Lifestyle Goals for Individuals
• Weight loss – at least 7% loss and maintenance
• Physical activity – at least 150 minutes/week
• Nutritional goals– Dietary fat goal: <25% of calories
from fat– Calorie intake goal: 1200-1800
calories/day

Recruitment/Screening Eligibility
• Goal: 48/year• Screening at monthly Just Move It-Oneida events• Pre-diabetes diagnosis
– Not eligible: pregnant women, active alcohol/substance abuse, End Stage Renal Disease on dialysis, cancer
• OCHC Medical Referral• Assessment/Data Collection• Readiness Assessment• Wellness Vision
Assessments
• Medical clearance• Clinical measures (weight, height,
waist circumference, and blood pressure)
• Lab tests (lipid profile, glycemic measurement– A1C, urine albumin:creatinine ratio)
• Clinical history• Prescribed medications
Diabetes Prevention Program Curriculum
• 16-week sessions with emphasis on:– Self monitoring techniques– Problem solving– Individualizing programs– Empowerment and social support– Frequent contact with coach and DPP support
staff
