(FY 2004)

FY 2004 is another banner year for the PHS Class’65 Foundation, Inc.. We were able to accomplish meaningful projects, sustain the growth of our scholarship program and raised additional funds.

We elected this year Fifteen (15) Members of the Board of Trustees in order to properly monitor continuing projects. Five Trustees which will serve for 3 years, another five for 2 years and the last five will serve for 1 year. We created nineteen (19) different committees to implement our plans and Programs for Foundation Year 2004.

01 Scholarship Awards

-To support poor but bright students complete their studies.

-To raise funds for high school, collegiate, vocational and graduate study scholarships

-To establish donor-to-scholar contacts as opportunities for guidance, counseling, expression of gratitude and mutual participation to the program.

-To monitor the progress of the scholars and their placement after graduation.

-To plan and implement teacher development program through value enrichment workshops/seminars.

02 Livelihood Projects

- To organize dedicated and reliable members, PHS teachers and parents who have the desire and capability to work particularly in community self-help projects. To assist selected leaders in initiating livelihood programs with the foundation giving assistance by way of expert advice and in special cases,they seed money to help themselves in productive enterprises

6-month Caregiver Course & Basic Computer Training Course for Out-Of-School High School Graduates of PHS.

Medical & Dental

Mission

-To extend specialized medical/dental skills and give free medicines to not less than 4,500 patients including 100 young boys aged 15 who will undergo circumcision (Operation Tule) where there is demonstrated acute need for such services.

-Provided medical consultations & surgical operations and gave free medicines to more than 4,000 poor constituents in support to the mayoralty candidacy of fellow 65er Andrea Domingo from March to May 2004 to the different BarangayHealth Centers of the City of San Fernando.

06 PHS Improvement

Project

-To implement a meaningful project that would contribute to the improvement of the Pampanga High School

- To plan and implement teacher development program through value enrichment workshops /seminars.-

- Sponsored the Mentoring the Mentors Seminar Workshop at the PHS BulwaganCapampangan that was benefited by 61 Principals, head teachers, master teachers of high school and elementary schools in the City of San Fernando.

Ways & Means - To raise funds (min. of P500,000.00) and other resources for the support of the various projects of the Foundation particularly for the scholarship and health related services

-Completed and raised a NetEarnings of more than P400,000.00 during the 2nd

Fellowship Golf Tournament hosted by PHS Class’65 Foundation at the Gen. Emilio Aguinaldo Golf Course last April 16,2004

THANK YOU TO ALL OF YOU FOR COMING TO OUR 3RD INDUCTION CEREMONIES AND FELLOWSHIP PARTY. I WOULD LIKE TO WELCOME OUR SPECIAL GUESTS, RODY AND MARILYN DE GUZMAN WHO CAME ALL THE WAY FROM SWITZERLAND JUST TO BE WITH US THIS AFTERNOON AND OUR INVOCATOR, MS SIMEONA STA. CRUZ WHO CAME ALL THE WAY FROM AUSTRALIA TO LEAD US A PRAYER. I WOULD LIKE TO WELCOME OUR GUEST OF HONOR AND INDUCTING OFFICER, “ANG SIGURADONG CONGRESSMAN KAHIT ANONG ORAS MAAASAHAN, DOC ERIC DOMINGO DOMINGO. LET US GIVE THEM A BIG ROUND OF APPLAUSE.

THE PAST FOUNDATION YEAR WAS REALLY AN EXCITING YEAR AND AS MS. LISA VALERIO PREDICTED, YEAR 2003 WILL BE A BANNER YEAR FOR THE PHS CLASS’65 FOUNDATION. I THANK ALL OF YOU FOR GIVING ME INSPIRATION TO DO MY WORK – BY YOUR WARM AND UNCONDITIONAL SUPPORT AND MOST OF ALL WITH YOUR KIND UNDERSTANDING FOR MY SHORTCOMINGS. I THANK GOD THAT HE HAS KEPT ME SAFE AND STRONG BOTH PHYSICALLY, EMOTIONALY AND SPIRITUALY.

I TRIED MY BEST TO MAKE ALL 65ERS LOOK GOOD, FEEL GOOD AND TO FEEL PROUD WITH OUR HUMBLE ACCOMPLISHMENTS . I HOPE I DID NOT DISAPPOINT ANYBODY AND I HOPE I MET YOUR EXPECTATIONS.

MANY OF YOU CONFIDED TO ME YOUR PROBLEMS, AND I AM HAPPY BECAUSE THIS SIMPLY MEANS THAT YOU HAVE ACCEPTED ME, NOT AS JUST YOUR PRESIDENT, BUT MORE IMPORTANT TO ME AS YOUR FRIEND AND HELPER. I LEARNED ABOUT YOUR UNENDING WOES OF NOT BEING ABLE TO ATTEND MEETINGS DUE TO OTHER COMMITMENTS, DUE TO CONFLICTS TO YOUR WORK, DUE TO FINANCIAL DIFFICULTIES, ETC.., YOUR RELUCTANCE TO INFORM OTHER MEMBERS TO ATTEND MEETINGS, YOUR RELUCTANCE TO VOLUNTARILY HELP IN FOUNDATION WORK, THE INTERPERSONAL CONFLICTS WITH OTHER MEMBERS.

BUT MANY MORE OF YOU ALSO SHOWED ME SUCCESS STORIES AND ALL OF THESE THINGS GAVE ME THE INSPIRATION TO WORK HARDER. I SAW SOME OF YOU GOING THE EXTRA MILE, PERSEVERINGDESPITE SUDDEN PROBLEMS WITH YOUR FAMILY, BUSINESS, OR EVEN PERSONAL HEALTH – NOT COMPLAINING, NEVER CYNICAL, BUT ALWAYS ENTHUSIASTIC – CHEERFUL, CONSTRUCTIVE AND SUPPORTIVE.

I AM SO INSPIRED BY A BATCHMATE WHO OPTED TO TERMINATE HER HOUSEMAID JUST TO SAVE THE WAGES FOR OUR SCHOLARS TUITION AND ALLOWANCES. THAT IS VERY TOUCHING INDEED KNOWING THAT SHE HAS SMALL CHILDREN TO BE TAKEN CARED OFF. A BATCHMATE DECIDED NOT TO BUY A CHRISTMAS TREE LAST YEAR JUST TO BE ABLE TO SEND HIS CONTRIBUTIONS TO OUR REUNION.

A BATCHMATE HELPED ME RAISE MONEY TO TAKE CARE OF THE ACCUMULATING HOSPITAL BILLS OF MY SISTER-IN-LAW WHO UNDERWENT A SECOND OPEN HEART SURGERY DUE TO INFECTION AND ANOTHER BATCHMATE SHARED HER HUMANITARIAN ASSISTANCE THRU HER SON’S CONNECTION TOTHE HOSPITAL.

MY FRIENDS FROM ROTARY ALSO EXTENDED THEIR USUAL SUPPORT AND ASSISTANCE. A PORTION OF OUR FEEDING PROGRAM FUNDS AND THE PHS TOILETS AND LAVATORIES WERE DONATED BY ROTARIANS.

MANY OF OUR BATCHMATES AND FRIENDS DID NOT THINK TWICE WHEN WE ASKED THEM TO CONTRIBUTE TO OUR SCHOLARSHIP PROGRAM. MANY SUPPORTED WILLINGLY OUR GOLF TOURNAMENT AND OTHER FUND RAISING PROJECTS.

A BATCHMATE MADE SURE THAT I HAVE FUNDS TO OPERATE THE FOUNDATION BY DONATING A SIZEABLE AMOUNT DURING OUR INDUCTION LAST YEAR.

A BATCHMATE ALWAYS ENTERTAIN MY CALLS PROMPTLY AND WONT STOP TEXTING ME UNTIL WE SETTLE AN ISSUE REGARDING FOUNDATION PROBLEMS. MIND YOU SHE IS CAPABLE OF USING HER CELLFONE EVEN WHILE DRIVING AT A VERY HIGH SPEED.

MY FELLOW OFFICERS AND MEMBERS, LET US APPLAUD OURSELVES FOR ACCOMPLISHING ALL OUR PLANS AND PROGRAMS LAST YEAR AND FROM HERE, WE SHALL HAVE LEARNED FROM THE RECENT PAST AND CAN PROCEED TO A NEW TRANSFORMATION THAT WILL MAKE OUR THIRD YEAR OF OPERATION EVEN MORE SUCCESSFUL THAT THE PREVIOUS YEARS.

IN EVERYTHING THAT WE DO, THERE IS ONLY ONE ESSENTIAL INGREDIENT, AND THAT IS ATTITUDE. ATTITUDE MAKES THE DIFFERENCE. THERE ARE LESS KNOWLEDGEABLE, SIMPLE, EVEN HAPPY-GO-LUCKY LEADERS BUT THEY DRAW OUT THE BEST FROM THEIR MEMBERS. I AM SAYING THESE BECAUSE I WANT THE NEXT SET OF OFFICERS AND LEADERS TO CONCENTRATE IN MAKING OUR ACTIVE MEMBERS TO GROW IN NUMBERS SO WE CAN ACCOMPLISH MORE PROJECTS IN THE FUTURE. AS YOU MAY HAVE KNOWN, LEADERSHIP IS A WEAVING OF RELATIONSHIPS. A LEADER MUST BE VULNERABLE TO THE SPECIAL GIFTS AND TALENTS OF HIS FOLLOWER. HIS CRUCIAL ROLE IS TO FOSTER WORK ENVIRONMENTS AND WORK PROCESSES TO MAKE DIVERSE PEOPLE AND ELEMENTS WORK TOGETHER. A LEADER MUST HAVE A VISION, PASSION, INTEGRITY AND MUST BE TRUSTWORTHY. THESE BASIC INGREDIENTS OF A LEADER ARE TRAITS YOU ARE NOT BORN WITH AND CAN’T CHANGE; THEY CANNOT BE TAUGHT EITHER. THEY CAN ONLY BE LEARNED FROM EXPERIENCE.

A LEADER’S PASSION, VIRTUES AND VISION BEGETS THE PASSION, VIRTUES AND VISION OF THE FOLLOWERS. WATCH A GREAT ORCHESTRA CONDUCTOR OR A BASKETBALL COACH IN ACTION. THE MEMBERS OF THE ORCHESTRA OR THE BASKETBALL TEAM ARE NOT DUTIFUL BUT ARE INSPIRED. MY FRIENDS, LEADERSHIP IS INDEED A PARADOX THAT WILL TAKE A WHOLE LIFETIME TO UNDERSTAND.

PHS ’65 FOUNDATION SPONSORSMENTORING THE MENTORS WORKSHOP SEMINAR

& ADOPT-A-SCHOOL PROGRAMMost of the participants thought that it was just another torturing seminar endorsed by the Department of Education.

But as the opening program progresses, they suddenly shifted their attention and interests. At the closing ceremonies, the revolutionaries, armed with new knowledge, skills and techniques signified their readiness to go forth to serve and “spread the revolution” as they recited their individual commitment to upgrade the quality of education in their respective schools as “better mentors.” The workshop seminar included 61 principals, department heads, master teachers and teachers from Pampanga High School, San Fernando Elementary School, San Juan Elementary School and Telabastagan Elementary School for four days (October 4–7, 2004) at the Bulwagan Capampangan aimed at igniting “The Education Revolution” which is anchored on the belief that education is the best weapon against poverty.

The Mentoring the Mentors Program is one of the programs of Education Revolution which brings back the nurturing dimension of teaching. It introduces deeper and broader value to teaching that is anchored on the need for teachers to be a role model for the young. The sessionscombined inspirational and practical activities that engage the teacher’s whole person.

Its objectives are to empower the participants as agents of change by developing their mentoring skills; design an action plan on mentoring their co-teachers/mentees; and to implement the mentoring program which is suited to the varying educational and community needs.

The interests of all the participants were aroused by the effective and convincing lectures of the mentoring experts from the Foundation for Worldwide People Power, Inc., Marie Eugenie Institute of the Assumption, University of the Philippines, Philippine Association for Teacher Education or PAFTE and Center for Leadership and Change, Inc.

Dr. Evelyn Mejillano, as national coordinator, talked on the Overview and Roles of Mentoring including Active Teaching and Learning, Dr. Celia Adriano, as professor and director for instruction in UP talked on Designing Effective Instruction, while Ms. Chinit Rufino, as program director started the mentoring process on the call for social transformation – to shift perspectives from an akin mentality to an atin mentality.

To imbibe the atin mentality is to take care of the last, the least and the lost. To win, to strive for excellence, but to extend an helping hand to those who are left behind. She emphasized that with this new perspective, you will be ready to influence your co-teachers to mentor the students to become better citizens who care about their community and their nation.

Leadership for Service to remind participants that the call for Education Revolution is a call for change in the way we educate the young. It is a call to make learning beyond the exercise of imparting and acquiring knowledge.

Also discussed was Building Win - Win Relationships with topics on Stephen Covey’s “Seven Habits of Highly Effective People” and ChiaraLubich’s “Art of Loving” to guide the mentors through the process of investing in relationships.

The commitments by the participants are a manifestation that the four day mentoring the mentors workshop seminar somehow ignited the flame that would bring about a brighter future for effective education in our country. We are supporting this effort to improve the recently published article that only 33% of students from the public secondary schools passed the bridge program being implemented by the Department of Education.

WHEREAS, Republic Act No. 8525, otherwise known as “An Act Establishing the ‘Adopt-A-School’ andProviding Incentives Therefore and for Other Purposes” was enacted to encourage the private sector to assist in thedelivery of better quality education to public schools in the country, particularly in the poverty-stricken provinces, through construction and facilities upgrading, provision of books and other related materials, nutrition programs andmodernization of instructional technologies as well as staff and faculty development;

WHEREAS, SCHOOL wishes to improve and upgrade its facilities and capability to deliver qualityeducation to its students;

WHEREAS, ADOPTING ENTITY and CO-ADOPTING ENTITY are ready, willing and able to assistSCHOOL;

WHEREAS, ADVISOR is ready, willing and able to assist in the formulation and implementation of asustained development program for SCHOOL;

WHEREAS, this Agreement between the ADOPTING ENTITY, CO-ADOPTING ENTITY and the SCHOOL will not be affected by the change in the administration of both entities;

NOW THEREFORE, for and in consideration of the foregoing premises and the terms and conditions herein set forth, the parties hereby agree as follows:

Section 1 – The Project

The SCHOOL shall submit a project proposal that will improve and upgrade its facilities and capability to deliver quality education.

The ADVISOR shall assist the SCHOOL in designing and finalizing the project proposal.

The ADOPTING ENTITY and CO-ADOPTING ENTITY shall review and evaluate the project proposal and shall make available its resources to implement the project, once approved.

The SCHOOL shall make available to the ADOPTING ENTITY, CO-ADOPTING ENTITY and ADVISOR any information or assistance necessary to evaluate and implement the project.

The SCHOOL shall mobilize its stakeholders, including the Local School Board and the Parent-Teachers Association, in implementing the project.

The SCHOOL shall ensure that the assistance extended to it by the ADOPTING ENTITY and CO-ADOPTING ENTITY shall be preserved and shall be used solely for the purpose it was intended.

Section 2 – Term

This Memorandum of Agreement shall have a term of one (1) year, subject to mutual extension by the parties for another one (1) year, and so on.

Section 3 – Capacity and Authorization

The parties herein warrant that they have the authority and power to enter into this Agreement.

Section 4 – Validation by the Department of Education

This Agreement shall be subject to review and approval by the Superintendent of Schools of Pampanga. In accordance with RA 8525, the Program shall be administered by the Coordinating Council created under Section 7 thereof. The same shall likewise be subject to periodic review by the Local School Board.

Section 5 – Termination / Dissolution

The term of this Agreement may be shortened only in cases where the Adopting and Co-Adopting Entities are dissolved before the end of such period unless otherwise terminated in accordance with Section 4 of RA 8525.
