OHS staff works as a team to provide U-M Ann Arbor campus and Health System faculty and staff with care that is convenient effective caring

FAA Physicals – (Federal Aviation Association) Pilots-1st, 2nd, and 3rd class (self-pay)
Drug Testing – specific to certain groups
Reporting/Scheduling New Employees
Department determines if employee needs to process through OHS via Assessment Tool on OHS website
Department completes New Employee Routing Form on OHS website for employees who need assessment at OHS and submits online
New employee calls 764-8021 to schedule new employee health screening.
Work Injuries Employee or Supervisor: Complete online Work
Connections Illness/Injury Report Form, print prior to submitting and have employee bring copy to OHS clinic

Physicals/Testing Hospital – OHS will contact to schedule annual
appointment Campus –
• Department - Contact OSEH at 615-2140 to determine if your employee needs to be in any program
• OSEH will submit request to OHS• OHS will schedule employee and contact via email
