This applicant information page is tied to employment and is used for permits and authorizations. Notice the different title/heading it has at the top; in Part 5, the county superintendent must sign the form prior to its being submitted to our office for processing.10Form 7All first-time applicants must complete

This form is the applicants consent to have their background and fingerprint results shared with their employing county. It must not only be signed by a notary public, but must also bear the seal of the notary public. Student Teachers must be re-fingerprinted prior to application for West Virginia Initial Teacher License Form 2Short-term and Long-termSubstitutes includingSchool NurseForm 2 is the application for substitute teachers, including substitute nurses. If applying as a substitute nurse, you must remember to mark the application type as Long term and write in school nurse in the endorsement area.43Form 2First-time ApplicantUse newest formInclude the hire date and date training completedMark whether the application is for a short-term or long-term substitute If long-term, be sure to include the proper endorsement(s) Also remember to submit the Form 7, if a first time applicant. Be sure to use the newest form; it must have the applicant information page that says it is for permits and authorizations that require county employment.When completing the Form 2, make sure to write in the hire date and the date training was completed. You must also note whether it is for a short-term substitute or a long-term substitute. If it is for long-term, you must write in the proper endorsements and grade levels. Remember, for each endorsement area you write in, the applicant must have a minimum of 12 credit hours reflected on his/her transcript. The transcript must be an original, official seal-bearing transcript which also relfects a minimum of a bachelors degree awarded and the date it was awarded.44

An applicant may use this selection only one time (1) time to obtain a Substitute Permit provided that the Certificate has not been expired for more than five (5) years prior to the date of application. If the Certificate has been expired longer than five (5) years prior to the date of application for the Substitute Permit, the applicant must follow the requirements as listed in 11.7.3 A. to obtain the Substitute Permit45

Again, this is what the newest applicant information page looks like that you would use for a substitute. It must have a superintendents signature in part 5 and the heading at the top indicating that this is for permits and authorizations that require county employment. Note the yellow arrow. If an applicant is renewing a permit, and previously disclosed (answered yes) background information, he/she must again mark yes in the same area. If nothing has changed, he/she may simply note in the margin of the application that WVDE has information on file for the incident. IF applicant has something new to disclose, he/she must include a narrative describing the incident.46Using an Expired LicenseAn expired West Virginia professional certificate or an expired or valid out of state teaching license may be used only once in lieu of the 18 hours of training. Certificate may not be expired for more than 5 years prior to date of application.An applicant may use this selection only one time (1) time to obtain a Substitute Permit provided that the Certificate has not been expired for more than five (5) years prior to the date of application. If the Certificate has been expired longer than five (5) years prior to the date of application for the Substitute Permit, the applicant must follow the requirements as listed in 11.7.3 A. to obtain the Substitute Permit

Example of documentation of renewal hours for substituteThis is an example of county-approved professional development for substitute renewal. If you create a form such as this, make sure there is a place for signatures that, again, verify the training(s).49First Class PermitA first class permit (FCP) or out-of-field authorization (OOF) may be issued when an educator who is not certified for a specific employment position is hired by the county into the position, when no fully-certified educator has applied.A FCP or OOF is based upon county employment verification before the FCP or OOF can be issued.50First Class Permit Five Year Limit. The position held is not subject to posting provided the educator meets the annual renewal requirements and completes the state approved program within five years. The State Superintendent may extend the five-year limit, if extenuating circumstances warrant the extension.

Unless position is RIFd51First Class PermitInitial First Class Permit: The teacher certification specialist at the appropriate college/university must verify on the Form 1/1A application that 25% of the state-approved license program for the endorsement area has been completed. If not yet completed, the county superintendent may choose to submit a waiver request with the Form 1/1A application. OR, see next slide.52First Class PermitFor educators already holding an appropriate Professional Certificate, if the 25% program completion requirement for a First Class Permit has not yet been met, the educator may be employed on an Out-of-Field Authorization (OOF) until the 25% program completion requirement is met.

For an OOF in a teaching endorsement area, a professional teaching certificate must be held; for an OOF in a student support area, a [professional student support certificate must be held, etc.53First Class PermitFirst Class Permit Renewal: Six (6) appropriate renewal hours verified by the college/university must be completed each year for permit renewal. An official transcript must be submitted with each Form 1/1A.Both the college/university and the county must complete & sign each Form 1/1A.

Form 4 Professional Certificate Renewal, Conversion, or Permanent for Teachers and Student Support

Form 4 Student Support includes the following:CounselorSchool NurseSchool PsychologistSocial Services and AttendanceSpeech Language PathologistSpeech Assistant**Student Support staff are exempt from the Beginning Educator Internship requirementIn order to be eligible for a conversion or a permanent certificate, the applicant must have at least two years of public school experience in a combination of their endorsement area(s) and have at least one year of public school experience in West Virginia.60Form 4 Professional Certificate Renewal for Teachers and Student Support If you are applying for a 3-year renewal, you must complete the following on Form 4:Part 1 Check RENEWALPart 2 If you select Option 1 or Option 3, you must also complete Part 3 ( the original signature/date of the recommending college/university) and submit the official seal-bearing transcripts reflecting your 6 hours for renewal.--If you select Option 2, three (3) of your college credit hours must be in instructional technology per Policy 5202Option 2-instructional technology applies to all applicants. All valid Professional Teaching/Student Support Certificates are now after the January 1, 2008, time frame.61Policy 5202Instructional TechnologyAccording to Policy 5202, Section 10.1.4. B Three of the six semester hours must be a course related to the improvement of instruction through the use of instructional technologies

Student Support Certifications do not require 3 hours of instructional technology, but they must have 6 hours of college credit related to the public school system.62 Policy 5202 Instructional TechnologyThis coursework should address: instructional technology operations/concepts the plan and design of effective instruction supported by technology the implementation of curriculum plans that include instructional technologies application of technology to assess and evaluate student learning

If using eLearning courses, the instructional technology classes are indicated by a double asterisk (**) on the eLearning list of courses. If you want to confirm that a class meets the instructional technology requirement, you can email the course title/syllabus to Lori Buchanan at [email protected] for confirmation.63 Policy 5202 Recognition of College Credit for Renewal and Salary PurposesSection 9.6.6Only unduplicated coursework related to the public school program as defined in 126-136-9.6.7, completed through an accredited institution of higher education, as defined in 126-136-4.5, and subsequent to the issuance of the license being renewed and within the five-year period immediately preceding the date of application may be used for renewal of a license. Form 4 Professional Certificate Conversion, for Teachers and Student SupportIf you are applying for a 5-year conversion, you must complete the following on Form 4:

Part 1 Check CONVERSIONPart 2 If you select Option 1 or Option 3, you must also complete Part 3 (the original signature/date of the recommending college/university) and submit the official seal-bearing transcripts reflecting your 6 hours for renewal.--If you select Option 2, three (3) of your college credit hours must be in instructional technology per Policy 5202Continued

Form 4 Professional Certificate Conversion, for Teachers/ Student Support Part 4 Provide the completion date of the Beginning Educator Internship Exemptions: Student Support Staff ; Teachers with a teaching license prior to January 1992; Teachers with 5 years of teaching experience out-of-state.Part 5 -- Verification of at least 2 years of experience (both must be within the applicants endorsement area(s) and one must be completed in West Virginia) One school year of experience = 133 days.

Form 4 Professional Certificate Permanent for Teachers and Student Support If you are applying for a permanent, you must complete the following on Form 4:Part 1 Check PERMANENT (an applicant must either hold a Masters degree or have held two conversion licenses)Part 4 Provide the completion date Exemptions: Student Support Staff ; Teachers with a teaching license prior to January 1992; Teachers with 5 years of teaching experience out-of-state.Part 5 -- Verification of at least 5 years of experience (two years must be within the applicants endorsement area(s) and one must be completed in West Virginia) One school year of experience = 133 days.

Out-of-State LicensesIf applicant opts to use a valid out-of-state license at the time of renewal, then applicant must use a Form 20A or 20S instead of Form 4.

Valid out-of-state license can only be used once.

20A Initial Teacher Licensure20S Initial Student Support Licensure

The valid out-of-state can only be used once. If the applicant used the out-of-state certificate to gain the initial licensure, the same OoS certificate cannot be used again. 68Form 8 Additional Endorsement

Form 8 Additional Endorsement If you are applying for an additional endorsement, please complete the following on Form 8:Applicants RequestCollege Recommendation--this must be completed by the issuing college/university and must have the original signature/dateAdditional Documentation **Submit the official seal-bearing transcripts reflecting your degree earned/awarded**Submit a copy of your Praxis II results (if applicable)

Form 8 Additional EndorsementIf the applicant holds a Professional Teaching Certificate endorsed in specified areas of special education, they can add an Autism endorsement with 6 college hours in Autism. No college signature is required and they do not have to take the appropriate Praxis II exam.

The areas of special education in which the autism can be added with 6 hours of autism: Emotional/Behavior Disorders; Multi Categorical Special Education; Mental Impairments (Mild/Moderate); Severe Disabilities; or Pre-School Special Needs71 Form 8Additional EndorsementThe endorsement grade level will match their current special education endorsement.

This certificate cannot be renewed.

ELI training is completed through the West Virginia Center for Professional Development. Applicants can register at www.wvcpd.org. 75 Form 19 Renewal Professional Administrative Certificate If applying for a renewal of your Professional Administrative Certificate, please complete the following on Form 19:Part 1 Check RENEWPart 1b Select one option:6 hours of college credit Masters +30 60 years of age or older (provide a copy of your birth certificate)Part 3 --Check requesting the renewal

Form 19 Administrative Technology Three of the 6 hours must be in administrative technology per Policy 5202.Policy reads, must be a course related to increasing the capacity of public school administrators to improve instruction through the support of instructional technologies. .should promote a vision for technology integration, assurances that administrators recognize curriculum, instruction, and environments that appropriate support the integration of technology, the use of technology to enhance professional practice and increase personal productivity, and the social, legal, and ethical issue related to technology integration

Form 19 Permanent Professional Administrative CertificateIf you are applying for a permanent Professional Administrative Certificate, please complete the following on Form 19:Part 1 Check PERMANENTPart 1b Select one option:6 hours of college credit Masters +30 60 years of age or older (provide a copy of your birth certificate)Part 3 Check requesting a permanent and verify 5 years of appropriate experience. Two years must be within a combination of the applicants administrative endorsements and one must be completed in West Virginia.

Form 26 Content Endorsement for Special Education

Form 26 Content Endorsement for Special EducationApplicants must currently hold a Professional Teaching or Alternative Teaching Certificate in:Autism 5 ADEmotional/Behavior Disorders 5 ADSpecific Learning Disabilities 5 ADMental Impairments Mild and/or Moderate 5 ADMulti-Categorical (LD, BD, MI) 5 ADHearing Impaired and Vision Impaired are being added to the revised Policy 5202.80 Form 26 Content Endorsement for Special EducationApplicants can use the appropriate Praxis exam to earn an endorsement in a subject area in place of coursework.

**First time applicants for Form 26 can apply for any/all areas (English, Mathematics, Biology, General Science, or Social Studies) without any course work. The initial and subsequent renewals are issued for only one year.Continued

If adding a Form 26 with a 20T, one content area must be completed in its entirety in order to be issued a 3-year certificate unless the applicant has an elementary education endorsement. If the content area is not completed, then the initial teaching certificate will only be issued for one year.81Form 26Second and third year applicants must have 6 hours of coursework filled in on Form 26 and have official transcripts documenting the coursework. ( eLearning courses can be documented on an official transcript OR on an eLearning certificate)

**At end of third year, if all coursework is completed in the endorsement area(s), the applicant will be issued a permanent endorsement.

**At end of third year, if all coursework is NOT completed, the endorsement area(s) will expire and cannot be renewed.

Advanced SalaryAll graduate level hours reflected on an official transcript of an appropriately-accredited college/university, related to the public school program or to the specific endorsement area(s) held, may be used for advanced salary on a professional certificate.IHE must be appropriately-accredited as required in Policy 5202, section 4.5.83 Advanced SalaryA maximum of 15 undergraduate level hours (including post-baccalaureate and other non-graduate level hours) may be used for salary IF completed after the effective date of the initial WV professional teaching, student support, or administrative certificate.Form 12
