La valutazione dello stato di nutrizione può consentire di programmare un intervento nutrizionale migliore e più efficace, di monitorizzarne l’efficacia e di modificare, se necessario, il tipo di intervento in relazione ai risultati ottenuti

Un biomarcatore è qualunque sostanza, prodotto metabolico o risposta cellulare, dosabile/misurabile nei liquidi/tessuti biologici.
Può essere indicatore dell’intake, dello stato di nutrizione e/o deglieffetti funzionali di un regime alimentare

ABNORMAL INDICES COULD BE DRIVEN BY INFLAMMATION RATHER
THAN MALNUTRITION
ALWAYS CHECK CRP
• Visceral proteins, like serum albumin, concentrations are good indicators of
disease severity and outcome.
• Inflammation is today considered the major reason for reduced serum levels
of visceral proteins.Thus, visceral proteins should not be used for either
screening or diagnosis of malnutrition
• The low grade of nutrition specificity is the major reason for this decision.
• The question of integrating a biochemical indicator of inflammatory activity, as
CRP, into the diagnostic format raised another discussion.
• Inflammation has a central etiologic role for catabolism and is an integral
feature of cachexia.
• Thus, inflammation is rather to be regarded as an etiologic factor than a
diagnostic feature of malnutrition. The same reasoning goes for indicators of
potential anabolic activity, e.g. insulin-like growth factor-1.

CONCLUSIONI
• La valutazione dello stato nutrizionale è una procedura compl essa, per laquale è necessario un approccio multidimensionale ed una no tevole capacitàdi contestualizzazione e valutazione clinica
• Alcuni parametri ematochimici sono stati proposti come ind icatori dello statodi nutrizione, ma i loro livelli sono influenzati da una serie d i variabili nonstrettamente nutrizionali
• L’infiammazione sistemica è frequentemente associata alla presenza dimalattie ad impatto nutrizionale e influenza in modo signific ativo i parametriutilizzati per la valutazione nutrizionale
• Ad oggi non esiste un “marcatore” di laboratorio unico e speci fico dellamalnutrizione per difetto
• Alcuni parametri funzionali sono influenzati dalla presenza d i sarcopenia,perdita di massa e funzione muscolare, che è una rilevante com ponente dellamalnutrizione per difetto
