diameter. A heavy masticator is defined as a wheeled or traced vehicle having between 350 and 600+ HP engine capable of supplying simultaneous power to the wheels/tracks and to all hydraulics. The heavy masticator can be equipped with a hydraulically operated rotary masticator head or a flail type power head. A heavy masticator is fully capable of masticating trees up to 20 inches in diameter and virtually all forest and range shrubs found in Nevada. All masticating equipment will be equipped with a Roll-Over Protection System and a Falling Object Protection System.

7. In section 3.3.2.2, the State describes Brush masticators as “light, medium and heavy duty”. Does vendor decide which mastication equipment will be used on each job? What factors does the State consider in the selection of which mastication equipment to use?

The vendor can make recommendations based on past experiences in different terrains and job specifics. If the state agency requesting bids want a specific type of masticator, that will be specified in each individual project bid. The State will then consider such factors as fuel type, topography, soil impact, proximity to water

sources, and others as specified in each individual project plan when bidding for equipment.

8. Are there any special technical formatting directions for the CDs? No. The CDs must contain the information as outlined in Section 9 of the RFP.

9. Section 9.2.2.6 says that, “Written responses must be in bold/italics and placed immediately following the applicable RFP question, statement and/or section.” What is the response information the State is looking for? There are no questions; rather just defined scopes listed. So, what “response” information is desired there? Section 9.2.2.6 is referring to the vendor’s response to the Scope of Work identified in Section 3 of the RFP. Vendors must acknowledge each question, statement and/or section in some way. A response to each question, statement and/or section is important because this is where the vendor will demonstrate and be scored on capabilities, experience, etc., for each specific scope as outlined.

ATTACHMENT A – CONFIDENTIALITY AND CERTIFICATION OF INDEMNIFICATION

Submitted proposals, which are marked “confidential” in their entirety, or those in which a significant portion of the submitted proposal is marked “confidential” will not be accepted by the State of Nevada. Pursuant to NRS 333.333, only specific parts of the proposal may be labeled a “trade secret” as defined in NRS 600A.030(5). All proposals are confidential until the contract is awarded; at which time, both successful and unsuccessful vendors’ technical and cost proposals become public information. In accordance with the Submittal Instructions of this RFP, vendors are requested to submit confidential information in separate binders marked “Part I B Confidential Technical” and “Part III Confidential Financial”. The State will not be responsible for any information contained within the proposal. Should vendors not comply with the labeling and packing requirements, proposals will be released as submitted. In the event a governing board acts as the final authority, there may be public discussion regarding the submitted proposals that will be in an open meeting format, the proposals will remain confidential. 10 ATTACHMENT C – VENDOR CERTIFICATIONS

Vendor agrees and will comply with the following: (1) Any and all prices that may be charged under the terms of the contract do not and will not violate any existing federal, State or

municipal laws or regulations concerning discrimination and/or price fixing. The vendor agrees to indemnify, exonerate and hold the State harmless from liability for any such violation now and throughout the term of the contract.

(2) All proposed capabilities can be demonstrated by the vendor. (3) The price(s) and amount of this proposal have been arrived at independently and without consultation, communication, agreement

24 RFP 1995 Technical Approach The task order received by Bordges Timber Inc will be read and reviewed by Tim Bordges. An on site visit will be made, during this visit we will look at tasks to be performed, i.e.; mastication , fuel removal, tree removal, chipping or grinding, clearing, seeding or whatever else would be asked to be done. We look for project boundaries, environmental concerns, i.e.; archaeological sites, streams, stream areas, ephemeral and intermittent streams which might be effected by projects use of equipment , i.e.; noise, dust , access points to site, soil conditions and hazards that might occur while performing the task needed. Safety is greatly valued by Bordges Timber, Inc. Tim Bordges would address what methods, manpower and equipment would be used. The project would start immediately upon the completion of all contractual obligations and conditions between the vendors and contracting officer. We will then request a pre operations meeting to discuss production rates and particulars of project depending on type of project. Usually we post safety signs, haul equipment to project site, have a meeting with crew to discuss hazards and job outline, boundaries of project and other sensitive measures needed to perform project. Bordges Timber Inc. has experienced crews available to perform fuel reduction tree thinning or wildlife habitat improvement as well as erosion control skills, highly skilled people have performed work for Plumas Fire Safe Council, White Hawk fuel reduction and Nevada fire safe council sewer treatment project in Zepher Cove both of these projects required whole tree removal log processing, top grinding and removal as well as mastication. Bordges Timber Inc. has the capabilities of chipping, grinding, piling of slash, and hauling of products, i.e. chips, slash, logs or firewood. We have timber fallers for trees that cannot be harvested by mechanical means, having worked for Pacific Gas & Electric downing hazard tree removal on Highway 50 summer home tract and numerous projects on Interstate 80 and Highway 20. Larry Reagen and Paul Mayben were the managers from Pacific Gas and Electric. We have done trees at Ice House Reservoir, Union Valley and Sacramento Municipal Utility District (SMUDA) campground in the Pacific Ranger district of El Dorado National Forest. We have made bear dens at Forest Hill Ranger Station District out of long butts available from timber harvesting. Bordges Timber has done machine piling for Sierra Pacific Industries from 1994-2001. Using dozers with brush rakes we averaged 600 acres per year. We had a project in Tahoe National Forest piling brush called Rock Pile which we sub-contacted for Paul Roen Company of Sierraville, CA. Bordges Timber has been fire fighting with dozers, water trucks for many years with the USFS, BLM and Cal Fire. We have years of hot line and fire line repair experience Our mobilization is based on an hourly rate and not by mileage because of extra legal width and weights. Some pieces of equipment require pilot cars and the unknown distance to projects. Bordges Timber is proposing to achieve fuel reduction, tree-thinning wildlife habitat improvement and erosion control with equipment and man power. There are some things that manpower still has the upper hand in. We will request a pre-operations meeting to discuss production rates, road work, and any other issues of concern. We intend to proceed from that point in flagging skid trails and landings. At the same time we will have a Cat 722, Faller buncher with bar saw and skid steer with 14” shear begin falling and begin falling appropriate timber and sub murch, placing them in at a 45 degree angle to skid trails where a Cat 518 rubber tire will grapple and skid doodles to landings with least amount of turning necessary. We will be placing sporax to all required stumps as directed. At that point a hydro log loader will sort the pile timber and trees. If log market provides opportunities to once again sell volumes of timber, we would gladly deliver logs to mills. The rest of the sub murch and small logs can be chipped and delivered to cogeneration plants in Carson City, NV and /or Loyalton and Quincy. A brief description of each section and past performance of that type of work will be included in this technical approach under sections.

3.1 Forest Management Hand Crew Services Specification

Bordges Timber Inc has done both forest health projects and fuel reduction projects and thinning projects over our 32 years of business. Our road side salvage projects have had hand piling and view strips in them where we picked up logging slash, limbs, and chunks and piled them as well as removing brush and hand thinning 3 – 9 DBH trees. These trees were fell limbed, cut into 3 – 6 ft pieces and hand piled into a tee pee shape piles for burning. A fire line was dug around the pile to mineral soil. On steep ground a ditch was dug on the lower side of pile to catch any roll outs from going down the hill, where the pile was burned. Bordges Timber has also done dozer piling of slash and debris and excavator piling and thinning. We have used 320 & 325 size excavators with rakes and thumbs to chunk and remove small trees in plantation and pile them for burning as well as jack pot piling of debris around and between larger trees. The larger trees were not to be damaged and a fine of $200 per tree was to be assessed, but there were none to be had. Bordges Timber has done wildlife improvement as well. On the El Dorado National Forest Georgetown Road, we placed a dozer and excavator into Long Canyon creek and created fish holes by placing rocks onto other rocks to make pools for the fish. We also took the slide and placed water bars in and rechanneled the channel from straight to S turn to displace energy from the water. This work was done under the direction of the Forest Fish Biologist. Bordges Timber has also taken down red fur logs and other unmurchable trees and placed them into a lean two type of structure for bears to den into above Hell Hole Reservoir. On some of our timber sales, there are nature nook areas of trees that we leave untouched for deer to fawn in and to get shelter in from the wind. Bordges Timber has worked on Sage Summit Hen with the Truckee Range District to do experimental stand improvement which follows the guidelines in GR220 where we leave 5 acres untouched and the outward from the center leaving a mosaic pattern of nature nooks which are separated by thin areas. All of Bordges Timber’s temporary roads, skid travel and landings are closed according to contract specifications. We use outslopeing, water bars, trees and rocks as barriers, ditching, scattering wood debris or straw as filter strips for erosion control.

26 3.2 Large Tree Removal Services Specifications 3.2.12.1 Class A: 1 faller, transportation, bucker, person to sweep area for possible improvements that might be damaged, i.e.; Power lines under and above ground, water systems, septic systems. Plants and shrubs to be protected, Evaluate area for possible flying debris from trees, broken limbs, limbs from other trees or shrubs that may be brushed upon during falling, placement of safety signs and safety. 3.2.12.2 Class B: 1 faller climber, 1 ground person. This type of work requires the same approach as above with safety being our first concern i.e.; looking for possible hazards, rotten branches, bees, electrical lines, nails or metal in trees, impact on ground. 3.2.12.3 Class C: Bordges Timber will be happy to bid on a case by case basis. There are many variables, i.e.; height, slope, accessibility, existing hazards, fences, power lines, septic systems, irrigation systems and buildings. Mobilization for cranes is expensive. The distance could add significant cost as well as type of soil for stabilization and access to project. 3.2 Large Tree Removal Bordges Timber Inc has completed many hazard tree removals from private grounds and USFS, S.M.U.D. and PG&E camp grounds. The tree removal around improvements is always a challenge. Septic systems, leach lines, pavement, water lines, BBQ’s, fire boxes, picnic tables and barriers all had to be protected along with clean up of slash. In 2010, Bordges Timber did such a job on the Plumas National Forest, called Hopper Corral Stewardship. This project included Honker Gove and Grizzley Camp Grounds. Karen Newsted was the site administrator for the project. This project included whole tree removal and total clean up. All tops were chipped for bio mass and the slash was removed and piled for burning. The project was successful. We have done many campgrounds on the El Dorado National Forest, Pacific District with Don Errington at Ice House, Wench Creek and Jones Camp Grounds as well as PG&E 43 mile tract on Hwy 50 and SMUDA camp at Union Valley. For Sacramento Utility District we used hand fallers, jacks and mechanical equipment to perform the tasks. Some were trees taken down in sections or chunks. We have used tires stacked up to soften a tree crossing the pavement. We have climbed and roped trees for pulling or pushing and total clean up was performed.

27 3. 3 - Forestry Equipment Specification Our people have numerous years in the Timber and Construction industries. We have a Forester on staff. My operators know what a SMZ (stream management zone) and equipment limits. They are light on the land and have finesse at operating equipment. They can surgically remove trees without damage to other resources. They are aware of the dangers of flying debris, dust, compaction of soil issues as well as erosion issues. They have worked in and around archaeological sites with both limited and avoidance operations. They are familiar with USFS rules/regulations and Cal Fire rules/regulations as well as the State of Nevada Dept. of Forestry rules/regulations. Bordges Timber has qualified timber operators to do forest health, logging, thinning and mastication projects. We provide a large range of equipment to do these changing projects. We have done meadow enhancement at 3 Ring Road for Nevada Safe Council. We have done Aspen stand improvement on the Plumas National Forest Thompson Aspen Stewardship. For Karen Newsted we have done forest health and thinning at Zepher Cove for Nevada Fire Safe Council. On the Larson National Forest, we have done 40 ac of brush piling using dozers, again on the Plumas N.F. we have done plantation thinning, using excavators with thumbs to thin and pile the trees. On the Plumas N.F. we thinned two campgrounds, Honker Cove and Grizzley Campgrounds. These campgrounds have numerous improvements that need protection and paved roads. We used rubber tracked equipment and machined felled the trees along with a hand feller, skidded the trees whole with rubber tire skidders and loaded the trucks with a rubber tire loader, then chipped all tops for bio mass and removed it. Bordges Timber’s crew and operators are as follows Steve Skochko, Jon Thurlander, Dennis Turpin, Jeff Edwardson, Matt Bordges, Steve Chasen, Ray Romine, Juan Lopaz, Tim Bordges, Bret Gibford, Chase Ryan, Mike Bordges, Gordon Powers and Ron Keller. type of skid steer perform best on flat or level ground. They can run on medium pitches of 20% but are not productive over 30% slope.

The tracked faller buncher Timbco 425 and CAT 722 have self leveling cabs. They can work on 40% slopes productively and pitches up to 50%. This equipment is specially designed to work on slopes. Both have single bar grozer tracks.

metric ton machines with FAE drum type heads. They work best on light and medium fuel loads. They have 24” street pads on them for ground pressure.

CAT 722 and Timbco 425 are 30 – 35 metric ton machines which can work on slopes up to 50%. They are equipped with single bar grozers and a reach of 20ft. They work best in medium and heavy fuels.

3.3.2.4 Forwarders

John Deere 1210 John Deere 1210 rubber tire forwarder is a 6 wheel drive machine which carries the processed stems left behind from the 1270 procession. This machine loads itself with stems approximately 1/3 of a truck load and carries them out of the wood instead of skidding the stem on the ground, so there is less ground disturbance.

3.3.2.5 Cut to length operators

John Deere 1270 John Deere 1270 D with 480 Head. This rubber tire processor is a low impact type of equipment. It has 6 wheel drive with a squirt boom with water 480 head which grabs the stem. A bar saw severs the stem, we push or pull the stem into a horizontal position, then process the stem by delimbing it, and measuring it into log lengths. This process is done in the woods. The 1270 has a reach of 20’ with 270 degree rotation.

3.3.2.6 Heavy Cranes LinkBelt HSP25

Linkbelt HSP 25 is a 25 ton rubber tire rough terrain crane capable of moving on and off roads.

It has a 70’ boom with two drums capable of lifting 25 tons.

3.3.2.7 Wheeler skidders Cat 518 w/100” log grapple rubber tire Cat 528 w/120” log grapple rubber tire Cat 525 w/100” log grapple rubber tire

Cat 528 is a rubber tire skidder with wench and 100’ log grapple. It has 174 HP with 24.5 x 32”

tires. This is a large skidder capable of skidding whole trees up hill. The skidder can work on slopes 35 – 40 % straight up and down.

29 CAT 518 is a smaller version of the 528 with only 130 HP and 23 x 26 “ tires with wench and

100” log grapple. This skidder can also operate on slopes of 35 – 40 % straight up and down. These rubber tire skidders are friendlier on rocks than metal tracks

CAT 525 is a rubber tire skidder with wench and 100” log grapple. This machine has 165 hp with 24.5 x 32” tires. It can work on slopes 35 – 40%.

3.3.2.8 Tracked Skidder

Cat D5H tracked log skidder with 100” grapple and winch Cat D7F Cat 287 Cat D7F with 100 log grapple is a dozer with a log grapple capable of skidding timber along

with road or skid road construction. It has 180 HP with 40ton capable of operation on 40 – 45 % slopes. Cat D5 tsk is a D5 dozer custom built for log skidding with a longer track frame. It has a 100”

log grapple and 6 way blade. This skidder has 145 HP and weighs 35 tons with 22 inch track pads and a ground psi of around 5 psi. This piece of equipment is light on the ground and produces a lot of production.

Cat 287 is a rubber tracked skid steer with log forks or grapple, good for low impact and close corners. It can run on streets, load trucks and skid logs. It has 90 HP and ground pressure around 3.5 psi and has 90 hp.

3.3.2.10 Chippers

Peterson 4310 grinder Peterson 4310 chipper is a whole tree 800 HP tracked chipper capable of self mobility. It can

produce up to 100T per hr. Wt is 30T, and has 18” single bar grozers. Above equipment expensed by the hour and includes fuel, maintenance, operator and replacement of wear items. Equipment transport, water for fire suppression and dust abatement will be negotiated on a project by project basis.

30 3.6 Hauling Services Bordges Timber has done trucking since 1979, when Tim bought his 1st log truck. Since then Bordges Timber owns 5 trucks and numerous trailers. We move most of all our own equipment on our low beds. We use the 10 wheel dump for aggregate and logs. For brush and stump removal we have 30’ end dumps, chip vans, walking floor trailers which unload themselves, log dolleys, 40’ log trailers, flat bed trailers and belly dumps. Bordges Timber plans to use Sierra Disposal for their container freight in 10- 20-30 yd containers. Bordges Timber will drop off containers at designated project site. The container will be able to hold wood chips or wood stems or cord wood. The containers are available in 10 – 20 – 30 cubic yards. The cost of container will be determined by distance to drop point, tipping fee, length of time used and rental fee. 31 3.9 Road Construction/Maintenance/Rehabilitation Specifications Bordges Timber has been doing forestry and construction work since 1986. Most timber removal requires sale layout and planning during this process. The project area is defined; environmental issues are mapped and mitigated. Within most projects there are roads to be built, culverts to be installed, aggregate base to be placed, bridges to be installed and erosion control measures to be installed. Bordges Timber has completed 1 mile of new road construction for NDF at Spooner State Park, called Spooner Road. We installed water bars, out slope the road, did cut and fill work, added rolling dips, padded the existing road, rolled and compacted roadway, removed trees, stumps and installed a gate. Most roads are flagged prior to construction, draws are mapped, slopes and the grade of road are reviewed, obstacles are flagged, the timber is removed, and stash is piled or removed. The organic 2-4 inches of matter is removed or incorporated within the fills, slopes cut, low spots and draws are filled, culvert are placed, the road is watered and compacted, then final graded. Bordges Timber has built roads for PG&E, USFS, SPI, Michigan Cal, State of Nevada and other private people. We are familiar with most all types of erosion controls devices i.e.; water bars, ditches, use of waldals, silt fence, straw or slash scattering rock filter strips, just to mention a few. We have done temporary crossings. Using logs we made log stringer bridges to haul over, French drains and used Geo Textile fabric in roadway and drains. We have installed new pipe gates and signs, placed concrete barriers, set box culverts and poured bridge embankments, placed rip rap constructed gaben basket. We have built engineered rock walls. On most Forest service sales prehaul and post haul maintenance is required, ditch cleaning, cross blading, line of sight brushing, culvert cleaning, armoring rolling dips, putting up barricades. Rocking road with AB, repairing asphalt, both permanent temporary erosion work from seeding seeding and strawing slopes to spreading slash or chips on slopes. We have had to cordaroy roads in the spring time to gain access to units. We have used geo-textile fabric with aggregate base over the top on native surface roads prior to applying AB. We have experience with new road construction performing all aspects, right away clearing, staking, timber removal and grubbing. Construction of road bed and key ways compaction of material used, construction of rolling dips inside of ditches, french drains, culverts and downspouts. Bordges Timber has built roads for Sierra Pacific Industries, USFS, and private land owners. Due to unknown types of projects our equipment rates are by the hour of operation. We are willing to lump sum bids or piece bid. We have additional equipment that may be used in construction maintenance or rehabilitation. All equipment is pressure washed for safety and weed abatement. Equipment that may be utilized: Back hoe Cat 446-Rubber tire hoe CAT 446 4x4 back hoe is a rubber tired tractor with a digging attachment on the rear and hydraulic stabilizers. It has an assortment of different size buckets from 12” to 48”. Its weight is about 25 k lbs. It has a front end loader and bucket as well. The bucket is 3 cubic yds. The machine is 4x4 with an extension hoe Roller Cat 563, smooth drum roller, 84” drum w/vibration, floatation tires CAT 563 smooth drum roller is an 84” smooth drum roller with vibration. It has 23’ x 26’ rear tires and approximately 90 hp. This machine is used to compact road surfaces.

32 Grader Cat 12G – 12’ mulboard and rippers Cat 143g all wheel drive grader, 12’ mulboard and wing front plow for snow CAT 12G is a 6 wheel grader with a 12’foot mulboard and ripper on the rear. It articulates in the center, has power shift transmission, 14:00 x 24:00 rubber tires, hp of 125 and a push block on front to help push trees. CAT 143 grader is a 6 wheel drive grader with a 12 foot mulboard and a ripper on the rear and push block on front, hp of 140. It has a power shift transmission and articulates in the middle. Dozers Cat D7H - semi U blade w/ripper Cat 7G – full U dozer and ripper Cat D5H tsk– 6 way dozer w/grapple CAT D7 dozer is a 64 k lbs machine with 171 hp engine. It has rippers and semi U dozer 11” wide. The semi U allows user to carry soil further without losses. CAT7G dozer is a 54 k lbs machine with 200 hp. It has rippers and a full U blade. Again this allows user to carry soil further with less loss CATD5H tsk dozer is a 40 k lbs dozer with log grapple on rear to move timber and rocks. It is equipped with 10’ 6 way dozers which also are able to cut angles without setting the dozer at that angle. The dozer is a straight blader. This dozer works nicely for water bars. This machine articulates in the middle and has a 3 yard g/p bucket. It also has a log fork available. It is mainly used to land or carry materials i.e. dirt and rocks. Dump Trucks Mack RL 700 Mack RL 700 10 wheeled dump truck has 430 hp engine with 15 speed transmission and 44 mack boggies. It is equipped with a 16 foot dumb box. It can carry 10 – 20 yds of material.

33 Belly Dumps Our belly dumps are pulled by a 3 axle tractor. These trailers are 28’ & 30’ long. They can carry 18 – 22 yds of material. The gates are single and adjustable. End Dumps The end dumps are 30 – 35 foot. One is high sided or high cubic with a barn door at the rear. The other end dump has an air lift tail gate. We use these to transport brush, debris, rocks, firewood and logs. There may be the need for other types of equipment which will be rented on a project by project basis. For many years Bordges Timber has helped Cal Fire, USFS and BLM to do fire suppression and fire line rehabilitation. In the last few years, this has been a big part of our business. We have worked for the Plumas Fire Safe Council, White Hawk Project, and the Nevada Fire Safe Council Sewer Plant Project. These jobs involved removing and sawing12” or greater logs as well as masticating and chipping the remaining fuels.
